Preschool Insight on your options. As an education director of a private preschool in Manhattan, I lead year-round tours for families. I’m often asked by parents as to what sets our school apart from the many others in our area — one of the essential pieces of information you should also obtain when choosing a preschool for your child. There are likely many thriving and nurturing preschools in your area. In your research, you’ll find options ranging from small private schools, Head Start programs, and nonprofits to large schools, which offer programs for 2, 3, and 4 year olds. Each preschool has its own unique personality and philosophy to meet differing family needs. I often advise parents to tour a handful of preschools when deciding what is important to their family unit. When browsing, consider where your child will be most likely to thrive and make social connections. Take note as to where you feel that instant sense of comfort, intimacy, and security. Other considerations to make could revolve around faculty size, extracurricular activities, availability of extended hours, and program philosophies. The National Association for the Education of Young Children has additional tips for questions to ask and observations to make when touring at www.naeyc.org. It educates parents about the features of accredited schools, and even if you’re not considering one, the website contains invaluable information about the preschool selection process. Considering a small preschool has its benefits. There is a sense of family among staff, parents, and the children that is undeniable. Feelings of true caring and loving relationships are fostered during school events, trips, and meetings. Getting to know each parent, grandparent, cousin, aunt, and uncle enables the school community to become part of an extended family. Staff members at small preschools have the ability to notice how even minor changes may affect a child or a family. They have the opportunity to collaborate with parents on important decisions and take caregivers’ feelings into account. Teachers can also learn about the individual child and cater to his particular learning style and interests. Regardless of school size, it’s important for educators to honor each child as if he were their own to cherish. I am extremely grateful to personally be a part of a smaller preschool community where I walk into a room and each child knows me by my first name. Hearing “Ms. Jennifer” being called brings a warmth to my heart that wakes up my daily passion for early childhood education. I hope all parents find a “gem” during their search for a preschool and have the opportunity to obtain the happiness I’ve experienced when working with a wonderful school. Addressing speech-language impairments. by Erica L. Fener, Ph.D.

o you think your child exhibits language impairments and developmental delays? If so, it’s important to intervene early. Early intervention for children with language impairments and developmental delays may make the difference between having an impairment that is mild – or perhaps even nonexistent after interventions – to having one that causes difficulty throughout his or her life.

What should you do if you think your child has language impairments and developmental delays? Have your child evaluated as early as possible.
If you think your child exhibits delays compared to other children his or her age, arrange for a professional evaluation as soon as possible. Small children generally overcome developmental or speech delays with early intervention much more easily than older children, so it’s imperative that you get help as early as possible.

Determine other causes for speech or language delay.
The specialist who evaluates your child determines whether other causes for language impairments or development delays may be present. These may include intellectual disabilities, hearing loss that cannot be corrected with surgery or other interventions, auditory processing disorders (whereby children have a problem decoding speech sounds), autism, speech apraxia, and so forth. Once the cause(s) of the child’s speech/ language/developmental difficulties are diagnosed, a proper therapeutic course to correct them may be established and carried out.
Address any physical deterrents to proper speech and language as soon as possible.
If your child’s hearing is impaired but correctable, take care of this as soon as possible. For example, if your child has a hearing difficulty that experts say is fixable with surgery, such as placing tubes in the ears to ensure fluid drainage, then have procedure done as soon as you can. If your child can’t properly hear, he can’t properly develop language; early surgical or other intervention that restores hearing encourages language development on a normal trajectory.

If your child has a cleft palate, or other facial structural abnormality that makes speech difficult, your doctor will probably recommend correction shortly after birth. If your child cannot physically create proper speech, even though he may be perfectly “normal” in every other way, no amount of standard speech therapy will help. Correcting physical difficulties as early as possible ensures that you’re making a difference for your child. parentguidenews.com

It’s important to note that your child should not be “punished” for speaking incorrectly. Instead, ask your child’s speech therapist or other professional to show you how to model correct speech around your child, and then entice him to respond appropriately. Similarly, playing games, singing songs, and reading books are all fun ways to develop language without making it seem like work. As a parent, it all falls to you, but the therapies you follow don’t have to be “necessary evils.” Instead, they can be fun ways to develop methods that fully engage your child and make him want to speak and communicate with others more effectively.
Consistency is important, too.
Depending on your child’s impairment, professionals will suggest programs to follow at home. They will also show you how to interact with your child so that speech is something with which he’s “forced” to engage. Parents often unwittingly “help” their children by letting them use noises or signals instead of actual speech to communicate wants and needs. Assisting children with language impairments and developmental delays is not easy, but coddling or otherwise enabling your child’s unwillingness to challenge himself is certainly ill advised.

ommunication disorders are among the most common disabilities in the United States. Roughly 40 million Americans have difficulty speaking or hearing — many of them are children. In fact, speech disorders affect 8 to 9 percent of young children, and hearing loss affects two in every 100 children. Many other children are affected by additional medical or developmental disorders that hamper their ability to communicate. These disorders are more serious than many parents realize. They may take academic, behavioral, emotional, and social tolls on a child. They can also impact the entire family by requiring established routines, straining relationships, and challenging interactions among all members of the household. Many of these struggles are unnecessary as communication disorders are treatable. Unfortunately, only a small percentage of the public is aware of the signs of communication disorders and the importance of early detection according to a recent poll of speech-language pathologists and audiologists (the professionals who treat these disorders). To address this, the American Speech-Language-Hearing Association (ASHA) recently launched the Identify the Signs campaign, which offers

information and resources for identifying and getting help with communication disorders. Here are just some of the helpful tips available at www.identifythesigns.org.

ypes of Communications Disorders
A variety of conditions fall under the umbrella of “communication disorders.� Categories include:
Speech/Language Disorders
When a child has trouble understanding others (receptive language), or completely sharing thoughts, ideas, and feelings (expressive language), then she has a language disorder. When a child is unable to correctly or fluently produce speech sounds, or has problems with her voice, then she has a speech disorder. Speech and language disorders include stuttering, selective mutism (inability to speak in certain situations), and childhood apraxia of speech (a motor speech disorder where the brain has difficulty coordinating the muscle movements necessary to speak).
Hearing Loss
Different types of hearing loss and degrees of severity exist. Hearing loss can be congenital (present at birth) or acquired. One common

illness in children that involves inflammation in the middle ear (usually associated with fluid buildup) is called otitis media, and it can result in hearing loss. This hearing loss can be permanent. Otitis media is the most frequently diagnosed disease in infants and young children; 75 percent of young children will experience at least one episode of it by their third birthday. The more frequent the episodes, the more likely permanent hearing damage will occur.
Medical/Developmental Disorders
Autism is in large part characterized by problems with communication and social skills. Other medical or developmental conditions that may result in communication problems include attention deficit/hyperactivity disorder and traumatic brain injury.

Each individual communication disorder has unique characteristics. However, the following are some general signs parents should notice as indicators of a potential speech-language problem, as well as the ages when these behaviors are first expected.

Many parents wait to act upon first noticing symptoms in their children. More than one-third of moms and dads (36 percent) will wait a year or longer to take action in the case of speech delay, and 31 percent of parents wait six months to one year for suspected hearing loss. Parents should never delay an assessment if they suspect a problem. Studies have shown that children who receive services prior to age 3 have better outcomes than those receiving services after age 5. Free services are also available to schoolage children through public school systems. Contact the local public school district for information about early intervention programs.

Juvenile Diabetes Facing your child’s diagnosis as a family.

ach year, more than 15,000 children are diagnosed with juvenile (Type 1) diabetes, and November is recognized as an awareness month for this condition in the United States. With juvenile diabetes, the pancreatic islet cells that make insulin are destroyed by the immune system; no insulin is made by the pancreas internally and must be administered externally. All parents should be aware of the warning signs that accompany Type 1 diabetes. A major change in disposition, energy level, or appearance should always be a cause for concern. Other symptoms can include: • Extreme thirst and frequent urination • Difficulty concentrating or holding attention • Drowsiness or lethargy • Increased appetite • Sudden weight loss • Sudden vision changes • Sugar in the urine • Fruity odor on the breath • Heavy or labored breathing • Stupor or unconsciousness If your child exhibits some or all of these symptoms, go to a physician immediately. The doctor may perform a random blood sugar test or a glycated hemoglobin (A1C) test, which determines blood sugar average throughout two or three months. If these result in a positive diagnosis, you should consult with your pediatrician and your endocrinologist. You should also take the time to prepare a thoughtful, compassionate discussion with your child about his diagnosis. Your response will serve as a guideline for how your child will react. Calmly provide all the information you can. Reinforce the facts and guidelines discussed by your physician. All caregivers and family members should provide a uniform message and instructions. Emphasize the positive while maintaining the serious nature of the situation. Because there is no cure for Type 1 diabetes, you should focus on the treatments that are available. Insulin is not a cure, but it does help manage the disease. Your child might be afraid that he will be unable to play or go to school, so help him to understand that the insulin will help with living a normal life. Have a family member demonstrate an example of taking medicine as part of a daily routine (like grandpa and his blood pressure pills) to de-stigmatize this new responsibility. It may be a shift for your family to start regularly eating healthy foods, but the importance of a nutritious diet for your child cannot be understated. Instill the value of healthy eating right at the onset of diagnosis. your needs, your neighborhood

Encouraging the entire family to practice healthy eating habits will prevent your child from feeling left out or viewing healthy food as a punishment. Counting carbohydrates (sugars, starches) to determine proper insulin dosages can become a game. (For example, ask your child, “How many carbohydrates are in one half-cup of mashed potatoes? Fifteen? You’re right!”) Encourage your child to learn carbohydrate counts with you. Look at the labels of snacks and other foods and remember portions. It may not be an easy habit to pick up, but it’s imperative for your child with juvenile diabetes, and it will benefit the whole family. Regular exercise is also important and a natural way to help regulate diabetes. If your family time is usually spent in front of the TV, switch to playing outside. Family bike rides, active games, or simple walks around the neighborhood will make everyone healthier, help relieve stress, and provide a new way to bond. You want your child to view a commitment to diet and exercise as positive asset, not a consequence of being sick. Monitoring blood sugar is often the hardest adjustment for a child. Again, positioning it as a game (“Let’s see what your glucose number is!”) instead of a burden may make it easier in the beginning. Spend as much time as it takes for your child to feel comfortable doing the finger prick to test her sugar levels. You might even have every family member perform a test to show your child that your family is “in this” together. Finally, educate your child on the signs of low blood sugar, such as shaking, sweatiness, hunger, weakness, being lightheaded, headaches, fatigue, blurred vision, or rapid heart rate. When these symptoms occur, your child should drink fruit juice or another source of sugar and retest her blood sugar in 15 minutes, repeating this process until the reading is normal. Make sure your child is always prepared by carrying a source of fast-acting sugar at all times, as well as a glucagon emergency kit. Facing juvenile diabetes is all about preparation and education. As both a doctor and mother, I have learned that children have a natural curiosity and a resilience that often surprises adults. With support and the tips above, living with juvenile diabetes can be manageable for you, your child, and your entire family.

Self-Care to Improve Self-Esteem Motherhood, at any time, takes a toll on your body. With four children of my own, I often find myself speaking to other women about their concerns. They are generally focused on the state of their bellies and bottoms, as well as how tired they feel. A broken night’s sleep is draining, and as you get older, your energy levels may lower. A woman’s metabolism starts to slow down in her late 30s. Plus, you’re busy looking after everyone in your family — perhaps at the expense of yourself. The good news is there are many ways to keep our energy and sparkle up — albeit with effort! Find your balance and old sense of self again by understanding the changes that have taken place in your body and mind since entering motherhood. Small steps will eventually change the bigger picture so that you can be happier and healthier. Exercise, nutrition, and physical well-being are important for you and your family. Involve the children in your goals and activities, and they will learn from your good habits. There is always time to exercise even if it means doubling up. Take the “stroll” out of the stroller and march forth; pick up the wrists and heels to turn a general errand into a walk and raise the pulse. Bend down and collect leftover clothes with an actual squat, use the buttocks to push yourself up. Squat while you clean your teeth, and you will clock as many as 30 squats a day. Muscles want to be exercised and properly used. They also need hydrating, so drink more water! Our muscles get affected by toxins just like other parts of the body. The more water we drink, the more we can flush the toxins. Of course, it helps to not take in as much toxins in the first place! Less alcohol and less caffeine may
sound less fun, but it is far healthier for you to reduce intake of both. A high-quality coffee that has been decaffeinated (it should say on the packet) tastes just as good as a caffeinated coffee. There are clever ways you can cut down on consumption without activating cravings. A nutritious eating plan and aerobic exercise will aid with toning and weight loss for your physical well-being. To target the belly, keep a positive connection to it, and do specific exercises such as situps and leg lifts (lie flat and slowly raise straight legs). The loose, “empty skin” (folds without fat that you can pinch) may never vanish, but it will reduce as the skin around it tightens up and draws in. By burning fat, you will regain more shape at the waist. Also try twisting exercises such as hula hooping as a fun activity to do with the children. Women can at times be susceptible to depression, and it is important to keep your brain exercised. Memory games with the children actually help us, too. My youngest son likes the shopping game: “I went to the store and bought an apple; I went to the store and bought an apple and a banana...” Simply go through the alphabet and repeat each item. This can be done with names, countries, etc., and can lighten up a tedious car journey. Crosswords and reading well-written books keep the mind open. I enjoy historical novels; absorb the facts while the fiction weaves a story thread around them to keep the reader hooked. Don’t stress about forgetting the odd little thing, our responsibilities and priorities totally shifted when we ventured into motherhood. Our brains had to open up a whole new division. Because remembering to feed the kids has come to the forefront, it’s okay to occasionally forget where you put the recipe for pecan pie! It always helps to take a deep breath and go back to where you were when you thought of it last.

Projects to Tackle Before Baby Arrives.

ou couldn’t be more excited about the new year ahead, because you’ve gotten some wonderful news: You’re going to be a mother in 2014! Right now you’re probably daydreaming about chubby cheeks, adorable onesies, and the bundle of joy you’ll be holding in your arms this time next year. You may also be feeling the urge to clean, organize, de-clutter, and redecorate. Congratulations: You’re nesting! In other words, you’re feeling the desire to prepare your home for the new arrival. Nesting is an important “stage” of pregnancy, and I know this from experience. I’m a modern-day princess, but I come from modest means and met my Italian Prince Charming, Adriano Pignatelli Aragona Cortes, Prince of the Holy Roman Empire, while on scholarship at Pepperdine University. Being a princess has given me access to great expertise, but I’m adamant that the most important choices I have made (and that you will make!) as a parent have little to do with money. Instead, they have everything to do with educating yourself, trusting your instincts, and eventually, paying attention to your child’s signals. Here are nine of my nesting tips to take into account:

Put paperwork in order. You might not think of paperwork as being a nesting activity, but even more important than a well-decorated nursery and organized closet are your child’s medical needs and daily care. Find a pediatrician, make sure your insurance is in order, and look into pre-admitting if you’re giving birth in a hospital. Tackle your budget. The exact amount of your baby-related budget varies depending on your personal choices, where you live, whether or not you’re a first-time parent, and much more. Do your best to figure up how much money you’ll spend on baby paraphernalia, during maternity leave, for future daycare, etc. If you’ll be a stay-at-home mom and you’re currently working, think about how the loss of income will impact your family. Factor in new expenses like diapers and formula.

Organize. You’re probably feeling the urge to organize everything: the pantry, your closet, the garage, your DVD collection, and of course, the baby’s room. Take a step back and prioritize the projects. Besides the ones that involve your baby’s room, clothes, and gear, I recommend cleaning out kitchen cabinets to make room for baby supplies. Also clean out junk drawers so that you’ll be able to easily locate things like screwdrivers, batteries, and other essentials for building baby toys and accessories.

Follow the fingertip rule. In other words, when you’re organizing and de-cluttering, keep the most important things at your fingertips. With a new baby, space is at a premium, so get tough and clean out things you rarely use. Put these items in big plastic tubs and store them out of the way. Get low. Look at your home from a baby’s point of view. Clean or replace carpets, change the air filters, get a new vacuum bag, etc., to improve the air quality in your house and make the floors a safe place to play. Since it will be a little while before your baby becomes mobile, you can hold off on baby-proofing the entire house. Finish baby’s room. Decorating, organizing, and stocking the baby’s room is a favorite activity for most soon-to-be moms. Get started as early as you want, and tweak to your heart’s content. Take care of any pesky chores you’ve been putting off. They’re not things you’re going to have the time, energy, and mental clarity to tackle with a newborn in the house. Figure out daycare options, take the car in for a tune-up, and get your gutters cleaned. Get help. The fact is, you can’t do it all. In some instances, such as heavy lifting and cleaning with strong chemicals, or if you’re on bed rest, doing it all might harm you and/or your baby. That’s why I encourage you to ask for or hire help. If you want to rearrange a room, paint, or deep clean your house, for example, your peace of mind alone might be worth the cost of hiring someone. If your budget is tight, but there’s work to be done, try to enlist friends and extended family.

Pencil in some time for yourself. This might be the most important nesting practice of all: Making sure that you are in order and at peace. It’s natural for moms and moms-to-be to put others’ needs first. But remember, this is the calm before the proverbial storm. Schedule a pedicure. Read a book. If there’s time, sign up for a class you’ve been wanting to take. Ultimately, smart nesting is all about planning, prioritizing, and ensuring you’re comfortable bringing your baby home. Now’s the time to tie up all of those loose ends before your bundle of joy arrives!

Spend Sandwich Day, November 3, munching on Joanne’s Super Hero Sandwich, which overflows with cold cuts and flavor, courtesy of www.allrecipes.com. Ingredients: •½ c. olive oil •1 Tbs. lemon juice •3 Tbs. tarragon vinegar •3 cloves minced garlic •2 Tbs. chopped fresh parsley •2 tsp. dried oregano •½ tsp. black pepper •1 c. chopped black olives •1 c. chopped mushrooms •1 loaf round, crusty Italian bread •½ lb. sliced, deli turkey meat •½ lb. sliced ham •¼ lb. sliced salami •½ lb. sliced mozzarella cheese •6 leaves of lettuce •1 sliced tomato Directions: In a medium bowl, combine olive oil, lemon juice, vinegar, and garlic. Season with parsley, oregano, and pepper. Stir in olives and mushrooms. Set aside. Cut the bread in half. Scoop out the inside of the bottom half, and leave an ½-in. outside wall. Spoon 2/3 of the olive mixture into the bottom. Layer with turkey, ham, salami, mozzarella, lettuce, and tomato. Pour remaining olive mixture on top, and replace the top half of the bread. Wrap securely in plastic wrap, and refrigerate overnight. Enjoy! 42 I parentguide News I November 2013

What You’ll Need: •18-in. plywood square •1-3 branches •L-brackets and hardware •Cable ties •Burlap •EZ-Glide surface protectors •Leaves •Gourds •Artificial birds •A pencil. What To Do: Place a branch on the plywood square and mark a line around the branch on the plywood with a pencil. Screw an L-bracket on either side of where you have outlined the branch, drill through the bracket holes into the branch base. Screw or bolt branch to brackets. Add up to two more branches, if desired, securing to original branch by wrapping a cable tie around the base of the branches. Hide screws, brackets and cable ties by wrapping a length of burlap around the base. Dress with leaves, gourds, and artificial birds. Add EZ-Glides to underside of base to protect furniture.
Classic Thanksgiving Fun
Your kids will love the classic activity of creating a one-of-a-kind paper turkey by tracing their hands! Try out the Hands and Feet Turkey Craft, courtesy of www.kaboose.com, for a truly fun time that also adds tracing feet to this classic kids activity! What You’ll Need: •Construction paper in brown, tan, orange, yellow, and white •Scissors •Glue •Wiggle eyes if you want a turkey that’s a little silly! What To Do: Trace your two feet (with shoes on) on the brown construction paper, then trace two hands on each color of construction paper. Then, cut out all the hands and feet tracings. Put the two foot tracings together with the heels one on top of another. Make it so the wider part of the feet are touching but also far apart enough to make the turkey body and head. Apply glue. Cut two turkey legs out of the orange scraps, then cut a diamond shape for the beak. Draw the eyes on white paper and cut them out, or use wiggle eyes. Make the wattle out of red construction paper. Glue each piece onto the body. Then, glue the red, orange, and yellow construction paper hands behind the brown body to make the tail. Finally, position the two tan hands on either side of the body to make the turkey’s wings and glue them in place, and you’ve got a cute turkey buddy for the holidays!
