COPD - CHRONIC OBSTRUCTIVE PULMONARY DISEASE What is Chronic Obstructive Pulmonary Disease (COPD)? COPD is not a singular disease, but a group of progressive lung diseases. The number one symptom is increasing breathlessness. Although it can occur at any age, it most commonly occurs in people over the age of 40. In 2011, 4% of Canadians between the ages of 35 to 79 self-reported having COPD, but 13% of Canadians had a lung function score indicative of COPD. Unfortunately, symptoms of COPD usually become noticeable when significant lung damage has already occurred. This month we will learn what COPD can look like and what can be done to help. What are the symptoms of COPD? The major symptoms include: shortness of breath, wheezing, and chronic cough with mucus and phlegm, especially in the morning. Other symptoms can also include: barrel chest, frequent lung infections, chest breathing (as opposed to from the stomach), problems sleeping and skin and nails with blue coloured hue. Some sufferers experience anxiety which can develop from the unease caused by laboured breathing. Morning headaches can also occur. It is thought that these headaches may be caused by a lack of oxygen due to laboured breathing through the night. Weight loss can also occur, as it is hard to eat when your chest feels tight. Lastly, a lack of energy and the inability to engage in everyday activities, can occur as you lack an adequate oxygen levels to function normally. If you are wondering if COPD may be affecting you, talk to your doctor. COPD is diagnosed by a demonstration of airflow obstruction through spirometry. Emphysema and chronic bronchitis are the two most common forms of COPD. Chronic bronchitis happens when the bronchial tubes of the lungs become inflamed making it hard to breathe. It is hard to move air through a very small tube. Emphysema is the result of the damage and destruction to the alveolis, the small air sacs in the lungs. These conditions are usually the result of smoking or exposure to other toxic gas. Although not as common, refactory (non-reversible) asthma and some forms of bronchiectasis also fall under the umbrella of COPD. Some medical researchers feel that COPD is a disease caused by a genetic defect. Recently, test has been developed which detects a protein in the blood called Alpha-1 Antitrypsin (AAT). AAT is a protein which helps to protect the lungs and to a lesser extent the liver. People suffering from an AAT deficiency have higher rates of lung diseases, which often happen at younger ages (30 to 40 years old). AAT deficiency is commonly called “genetic COPD�, but it is quite rare. Although Alpha-1 lung disease is incurable, it can be helped by the same drugs used by other COPD suffers. There are also treatments specifically designed to help treat people with AAT. Currently, there is no cost to test for AAT deficiency in Canada. OSTEOPOROSIS

What is osteoporosis?

Osteoporosis is a disease that causes bones to lose mass and become thin and brittle. Weaker bones mean there is more risk of breaks, which can result in pain, deformity, and other serious consequences. As people age their bodies start to re-absorb calcium from their bones, leading to some loss in bone density. Osteoporosis occurs when the bone loss is excessive. Fractures from osteoporosis are more common than heart attack, stroke and breast cancer combined. At least 1 in 3 women and 1 in 5 men will suffer from an osteoporotic fracture during their lifetime and over 80% of all fractures in people older than 50 are caused by osteoporosis.

What are the symptoms of osteoporosis?

Osteoporosis is sometimes called the “silent disease” because many people do not notice symptoms until they break a bone. Symptoms for older people may include: • fractures of the hip, wrist, or vertebrae (back bone) • back pain • loss of height • vertebral collapse, which shortens and curves the spine

What are the risk factors for osteoporosis?

Osteoporosis affects about one in four women over the age of 50 and one in eight men over 50. You are more at risk of osteoporosis if you: • are 65 years or older • are of Caucasian or Asian descent 6 PEOPLES Health Join us on

• have a history of osteoporosis in your family • have a thin (small-boned) frame and low body weight • do not have a period for more than three months (unless you are on birth control pills) • started menopause before age 45 You have an increased risk for osteoporosis if you: • eat lots of high-protein foods • drink a lot of alcohol or caffeinated beverages • smoke • do not get enough calcium or vitamin D • do not do much weight-bearing exercise Certain drugs or other products may also increase your risk of osteoporosis. If you are taking medications, ask your Peoples Pharmacist about risks. Diseases that may increase risk Your risk for osteoporosis may increase if you have: • rheumatoid arthritis • hyperthyroidism • bulimia or anorexia nervosa • chronic kidney or liver disease • chronic depression • chronic obstructive lung disease

How is osteoporosis diagnosed?

Talk to your doctor if you have risk factors for osteoporosis. There are several ways to determine how much bone mass you have lost. Your doctor may recommend a heel ultrasound, bone density scan, or other tests.

body. Follow these basic guidelines: • Eat plenty of fruits and vegetables — at least 5-10 servings a day. This will give you nutrients that help keep your bones strong and prevent fractures. • Limit your daily salt intake to less than 2100 mg. • Limit your daily alcohol intake to two beverages or less. Maximum weekly intake should be 9 alcoholic beverages for women and 14 for men. • Limit your daily caffeine intake to three cups of coffee, tea, or soda. • Quit smoking. Other things you can do to help prevent osteoporosis include getting enough calcium, getting enough vitamin D, and staying physically active. Stay physically active You can help prevent osteoporosis by doing some weight-bearing physical activity most days of the week — exercise at a light to moderate intensity for at least 30–60 minutes. Weight-bearing activities include walking, low-impact aerobics, and dancing. Weight training and other resistance exercises can help maintain bone density as well. Before starting regular exercise, fill out a Physical Activity Readiness Questionnaire (PAR-Q). Talk to your doctor if you are over 69. COPD. Often an increase in shortness of breath and change in mucus and phlegm production. Flare ups are often be triggered by an infection or exposure to an airborne environmental pollutant. Inhalers are often used to treat COPD exacerbation. These inhalers contain a single type or combination of medications, which can include bronchodilators, corticosteriods or both. Bronchodilators open your airways and help to increase airflow. Corticosteriods help to reduce airway inflammation. You and your doctor can work together to figure out which type of inhaler will work best for you. There are also Pharmacists that have taken extra training to help their patients with asthma and COPD. An important part of COPD treatment is the proper use of your inhaler. According to the American College of Chest Physicians, one in three patients with asthma or COPD misuse their inhaler. The good news is that your Peoples Pharmacist can teach you how to use your inhaler properly. A study in the Annals of the American Thoracic Society, found that patients become proficient inhaler users when offered interactive, hands-on instruction. The study tracked 120 asthma patients after they were discharged from hospital. After 90 days, only 11% of the group who received training misused their inhalers, while 60% who were given only brief instructions misused their inhalers. Even more significant was the fact that only 17% of those who received training had asthma emergencies or needed acute care, while 36% of the other group did. These findings highlight the fact that patients with COPD, just like those with asthma, need to know how to effectively use their inhaler. Make an appointment with your Peoples Pharmacist to get some hands-on training and to get the most out of our inhaler. A newer, non-drug treatment option to help with COPD symptoms is the Aerobika, which is an oscillating positive expiratory pressure therapy system. When you are suffering from COPD your airways can become plugged with mucus. As we know air can't flow through a blocked tube. The Aerobika holds the airways open with air pressure, this device creates little pulses of air which help to thin and loosen the mucus, thus leading to a clearer airway. So what can you do to help with your COPD symptoms? Smoking cessation is the single most important thing you can do to help with COPD. The good news is there is help available. The B.C. government will pay for the cost of nicotine replacement therapy (NRT) products or might contribute to the cost of some smoking cessation prescription drugs for eligible B.C. residents. Each calendar year, eligible BC residents can receive a single continuous treatment of NRT for up to 12 weeks in a row. All you need to do is visit your Peoples Pharmacy and determine what NRT is best for you. You would have to visit your doctor for a prescription if you want to try a smoking cessation drug. With the support of your pharmacist, the appropriate medication and sites like QuitNow.ca, you greatly increase your chances of quitting. QuitNow.ca supplies one-on-one support and many resources to help you quit smoking. Get started today. Visit your Peoples Pharmacist to create a personalized smoking cessation plan you can follow. Another avenue to try to help to alleviate COPD symptoms is pulmonary rehabilitation. This is an exercise program available for people with COPD. To be most effective it must be done 3 times a week for a minimum of 8 weeks. It includes endurance exercise, like biking and walking, as well as strengthening exercises, like weight lifting. With all of the possible positive effects of exercise, like elevated mood and a stronger body, this program is worth trying. Talk to your doctor about getting a referral to a pulmonary rehabilitation program. As with any disease education is paramount. Talk to your doctor or Peoples Drug Mart Pharmacist if you believe that you, or someone you care about, may have COPD. Educate yourself on the proper use of your inhaler. A HEART HEALTHY LIFE According to the Heart and Stroke foundation, heart disease touches the lives of about 1.3 million Canadians. It may be characterized by abnormal heart rhythms, abnormal blood pressure, blood vessel disorders or problems with the function of heart muscle itself. Not all symptoms of heart disease are clear-cut and some may be ignored or attributed to other factors. Symptoms may include but are not limited to chest discomfort, pain that radiates to jaw, back, throat or arm, sweating, fatigue, dizziness, nausea, indigestion and more. Consult your doctor if you experience new symptoms or they become severe or frequent. The good news is that through dietary and lifestyle strategies, your risk factors for heart disease can be greatly reduced. Much awareness around good reasons to quit smoking focuses on cancer prevention but smoking also drastically undermines hearth health. The chemicals in tobacco can damage heart and blood vessels and inhaling carbon monoxide in cigarette smoke competes with vital oxygen in the blood supply. If you smoke, do your heart a favour and breathe a sigh of relief as your risk factors begin to drop significantly after 1 year as a non-smoker. Move your body 30 minutes every day and you strengthen your heart, reduce stress, boost metabolism, lower blood pressure, manage weight and get a better nights sleep. Each one of these by-products of exercise supports a healthy heart. A gym membership is not the only way to exercise. Move your body through walk, dance, housework, stair climbing, cycling and driving less. Finding a variety of ways to increase activity is optimal as moderate daily exercise is one of the best medicines we have in the pursuit of optimal health. Check with your doctor before embarking on an exercise program and start slow. Gradually increase duration, frequency and intensity as endurance builds. Your diet is frontline in preventative strategies. You will find most of your heart healthy foods in the outside aisles of the supermarket but just because it is there, does not automatically deem it healthy. Keep the following nutritional tips in mind when aiming to nurture your cardiovascular system: 1. Put the saltshaker away and keep your sodium intake

under 2,300 mg daily. If you are in a high-risk group for heart disease, your doctor may suggest a much lower sodium intake than the RDA. Sodium is prevalent in processed foods as a preservative and flavour enhancer. Eat fresh, unprocessed foods and you have more control over your sodium intake. Use fresh herbs and spices to jazz up your recipes. If you are used to and enjoy the taste of salt, you will have to give yourself time to adapt as food may taste bland at first. Try using a squeeze of fresh lime or lemon juice if you need a little zing. 2. Limit unhealthy fats and avoid trans fats. Trans fats are created in the process of hydrogenation, which takes a liquid fat and turns it into a solid. Hydrogenating fat is a popular practice adopted my many food manufacturers to create a longer shelf life on their products. Fats to be eaten in moderation are generally saturated; saturated fats are solid at room temperature and found mostly in animal products. Coconut oil is also a saturated fat but with a very low melting point. Saturated fats have long been deemed evil in the world of nutrition but new studies suggest we may need to rethink this. In the meantime, keep in mind that nuts, seeds, avocados, fish oils, olives, olive oil and high quality cold pressed vegetable oils are your heart healthy options. Avoiding fats heated at high temperatures is also good practice. 3. Build menu plans using plenty of nutrient rich fresh fruit and vegetables. These foods are high in phytonutrients, antioxidants, vitamins, minerals and fibre. They provide bulk and are low in calories. Limit fatty cuts of meat and get your protein from leaner sources like fish, poultry, tofu, beans, legumes, eggs and low fat dairy. Choose fibre rich whole grains and don’t limit grains to just wheat. Quinoa, brown rice, oats, wild rice, barley, corn and bulgur are packed full of good nutrition. Obesity increases risk of heart attack and stroke as well as other conditions that may contribute to an increase risk of heart disease. Keep sugar to a minimum, watch your portion size and keep an eye on the scale. Your heart does not have to work as hard if you are maintaining a healthy weight. Eat well, exercise often and your heart will benefit! Good Health to You!

Meatless Moussaka

Combine first 5 ingredients in a large bowl. To assemble, layer ingredients in a greased 5 to 7 quart (5 to 7 L) slow cooker as follows: 1. 2 cups (500 mL) lentil mixture 2. Half of eggplant 3. 2 cups (500 mL) lentil mixture 4. Remaining eggplant 5. Remaining lentil mixture Cook, covered, on Low for 8 to 9 hours, or on High for 4 to 4 1/2 hours. Sprinkle with cheese. Cook, covered, on High for about 10 minutes until cheese is softened.
