Letter from the Editor U This letter is being constructed in my yard, where curious catbirds, cautious cardinals, and skittish wrens have been popping by to feed themselves and their young. Juvenile songbirds, sticking close to mama and papa bird, are trembling their wings as they beseech for food. They’re still relying on their parents for support, toeing (“beaking?”) the line between needy childhood and independence. STOP Bedwetting with these helpful tips.

by Jacob Sagie, Ph.D. It’s a milestone towards which every parent strives: The day when they’ll finish buying diapers or nighttime pull-ups for their child. With this milestone comes passage into an age of maturity and responsibility for the child, not to mention a welcome reprieve on our bank accounts. So what happens if that time seems never to be coming? What if your child has long ago passed his 5th or 6th birthday, and he is still not getting through the night without accidents? Or what about when he hits the double digits? You’ve already asked the pediatrician what to do seemingly countless times and were told to simply let the child grow out of it. But how long should you wait before taking another course of action? Parents come to me all the time after having listened to their family doctor and waited. But when your child is suffering from heightened stress, frustration, and embarrassment, why should you merely wait? Why add months or even years of distress to his life? As a parent, I was once confronted with just this situation. My son, at age 6, was still having accidents at night. Completely dry and able to go the whole day without any incidents, nary a night went by when we didn’t have to change the sheets. We were exhausted not only because of the middle-of-the-night housekeeping duties, but also because, as much as we tried, we couldn’t find any real solution for his “problem.” Our doctor sent us for many tests: blood tests, urine tests, you name it. But nothing yielded any results. The reason? Because in

nearly 99 percent of children facing bedwetting, nighttime accidents are not a result of a medical condition. Bedwetting may be the result of a learning fault of the brain’s reflex system that occurs when the child is in a state of very deep sleep. These are otherwise healthy children who, through no fault of their own, aren’t capable of recognizing the signals that the bladder is sending to their brains. It pains me when I hear of children who are being punished for bedwetting, made to change their own wet linens to “teach them a lesson” with the intent of forcing them to learn to wake up and go to the bathroom. Confronting this ignorance requires that parents become educated and effectively assist their child in overcoming this condition. Within pediatric medical circles and the parenting community there is an overwhelming lack of credible information and resources available to those parents who are desperate to do something. Commonly repeated “helpful hints” are often wrong and can even be detrimental to the child. For example, I bet you’ve heard that you should take your child to the bathroom before you go to bed yourself. But by doing this, you’re taking the responsibility away from the child, and he will fail to train his brain to recognize the necessary signals. There are people, myself included, who have a focused practice to fix bedwetting. Safe and proven methods have already treated millions of children around the world and are available to the millions more who are still suffering.

So what can we do? •Educate. Knowledge is power, and it will help guide whatever decision you make. •Do not ignore the problem. The condition exists, and your child needs to feel that he has a support system. •Be available to your child. He is the primary sufferer of this whole ordeal and needs to know that he is not alone. •Show your child information available online about other kids who have been dealing with this problem. Let him see that there is nothing wrong with him, and that bedwetting can be overcome. •Don’t compare your children. Highlighting that a younger sibling is not having accidents just makes the affected child feel worse. •Seek professional help when your child shows concern for the condition. Dedicated bedwetting clinics and online resources are available for exactly this purpose and have safe and proven treatment options.

These scenarios describe what it is like to have an auditory processing disorder. Some children are sensitive to sounds (hyperresponsive to auditory stimulus) and some children ignore sounds (hypo-responsive to auditory stimuli). Others have difficulty filtering sounds and hear all sounds at once. Many people diagnosed with autism spectrum disorder have to deal with these auditory issues 24/7. In a world where one out of 88 people is affected by autism, there are a multitude of interventions to manage each and every symptom. To manage the auditory aspects, listening therapy may assist with retraining the brain to process sounds. Listening therapy combines electronically altered compact discs — which vary in musical style, sound quality, and level of enhancement — within a sensory integration frame of reference. Sensory integration recognizes that there are three steps to functioning: sensory input, or taking cues from our environment all at once through the senses; processing input, in which we understand the stimulations and store the experience; and producing an adaptive response, like answering a question or speaking at the correct volume. Listening therapy programs are individualized to address each child’s specific issues. The music, listening time, and type of modulation used vary. The accompanying individualized activity program fluctuates, too. A child may be asked to sit on a moving swing and toss a bean bag into a bucket, repeat a list of directions next to an open window that lets in outside noises, pick plastics letters out of putty to stimulate hand strength, or countless other activities. A child may also be very active while listening to CDs, working on suspended equipment and 3-D surfaces that challenge postural organization,

motor planning, and higher level sensory integration skills. The use of sound and music is so intimately connected to movement that children in listening programs are often compelled to move and explore the environment in new ways. Current clinical work suggests that listening therapy decreases the time necessary to meet treatment goals. When incorporated into a sensory integrative treatment approach, changes are typically observed in: attention, organization of behavior, self-regulation, timing, development, refinement, articulation, fine motor skills, and more. Since many of these skills support communication, improvements are seen in many components of communication, too. Children may obtain a greater range of nonverbal communication, greater emotional and verbal expression, and improvements in pragmatic language. A typical, home-based listening program lasts for 30-minute intervals, two times a day, for two through 16 weeks. The time frame between daily listening sessions is about three hours. The basic equipment needed includes a CD player, special headphones, and CDs. The CDs for each individually designed listening program are obtained from a trained and qualified provider, who usually stocks a library of them for clients to borrow.

Currently, all in-clinic staff at the Holistic Learning Center in New York is trained in listening therapy. Timeout Why you shouldn’t scream at kids playing sports. by J. Mark Hart On every youth baseball field in America, a fourthgrade infielder lets a routine ground ball go through his legs into the outfield, causing an error. In response, an adult — coach or over-involved parent — yells at the child. The depth of fury in the scream suggests the boy did it on purpose, but we all know he did not. It was a simple mistake, which happens often in baseball. So, why the uproar? Screaming has become the norm in youth sports. Why do we think it is acceptable to yell at children who have no power to answer back and are simply playing a game that is supposed to be fun? What does that say about the parents and instructors? What is it teaching our children?

Examining this behavior poses hard questions for the screamers and the rest of us who enable them with our silence. Let’s take a look at what is really going on in such a scenario. •It’s likely the boy feels bad already about making a mistake in front of his teammates, opposing players, umpires, and grownups on both sides. Adding to the embarrassment and hurt pride with a loud display makes the situation worse. •Publicly demeaning the player isn’t going to enable him to make the play next time. •Screaming may make an adult feel better, but isn’t the child’s mood most important? •Odds are, the kids engaged in youth sports are just enjoying themselves and not going to become professional athletes. Likewise, their parents aren’t going to become professional coaches. So, if there’s a missed play or mistake in a childhood game meant to be fun, what’s the big deal? The next time you encounter a public shaming in youth sports, consider that the following factors involved in this behavior: •The screamer has lost his perspective on youth sports and their purpose. •A parent may have boundary issues, which are expressed through being too invested in his child’s performance. These actions don’t actually reflect on the parent. •A coach with boundary issues may be too invested in his team’s performance because he thinks it means something about him. It does not. It’s the coach’s job to offer quality coaching regardless of the team’s results. •The person yelling does not have a powerful way to relate to failure and is focusing on his own feelings rather than those of the child. People who can’t control their volume and discouraging words at youth sports events should explore three main points: 1) Get some perspective and mend boundaries. If a kid hits a home run, it’s his home run. If he flubs a grounder, he flubbed it. If he gets a scholarship, it’s his scholarship. None of these accomplishments or failures belong to the parent or coach. 2) Learn from what happened and improve if possible. Emotional reactions to failure are a distraction. The child still has to make the next play. After a foul play, the pitcher grabs the ball, gets the sign, and hurls the next pitch regardless of anyone else’s emotions. If a person is hooked by emotions, he cannot fully coach during the subsequent play; if the players adopt this same attitude of fixation, they cannot fully execute on the next play, either. 3) Craft a broader, more effective understanding of youth sports, as well as life itself. Sports offer a wonderful opportunity for children to learn how to set goals and work constructively toward their accomplishments. They discover how to deal with frustration and failure. Sports can teach grace under pressure, social skills, and how to contribute to a team. Making a mistake in public, modulating an emotional response to it, learning from it, and refocusing on the goal is a recipe for success on and off the field. The positive lessons learned from youth sports stay with children in the classroom and in their adult careers. Sports are supposed to be fun. That’s why children play them. With a little reflection and awareness, it’s possible for adults to have fun with youth sports, too.

During the school year, there are many tasks that need to be repeated daily or weekly, all year long. Create organized and efficient systems with specific time frames to complete these tasks. This takes the lastminute frenzy and fear of forgetting completely out of the equation. If you plan to make your child’s lunch every day, try scheduling this task as the last thing you do before you go to bed each night. Commit yourself to this routine, and soon it will become a habit. Organize everything you need to complete this task by designating one corner in a cabinet for all lunch-making supplies: containers, groceries, and notewriting materials. Save a corner in the fridge for all lunch-related perishables. Now, right before you go to bed, you’ll automatically know what you need to accomplish and how to do so without a second thought.

Before the school year begins, create a family calendar with upcoming events. Sports activities, holidays, birthdays, dances, parent-teacher conferences, and plays are always mapped out by the school well before the start of the semester. If you have an active awareness of what the next month or season brings, you can prepare your family to forgo frantic scrambling. And with a solid overview of the ebb and flow of the calendar, you can fit in extras — like doctor appointments and vacations — even during busy periods. For example, if you know October is a particularly hectic extracurricular activity month, and you’re also aware that your child needs a physical, you can go ahead and schedule the physical for November, when the calendar is less packed.

Apply the same principles of systematizing and organizing as described above to tasks for which the kids are responsible. Inviting your children to co-create systems they‘ll use gives them a sense of ownership and signals your respect for their growing decision-making abilities. For younger children, pick a simple task such as deciding what to wear to school. Work with your children to designate a time to choose each part of their outfit, and lay the clothes out together before bedtime. Ask older children what time of day they might like to work on their homework on a regular basis. Help choose a homework area that’s quiet and available to them at that time.

hen it comes to your kids’ health, there’s nothing you wouldn’t do or overlook. You schedule annual physicals right on time and visits to the doctor in between when your child is sick. You buy organic milk, eggs, and veggies and look for the best sports classes. But, parents are so focused on their children’s health needs that they often forget about their own. 48 I parentguide News I August 2014

According to a recent survey by the International Food Information Council, although parents with children younger than 18 are extremely focused on the nutritional needs of their offspring, they are far less likely to think about their own health. And, in another recent survey by the American Heart Association, 12 percent of its website visitors said that they were too busy taking care of others to take care of themselves. Busy parents should realize that caring for yourself is just as important as caring for your kids. It’s in the best interests of the children to care for your own health. The airlines have it right: Put on your oxygen mask before putting on that of your child. Neglecting your needs may not only lead you to feel depleted and exhausted, but it can also put you at risk for getting sick. It’s important to balance taking care of your children with taking care of yourself. Here’s how to create that balance:

exam, an OB/GYN exam, an annual mammogram (starting at age 40), and a colonoscopy if you are turning 50 and you’re at average risk. A colonoscopy should be performed earlier and more often for people at increased risk, including those with a personal or family history of polyps or colorectal cancer, inflammatory bowel disease, or a hereditary syndrome such as familial adenomatous polyposis or hereditary nonpolyposis colorectal cancer (Lynch syndrome). The screening for colorectal cancer starts at age 50 because the risk for colon cancer increases with age, and more than 90 percent of cases occur in people ages 50 and older. Most colorectal cancers arise from polyps that are precancerous but progress to full-blown cancer in 10 years or more. A colonoscopy allows your physician to remove polyps before they become cancerous and to identify cancer early, when it’s most treatable.

Eat a well-balanced, low-fat diet with lots of fruits, vegetables, and whole grains. Choose a diet that’s low in saturated fat and cholesterol, as well as moderate in sugar, salt, and total fat.
