Fitness Foundations: How to Save the Healthcare System By Kelly Farris, ACSM CPT Three hundred eighty one billion (that’s billion with a B) healthcare dollars will be spent on preventable diseases this year. Chronic diseases, like type 2 diabetes, heart disease, and high blood pressure, are responsible for seven out of ten American deaths. Cardiovascular exercise can prevent more than forty chronic conditions. If cardio exercise was in a pill, it would be the most prescribed drug in the world. It would be a wonder-drug that costs next to nothing and is available to every person. Lack of exercise will cause 7 million early American deaths this decade. It is abundantly clear that a healthy lifestyle must be part of the healthcare equation. The American College of Sports Medicine, the largest sports medicine and exercise organization in the world, recommends 30 minutes of moderate physical exercise 5 times per week. Lack of time is the number one excuse for not exercising. Do you hangout on Facebook, watch TV, surf the web, read endlessly for hours? If you answered yes to any of these, then you do have the time. It’s all a matter of how you use it. So how do you get moving? The immediate solution would be to walk straight out your front door for 15 minutes, then turn around and walk back. Visit one of our fabulous parks. Join a gym that offers a variety of cardiovascular exercise options such as treadmills, bikes, and ellipticals. If you don’t like exercising alone, look for cardio group fitness classes, like spinning, Zumba, or kickboxing. Any class that gets your heart rate up and keeps it up is considered a cardio class. Classes are a fantastic option for cardio exercise. What class is right for you? In my gym we encourage you to try everything. All classes can be modified to all fitness levels. There are many hybrid classes out there that offer strength and cardio together. Yoga, Pilates, and flow classes are a nice addition to your cardio exercise, but they cannot replace it. Get out there and get your heart pumping!

On the Cover: NG3 Works School by School to Make a Difference to the Next Generation By Heidi Campbell Character. Community. Change. Those three words represent the tenets of the non-profit organization known as NG3, which is inspiring the “next generation” of student athletes to build relationships and become true servant leaders in their communities. President and founder Matt Williams has made this organization his full time mission. He recruited former New Orleans Saints offensive lineman Jon Stinchcomb to direct the operations of the organization. Williams and Stinchcomb are both local alums from not only Brookwood and Parkview High Schools respectively, but UGA as well. They are working together with seven school-level directors and many local volunteers to make NG3 a household name. “NG3 is about character development, creating small mentoring groups, and providing opportunities for kids to lead and serve,” explains Williams. Williams admits that he kind of fell into the concept of NG3. He had been working with another non-profit when then Brookwood coaches Mark Crews and Dave Hunter asked if he would consider joining their football team as a kind of third party for the kids. “At the time,” Williams recalls, “I didn’t know what that was supposed to look like, so I just started showing up. Every day. Little by little, we started doing some new things with character lessons and voluntary Bible studies, until I felt comfortable enough asking to do it with the 9th grade team. From there, the goal was to get the kids into smaller groups and mentor them off campus.” Williams has fine-tuned the organization so that each high school has its own director. These school directors, most having a ministry background, work daily with the coaches and athletes to build strong, trusting relationships. The focus is on character development, integrity, respect, perseverance, and core values. These school-based meetings offer inspiration to connect students from all faiths. Once relationships are established, the directors create “huddle groups” that meet off campus. These groups, consisting of ten to twelve athletes, are paired with community volunteers who act as guides for faith-based discussion groups. “The heartbeat of our program is the

mentorship groups,” explains Stinchcomb. “We want to find the right folks who will reinforce good values and be mentors to these kids. The entire organization is dedicated to building relationships, giving kids a positive voice in their lives, and serving the community.” NG3’s mission has caught on in seven major high schools, and there are a number of schools who have requested an NG3 presence on their campuses. Williams has always wanted to make NG3 a community staple all across the country, but wants to “grow smart.” Stinchcomb is thrilled to be a part of this growing mission as they expand to other schools in Gwinnett and Walton counties. “I finished playing football in 2011, and was fortunate enough to be able to Continued on page 16

I love family and community. This is one of many reasons I enjoy publishing two monthly magazines dedicated to these topics. I believe there is more good in the world than is ever reported. Moreover, many of us view our new world as overloaded with information moving at warp speed – quite impersonal and negative in nature. As a side note, I experimentally cut off my own access to my personal Facebook and Instagram feed for six weeks. I will let you know of the results. It is part of a study I am doing on human behavior and how much is too much connectivity. If you watch traditional news (and how can you miss it), you’re shown a world that looks out of control and about to combust. If I did not know better, I would think there weren’t any good, loving, and caring people left. But let me tell you something. There are more amazing people than you can ever imagine. Yes, we do have a lot that I grieve over. The senseless killings in recent months make me sick. Terrorism and no concern for life make me ill as well. Moreover, the “blanket judgement” of people based upon their skin color, nationality, sexual orientation, religion, creed, gender, etc. is something that deeply bothers me. I believe most people (at their core) seek unification, not division. I desire peace, love, harmony, laughter, and a better world for my children and future grandchildren. You see, the Our Town community is simply a microcosm of all that is happening around America and the world. Right here in our own communities we have our own opinions, beliefs, values, and worldview. You know UNTIE (coming undone) and UNITE (coming together) are not that different. Thus, I challenge each of us to focus on what unites us, not divides us. As human beings, made of flesh and blood, we are all here on this earth for a short time. There are so many wonderful things going on around the world and community that are never covered. In the Our Town community, we have amazing people doing incredible things to make our community awesome. We have super families, great schools, amazing teams, wonderful parks, and people of integrity and character. Our diversity is one we embrace and one that makes us stronger. We must never give in or let up. We must work even harder (each and every day) to care about each person we meet. Will you help me challenge others to think of ways to build our community, to become even better, to refrain from criticism, and to focus on solutions? Remember, every human has emotions: joy, love, anger, frustration, anxiousness. Together we can make a difference and build stronger bridges of understanding between one another. I could not be more proud of where I live, work, and play. People need encouragement and are eager to hear a kind word. People need to laugh and they want to smile. Let’s make another person’s day amazing. Let’s rock his or her world. A community is strengthened one person at a time. I am so glad you are with me as we continue the journey of my town, your town, OUR TOWN!

“We will never get into the restaurant business! This is crazy!” the Chen brothers – whose families owned restaurants in New York and Connecticut – told each other as kids. But as Ken and Jimmy got older, they saw an opportunity to not only “get into the restaurant business,” but to also do things differently. The Chen brothers observed how things “were always done” in Chinese restaurants, and they were determined to change the things that bothered them when they opened their first Ricebox Chinese Restaurant in Gwinnett in 2014. To them, it often appeared that customer service in Chinese restaurants was bad because of a prevalent language barrier. “We wanted to overcome that and offer better customer service,” says Ken. Next on the list of improvements was simplifying the menu. Chinese restaurants often have huge menus which confuse people, so customers just order the same few popular dishes. The Chens pulled together the most frequently ordered items to create their simpler menu, including Chinese and Thai cuisines. The third and perhaps most impactful change the Chens implemented in their new restaurants is the cooking method. “Over 90 percent of Chinese restaurants cook food in oil – everything goes through oil before it’s cooked,” the Chens note. “We asked why, and were told, ‘because that’s how it’s always been done.’ We questioned that. We want our food to be healthier.” The Chens challenged the hundred-year-old standards of Chinese restaurants in the US and created a better plan for a healthier experience. “Steaming takes longer, but it is healthier,” the Chens say, “and we never used canned products. All our vegetables and produce are fresh, and whenever possible, we get them from local farmers’ markets.” They are on a mission to change the perception that take-out Chinese food is greasy. Each menu item is carefully considered, too. When the Chens add a new dish to the menu, they give it three months. If the item doesn’t sell more than three times, they consider revising or removing it completely. That way the menu is always fresh, updated, and easy to understand. Additionally, the Chens have listened to their customers’ requests for gluten-free dishes, including the favorite General’s Chicken. Their revised recipe no longer uses flour for the breading, and soy sauce without wheat has replaced the traditional version. Ken says the glutenfree soy sauce is more expensive than the “regular” kind, but he and Jimmy believe it is worth it. “We personally approve all the ingredients in our supplies. That way we can be sure we are offering the best gluten-free options.” Ken and Jimmy are also of the philosophy that in order to gain the support of the local community (now with a location in Centerville and a second in Lawrenceville), they must first support their neighbors. “We are not just here for business,” Jimmy says. “We care about the community. We want to help our community as a whole; to give back any way we can.” The Chens and Ricebox give back by hiring local people and buying produce from local growers whenever possible. Finally, they are involved with charities and co-ops in Gwinnett County. Among them are Give Hunger the Boot, a program that supports the South Gwinnett Continued on page 13

Cornerstone Coworking: Coworking, Meeting, and Event Facility By Daidra Annis Small business owners have a friend at Cornerstone in Lawrenceville. Located at the corner of Crogan and Culver Streets, the property was built on the Historic Square a little less than three years ago, and offers much needed space and resources to small businesses. The project was started by Novologic when it had outgrown its own space. In the process the need for independent office space in Lawrenceville was identified. And, as they say, the rest is history and Cornerstone Coworking began. Taking its cue from the virtual office trend, Cornerstone is a one-stop shop for its members’ office needs, providing everything from office space and conference rooms, to an open common area, reception, and mailbox services. Diabetes is one of the country’s fastest growing epidemics. According to the Centers for Disease Control, 29 million Americans are affected with diabetes and 86 million are living with prediabetes. Unfortunately, 90 percent of those with prediabetes are unaware of their diagnosis. The good news, you may ask? Both conditions can be effectively managed with lifestyle changes. Diabetes occurs when the body is not able to efficiently use glucose, its primary fuel source. The pancreas produces insulin. Insulin pushes glucose into various cells in the body and allows those cells to use glucose to fuel various cellular processes. In diabetes, the body does not recognize insulin. The pancreas has to produce more insulin than normal, resulting in pancreatic burnout, increased fat deposits, and ultimately prediabetes turning into type 2 diabetes. Type 2 diabetes is one of the leading causes for death in America. It is one of the number one causes for strokes, heart attacks, kidney failure, limb amputation, and blindness. Patients are typically diagnosed with Type 2 diabetes eight to ten years into the disease process. With better diagnostic tools available, clinicians are now able to identify patients earlier in the disease process. Patients at risk for diabetes can be identified when repeated fasting glucose measurements are above 100 mg/dL or when A1C measurements are above 5.7%. Diabetes is diagnosed when repeated fasting glucose measurements are above 125 mg/dL or when A1C measures above 6.4%. Continued on page 20

Crane’s Corner: Political Insights from Bill Crane By Bill Crane The closest presidential election in US history was the 2000 contest between Vice President Al Gore and then Texas Governor George W. Bush. As you will recall, the tight contest hung by the twenty-nine electoral votes from Florida after a strong ground-game by the GOP had moved Ohio and Virginia back into the Bush win column. Winning Florida would give Bush 271 electoral votes, with 270 required for victory in the Electoral College. Without expending the entire column recounting those days and weeks, the US Supreme Court interceded and finally ended the Florida Supreme Court directed recounts that were underway in several counties. The final tally gave Bush a statewide margin of only 537 votes among sixmillion cast in the state, and he was later awarded all of Florida’s electoral votes. The Electoral College selected Bush as victor by a margin of one vote. Election laws in most states have mechanisms in place for close contests. Most allow for a recount, at the cost of the state or local jurisdiction, if the margin is less than a single percentage point. It is not unusual for there to be close contests, particularly in local elections, but it is rare that recounts change the outcome or result. In Georgia alone, there are more than 3,000 voting precincts, spread across 159 counties. Each precinct has dozens of individual voting machines and chip enabled voting cards. These machines are each tabulated at the precinct level, relayed to the county, tallied, and then relayed to the state. The state systems are fifty closed loops. There is no central cloud, main frame, or national computer tabulating the results waiting to be hacked. Voters in each state select a slate of Electors, as there is actually no direct popular vote for President. These Electors are awarded and sometimes directed by varying state laws, with the bulk giving winner-take-all to that state’s popular vote winner by plurality. But even losing by only one Electoral vote, in a state where the President-elect’s own brother was the governor, Vice President Gore did not allege a conspiracy nor suggest that his supporters might take to the streets in protest of the result. In fact during the most heated moments of the recount, Gore and Bush each urged their respective followers to remain calm and let the system play itself out, while sending a flood of lawyers and election experts from each side to Tallahassee, Florida. As of this writing, with early voting underway in more than twenty states, and that number rising above thirty with early absentee voting, the contest polling is not close. Of the 100 national polls since the two national political conventions, Clinton has led in 86. Of the 14 where Trump has led, that lead has most often been within the margin of error. And I suspect in each of those states the down ballot winners and losers will each accept the will of the voters, await recounts as they are entitled in contests separated by less than a percentage point, and then either move in or move on from the office they held. And that is the way we do things in the world’s most established and respected republic. The republic survives and the will of the people is followed. Are you listening, Mr. Trump? Medical Matters: Wide Awake Hand Surgery By David I. Katz, MD Hand surgeons have the ability to perform surgery on the arm without general anesthesia, sedation, or the need for a tourniquet. Local anesthetic is gently injected into the affected area of the wrist, hand, or fingers – which is similar to how dental or dermatologic procedures are performed. During surgery, the patient remains alert and comfortable, while afterward patients can get up and go home without any of the side effects of sedation. This form of surgery has come to be known by its acronym WALANT, which stands for: wide awake local anesthesia no tourniquet. WALANT offers significant advantages for patients. While modern anesthesia is one of the safest areas of medicine, there are always potential side effects and risks with sedation and general anesthesia. This is especially true for individuals with multiple medical problems. Less sedation is safer than more sedation, and the safest sedation is no sedation at all. Before receiving any surgery involving anesthesia, many patients are required to go to the hospital or surgery center for tests and/or lab work. With WALANT, there is no need for preoperative testing since patients do not receive sedation. This eliminates the need to take even more time off work or school before surgery. The night before surgery typically means having nothing to eat or drink after midnight. This requirement is important for patients receiving sedation or general anesthesia. However, with WALANT, patients can eat and drink normally the morning of surgery as they will not receive any of the typical anesthesia medicines. This is particularly helpful for diabetic patients and patients on blood thinners, as they can follow their typical diet and medication regimens around the time of surgery. Continued on page 20

For the Love of Tennis: Meet Shawn Arnette of Courtcraft Tennis By Lisa Brittain Two years into owning and running Courtcraft Tennis in downtown Lilburn, Shawn Arnette is living the fulfillment of his combined career experience in corporate finance and as both a tennis pro and a collegiate tennis athlete. In a recent interview, Shawn stated his purpose in opening this tennis shop was to provide a full service experience to tennis enthusiasts from beginner to avid competitor. Originally from Brevard, North Carolina, Shawn first played tennis with his father in their yard. According to Shawn, his dad kept their grass cut so short they could hit a tennis ball back and forth. He teasingly tells people that he began his tennis career on a grass court. After going away to college at Appalachian State in Boone, Shawn landed a summer job at a resort. There he learned to maintain the clay tennis courts. Because of his daily interactions with the tennis pro, tennis equipment, and tennis enthusiasts, Shawn developed a real love for the game itself. He made use of the available equipment and courts to practice the game every opportunity he could find. Once back at school, Shawn pursued his passion for the game. He practiced alone on the tennis courts, read books and articles to learn more of the details and mechanics of the game, and played as often as he could find a partner. Eventually, as a walk on his senior year, Shawn

earned a spot on the varsity tennis team. After graduation, Shawn set his sights on becoming a tennis pro. His first job was in a local sporting goods store in Boone. This is where Shawn learned a valuable skill, which he still employs today at Courtcraft: stringing racquets. It was a fortunate connection with a customer during that first job that led to an assistant tennis pro position at a local area tennis resort. Very soon afterward, he accepted an open position as Director of Tennis for Fairfield Sapphire Valley in Cashiers, NC. The sunshine and warmth of Florida called to Shawn sometime later. Living in Sarasota, he became certified through the United States Professional Tennis Association. During the five years he lived in Florida, Shawn coached others to share his passion for tennis. Finally, he became the Head Tennis Professional at the Royal Racquet Club in Clearwater. After several years of coaching all day in South Florida’s summer heat, Shawn decided to relocate to Georgia, where he pursued his post graduate education at Georgia State University while continuing to teach tennis. He eventually earned an MBA. According to Shawn, his first “real job” was with Oglethorpe Power in Tucker, GA in corporate finance. In time, Shawn worked his way to a senior vice president of finance. However, he never lost his appetite for tennis. Shawn was constantly conjuring up plans to combine his love for and expertise in tennis with his business knowledge and skill. He desired to become an entrepreneur. One day while getting his hair cut at the local beauty salon in downtown Lilburn, Shawn noticed a “For Rent” sign on the building across the street. The wheels began to turn with regard to opening a tennis specialty shop. Shawn’s dream became a reality two years ago when he opened Courtcraft Tennis. Shawn desires to add value to the Gwinnett community through customer service and tennis expertise. He has already developed quality tennis relationships with local high schools – Parkview, Brookwood, Providence Christian, and Greater Atlanta Christian. Shawn looks forward to developing tennis relationships with more local schools and parks through coaches and tennis fans alike. NG3 Continued from page 5 choose what I do for a living,” Stinchcomb shares. “I feel very strongly that great blessings equal great responsibilities. I felt driven to use my platform to make an impact. So many people went above and beyond to invest in me, and now I can do that for the Next Generation through NG3.” The students involved in NG3 have worked on numerous philanthropic community projects. The students at Parkview High School, for example, have worked with Habitat for Humanity to rehabilitate homes. The Brookwood huddles have worked with the Miracle League and adopted families during the holidays. Others have worked in local food pantries and helped with the Cardiac Step Down programs. “The goal is to show these kids that they don’t have to go far to make a difference in their community,” says Stinchcomb. “We try to offer lots of local opportunities for them to discover the joy of giving.” Donations and foundation grants help to support the NG3 programs at the school level. Several of the schools share the proceeds of Christmas trees sales in December. The organization hosts a yearly fund raising banquet which includes, for $50 a person, dinner, a live and silent auction, and a guest speaker. Last year’s banquet was held at 550 Trackside, a refurbished train depot in downtown Lawrenceville, and featured speaker Chris Conley, a wide receiver from UGA who currently plays in the NFL for the Kansas City Chiefs. The continued success of NG3 relies on the help of dedicated volunteers. “We need help!” declares Williams. “We need people to come lead small groups, donate financially, and get involved with our staff. We’re only as good as our people, and we need a lot of them!” Williams and Stinchcomb are excited about the future of NG3. Former student, Will Bearden, felt so moved by his experience with the group that he helped create a pilot NG3 Student Leadership Program at Archer High School, where juniors and seniors meet weekly with underclassmen to talk about the importance of good character and a service oriented life. “This job is so rewarding,” Williams shares. “It’s fun to see our staff be successful with kids and communities. One of my favorite things is when kids come back from college and talk about what it meant for them and how it changed them. There’s nothing like making a difference in someone’s life. You can’t buy stuff like that.”

Travel Tips: 48 Hours in NYC By Karen Tanner-Holt Amazing what you can do and see in New York City in just two days. Arriving before noon on Friday gave me time to explore before dinner. I decided on a self-guided walking architectural tour. Heading east, I started with Radio City Music Hall, past Rockefeller Plaza, and then down the street to St. Patrick’s Cathedral. A few blocks south and it was hello Grand Central Station. Lastly, as I headed back to the hotel, I wandered into the beautiful and not to be missed New York Public Library. Up next, dinner at the Central Park Boathouse. What an amazing view! A bit pricey for dinner, but the food, view, and good company made this more than worth it. Saturday morning dawned beautifully for a visit to the Statue of Liberty and her neighbor Ellis Island. As the ferry approached Liberty Island, I was mesmerized by the beauty of Lady Liberty’s patina and the freedom for which she stands. I can only imagine how the weary immigrants must have felt upon seeing her: awe, fear, hope, relief? I felt pride and an unexpected pang of gratitude for the French who sent her our way. Next up, a Broadway show. As a lover of musicals, I was beyond excited to see An American in Paris. I was not disappointed. I felt like I had been to a musical and a ballet all rolled into one. The evening ended on a happy note with a couple of slices of NY pizza and a discussion of the show. Sunday came much too fast, but there was still time to wander through Central Park and enjoy the many talented performers scattered about the lanes as we headed to the Guggenheim Art Museum. The Guggenheim is a wonder from the mind of Frank Lloyd Wright. As with all museums of this type, the traveling exhibits combined with the permanent exhibits create a unique experience each time you visit. Time to head home, but not before having a little scoop of gelato from a vendor outside the museum. This was probably the best gelato I’ve had since Italy. What an amazing 48 hours! My daughter tells me that her dad listens to the comedy channel “to keep him from getting mad at all the stupid drivers” while he is stuck in traffic each day. Note to self: Think twice before calling the ex during his commute. Be kind to someone. My boyfriend, who commutes daily in Atlanta traffic and was raised in the South, came up with this one. He says that unlike New York drivers (where I hail from), Southerners are polite and let other drivers in ahead of them. It’s true. As a native New Yorker living in Atlanta, if I don’t allow someone to merge in front of me, or wave them to go first at a four-way stop, it is considered extremely impolite and will adversely affect my karma at the very next intersection. Next time you find yourself in traffic, blast the Beethoven, keep a sense of humor, and voila! Your commute won’t be so bad after all. Community Member Spotlight: Meet Amanda Pilz By Daidra Annis “Take care of your body. It’s the only place you have to live.” ~ Jim Rohn It seems like everyone is jumping on the “Wellness” bandwagon lately, but for Amanda Pilz, wellness has always been a way of life. The CGrayson • Grayson resident and owner of ProMotion Physical Therapy and The Locally Owned & Wellness Studio at ProMotion has committed her life to improving theOperated Since 2001 wellness of others. Born and raised in Pennsylvania, Amanda is a lifelong athlete who experienced the benefits of physical therapy first hand as a collegiate gymnast. Her awareness of its benefits led her to Duquesne University, where she found her passion in teaching others to take ownership of their well-being through physical therapy. Amanda and her husband Matt came to Georgia because of a job relocation. In 2001 ProMotion Physical Therapy was founded. What began as a one room office within a fitness center eventually expanded to include a location in Loganville and in Grayson. In 2014, ProMotion combined into a single facility in downtown Grayson to focus on the health needs of the community. Having one location has allowed Amanda to add The Wellness Studio, which offers all things health and wellness related. Some of the services include beginning to more advanced level exercise classes such as strengthening, flexibility, and cardio activities, yoga, Zumba, “Wellness Talks” given by her professional physical therapy staff, and community educational workshops on topics like meditation, essential oil use, juicing, and even the occasional couples’ massage! The idea for The Wellness Studio came from Amanda’s desire to help patients who have completed physical therapy and were looking to continue their healing process after discharge, but were not comfortable in the traditional settings of a gym With four children in the Grayson Cluster Schools, Amanda has been a Ram Supporter for many years. In a show of support to those who serve Grayson’s young citizens, Amanda rewards Starling Elementary’s teacher and volunteer of the month with a Class Pass to visit the studio. In 2015, she expanded her programs to include the younger members of the Grayson community. Additionally, Daycare Yoga launched in 2016 and is similar in feel to a summer camp program. Three classes are bussed to the studio each week, with as many as seventy children participating. Another children’s program, the Young Yogis Camp, teaches health, wellness, and centering. A key concept of this camp is how to manage stress, a skill that children certainly need as they grow and develop into adulthood. For the more mature folks in the community, Yoga in the Park debuted this fall at Grayson City Park. Yoga for Seniors is another free program that is now offered at the Grayson Senior Center. Wellness for every individual is the focus for everything Amanda does. Her favorite part of her job is serving others and making an impact in their lives. She is proud that ProMotion Physical Therapy provides an environment where her patients can be guided through individualized care. One-on-one therapy time is uncommon in the industry, but that’s the only way it’s done at ProMotion. Amanda believes that this practice allows her team to better guide patients through treatment and to communicate better with physicians who have a thorough understanding of a patient’s history and the environmental factors that may affect healing. As Amanda celebrates her fifteenth year in business, she recognizes how fortunate she is to be where she is today. She credits her success to a wonderful group that includes 11 staff members and 10 wellness instructors, and of course, through word of mouth/patient referrals, the backbone of any small business. The team at ProMotion Physical Therapy and The Wellness Studio will be marking the occasion with a celebration week for patients and staff. Two pivotal trials, the Diabetes Prevention Program (DPS) in 2005 and the more recent YMCA Diabetes Preention Program, proved that diet and exercise can prevent or delay the progression of Type 2 diabetes. DPS randomized 3,234 patients with impaired fasting glucose (defined as a fasting glucose level between 95 mg/dL – 125mg/dL) to three groups. One group was given placebo, the second group initiated lifestyle changes which included a 7% weight loss and 150 minutes of vigorous exercise per week, and the third group was given metformin 850 mg twice daily. DPS showed that the lifestyle modifications group outperformed the metformin group! Ask to be screened for prediabetes and if found, start lifestyle modifications immediately. Diet and exercise are proven strategies to delay progression of diabetes. Just Do It!

Surgery with sedation commonly involves starting an IV in the arm. For patients undergoing wide awake hand surgery, there is typically no need for an IV. Discomfort is limited to a single brief prick of a small needle to numb the hand. WALANT also eliminates the need for a tourniquet on the arm which can cause some degree of soreness after surgery. Lidocaine and epinephrine (the typical combination of medicines used in WALANT) are two of the safest and most widely tested injectable drugs used in humans. Since 1950, dentists have injected literally billions of doses of these same two medications in their offices with no preoperative testing, no IV insertion, and very few side effects. Over the last twenty to thirty years, hand surgeons have established the safety of injecting lidocaine and epinephrine into the wrist, hand, and fingers. During the procedure, patients who are awake and alert have the added advantage of being able to talk to their surgeon. This time is well spent discussing the postoperative care of the hand and how to avoid complications. Patients should feel free to ask their surgeon questions about the procedure as well as things to look out for afterwards. At times patients may even be able to see their anatomy which can help with their recovery. None of this is possible when asleep or sedated during a typical operation. Without sedation, patients avoid many of the possible undesirable side effects associated with anesthesia and opiates. After wide awake hand surgery, there is no nausea, vomiting, urinary retention, or constipation. This means less time in recovery at the hospital or surgery center. Most patients are able to simply get up and go home after WALANT. Wide awake hand surgery has meant greater convenience, less expense, and lower risk for many patients. While WALANT is possible for a variety of procedures, it is not feasible for all surgeries. Additionally, some patients may not be suitable candidates to undergo wide awake hand surgery – and other individuals may simply choose to have anesthesia for their operation. There are many different factors that go into the decision to have wide awake hand surgery. If interested, feel free to ask your surgeon if it is a good option for you!

Legal Lessons: Avoiding Real Estate Title Problems after Death or Divorce

By David Walker A real estate closing is about to happen when a title problem appears. The seller is a widow, but the title to her home is still under the names of both her and her deceased husband. A deed is needed from the executor of his estate in order to allow her to sell the home, but her husband’s will has never been probated. This can take time, so the closing may have to be postponed or it may fall through. This event happened recently, and our office was asked to handle the problem. We were able to probate the will by obtaining the consent of all the heirs, including out-ofstate children of the deceased husband. Fortunately, they were willing to consent, so the process did not take too long and the closing did take place as planned. Problems like this often occur if the seller is a widow or widower and the title of the home has never been updated to remove the deceased spouse’s name. Or, the seller may be divorced and the former spouse’s name has never been removed from the title. In either of these situations, the problem could have been avoided by preparing and filing the deeds necessary to update the title soon after the death or divorce. Any person who inherits property, or is going through a divorce, should obtain legal advice to check the title to their home to see if legal work needs to be done. In the situation of a widow or widower, if the house was owned jointly, the will would need to be probated, or the estate would need to appoint a personal representative or executor who can sign the necessary deed. In a divorce, the party receiving the home or other real estate should have that stated in the divorce settlement and any necessary deed should be signed and recorded at the time the settlement documents are signed. Our office handles these types of matters and would be glad to be of assistance.

Pet Perch: Fish Oil for Dogs By Terie Hansen There is a lot of talk about fish oil and the benefits for both humans and dogs. What makes fish oil so special, and is it really that beneficial for your pet? Yes! Fish oil contains Omega 3 fatty acids (beneficial fats) that can be a healthy supplement when given to your dog regularly. Here are the main benefits of fish oil: • Bone and joint health – helps lubricate joints, thus allowing them to move more freely • Skin and coat – adds moisture to dry, irritated skin and improves texture and shine of coat • Increased energy • A lower risk of cancer because of its antioxidant properties • Improved immune system • Anti-inflammatory – especially helpful for dogs suffering arthritis or post orthopedic surgery • Supports healthy brain aging and cognitive function Fish oil can be helpful in keeping your dog in top shape well into old age, so it’s best to start now rather than wait until your dog exhibits health issues. Oil from fish contains eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA); both are omega-3 fatty acids. Knowing how much to give your dog is important. A good rule of thumb is a daily dose of 20 mg of Omega 3 per 1 pound of body weight. So a 50 pound dog would take 1,000 mg of Omega 3 (EPA & DHA). This might entail giving 2-3 softgels a day, depending on the size of your dog as well as the amount of Omega 3 in each softgel. Don’t confuse EPA (a specific fatty acid) with EFA (a general term for essential fatty acids). EFA includes EPA & DHA (both Omega 3) as well as Omega 6, ALA, CLA, and other fatty acids. Be sure to read the label to see what is really in there. Pets need a supplement high in Omega 3, without any additional EFA. Check with your veterinarian to make sure your dog can ingest fish oil.
