Leg Problems Managing Menopause Varicose Veins and DVT’s For once, I’m going to begin an item by praising my editor – here’s hoping the shock doesn’t do him in – because it was in fact he who suggested that I do a column on “leg problems”, and the reason that directive has drawn the faintest bit of praise from me is that to be completely honest, I never would have considered doing a column on that topic, mainly because for me, “leg problems” is just too broad a topic (and let me quickly say that I don’t mean “broad” in the way some people use that word but rather “broad” in the sense of being wide and big). But in light of a recent event – the tennis star Serena Williams having developed a life-threatening blood clot in her lungs that started as clot in a vein in one of her legs – at least one type of leg problem, namely, a blood clot in a deep vein of the leg, has suddenly become a very newsworthy health topic, so it is indeed worth spending a column talking about at least a couple of “leg problems.” But before I get to the very serious problem of clots in the deep leg veins, let me first discuss a “leg problem” that is not life-threatening but is nonetheless very troublesome to a great number of people, namely, varicose veins. Varicosities are enlarged, tortuous and essentially “dysfunctional” veins, “dysfunctional” not like in Charlie Sheen behavior (actually, that’s beyond dysfunctional) but rather “dysfunctional” in that varicose veins aren’t working properly mainly because the one-way valves in those veins – valves that are there in order for blood in the veins to travel in only one direction, namely back to the heart - have stopped working properly.

What may surprise some of you, then, is that varicose veins can actually occur anywhere in the body including, interestingly, in the scrotum (where the problem is called a varicocele), although I`m pretty sure that most of us don’t instantly conjure up a scrotum when the topic of varicose veins comes up but rather we tend to first think of varicose veins in the legs. Risk factors for varicose veins include your genes (if one of your parents – and yes, men get varicose veins, too - had varicosities, you have a greater chance of also ending up with them), and anything that backs up blood flow in the lower extremities such as a pregnancy, tumours in the abdomen, obesity, injury to the leg, and any activity or job that demands standing for long periods of time, which is why, of course, the most common image of a varicose vein sufferer is a waitress (probably one named Flo who talks tough but is a sweetheart in secret), although before I get my next restaurant serving of ham and eggs (forgive me, rabbi) dumped in my lap, I want to quickly add that lots of restaurant servers have great legs free of varicose veins. Many people with even extensive varicose veins don’t complain of any symptoms at all, but much more often, varicose vein sufferers complain of discomfort in the legs, especially when standing, or in some cases, significant pain, a feeling of “heaviness” in the legs, itching, ankle swelling, and (although it’s not actually a symptom), a great many people with varicose veins lament loudly that their veins are “ugly” and that their varicosities have led to an increase in “spider veins,” those thin blue much smaller blood vessels which so many people seem to find particularly cosmetically disquieting. Allergy Season It happens every year; it causes peoples’ eyes to water, induces lots of frustration and maybe even a few sleepless nights. No, it is not the yearly filing of income tax. It is seasonal allergies. Normally, we associate allergy symptoms with runny noses, sneezing, and watery eyes. It can't be that bad, right? If these symptoms lasted for only one day it might be manageable, but these symptoms can last for up to 4 months. Sometimes people with allergies suffer from headaches, sinus pain/infections, and constant fatigue. This month, I will discuss herbal supplements and other non-drug measures to help soothe those allergy symptoms. Whenever I’m being asked about allergies, I recommend several lifestyle changes before a supplement. I feel the best basic help for any condition is to drink lots of water. Extra water will make any nasal/sinus secretions thinner and easier to clear. Saline nasal rinses are also found to be helpful in removing pollen and nasal secretions. There is a new and easy to use sinus rinse product called ‘Sinus Rinse’ by Neilmed. Ask your Peoples Pharmacist for more information. Washing one's hair before bed is also a good idea. This will remove any pollens that are trapped in the hair and prevent them from falling onto the pillow during the night. This way, you won't be sleeping with a lot of pollen close to your face and nose. I recommend changing the pillowcase every few days. Avoidance of certain allergenic foods will often help. The most common culprits are milk and other dairy, eggs, wheat, nuts and artificial dyes. There are other simple steps one can take to reduce their exposure to the allergens, and these may help with allergy symptoms. The easiest one is to keep the bed6 People First peoplesdrugmart.com

room clear of dust and/or pet dander. We spend around 8 hours a day in our bedroom, so it should be an allergen-free sanctuary. Pets should not be allowed to spend extended amounts of time in this room. The carpets should be vacuumed, with a vacuum cleaner that uses a HEPA filter, and surfaces should be dusted regularly. If possible, an air filtration device should be kept in the bedroom. At one time portable HEPA filters were very expensive and bulky, but not any more. You can find them at almost any department store. I got one this year and find it is helpful in removing dust and particulates from the air in my house. My favourite supplement recommendation for allergic rhinitis is quercetin. Quercetin is a natural flavonoid found in many fruits and vegetables. The best sources include apples, green tea and onions. During an allergic reaction, your body’s immune system recognizes a foreign substance and tries to get rid of it. It does this by causing a part of the immune system, called mast cells, to break apart and release histamine and other inflammatory compounds. This causes a lot of congestion and prevents the spread of the foreign substance. This sounds like a great idea, but with seasonal allergies there tends to be an over-reaction to allergens. How quercetin and other flavonoids work is by preventing the breakdown of mast cells and the subsequent release of histamine. This is referred to as mast cell stabilization. Vitamin C and the synthetic flavonoid cromolyn sodium also can act as mast cell stabilizers. I recommend more soluble form called quercetin chalcone, taken 250mg two or three times a day. Rarely, side effects of headache are seen. A recent study suggests that Pycnogenol (French maritine pine) can help reduce severity of allergic rhinitis. Healthy allergy sufferers took Pycnogenol 50mg twice a day 5 weeks before allergy season. Once allergy season hit, the Pycnogenol group reported lower amounts of Continued On Page 8

itchy, watering eyes and stuffy noses than the group that took nothing. The Pyconogenol group also took slightly fewer doses of anti-histamines than the other group. While this study was small and the evidence preliminary, it does offer hope for seasonal allergy sufferers. A supplement for allergic rhinitis is butterbur (Petasites hybridus). In one study, a special extract of butterbur was as effective as over the counter anti-histamines in treating seasonal allergies. This supplement has also been found to be helpful in preventing migraine headaches. If you are thinking about trying butterbur, you must find a particular formulation. These extracts have been processed to remove all the toxic pyrrolizidine alkaloids (PA) that are naturally present in butterbur. The most common trade name product is called Petadolex. Talk to your Peoples Pharmacist for more information about Butterbur. Butterbur is thought to work by preventing the formation of compounds called leukotrienes. Leukotrienes are inflammatory substances produced by the body in response to allergic reactions. I like vitamin C; whenever I read about it, I learn some-

thing new. Did you know that humans are one of the few animals that don’t produce their own supply of this important vitamin? If you ever want to learn more about vitamin C, or any vitamin, visit the website for the Linus Pauling Institute (lpi.oregonstate.edu). It is thought that vitamin C can act as a mild antihistamine. I recommend that people take about 2 grams of extra vitamin C if they suffer from seasonal allergies. Some people find that it helps with the symptoms of allergic rhinitis. Higher doses of vitamin C might cause stomach upset and diarrhea. Whenever people ask me about vitamin C, they sometimes ask which form is best. There are several ‘flavours’ of vitamin C: ascorbic acid, Ester-C and various mineral ascorbates. There is little difference between the forms of vitamin C in terms of absorption and usefulness in the body. Although, it is true that the mineral ascorbate forms might be less irritating to the stomach because they are not as acidic. Acidophilus and other probiotics are often overlooked when it comes to seasonal allergies. There are several ways that these ‘friendly’ intestinal bacteria can help reduce the symptoms of seasonal allergies. The first is that Continued On Page 10

There is growing evidence that throwing medications (prescription drugs and non-prescription/over-the-counter drugs) in the garbage, or flushing them down the toilet or the sink, may have a harmful effect on the environment, and indirectly, on human health. health news Being 'Housebound' Linked to Alzheimer's in Elderly Seniors who are "housebound" seem to have nearly double the risk of developing Alzheimer's disease, a recent study suggests. The research doesn't prove that being confined to the house causes dementia, and other factors could explain the association. Still, the findings raise questions about the possible cost of isolation. The study noted that underlying brain disease may also explain the results -- that is, people may not be getting out as much because the insidious workings of Alzheimer's or another form of dementia may affect the way one moves through the world long before they affect memory or speech. The study, published in the American Journal of Geriatric Psychiatry, looks at something known as "life space." Life space is actually a measure that has come into vogue with gerontologists recently, it's been a measurement of mobility, figuring out whether people are getting around their environment, how much they're seeing that's different from their couch or bedroom or living room.

Researchers followed 1,294 seniors from two separate studies of older adults whose health was being tracked over time. The researchers found that people who reported that they never left their home environment during a given week were about twice as likely to develop Alzheimer's disease in the five years of follow-up as those who traveled out of town. The study also found that those who did not go beyond their driveway or front yard were also more likely to develop mild cognitive disorder, which can be an early manifestation of Alzheimer's. There are some caveats to the research. Some of the participants lived in retirement homes and may have led full lives without needing to leave the buildings where they live; however, they were still counted as being housebound. This study is consistent with previous findings that physical activity, intellectual and social stimulation are important to delaying cognitive decline.

these bacteria have been shown to reduce intestinal permeability. This is thought to be helpful by reducing the amount of allergens absorbed from the intestines. There is some research to suggest that acidophilus can help modulate the immune system. This means that if people have an overactive immune system, acidophilus can help calm immune function. It can do the opposite in people with a weakened immune system. I take a supplement that contains about 4 million live cells daily. I have been giving acidophilus to my little one since she was about one year old. I feel that this is a very safe supplement. Another idea is to watch the newspapers or weather channel for your local pollen count. This might help you plan your week in order to avoid days when your allergies might be at their worst. This may be especially helpful of people who exercise outdoors. Another tip, my wife mentioned to me, is the miracle of the vicks inhaler and tiger balm. At night, when symptoms might be bad, place a LITTLE bit of tiger balm under the nose. The aromatic vapors will help to keep the nasal passages open so you can rest better. Also, keep

a glass of water by your bedside, in case you wake up with a dry mouth in the middle of the night. This happens quite often with people who have allergies. Seasonal allergies can be very troublesome and tiring for those who are affected by them. When the symptoms of fatigue, watery nose and eyes or sneezing get your down, visit your Peoples Pharmacist. They can help you choose the right anti-histamine or supplement to treat your symptoms. It is also a good idea to keep a journal of your allergy symptoms; even just make notes on your calendar. This will tell you a few important things; when your allergies hit, what has worked and what has not. Many of the natural supplements work best if they are taken in advance. This is especially true of homeopathic treatments for allergies. If I had a nickel for every time someone has said that they have forgotten what they took last year that worked really well, I wouldn’t have to write these articles anymore. don’t lead to any worse health consequences, although occasionally a clot can develop in a varicose vein, and on rarer occasions, that clot can extend into a deeper vein in the leg, which in turn leads to that severe, lifethreatening complication known as a deep vein thrombosis (or a DVT), which is what Serena Williams suffered as a result, reports said, of an injury to her leg that required some surgery and some subsequent rest (more on DVTs to follow). The treatment of varicose veins really depends on a person’s level of discomfort and on their cosmetic concerns, so that some people opt for therapy with what most of us would call a “mild” case (mainly because you’d need a magnifying glass to see the varicose veins) while others will put up uncomplainingly with veins that look like the Los Angeles highway network on a day the traffic is over-flowing. The simple you-can-do-this-yourself treatments include not standing as much, elevating the legs when sitting, and compression stockings which have been shown to be quite effective. Beyond that level of self-treatment, the plethora of choices involve some way to harden the veins or stripping the veins, but as always, before proceeding with a particular treatment, be sure to ask a lot of questions – and not just about the cost of the therapy but also about the success rate, the recurrence rate, the rate of complications, etc.. And now a few words about DVTs. The most common risk factors for DVTs include obesity, surgery, prolonged rest, having cancer (sometimes a DVT is actually the first sign of a hidden malignancy), the use of certain medications such as some hormones for menopause and birth control pills, injury to the leg, having had a previous DVT, and the most notorious precondition these days, prolonged sitting in a confined space, the so-called economy class syndrome (or ECS), although it’s very important to point out that you don’t have to fly in economy class to get a DVT (in fact, those more-money-than-brains flyers in business class can also get DVTS) and you don’t even have to fly to get one – DVTS can also occur in people who just sit for several hours in a car or in a bus. Symptoms are pretty straight-forward: any unexplained calf pain, especially in someone with one of those risk factors and especially if the pain is accompanied by leg swelling, must be immediately checked out

Because if a piece of the clot breaks off, it travels upwards to the lungs – as what is called a pulmonary embolus – and for reasons that are still not totally clear, pulmonary emboli are life-threatening complications that must be treated vigorously and instantly. And finally, there’s the matter of prevention. The very least we should all do, even those of us who don’t think we’re at risk (I’ll bet you anything, for example, that Serena would never have considered herself as being at risk) is to reduce our risk factors as much as possible (see why I get paid the small bucks?), and to take extra precautions in unavoidable higher-risk situations (like when we’re flying for a few hours) by wearing support stockings, doing leg exercises, staying well hydrated, and getting up from a sitting position frequently (or at least as much as possible without incurring the wrath of the flight attendants whose job it is, poor souls, to shoo you back into your seat as soon as you get up). The Benefits of Exercise I was not an active child. My early family life centred on more sedentary activities like game playing and television watching. It wasn’t until I set off into the world as an independent young adult that I began to explore various ways to move my body. At first it felt like hard work and I would do short spurts of activity followed by long periods of inertia. But I stuck with the process and exercise eventually became a way for me to become fully present in the world. Exercise is so much more than just an engine to burn calories; it is essential on many levels for optimal physical and emotional well-being. Have you ever taken a nap during the day and woken up more tired than you were before you closed your eyes? If you want more energy in your life you don’t get it by doing less - you get it by doing more. Seems backwards I know, but exercise plays an important role in fighting low energy. This fatigue response to inactivity is likely an evolutionary carry over; it was meant to conserve energy if we broke a leg and could not forage or hunt for our dinner. We rarely give enough credit to just how marvellous our body systems are. You can test this for yourself by going out for a short walk next time you feel that 3 o’clock nap calling. And speaking of sleep, you can actually improve your chances of a deep sleep at night by exercising in the morning. Exercising too close to bedtime can raise body temperature and stimulate production of hormones that are not conducent to a good night’s rest. Exercising and deep sleep are related, so if morning is not your thing then just make sure you allow a few hours between your workout and hitting the pillow. A cool shower can help bring your body temperature down and evening meditation can help you sleep better. Exercise is also Mother Nature’s natural mood enhancer. Movement releases those feel good endorphins and increases adrenalin, seratonin and dopamine levels. This can have a positive effect on those who are dealing with anxiety or depression. An added benefit is that regular daily exercise can also help you feel sexier overall. This may be attributed to

the endorphin release as well an increase in confidence due to a stronger and improved body image. There is a huge market for those who would sell us products and services that promise the fountain of youth and yet the secret to anti-aging is within our own DNA. On a recent study involving mice, researchers concluded that regular exercise does have a positive anti-aging effect on every cell in our body. They also saw a correlation between moderate exercise and fewer incidences of the many diseases we see in the aging population. I guess that is just more confirmation for the “use it or lose it” theory. And then of course there is the calorie burn benefit. Obesity is becoming a global concern and an epidemic in our own country. Our children are also at risk as childhood obesity rates are skyrocketing. Much of this can be attributed to dietary choices, and an unhealthy relationship with food; a sedentary lifestyle is also a contributing factor. We need to step away from the screens, get up off the couch, take the stairs and walk to the store instead of driving. In our society, appliances do the hard labour, fast food has replaced home cooking and we have effectively “convenienced” ourselves into a dangerous position as far as the natural world is concerned. Obesity increases the chance of developing a number of health problems including diabetes, heart disease, stroke, and cancer. The bottom line is that obesity reduces quality of life and accelerates the aging process. So the really good news is that you don’t have to train like a marathoner to reap the benefits of exercise and you can begin at any age. First let your health care practitioner know of your intention and ease into a regular program by gradually increasing duration as your endurance builds. Walking is my number one recommendation for most but for those who may have weight related joint issues, the local swimming pool is a great place to start. Many fitness centres offer joint friendly water aerobic classes. I encourage you to seek out a variety of activities that are fun and enjoyable. In this way you will be more inclined to do them on a regular basis. Good Health to You!
