Dr. Art Hister ERECTILE DYSFUNCTION

I have to admit that I still find it very hard to talk about “erectile dysfunction” but before you start imagining stuff that - trust me, readers - you really don’t want to get into, let me quickly add that all I meant with that opening is that I still have a very tough time accepting the term “erectile dysfunction” (generally defined as the inability to develop or maintain a penile erection during sexual intercourse) instead of what I consider the much more imaginative, more descriptive, and certainly more widely-recognized lay term for the same problem, namely “impotence.” Despite my views and much to my chagrin, however, the term “erectile dysfunction” has certainly caught on and has now nearly completely replaced “impotence,” even in the public domain, a change that reflects a very interesting trend in the use of medical language over the last couple of decades, a trend that stems from changing social mores that have resulted in many previously largely “hidden” medical conditions coming to public prominence (“out of the closet,” as it were). And as part of the process of helping us adjust to those conditions (to accept them without shame, and just as important, to help us discuss these conditions in public), health experts (aided by drug companies) have developed new terminology for many old health “problems,” labels that the experts feel are more publicly palatable (not to mention, way more flexible and hence more profitable, for both the drug companies and the experts) than

the old ones we’ve always relied on. So, for example, “social anxiety disorder,” a condition that just sounds like it’s screaming to be treated and followed up for long periods is now often offered as a diagnosis instead of what we all used to know as “excessive shyness,” which was simply considered to be a (sometimes unfortunate) variant of normal. Worse, battalions of “pre”-somethings are now charging into the medical literature. For example, “pre-hypertension” is now commonly used as the term-of-preference for what used to be called high normal blood pressure. And since someone with prehypertension sounds like he’s standing on the edge of an abyss, pre-hypertension clearly demands treatment, whereas high normal blood pressure was frequently accepted as just something that “we should keep an eye on.” Anyway, as any T/-watcher can tell you, “erectile dysfunction” (often just D) is now pretty much the standard for what we all used to call impotence, so I’ll live with it P I mean live with the term, of course. “But what can you tell us, Art, about this incredibly well-advertised problem that we don’t already know?” you may well ask, “besides, perhaps, explaining why the people in D drug commercials always have those silly smiles on their faces when they’ve invariably forgotten to turn off the lawn mower or are disturbing everyone by taking their seats late for the opera or why an amazing numContinued On Page 11

*uestions, questions, questions: It seems like all a Pharmacist does during a workday is answer questions. ven when we are not working, we are still answering questions. *uestions about: medications, medical conditions, pets, costs, side effects and more. The next question is: do we really mind all of these inquiries? The answer P luckily P is no, not at all. Helping people is what we do best. People might feel a bit awkward about asking questions that they believe are simple or trivial. Nonsense! The only dumb question is the one never asked. Peoples Pharmacists love answering questions. It provides us with an opportunity to share and help our patients. LetNs look at the most common prescription questions that I get asked. What is the shelf life of prescription drugs? Prepackaged, over the counter medications will have an expiry date stamped on them. You really shouldnNt have any unused prescription drugs around the house. Many of these will not have expiry dates because they were intended to be used within three to six months. My general rule is that if more than two years has passed since the date of dispensing, the medication has passed its expiry date. If you have medication to be used at a later date, for travel or just-in-case, ask your Peoples Pharmacist to mark the expiry date on the bottle. Any medication that has pasted its expiry date should be disposed of properly. This nicely leads to my next question: How should I dispose of old medications? medications to ANY BC pharmacy for proper disposal. If a medication or over the counter product was purchased in a pharmacy, chances are that it can be returned there. The exceptions are cosmetics, sharps and household chemicals. Some pharmacies accept needles and other medical sharps for disposal, but it is best to check with your local Peoples Pharmacist about this service before bringing any in. Another small bit of advice; always purchase small bottles of over the counter medications unless you use it regularly. While it is true that larger sizes can be more economical, these savings are lost when 9 of the bottle expires. I dispose of many large supplement bottles with only a few tablets missing. How should prescription medications be stored? The first part of this answer is: always out of the reach of children and pets. They should also be kept in a dry place, away from heat, humidity and light. Perhaps the worst place to store medication is the bathroom. It is generally bright, moist, warm and cabinets are easily accessible by anyone. I think the best place to store medications is in an upper cabinet in the kitchen. They should also be stored in their original containers. Do not combine different medications into the same bottle. The exception to this is if you place them in a weekly pill box (dossette), which is a great idea. should I do? This is perhaps the most troublesome and least asked question. While taking medications regularly might be necessary for maintaining your health, it is not a natural act like eating or sleeping. The solution is to make taking your medication part of your daily routine. (ne trick to begin any new habit is to attach it to an already established habit. If you always make coffee in the morning, place a sticky note on the coffee machine to remind you to take your medications. If you are a hi-tech person, download one of the many free medication reminder apps for iPhone or Android. Do you always wear a watch? Place a bright red elastic band around it as a reminder. (ften it only takes a week to establish a new habit; both good or bad. If you take multiple medications, make a list of which ones should be taken at which time. Ask your Peoples Pharmacist for a medication review. This service is free and can be helpful to learn about your medications and when they should be taken. Are generic medications just as good as the brand

name drugs? In a few words, the answer is: yes. In Canada, when a new drug is introduced to the market it has about years of patent protection. This means that only the patent holder, the brand name drug company who created it, can produce and sell it. After the patent expires, other generic drug manufacturers can apply to Health Canada to sell the same drug. These generic companies must prove scientifically that their generic drugs are just as safe and effective as the brand name product. BC Pharmacists are allowed by legislation to substitute a generic drug for the brand name drug; we usually ask you first. Most people welcome the idea of saving money by switching to a generic drug. What can you do if your medication tastes bad? This can be a problem for little children. Little ones often have more sensitive taste buds and things tend to taste stronger. Here are a few tips that everyone can use to get bitter medicine down: • Sip or suck on something very cold before taking medication. Influenza, often called the flu, is an infection of the nose, throat and lungs caused by an influenza virus. Many people use the term “the flu” to refer to any illness caused by a virus, such as ‘stomach flu’ or the common cold, which are different than influenza. The flu is not always a harmless illness. It can cause serious health risks, including death. A person with influenza is also at risk of other infections. These include viral or bacterial pneumonia, an infection of the lungs. People at risk. The risk of complications is greater for seniors 65 years and older, very young children, and people who have lung or heart diseases, certain chronic health problems, or weakened immune systems. What are the symptoms? Influenza symptoms can include fever, headache, muscle pain, runny nose, sore throat, extreme tiredness, and cough. Children may also experience nausea, vomiting, or diarrhea. Symptoms can begin about one to four days after a person is first exposed to the influenza virus. Fever and other symptoms may last up to seven to ten days, with the cough and weakness lasting up to two more weeks. How to fight a flu - Drink fluids: Increase your daily amount of water, juice and soup. - Get plenty of rest for seven to ten days - Take acetaminophen to reduce a fever. If your symptoms do not improve, see your doctor; you may be suffering from complications of the flu.

PREVENTING THE FLU Wash your hands frequently Twenty seconds of hand washing with warm water and soap helps remove bacteria and viruses. Wash before and after eating, after using the bathroom, after coughing or sneezing, and after touching surfaces that may have been contaminated by other people. Cover your coughs or sneezes Use a tissue, or raise your arm up to your face to cough or sneeze into your sleeve. If you use a tissue, throw it away and wash your hands immediately. Keep surface areas clean Disinfect doorknobs, light switches, telephones, keyboards and other surfaces regularly. Avoid touching your, mouth, nose and eyes Germs quickly spread this way! Maintain a healthy lifestyle Make sure you get plenty of rest, eat a proper diet, exercise, and take time for relaxation. Stay home, If you get sick. Wait until you no longer have a fever and your cough is improving. Get a flu shot. Your best way to protect yourself againt the seasonal flu is to get immunized every year.

sense of taste. Popsicles or ice cubes are suitable. • Mix it or chase it down with something very sweet. This sweetness will distract your taste buds and make the bitterness taste less. Use ice cream, honey, milk or fruit juices for this trick. • Take your medication with a big glass of water. This will ensure that it will get flushed out of your mouth. • Brush your teeth afterwards. • Drink a glass of an acidic beverage after your medication. This acidity with help to mask the bitter flavour. Cola and orange juice tend to work best. What is the best way to travel with medication? With a little planning, traveling with your medications is relatively easy. Before you go, check with your travel agency, or foreign embassies, to ensure your medications are allowed in the country. Usually this is only an issue with medications for pain, i.e. narcotics. Only take enough medication to last for your trip, plus a few days extra in case your flight is delayed. If you need a smaller vial, ask your Peo-

ples Pharmacist for one. Always, always, always keep your medications in its original and properly labeled container. Never combine two medications into one container in order to save space. It might be helpful to have a signed copy of your prescription. If you require syringes to use your medication, ask your Physician to write you a letter stating this fact. ber of people in those ads seem to have time for sex at times of the day that the rest of us reserve for other obligations.” To be honest, I have no way of explaining any of that, but I can tell you a few things about ED that are very important, starting with some of the causes. Much like stage fright, temporary ED can clearly occur for purely psychological reasons (“Yikes! Am I really up for this?”), and of course, the more a man focuses on his inability to perform, the less likely he is to be able to rise to the next occasion. Temporary ED can also be due to the use of recreational drugs (“Well, I certainly wouldn’t use the word groovy to describe what just happened, Mike.”), overuse of alcohol, fatigue, or even just lack of interest at the crucial moment (“Sorry, hon, it’s just that I still can’t stop thinking about what the Bruins did to the Canucks.”) More frequent instances of ED can be caused, of course, by many of the same factors that produce temporary ED but it can also be due to other important causes such as the presence of co-existing chronic illnesses, especially diabetes, neurological problems, and any condition that produces chronic pain, and as the side effect of many drugs, most frustratingly, many of the drugs that are used to treat those chronic illnesses that I just mentioned, such as for example, drugs for high blood pressure and drugs to treat depression. ED is also strongly linked to unhealthy lifestyle practices, particularly smoking, being significantly overweight, and this one may surprise you, biking (“ Serves them right,” a female friend of mine once said, “for wearing such ugly spandex outfits”). ED is also associated with many benign prostate problems, it’s a common complication of prostate surgery, it can be caused by hormonal problems, and on and on. But most important, by far, I think, is ED’s strong connection with co-existing heart disease because in a huge number of men, ED is actually a strong signal that their blood vessels are very unhealthy.

So taking all that you read in the preceding paragraphs into account, you should easily see that a man with ED must think of his wilting willie as his own personal canary in his own personal mine (I suppose I could have employed a better image, but I think this one may stick with you), that is, ED should invariably be taken as sign of other potential problems, especially of co-existing heart disease. All of which means that despite what you may deduce from those TV ads about how effective Cialis and Levitra and Viagra and the other drugs (which are known as PDE5 inhibitors) are in counter-acting ED, and they are generally quite effective, ED must never be treated as a simple symptom to be swatted down (or up?) with a simple remedy. Rather rule number one about ED is this: a man with ED must always get a full check-up to figure out what else may be wrong, which brings up a very important corollary of that rule: if you have Ed and you’re reluctant for whatever reason to see a doctor about it, never get around that reluctance by simply ordering those drugs on the net, not only because what you get often shipped is about as close to resembling a real ED drug as I am to Prince William, but also because you really need to muster up the courage to see your doctor about your problem. When it comes to treating ED, many studies have shown that the most important first-line therapy is to lose weight, do some exercise, and stop smoking. Once you’ve changed your lifestyle in a positive direction, if the ED is still kicking in, then that’s when you should consider getting one of those drugs for ED, but be warned that PDE5 inhibitors come with several potential limitations, so as always, a good source to tap for information is your pharmacist, who is likely as familiar with ED drugs as your doctor is. Actually, I’m not sure that came out the way I meant it to, but I think you know what I mean. K2 & CALCIUM Now some of you may be wondering what the world’s second highest mountain has to do with calcium and I suppose it would only be fair to say that if you are of a mind to climb mountains you may wish to read on. Calcium certainly is an essential nutrient that is needed to maintain the healthy bones and muscles that will power you toward those lofty peaks you aspire to; but I am not here to talk about tall mountains. The K2 I will be writing about is a once overlooked vitamin that has landed a lead role in the calcium show. The fat-soluble vitamin K family consists of vitamin K1 (phylloquinone), vitamin K2 (menaquinone) and vitamin K3 (menadione) which is a synthetic form of the vitamin. Vitamin K1 is widely known for its blood clotting effects and makes up the bulk of the vitamin K in our diet; we get K1 from leafy green vegetables like kale, spinach and broccoli. Vitamin K2 is a relatively new kid on the block and is generating a buzz in recognition for the role it plays in calcium metabolism and heart health. The best source for K2 is found in a fermented soybean product called Natto, which is not generally too appealing to the Western palate; other sources are eggs, meat, and aged or fermented cheeses. Beneficial bacteria in our gut also produce a small amount of Vitamin K. For years now we have been fighting the battle of bone loss by throwing mega doses of calcium supplements into the daily regime. And while some supplemental calcium may certainly be an asset in both the prevention and treatment of bone loss, too much of a good thing can create an imbalance and lead to other problems. One of the ways in which the body deals with too much calcium is to deposit it in the soft tissues, joints and vital organs. These calcium deposits can contribute to heart disease, cancers and painful conditions such as kidney stones and inflamed joints. Boron, zinc, fluoride, copper and many other trace nutrients are needed in lesser amounts but are no less vital. Quite recently, vitamin K2 was discovered and has been upgraded in importance for its role and relationship with calcium. This could be the best little discovery yet in the area of bone health. One of the main wonders of Vitamin K2 is that it acts like kind of a calcium traffic cop. Simply put, vitamin K2 will direct calcium to the areas where it is supposed to be and prevent it from building up in areas where it does not belong. There are studies that link K2 to improved bone density and the ability to break down calcium deposits in the arteries; there is also some evidence that it may help with certain cancers and slow the onset of dementia. Vitamin K deficiency is more likely going to be a risk factor for those who rely heavily on processed foods or those who may have chronic inflammation in the digestive tract, which can interfere with the optimal assimilation of nutrients. Long-term use of antibiotics and some prescription drugs is also disruptive to the digestive ecosystem and can affect absorption of essential nutrients. Remember that optimal health is all about balance. Mega doses of calcium may have therapeutic value over short term, but ideally we should strive toward getting most of our calcium from our diet and using supplements when a little extra boost is needed. Instead of upping your calcium dosage, perhaps the answer lies in taking vitamin K2 along with other synergistic nutrients so the calcium we do ingest is well utilized.
