Dear Colleagues, 2012 was the most successful year in the history of IPRAS: we created more membership values than ever before, - without raising the dues. Never before in our history did we have to close the registration of a congress as we had to in the 1. Congress of the International Society of Regenerative Plastic Surgery, our new daughter society, in Rome: 2 months prior to the opening we could not take any more participants! The first IPRAS course about Fat Grafting, held along with the congress of the Romanian and Hungarian Societies of Plastic Prof. Marita Eisemann-Klein President of IPRAS Surgery, also turned out to be a big success. Regenerative Medicine with the enormous potential of adipose tissue derived stem cells creates a brilliant future for us plastic surgeons, - thanks to our pioneers in research and clinical work such as José Guerrerosantos, Abel Chajchir, Sydney Coleman, Roger Khouri, Gino Rigotti, Peter Rubin, Norbert Pallua, just to name a few. The founding of the IPRAS Academy of Regenerative Medicine, the IPRAS Academy for Laser Treatment, the IPRAS Academy for Aesthetic Surgery Training and the IPRAS Trainees Association will take place along with the 10th anniversary of the IQUAM (former EQUAM) Consensus Conference in Athens, November 1st – 4th. This event, which you should not miss, is a strong commitment to quality optimization, critical scientific evaluation of techniques and devices AND to interdisciplinary cooperation. All academy members contribute not only their expertise and time, - they accept not to be reimbursed for their expenses. We thank all of them as well as our Board of Directors and EXCO members for their unique support enabling us to strengthen IPRAS everywhere in the world. And the group of contributors keeps growing: IPRAS meanwhile is governed by 170 excellent plastic surgeons from all over the world, - in our Board of Directors, in our Executive Committee, in our Board of Trustees, in our Subcommittees and in the decision making body – the Council of National Delegates. Our strength and power is based upon a fundamental training in all fields of plastic surgery, - burns, reconstruction and aesthetic as well as hands in some parts of the world. And even if some of us end up performing aesthetic surgery only – the memories of sleepless nights taking care of patients with life threatening injuries will always make us feel close to those who shared this experience with us. We shall remain plastic surgeons, - always. We should never give up on our plastic surgery roots - they keep us together, they are our identity, our history and our future! Cordially yours

President of Germany honors IPRAS for IPRAS WomenforWomen Trust On September 4th, 2012 the President of the Republic of Germany invited the initiators of new foundations to his residence Bellevue Castle in Berlin. In a personal conversation with IPRAS President Marita Eisenmann-Klein and IPRAS Deputy General Secretary Andreas Yiacoumettis President Joachim Gauck expressed his gratitude and enthusiasm for the IPRAS humanitarian project and asked them to provide him with more information. In an empathic speech, in which he expressed his pride to represent a country where citizens commit themselves to humanitarian work and idealistic projects, he again referred to the work of IPRAS WomenforWomen. Joachim Gauck is a former Lutheran priest, who became famous for his regime criticism in former East Germany. He was elected to be the President of Germany in 2012.

Although there may be several differences with relation to our practices, our patients and our everyday lives in the local areas of the world where we live and practice, we are all united through Plastic Surgery. These ideals which bond us, illuminate our paths so we can provide the best available technique, technology and surgical performance to our patients. As I said more than once before, I am always amazed by the easiness of communication with which, we can all share all this in the IPRAS world. This Journal itself is one of the major tools of this communication ability in the Plastic Surgical world – thousands of people receiving practically simultaneously all the “news that are fit to print” on our Specialty , where members of our National Societies share their opinion and thoughts on most varied subjects. IPRAS executive office staff is continuously and actively communicating with members in all corners of this planet, sending the latest news as demanded by National and other related Societies of Plastic Surgery. With state of the art equipment, the office at IPRAS can send any size of media or news to all our 37,000 members in a couple of hours, and as we are all reading this, the latest news on our forthcoming Chile Congress are also out. This Congress will be the epitome of communication in Plastic Surgery – over 1,500 presenters will be sharing their experiences and ideas with people from over 100 countries. As we have been promoting the Chilean

Congress on several meetings around all continents for the past several months, we have been happier and happier as more and more plastic surgeons share with us their intention in coming to IPRAS 2013. We have also become amazed at these different meetings, how the industry members are also very much looking forward to be there. We are certain that presence will be of ubiquitous as well as of generalistic and speciﬁc origins, as we will have practically all topics of our everyday practice depicted, with local and global aspects considered by a most varied group of conﬁrmed presenters, including members of our Co-Opted Societies. Another aspect of communication to be seriously considered is networking – the ability to construct connections – which will be at its pinnacle in Chile. One will be able to communicate and make permanent connections with people (plastic surgeons and industry representatives) from all continents, from practically everywhere, and all will be able to do so speaking one single language: Plastic Surgery!!! If you have not yet made your mind about coming, I urge you to act promptly and to register. Bring your family – make also a vacation out of this trip! As next February comes, you will see upon arriving to Santiago that you have taken the right decision and you and your family will be most happy that you have come. Upon leaving, your luggage will be full of new experiences, all worthwhile to have lived them!!! See you there,

EDITORIAL

“WE MAKE LIVES BETTER”

A few days ago I was sitting at my desk in my home office and I glanced at a painting hanging close by me on a wall. It was a still life, and a photograph of the artist was hanging next to it. The artist was my grandmother and the date on the photograph was 1887. She and my grandfather lived in a rural area three hours east of Houston (now forty-five minutes by freeway) and he was what we called a “country doctor’, which is now known as one in Family Practice. Many fond memories flooded my head as I glanced both at the painting and photograph and at the photograph of their home, which I’ve attached. I vividly remember her capturing a chicken for dinner and twirling it by the neck before plucking. I remember helping her milk the cow, and I remember putting wood into the oven for the roasting of the denuded bird. She pursued excellence in homemaking and art. My grandfather’s medical office was across the road. He had returned to Texas from medical school at what is now the University of Tennessee in 1898 and began practicing on the Texas Gulf Coast primarily because of a yellow fever epidemic. I never saw any of the yellow fever patients but I regularly saw someone come and rap on the door during one of our family dinners...without fail..at every dinner at least one person would come with a problem. He either left immediately or arranged a visit at a later time. They came because they trusted him to, in some way, make their lives better through his knowledge and constant pursuit of excellence. It was at that time..somewhere between seven and eight years of age ..that I knew I must be a doctor

because of the great reward..making lives better. After that long introduction please fast forward in time to the preceding June of this year. It was a busy time for me, beginning with a meeting in Amelia Island, Florida, of the Southeastern Society of Plastic Surgery where I had been honored by the invitation to give the Upchurch Lecture. Following Florida I traveled to Stockholm where I gave the Keynote Address at the Beauty Through Science meeting headed by Per Heden; then to Ekaterinburg, Russia, where I assisted Sergey Nudelman with his excellent semi-annual meeting of aesthetic surgery: and then to Moscow for a meeting put on by Natalia Manturova. Following Russia came Paris, by invitation of Eric Auclair ,and several presentations although my memory is not totally clear because of increasing hip pain. The hip pain was resolved on 3 July with a total hip joint replacement, and within a few days I was walking without assistance and playing golf by 22 August....and in September traveling to Geneva for ISAPS , Romania (thanks to Dana Jianu) for their national society meeting, and then to Spain for the aesthetic surgery meeting of Jose Luis Martin Del Yerro. So what’s the purpose of all this reporting? Well, it passed my mind that all those meetings and all those speakers, and all those attendees...were there for the purpose of pursuing excellence. I don’t remember a single word in any of the many sessions I attended that had anything to do with making money...it was nothing but how to do better work. How to do better work...why do better work”? The answer takes me back to my grandparents. Excellence is its own reward whether it be cooking a de licious chicken dinner...or treating a wound or illness. In the latter case...my grandfather was seeking the same thing I was seeking, and all the attendees at the many meetings I attended the past few months were seeking as well....to make lives better. To make lives better...my inspiration for over seventy years...and, Dear Reader, I’m certain it’s yours as well. Introduction In my professional career, I had the opportunity to participate in the Board of Directors Committee of several Associations, Federations, Chambers of Commerce, Sports Clubs, such as the Hellenic Association of Professional Congress Organizers (HAPCO President), the Hellenic Association of Travel & Tourist Agencies (HATTA member), the European Federation of Professional Congress Organizers (EFAPCO President), the GreekRussian Chamber of Commerce (Treasurer) and several other smaller (less important)sports clubs and institutes. From all my experiences that I have acquired throughout more than 20 years of such activities, I have come to know how difficult it is to keep political intentions and personal interests away from everyday management practices. This is why, at this period of my career, I really feel proud of being a member of the IPRAS Board of Directors. The reason is that IPRAS is from the very few large organizations whose strategies and practices are kept far away from political intensions and individual interests. I am sure that it is one of the very few scientific organizations in which so many people, in fact more than 170, are closely engaged with the everyday operation of our Confederation, intending only to serve and promote the science and art of Plastic Surgery. 101 National Delegates, more than 50 Executive Committee members, 13 Subcommittees with more than 45 members and finally 9 members of the Board of Directors work, away from political or vested interests and malice. This is wonderful and rare. We are all obliged to keep it as it is. I take the opportunity to start this 3-month report this way because almost all our planet continues to suffer from these political and individual interests and intentions. I don’t know how we can avoid the sometimes catastrophic results of political mistakes, vested interests or political

strategies. But I do know how important it is to keep scientiﬁc organizations like IPRAS distanciated from such practices.

IPRAS Management Ofﬁce Report The period of July – September 2012, was full of interesting actions for our Confederation. Starting with our humanitarian work, we had another successful mission to India, at the beginning of September, where amongst others, Dr. Constance Neuhann-Lorenz, Dr. Marita Eisenmann-Klein, Dr. Georgia Spyropoulou, Dr. Hansel Africano offered their volunteer services. WomenforWomen is indeed a very important and globally recognized humanitarian body, purely initiated, operated and governed by IPRAS. Recently, during an ofﬁcial ceremony in Berlin, our President, Dr. Marita Eisenmann-Klein and our Deputy General Secretary, Dr. Andreas Yiacoumettis, received congratulations and were honored with a plaque from the President of the Republic of Germany, Mr. Joachim Gauck, for the activities of IPRAS WomenforWomen.

Our Executive Director, Mr. Zacharias Kaplanidis had the opportunity to travel to Egypt in late-August and meet the President of the Panafrican Section, Mr. Ahmed Noreldin and other ofﬁcials of the Egyptian National Association to discuss the main issues regarding the Panafrican Biannual congress, which in 2013 will take place in Egypt (El Alamein), together with the annual national congress of the Egyptian Society. No doubt it will be a very successful and interesting event and we would like to thank our Deputy General Secretary, Mr. Ahmed Noreldin and the members of the Egyptian National Society for their wonderful hospitality and their assistance. The only bad thing during that time, was that Greece lost to Germany in the European League by 4-2!. An interesting and very useful trip was to the small but promising country of Montenegro, where Mr. Kaplanidis with our Executive Committee member from Serbia, Dr. Miodrag Colic, looked into the prospect of Montenegro forming an Ofﬁcial National Association and becoming a full member of IPRAS. Thanks to the great help from Dr. Colic and Dr. Aleksandra Bozovic Celebic, we are very close to realizing this target and very soon, perhaps

during the IQUAM Congress in Athens, Montenegro will become the 102nd IPRAS member. At the same time, Dr. Miodrag Colic, Dr. Alexandra Bozovic Celebic and Mr. Zacharias Kaplanidis worked in Budva, the beautiful sea side city of Montenegro to prepare the organization of the 8th Congress of the Balkan Association of Plastic, Reconstructive and Aesthetic Surgery, which will take place in September 2013. It was really a great trip with a lot of promising results. Mr. Zacharias Kaplanidis, also had the opportunity to spend a few days in Moscow at the beginning of September and visited the President of the Russian Association of Plastic Surgery, Dr. Nikolay Milanov and Moscow University Officials, in an effort for IPRAS to collaborate closely with the Russian National Association and the public officials for educational courses in several fields of Plastic Surgery, such as Aesthetic Surgery, Regenerative Surgery, Reconstructive, Burns and Microsurgery. An important event has already been decided between the Russian National Association and IPRAS/ISPRES for the coming December in Moscow (December 12th & 13th). September, as always, was full of congresses worldwide. During the period of 4-8 September, IPRAS honored its co-opted society, participating officially, in the very important and successful event of the biannual ISAPS congress in Geneva. Our General Secretary, Dr. Nelson Piccolo, our Executive Director, Mr. Zacharias Kaplanidis, the assistant Executive Director, Mrs. Maria Petsa and the Commercial Director of ZITA Congress, Mr. Gerasimos Kouloumpis, were there to talk with the ISAPS officials as well as to promote the vision and activities of IPRAS. The strong IPRAS representation at the ISAPS most important event passed the important message of our President, Dr. Marita Eisenmann-Klein, that IPRAS and ISAPS, historically, are very close relatives and staying like this will be for the benefit of Plastic Surgery. In a very emotional official event the past president, Dr. Jan Poell from Switzerland, “passed the torch on” to the new President, Dr. Carlos Uebel from Brazil. We

congratulate both of them for what they have done and we wish to the new President to continue managing ISAPS in the future with great success. IPRAS will always be close to Dr. Carlos Uebel and to the new Executive Committee for educational and other purposes, promoting the science and art of Plastic Surgery. Immediately after Geneva, on September 9th to 10th, the IPRAS ofﬁcials and the IPRAS booth, travelled to the beautiful city of Sinaia in Romania. The 12th Congress of the Romanian Association of Plastic Surgeons in co-organization with the Hungarian Association of

Plastic Surgeons, was indeed a great success. The IPRAS educational course opened this event the first 2 days and a number of great plastic surgeons from around the world, such as: Thomas Biggs (USA), Yilin Cao (China), Sydney Coleman (USA), Marita Eisenmann-Klein (Germany), Andreas Foustanos (Greece), Alexandru Georgescu (Romania), Brian Kinney (USA), Michele Masellis (Italy), Hanno Millesi (Austria), Maurizio Nava (Italy), David Netscher (USA), Nelson Piccolo (Brazil), Gino Rigotti (Italy), Manuel Garcia Velasco (Mexico) and Fu Chan Wei (Chinese Taipei), offered without any doubt a great scientific program and added value to the whole event. The Romanian Plastic Surgeons participated in the IPRAS educational course, exchanging experiences and knowledge with their international colleagues. At the end of the congress all participants were more than happy from the scientific program, from the organizational level and from the high hospitality efforts of the local organizing and scientific committee. We would like to congratulate and thank the President of the National Association, Dr. Ioan Petre Florescu, who was also Congress President, the Honorary President of the National Association, Dr. Ioan Lascar, Dr. Dana Jianu, Dr. Carmen Giuglea, Dr. Crenguta Coman and all the members of the Romanian Association for their hospitality. We really enjoyed this scientific event and we would also like to thank all the exhibitors and sponsors for their valuable contribution, such as: Mentor, Allergan, Peak Medical, Rombiomedica, BMW, Solartium, Bustiere Medicale, Brava, Arion Laboratories, Tulip Medical Products, See Try Buy, Teosyal, Zdrovit, Maramed, Contractubex, Restylane, LC Rhea Medical, Meda Pharma and Servier. The beautiful city of Innsbruck was our next immediate destination, where the well established international congress for Laser took place. The President of this event, which takes place every 2 years, is our plastic surgeon, Dr. Katharina Russe-Wilflingseder. For obvious reasons, IPRAS had given an official endorsement and our President, Dr. Marita Eisenmann-Klein, was officially invited as the IPRAS President and as a faculty member. The very interesting scientific program included some of the best scientists of the sector, such as: R. Rox Anderson (USA), Mathew M. Avram (USA), Christine Dierickx (Belgium), Marita EisenmannKlein (Germany), Merete Hædersdal (Denmark), Suzanne L. Kilmer (USA), Albert J. Nemeth (USA), Katharina Russe-Wilflingseder (Austria), Emil A. Tanghetti (USA), Christopher B. Zachary (USA), Wolfgang Bäumler (Germany), Thomas Hintringer (Austria), Constance Neuhann-Lorenz (Germany), Leonardo Marini (Italy), Serge R. Mordon (France), Uwe Paasch (Germany), Alexis Sidoroff (Austria), Stephan Spendel (Austria), Rolf-Markus Szeimies (Germany), Peter Waldenberger (Austria), Gottfried Wechselberger (Austria) and Andreas Yiacoumettis (Greece). There is no doubt that apart from dermatologists, ophthalmologists, etc., plastic surgeons play a very important role in the science and art of laser procedures. Taking into consideration the important role of plastic surgeons in laser applications and therapies, our President, Prof. Marita Eisenmann-Klein, our Deputy General Secretary, Prof. Andreas Yiacoumettis and the President of the Innsbruck event, Dr. Katharina Russe-Wilflingseder, worked during this event on the formation of a Laser Academy under the official umbrella of IPRAS. We also believe that it is a great opportunity for our Confederation to coordinate the efforts

of plastic surgeons and invite the best of the specialists of the Laser field from other sectors (e.g. dermatology), to unite their efforts for the progress of Laser Surgery and Procedures. We are confident that during the IQUAM congress in Athens, the IPRAS Executive Committee will approve the official formation of this Academy of Laser, under the IPRAS umbrella and with the collaboration of excellent scientists from some other specialties. We congratulate Dr. Katharina RusseWilflingseder for the exceptional scientific program. We congratulate and thank the above-mentioned high level international faculty and we are looking forward to the next congress in Innsbruck in 2014, hopefully with the official support of the IPRAS Academy of Laser. With Innsbruck we come to the end of the most important IPRAS management office activities during the period of the last 3 months. There is no doubt that many other activities took place during this time, but there is no way to even briefly refer to most of them. The AAPS is seeking applications for the John D. Constable International Traveling Fellowship For the sixth consecutive year, The American Association of Plastic Surgeons is pleased to announce the John D. Constable International Traveling Fellowship in Plastic Surgery. True to its mission, the award provides a unique opportunity for an international plastic surgeon to study and train with AAPS members and leaders in American plastic surgery. Over the past five years, plastic surgeons from Iraq, China, Egypt, Malaysia and Vietnam have been selected as recipients and have benefited from this exceptional experience. In their final reports, they have all expressed a profound gratitude for the knowledge and experience gained and the sense that the experience has indeed, changed the course of their lives and careers. They have returned to their respective countries with an enhanced ability to treat patients and train their colleagues. The fellowship invites an international plastic surgeon to come to America under the auspices of the American Association of Plastic Surgeons. The goal is to improve the fellowâ&#x20AC;&#x2122;s understanding of American plastic surgery and to promote good will and academic interchange among surgeons of the international and American surgical communities. The chosen fellow will be in the United States as an observer for a period of 6-12 weeks under the sponsorship of members of the American Association of Plastic Surgeons.

Eligibility Candidates must be fully trained in their respective country in plastic surgery and a member in good standing of their national society. They must be able to communicate well in both written and spoken English and must be sponsored by

two members of their national society where appropriate.

Dr. Jorge Herrera Buenos Aires, Argentina

Dr. Biggs: Dr. Herrera, you honor us by appearing as this issue’s Pioneer. As you know, this is a title we give to those surgeons who, through the entirety of their career, have pursued excellence and have transmitted this desire to other surgeons who have sought them as mentors. Please tell us a bit about your professional life, and begin with your educational background. Dr. Herrera: I received my M.D. degree in 1967 at the Medical College Buenos Aires University. During the beginnings of my career, I decided I would follow my father’s specialty, Dermatology. He was at that time dedicated mainly to dermatologic surgery. To gain better training in surgical skills, I entered a General Surgery Residency at the Rawson Hospital in Buenos Aires, a school founded by two worldwide renowned surgeons, Ricardo and Enrique Finochietto Dr. Biggs: How and why did you get into plastic surgery? Dr. Herrera: After this four years training in general surgery, dermatologic surgery felt rather limited and, thus I oriented my practice towards plastic surgery. I served as a resident in plastic surgery at the same hospital where I had done my general surgery training, Rawson Hospital in Buenos Aires, finished that training in 1974, and remained in the staff there until 1979. It was during these years that, thanks to the teachings of important Argentine plastic surgeons, I acquired my first knowledge in this field, with special interest in reconstructive surgery, with emphasis in the treatment of burn sequelae and oncologic facial surgery. Dr. Biggs: But like all our Pioneers your educational adventure didn’t stop there. Tell us more. Dr. Herrera: In addition to attending all the annual congresses in Argentina I participated in many others throughout Latin America, and eventually the world over. I made particular trips to have advanced training with outstanding people. In the U.S. with J.M. Converse, Thomas Rees, and Sherrell Aston in NYC; Ralph Millard in Miami Florida; and John Owsley, and Bruce Connel, in California. In Spain I studied with Jaime Planas in Barcelona, and in Brazil with Ivo Pitanguy, Liacyr Ribeiro, and Ronaldo Pontes. In more recent years my main contributions have been in several papers and lectures: face lifting with personal variations in SMAS treatment, short scar face lift, blepharoplasties, management of mammary implants’ complications, reduction mammaplasty with vertical scar, among others. The research and writing of these papers have been huge educational experiences. The friendship with Ribeiro has been and continues to be greatly rewarding. Dr. Biggs: Tell us about your professional positions.

Dr. Herrera: In 1980 I became Associate Professor in the Plastic Surgery Division of the Universidad del Salvador School of Medicine, Buenos Aires, and in 2005 to the present, Head Professor and Chairman Plastic Surgery Division of that same institution in Buenos Aires. Dr. Biggs: What professional organizations’ positions have you held? Dr. Herrera: In 1982 I was Secretary of the General Sociedad Argentina de Cirugía Plástica. In 1983 I served as President of the Sociedad de Cirugía Plástica de Buenos Aires. In1996 I became President of the Sociedad Argentina de Cirugía Plástica y Reparadora, in 1999-2001 I was the National Secretary of ISAPS. From 1992 to 2002 I was Director of the Chapter of Aesthetic Plastic Surgery –Iberolatinoamerican Society of Plastic Surgery, in 2005 I was the Master Surgeon of the Sociedad de Cirugía Plástica de Córdoba, and between 2010-2012 the 3rd Vice President of ISAPS. Dr. Biggs: This is a monumental list of positions and responsibilities. What are some of your memorable educational experiences? Dr. Herrera: During the ‘70s and my travels throughout USA and Europe, I had unique experiences with all the men I mentioned before. I remember J.M.Converse illustrating for me his technique of folding a forehead ﬂap for nasal reconstruction, using his tie to demonstrate it! I also remember Ralph Millard instructing me on the basic principles of Plastic Surgery and on excellence in cleft lip and palate surgery. His friend, Dr. Hector Marino, a pioneer in plastic surgery in our country, recommended me to him. Dr. Marino founded in 1974 the ﬁrst Plastic Surgery Post-Graduate Specialization in Argentina, of which I am the current director. Dr. Biggs: What are your more current interests? Dr. Herrera: In this career – which is divided into six units: Burns, Limb Surgery, Maxillofacial, Oncologic, Pediatric and Aesthetic Surgery - our objective is training future Plastic Surgeons with solid knowledge in general surgery, a condition that we consider most important for a good performance in the specialty. For a promising and true leadership of our specialty, it is of the greatest importance to pursue the development of intensive continuous medical education programs, thus achieving an integral teaching of our surgeons, which in this way will differentiate them from uncountable intruders, who under the name of aesthetic medicine invade our specialty with a limited knowledge of a few techniques. Dr. Biggs: Thank you ,Dr. Herrera. You exemplify beautifully the type of plastic surgeon that we designate as a “PIONEER”. Issue 10

Dr. Carmen Giuglea Bucharest, Romania Dr. Biggs: Congratulations for being chosen as our Rising Star and welcome to the interview. Dr. Giuglea. Thank you. Dr. Biggs: Tell us about you early years. Dr. Giuglea: I was born on December 13, 1967 in Bucharest, in a family of intellectuals who had only one goal: to offer me the best conditions of living and education, as best as possible during the communist era. I finished high school in Bucharest as the best in my class and decided to pursue medical school, which I finished in 1993, without really knowing where I was going. Dr. Biggs: How did you get into plastic surgery? Dr. Giuglea: It was a great chance to meet in 1994, during my staging year, Prof. I.P. Florescu, and to work with him. He had just moved into a new emergency hospital at that time and was trying to become known personally and professionally. Being alone and needing help he was happy to have me there as a staging doctor, not very skilled but dedicated and willing to learn. That six-month period was my first contact with this specialty and I fell in love with it forever. Dr. Biggs: What followed then? Dr Giuglea I was convinced that I wanted to follow this specialty and I managed to enter the residency in 1994. During the residency I followed all the required stages (general surgery, orthopedics, ENT Surgery, ophthalmology, OMF surgery), but the majority of time (4 years) I trained in all fields of plastic surgery: reconstructive, microsurgery, burns, aesthetic surgery. I became a specialist in 1999 and senior doctor in 2004, all my activity taking place in an emergency hospital where I had the chance to collaborate with other specialties like neurosurgery, general surgery, orthopedics and rehabilitation, solving the most difficult cases, at the border of these specialties and plastic surgery. This involved extensive tumors of the head and neck with skin, soft tissues, bone, dura mater, and brain invasion. It also involved large skin and soft tissue defects after trauma of the upper and lower extremity, which were operated together with orthopedic surgeons. We did breast and chest wall reconstructions and pressure sores as well In the emergency arena I performed finger and hand replantations, revascularizations of the upper extremity and lower leg, complex trauma of the face, upper and lower extremity and also burns. Dr. Biggs: What academic avenues did you follow after all that? Dy. Giuglea I pursued my PhD on free tissue transfer as a reliable method for covering large defects after extensive tumors of the head were removed and was awarded that degree in 2007. Since 2011 I have been a Lecturer at the University of General Medicine, because I decided to start my didactic career. Dr. Biggs: What course has your career taken since then? Dr. Giuglea: After working for 15 years in the same hospital,

last year I took the decision to be on my own and I moved to another emergency hospital, where there was a plastic surgery department but a not so developed one. I took this decision considering that I was ready to move on, that I had enough experience, having dealt with each case of plastic surgery, and enough energy to build a new team in a new place. In the past one and a half years I have managed to bring two more colleagues to work with me and to transform the department into a real clinic with 25 beds. Dr. Biggs: What is your biggest wish now? Dr. Giuglea: I believe my biggest wish at this moment is to fulfill my dream of building a professional surgical team, made of true people with proper working conditions, surrounded with respect and harmony. I consider that our professional part of life is taking the most of our time and therefore should occur in a friendly atmosphere, and also be organized for the best efficiency. I also wish that our clinic will become one of the best in Bucharest and all the country. Dr. Biggs: Tell me more. Are you interested in aesthetic surgery? Dr. Giuglea, I will continue by telling you also that aesthetic surgery was one of my constant concerns. I began my private activity 10 years ago, more timid at the beginning, not having a powerful aesthetic surgery school at that time, but I found my courage and became engaged in all aspects of aesthetic surgery. This involved breast surgery, liposuction, rhinoplasty, face, and of course the very new trend of fat grafting. I have been trying to educate myself constantly, going a few times a year to the most interesting scientific meetings and workshops all over the world. Besides that I am constantly participating in scientific studies presenting papers at scientific meetings, and being involved in research studies (7 phase II and III international and multicentric studies as a subinvestigator and one study as the principal investigator). In addition I am involved with students and residents in plastic surgery. Dr. Biggs: Do you have interests outside of plastic surgery? Dr Giuglea, I don’t want to finish before telling you that the most important persons in my life are my children, Diana and Horia, for whom I would like to be not only an example, but also a close friend. I consider no effort too big for them. In the end I want to thank you for this opportunity you gave me, to share my life experience and I hope the future will bring us more opportunities to improve ourselves and fulfill our dreams. Dr. Biggs: Thank you Dr. Giuglea. You make us proud by being our Rising Star…but this also means we expect very much from you in the future…and I’m confident you won’t disappoint us.

Dr. Gerard Illouz Paris, France

Dr. Biggs: Of course the whole world knows of you and your great contribution to plastic surgery. Please tell us a bit of your background and how you got into liposuction. Dr. Illouz: My origin was in Oran, Algeria, where I had my early training... then to France. I received all my medical schooling at Montpelier and surgical training in Paris with services in many hospitals and working under many outstanding surgeons (Prof. Charrier, Prof. Azerad, Dr. Lebeau, Dr. Gerard Lemarchand, Dr. David, Dr. Lance, Prof. Mondor, Prof. Olivier, Prof. Oudot, Prof. Couinaud, Prof.Duhamel, Dr. Monsaingeon, Dr. Lazard, Dr Auclair, and prof. Redon). Dr. Biggs: By your CV it’s apparent that you were trained in an extensive array of surgical specialties, which is the epitome of plastic surgery. How did you get interested in liposuction? Dr. Illouz: The discovery of liposuction was a “love story”. At that time, in 1977, I had a girl friend, very beautiful, and already a well known actress who was accustomed to wearing huge décolletés in the back. She developed a lipoma on her back and asked me to remove it without any visible scar. It was a challenge and because of that I discovered liposuction, my first case was a lipoma. Dr. Biggs: Then you set the world on fire with your report of your experience in 1982. Dr. Illouz: Yes, and when the American Society sent a task force to France to see what we were really doing and then went home with an official approval things changed rapidly. I published my book, Body Sculpting By Lipoplasty, and from then on the world followed. Dr. Biggs: Tell us about the evolution that is now taking place. Dr. Illouz: The surgical world has moved and will move even further into less invasive procedures. The work I’ve been doing for the past twenty years with stem cells is

becoming more accepted and I feel we’ll see less and less use of implants…breast, calf, chin, buttocks..all kinds. Dr. Biggs: What is your most recent work? Dr. Illouz: I have written my autobiography, On Seeking Aphrodite, which is my story. I’m in love with beauty and I’ve been in constant search for it. I have a picture of the cover of the book for you. Dr. Illouz: It’s a great pleasure and an honor to present you as our Senior Ambassador. You have made a significant leap in the evolutionary process in plastic surgery.

Report-Women for Women Camp Jalandhar, Punjab, September 2-September 9, 2012 The Location The September 2012 WomenforWomen mission by IPRAS, took place in the Shangara Singh Hospital, property of Dr. Shangara, father in law of our wonderfulhost Dr. Chanjiv Singh. The hospital is located in the city of Jalandhar with a population of approximately 1 million people, in the state of Punjab in the Northwest of the Republic of India. The Jalandhar Doab (the region surrounding the city between the Beas and Sutlej rivers) also marked the easternmost territory of the empire of Alexander the Great in ancient times. Our team arrived in Amritsar airport on September 2nd 2012. Saarvee, the wife of Dr. Chanjiv was waiting for us along with her sister and gave us the warmest welcome. Since that very first instant she started to look after us and was always eager to keep us happy and comfortable. We hopped on a bus arranged for us and started our journey by visiting the House of Chanjiv’s parents in Amritsar where they offered us traditional delicacies such as rosepetal sherbets and cold drinks and explained briefly their origin and religion-Sikhism. Our next stop was the famous Gold Temple of Amritsar, a beautiful traditional artwork Sikh Temple that is all Gold Plated. We left Amritsar in the afternoon and after a 3hs bus-drive we arrived at Jalandhar. Dr. Chanjiv was waiting for us at theat the Radisson Hotel to discussed our mission schedule. The hotel rate was graciously reduced, thanks to Dr. Chanjiv’s intervention as a donation for WomenforWomen by IPRAS. Shangara Hospital is a small private hospital with two OR´s for greater interventions and one for smaller ones. During our mission all the regular activities were put to a halt for which, we were especially grateful. So we found the whole staff and equipment at our disposal. All patients who presented were women and children having suffered fire and acid burns from 6 months to 20 years

earlier with severe scarring. Most of them had never received relevant treatment before. The Team Our team consisted of four Surgeons: Team Leader, Dr. Constance Neuhann-Lorenz (Germany, Munich), Prof. Marita Eisenmann-Klein (Germany, Munich) who could not join us since the beginning due to her obligations as President of the IPRAS but still came to Jalandhar on the 5thSeptember, the day of her birthday, Dr. Hansel Africano (Colombia, Bogota), Dr. Georgia-Alexandra Spyropoulou (Greece, Thessaloniki) and Dr. Chanjiv Singh (India, Amritsar). Photographic Documentation was performed by the famous photographer Christopher Thomas and his producer and curator Ira Stehmann. Ira also covered the whole mission as a journalist and plans to arrange in the future an exhibition about this mission in Munich for fundraising to help support the WomenforWomen cause. We operated under general or regional anaesthesia thanks to the help of anaesthesiologists Dr. HS Dhanju, Dr. Suman Sharma and especially Dr. Baljit Bajwa Professor of Anaesthesiology, who impressed us all with his skills in blocks and difficult cases such as one year old children with facial burns and nose obstruction. The patients were looked after by resident Dr. Amarjot Singh, OT assistants Kailash and Abhimanyu, anaesthesia assistants Manjit Singh and Rajdeep Singh and OT nurses Kulbir Kaur and Palwinder Kaur. The Mission The surgical mission started on September 4th. We went to the outpatients Department of Shangara Sing Hospital where prescreened approximatly 50 patients had been screened for us to re-evaluate. 20 of these patients finally were scheduled for

surgery. The majority of patients displayed sequelae of flame burns and burn contractures on various parts of their bodies. We performed surgery in 16 patients with facial, neck and upper arm burns. We evaluated 3 children with syndactyly and polydactyly, one child with a lower lip cyst and one patient with scar on the left cheek from previous abscess excision. Patients were all female except from the 3 children with severe syndactylies. We started the operations after unpacking our numerous surgical supplies of 100kg! These were donated thanks to the generosity of various German sponsors, who had also previously donated requested supplies to Dr. Constance Neuhann-Lorenz for WfW by IPRAS. She carried them from Germany with her luggage. Lufthansa Air Company as well as Jet Airlines graciously donated free air transport. Medicines came from BapuInder Singh Charitable Trust, and Dr. Africano also brought dressings and bandages from Convatec Company in Colombia. Furthermore, Panthenol creamswhich were donated from (Adler Pharmacy/ Dr.Koch, Munich) and they were given to patients that we chose not to operate upon. We operated simultaneous acture release and scar release with z-plasties. On Friday morning we saw the follow up cases and performed dressing changes. There were no complications so far, patients and families were grateful that their quality of life was improved. Further postoperate care was discussed with the local team and documented on the WfW postop documentation sheets, which were also left with the patients to arrange the dressing changes after our departure. We agreed with the local team to maintain online contact in case of any late onset complications. Chirurgie Sans Frontieres Congress Coincidentally on September 7th and 8th, a Congress of Surgery without Borders took place in Jalandhar, which we were invited to attend. Surgeons from Buthan, Sri Lanka, Myanmar, Tibet and Pakistan attended this Congress, which is very exceptional, as it is difficult to attain visa to India from these neighboring countries, especially from Pakistan. The Smile Train was co-sponsoring the Congress and we were able to exchange views with their representative on philanthropic missions. Dr. Marita Eisenmann-Klein and Dr. Constance Neuhann-Lorenz invited speakers in the Plastic Surgery session, were awarded the Honorary Fellowship of the National Society of Surgeons of India (Dr. Marita Eisenmann-Klein) respectively of the National Society of

Plastic Surgeons of India, Northern Section (Dr. NeuhannLorenz). During the opening ceremony of the Congress we expressed our gratitude for the generous hospitality of all our local hosts and two of us - Mrs. Spyropoulou and Mrs. Africano- wore the traditional local sari dress to honor the people of Jalandhar. Mission Overview Our mission to Jalandhar was a great success! We were able to improve the lives of these patients and exchange views with surgeons from other parts of the world on how we do things. Moreover, we felt grateful and appreciated how lucky we are to live in a safe and comfortable environment back in our countries. Many thanks to everyone who contributed to the success of our mission! Dr. Varaztad H. Kazanjian “The miracle man of the western front”

Varaztac Hovhanes Kazanjian was born in the heartland of ancient Armenia in Erzinga in 1879. When he was a child the family moved to Sivas (in Saebasti) a where he attended school. At the age of 15 he moved to Samsoun on the Black Sea. It was a time of great peril for the Armenians and he sympathized with an Armenian revolutionary party. He was soon being sought by the Turkish authorities for “passing forbidden political pamphlets in the dead of night. “To save his life, he was smuggled out of Turkey by an uncle”. He was sixteen when he arrived in New York in November of 1895, “with a bundle on my back and nothing else, not even a passport”. He was sent directly to Worcester, Massachusetts which had the largest colony of Armenians on America (nine hundred). Their largest employer was a wire mill where young Varaztad worked as a wire drawer until 1903. It was brutal work, “the milled sucked the blood and flesh of the workers.” His passion for knowledge was so intense that he immediately began night school. “For seven years,” he said, “ I did not miss a single night.” He became an avid reader, taking books to work. Many years later, he recalled, with helplessness, their hardships, their nostalgia for their homeland.” He developed a fierce determination to succeed. On an outing in 1990, when careers were being discuses, one of his fellow workers said, “You take dentistry, that’s a good profession.” He approached the Dean at Harvard Dental School in 1902 about matriculation and was admitted on probation. In June 1905, he received the degree of Doctor of Dental Medicine. His dental practice in Boston empathized maxillo facial prosthesis. On the outbreak of the First World War, he joined the First Harvard Unit with the rank of honorary lieutenant in the British Royal Army Medical Corps, and served in France as a dentist from 1915 and 1919. He treated the severe facial injuries sustained during trench warfare. “Give that man Kazanjian anything he wants,” his superior said. His innate skill, extreme patience and drive for perfection earned him the reputation of the miracle man of the western front. The major Kazanjian was invested by King George V as a Companion of the most Distinguished Order of St. Michael

and St. George. Had he been a British subject, he would have been known as Sir Varaztad. The foundation of modern plastic surgery had been laid in the battlefield hospitals of France and this immigrant Armenian-American dentist was one of its architects. He crossed into a no man’s land which only a few had bridged. Dr. Kazanjian set the standard and formulated the principles of treatment that were followed so successfully by others in World War I and even World Was II. Upon return from France in 1919, at the age of 40, he entered Havard Medical School and was given credit for two years. In his final year, his position and stature were dramatically changed. A clinic for medical students conducted by two distinguished visiting British surgeon was being held at the surgical amphitheater of the teaching hospital. As the demonstration was proceeding, one surgeon in glancing up paused in disbelief. These two British, surgeons rushed up and physically dragged “the quiet, black-haired little dentist” down from the amphitheater. Kazanjian was presented as the one who had taught the British surgeons the very technical skills that they were demonstrating. In Boston, he went on to become one of the world’s most famous plastic surgeons and Harvard’s first professor of plastic surgery. He operated without any wasted movements, very decisive and fast. He was ambidextrous. In collaboration with his former student, Dr. John Marquis Converse, he wrote in 1949 the classic plastic surgery textbook, The surgical Treatment of Facial Injuries. Humility with dignity were Dr. Kazanjian’s most notable traits. He was quit and unassuming: his bearing was shy and approached the apologetic. He was never known to negate a colleague’s work. His kindness, warmth and modesty became legendary. Dr. Varaztad Kazanjian died on October 19,1974, in his 95th year and I attended the service at Harvard University. “He had been recognized by ‘kings, and cardinals, presidents of great universities, noted political leaders, and the press of the world,” his son said. ISPRES BERLIN 2013 Dear colleagues, We are pleased to announce that the second congress of the International Society of Plastic Regenerative Surgery (ISPRES) will be held on June 7-9, 2013 at the Steigenberger Hotel, in Berlin, Germany. The first Congress was held in Rome March 9-11, 2012 and was an unprecedented success, attended by plastic surgeons from 55 countries and selling out three months before the meeting. ISPRES was founded to stimulate the study and discussion of those forms of plastic surgery that utilize fatty tissue, stromal vascular fraction, growth factors and adipose derived stem cells to repair, remodel and enhance the human body. FORMAT: The format for the upcoming Berlin 2013 Congress will be similar to the previous meeting in Rome 2012. Plastic surgery scientists will review our rapidly expanding knowledge of the science of fat and the components of adipose tissue. We will highlight the elements of translational research, which allow us to better understand what is happening in clinical applications. While understanding the science of adipose tissue is important,the ultimate goal is to use the knowledge obtained from the Congress to improve surgical clinical practices. Sessions will highlight the use of transplanted adipose tissue, stromal vascular fraction, growth factors, and adipose derived stem cells in the following clinical situations (see topics). Standard and alternative methods of processing fat grafts will be presented throughout the meeting.

Of course, safety issues will be emphasized throughout the sessions. Discussion of the standardization of nomenclature for this new branch of plastic surgery will take place. Finally, we will discuss the regulatory aspects of adipose tissue, growth factors and stem cells, and the role of our professional societies and governments world-wide. ABSTRACTS: During the Rome 2012 ISPRES Congress, abstracts of unprecedented quality were accepted and presented through-out the meeting in almost every session. We plan to use the same format to encourage introduction of new ideas and speakers in Berlin 2013. AESTHETICS IN FACIAL TRANSPLANTATION Presented at the 10th National Congress of Northern Cyprus Turkish Society of Plastic, Reconstructive and Aesthetic Surgeons in September 12-16, 2012 Selahattin ÖZMEN, M.D., Assoc. Prof., Gazi University Hospital, Department of Plastic, Reconstructive and Aesthetic Surgery & Hand Surgery With the advancing techniques and the increase of experience, we, plastic surgeons often find ourselves trying to achieve better results with advanced procedures in the field of microsurgery. The search for the “ideal” technique continues, especially for the challenging cases. We all are aware that facial deformities deserve a space in this category. As with most reconstructive algorithms; skin grafting, local flaps, tissue expansion, prefabrication and free tissue transfers are the current reconstructive routine for the treatment of severe facial deformities due to trauma, burn injuries, gunshot wounds or cancer. Nevertheless, reconstructive results of these procedures are usually unsatisfactory. There is very little room for camouflage when the face is at stake: skin discolorations, uneven textures, contractures that form especially around the mouth, nose, ears and eyes cause an unnatural, expressionless, almost mask-like face, are easily noticeable at conversation distance in most of the cases. These cases often require a significant number of reconstructive attempts, but even so, the results are not pleasing neither for the patient nor the surgeon.

Composite tissue allotransplantation became a reality in 1998 with the successful transplantation of a hand (1). Since then, many experimental and clinical studies concerning composite tissue transplants have been carried out for the reconstruction of the hand, larynx, abdominal wall, knee joint, tongue and so forth (2-10). These successful results gave the courage to think of the possibility of a partial or total human face allotransplantation. In November 2005 the first partial face transplantation took place in France led by Devauchelle and Dubernard (15). Since then a total number of 24 face transplants have been performed, and tailored facial allografts have restored aesthetic as well as functional subunits of the faces of these patients with the transfer of soft and skeletal tissue as we are informed through scientific and social media. With the success of face transplantations performed, this procedure is set to be the “ideal” technique for patients suffering from severe facial disfiguration. Even though the vascular anatomy of the face is well-known and many plastic surgeons have the microsurgical skills and anatomical knowledge to perform face transplantation surgery, should it

become a routine procedure at the reconstructive algorithm? In reality, face transplantation is more than a matter of technical advancement. This operation is a reconstructive as well as an aesthetic procedure. Therefore all aspects including aesthetic appearance of the patients should be considered. We reviewed the results of the performed facial transplant in this presentation. The surgical aspect is probably one of the few aspects that have been resolved. The side effects of current immunosuppressive medication and the physcological impact it would have on the patient and family members must be seriously considered. Until that risk-free day comes, recipient candidates and their families should be fully informed about the risks of the procedure and immunosuppression and undergo extensive counseling in the preoperational phase. Face transplantation should still be a possibility for those who have run out of other options.

Magic Toe Amputation

Background: 80 percent of diabetic foot wounds occur in the forefoot. Most of these wounds are associated with bone and joint involvement. Toe amputations and ray amputations are the most frequently performed surgical interventions for the lesions limited to forefoot. Most patients and their relatives perceive the toe loss as an important and negative turning point in their lives. They cannot get over the fear of and concern for repetitive amputations. Due to the same reasons, many diabetic patients refuse surgery although they have advanced deformities that

may cause dangerous injuries. As a matter of fact, many of our patients wanted to learn about the possibility of prosthetic toes. Understanding the anxiety and fear experienced by these patients, we defined a new alternative surgical procedure, which protects the external appearance of the toe. This surgical procedure is not perceived by patients as one that completely modifies the anatomical figure of the foot, ridding the patients of their toes. Therefore, we called this new operation “The Magic Toe Amputation”.

Method:

Conclusions:

Between 1994 – 2011, 42 diabetic patients with 50 lesions were treated with “Magic Toe Amputation”. Of the 50 toes treated with the defined procedure, 6 were great toes, 9 were second toes, 9 were third toes, 6 were fourth toes and 20 were fifth toes.

Magic Toe Amputation served as a reliable and successful alternative for patients who were anxious about losing their limbs. In time, it proved applicable to a greater number of indications and one that we relied on frequently for patients with diabetic foot. We believe that the Magic Toe amputation is as much an operation that can be used for proflactive purposes in the future as it is used for mainly treatment. We believe that any surgeon, who works with diabetic feet, can apply Magic Toe Amputation to the patient successfully as long as they know how to keep the tissues intact, and have the dexterity to re-form and relocate the tissues.

Results: The same or very close to the original toe length and thickness were sustained in all cases. Nails were included in the excision material in 45 cases; in 5 cases nails were protected. The new toe had sufficient strength and elasticity although phalanxes were removed.

Limitation in Facial Allotransplantation 1: Alternative reconstructive techniques for pediatric patients Presented at the 10th National Congress of Northern Cyprus Turkish Society of Plastic, Reconstructive and Aesthetic Surgeons in September 12-16,2012 Murat Topalan, M.D. Prof., Istanbul University, Istanbul Medical School, Department of Plastic, Reconstructive and Aesthetic Surgery, Istanbul.

Facial skin has a unique character with its color, pliability, texture, and vascularity. Established methods of reconstruction are far from restoring these elements. The skin of the neck and of the supraclavicular region is structurally most similar to that of the face. Facial transplantation, as it is well known , is the finest treatment method for patients suffering from burns, trauma or functional losses after tumor excisions since it promises to restore for facial transplantations is encountered in children. In this group of patients of young age, facial transplantation is illegal. In a pediatric case, the family is always the decision-maker, and the decision for the treatment method is highly correlated with the education level and socio-cultural status of the family. The use of immunosuppressives by the children, due to their side effects, is highly problematic. In this group of patients, unless vascular flap that will grow in sync with the growing organism of the child is constructed, after repeated operations and functional losses a face with a patchy look will be inevitable. Child patients with facial deformities suffer from serious psychological problems as well. The integration of these patients to the society can be

possible only when their facial deformity is reconstructed in line with what will be accepted by the society. With this purpose in mind, we perform reconstruction with four surgical techniques on young patients suitable to be operated. These can be summarized as follows: 1. Transfer of skin of the neck to the face with temporal facial prefabrication, 2. The prefabrication of one-sided radial forearm flap under the neck and supraclavicular skin, 3. The bilateral performance of the same operation to be able to change the full facial skin (Figure 1 a-b) and 4.The prefabrication of the omental, the free flap to the chest wall (figure 2 a-b). By placing expanders under all these flaps, thin, highly vascular and large flaps can be obtained. In this report, the surgical techniques are explained and some results 10-12 years after the operation are sampled. The objective should always be a reconstruction that will provide these children with a new condition in which they will be exposed to the society with no problems and in which they will continue their education. With prefabricated flaps and techniques relayed and explained here, the restoration of the specific aesthetic units of the face, of the half of the face and of the complete face is possible.

LIMITATION IN FACIAL ALLOTRANSPLANTATION

Facial allotransplantation has limited application in the adult population and at the current stage of knowledge, is reserved only for patients with severe deformities, which failed to respond to conventional reconstructive techniques. Besides the fact that over 24 facial transplantations have been performed worldwide, we are still evaluating outcomes of transplanted patients. Despite the effort and technical feasibility, 3 patients have lost their lives following facial transplantation, and all patients have suffered from episodes of rejection and more or less severe side effects of lifelong immunosuppression required to support the facial allograft. The debate of whether face transplantation can be performed in children with severe deformities is open. Currently, the inclusion criteria for face transplantation, includes patients between 18 and 65 years of age. The question is why a severely deformed child of 14, 15, or 16 is not eligible, whereas the young adult, who is 18, would be eligible for facial transplantation. These are the questions which the medical society, ethicists, and childrenâ&#x20AC;&#x2122;s families have to discuss and become comfortable with answering. Dr. Murat Topalan should be commended for the presented case of conventional reconstruction following severe burn in a child. Based on our IRB protocol at Cleveland Clinic, we were considering for face transplantations only the patients who have undergone conventional reconstructive procedures in order to justify face transplantation with lifelong immunosuppression. The case presented by Dr. Topalan encourages staged procedures for reconstruction of patients with facial burns and could be applied not only to children, but also to the adult population. If, as presented by the authors, aesthetic and functional results can be restored in staged conventional reconstructive procedures, this should always be considered before facial transplantation is attempted, specifically in children. Considering the outstanding results documented by many centers worldwide offering conventional reconstruction of patients with facial burns, we may, in the near future, want to revisit the idea of considering burn patients as candidates for facial transplantation. 16TH ASEAN Congress of Plastic Surgery “The Pearls of 30 Years” Gerald C. Sy, MD, FPCS, FPAPRAS, MBA Publications Committee Chairman Organizing Committee of the 16th ACPS

The Philippine Association of Plastic, Reconstructive, and Aesthetic Surgeons, Inc. (PAPRAS) is the only society in the specialty of Plastic Surgery duly recognized by the Philippine Medical Association and the Philippine College of Surgeons. PAPRAS hosted the recently concluded 16th ASEAN Congress of Plastic Surgery as its culminating event for the 50th Anniversary celebration. The muchtalked about event was held last March 11th-14th, 2012 at the powdery white sand beach of the world’s 2nd-top beach destination, Boracay, Aklan. To further ignite the enthusiasm for the event, 28 internationally renowned speakers from 13 countries (Australia, China, Finland, France, Italy, Japan, Malaysia, Philippines, Singapore, South Korea, Taiwan, Thailand, and the United States) were invited to share their expertise in different Plastic Surgery topics. With the Congress theme “The Pearls of 30 Years”, 11 general topics were namely prepared:

Cleft Lip/Palate, Body Contouring , Breast and Trunk Reconstruction, Aesthetic Breast, Wound Healing, Aesthetic Rhinoplasty, Nasal Reconstruction, Periorbital Rejuvenation, Facial Rejuvenation, Face Transplant, and Stem Cells and Tissue Engineering. In celebration of their 30 years of service in the Philippines, Operation Smile International with Dr. Irene Tangco started Day 1 of the Congress with management of issues of various craniofacial anomalies, from cleft lip and palate to craniosynostosis reconstruction and encephalocele management. The International Society of Aesthetic Plastic Surgeons (ISAPS) Symposium with Dr. Susumu Takayanagi played host for the topics of Day 2 of the Congress. Speakers from Australia, Japan, Finland, France, Philippines, South Korea and Taiwan happily shared their expertise in the management of aging facial skeleton, blepharoplasty,

augmentation mammoplasty, abdominoplasty and orthognathic surgery. The day ended with more enjoyment, as Dr. Alexander de Leon, President of PAPRAS officially welcomed the guests and invited them to a night of camaraderie, paired with delicious food. Entertainment was provided by local dancers who introduced a bit of Filipino culture through folk dance. The third day was crammed with plenary sessions on topics such as body contouring, breast and trunk reconstruction, wound healing, stem cells and tissue regeneration, facial rejuvenation, head and neck reconstruction and aesthetic rhinoplasty. Thereafter, the participants were treated to dinner and a delightful fire dance performance by the sea. The fellowship night also brought out a lot of surprises from the delegates, as representatives from each country acquainted us with their own culture through song, dance, and even martial arts! It was definitely a night to remember. The Congress concluded with more plenary sessions on the topics of aesthetic breast surgery, the rare case of facial transplantation and periorbital rejuvenation. The 16th ASEAN Congress of Plastic Surgery has satisfied the delegates continuing quest for knowledge and camaraderie. With the Philippines Department of Tourism campaign “It’s More Fun in the Philippines”, the Congress indeed also proved to be a pleasant mixture of work and play for Plastic Surgeons from different parts of the world.

DGPRÄC & VDÄPC Congress 2012 in Bremen: Humanity – a keystone of Plastic Surgery

Bremen – Premiere in the Free Hanseatic City of Bremen. For the first time the smallest German Federal Land welcomed the largest German Society of Plastic Surgery. “Humanity” was the motto of the 43rd Annual Convention of the German Society of Plastic, Reconstructive and Aesthetic Surgeons (DGPRÄC) and the 17th Annual Convention of the Association of German Aesthetic-Plastic Surgeons (VDÄPC). From September 13 to 15, 2012, more than 700 Plastic Surgeons and other professionals visited Bremen Congress Center to learn more about the latest in Plastic Surgery. 220 scientific lectures, 153 posters, ten workshops, seven Lunch Symposia, two hands-on classes, three Care Symposia and a “Best of Session” provided a basis for intensive scientific exchange. “Humanity is the basis of all medical action”, Congress President Prof. C. Can Cedidi explained this year’s motto. “We became physicians to heal people. Unfortunately, in a time of economic and organisational pressure this truth is getting out of sight. If, for example, a health insurance does not see the necessity for a certain operation I am seriously influenced in my medical autonomy of decision and often forced to restrict my therapy.” This conflict was reflected in many lectures, for example in a paper dealing with funding of palliative coverage of tumor defects (FV97).* Due to the 2012 congress motto

“Humanity” the voluntary work of Plastic Surgeons in developing countries was the centre of attention. Lectures referring to burn contractures during “Interplast Germany” missions (FV21/FV22) or to the new demands for helping in war zones (FV23) stirred the audience’s interest. The current state of research in Reconstructive, Aesthetic, Burn and Hand Surgery was the topic of many sessions. The scandal about defective breast implants by French producer “PIP” was also subject of several papers dealing with “bleeding” implants (FV58) or the validity of sonography examination of PIP implants (FV59). A special patients’ forum with top-class experts gave PIP patients the possibility to ask for risks and possibilities.

New board members The DGPRÄC general meeting elected Prof. Dr. Peter M. Vogt new President of the society. Vogt had been President since 2010 already. Prof. Dr. Hans-Eberhard Schaller was elected new Vice President again. The extended board also received new members. Prof. Dr. Henrik Menke will be Representative of Medical Directors for another turn, Dr. Christoph Czermak will serve as the new Representative of Residents. The members also decided to make Prof. Dr. Raymund E. Horch Representative of Academic Medical Directors for a second turn. Jonas Kolbenschlag became Representative of Associate Members, Prof. Dr. Ulrich Kneser was elected Head of Reconstruction/Microsurgery Section. The Burn Surgery Section is now chaired by Prof. Dr. Bert Reichert, Doc. Dr. Dr. Johannes C. Bruck became Head of Aesthetic Surgery Section. Prof. Dr. Michael Sauerbier was elected new Head of Hand Surgery Section. Prof. Dr. Christoph Heitmann will represent DGPRÄC in the German Society for Senology (DGS).

DGPRÄC Honorary membership for extraordinary commitment: Prof. Lemperle (r.).

Following the ofﬁcial congress motto the 2012 DGPRÄC Honorary Membership was given to „InterplastGermany“-Founder Prof. Dr. med. Gottfried Lemperle for his long lasting humanitarian commitment. Prof. Lemperle imported the Interplast idea to Germany in 1980 by founding an own section. Since then, Interplast members have travelled developing countries every year on their own expense. The volunteers perform plastic surgery on patients with face malformations, burn scars or war wounds. Between 1977 and 1994 Prof. Lemperle Issue 10

was head of the Department of Plastic Surgery at Markus Hospital in Frankfurt/Main. Today he is teaching at University of California in San Diego, USA. Prof. Lemperle was President of DGPRÄC 1985 to 1988, received the Federal Cross of Merit in 1997 and was given the Dieffenbach medal in 2010.

Awards and prizes “Fascination Plastic Surgery – based on my own experience” was the title of this year’s Dieffenbach lecture. Prof. Dr. med. Edgar Biemer, Dieffenbach award winner 2012, took the audience on an excursion through decades of plastic-surgical know-how. In 1973, Prof. Biemer joined the Department of Plastic and Reconstructive Surgery at the University Hospital “rechts der Isar” in Munich.

Following studies in Scandinavia, United Kingdom and USA he became head of the department in 1986 until his retirement in 2007. Prof. Biemer was President of DGPRÄC from 1993 to 1995. In 2008, he performed the ﬁrst double arm transplant on a patient with twosided traumatic arm amputation. Prof. Biemer has been honorary DGPRÄC member since 2011. DGPRÄC has awarded the Dieffenbach medal since 1989 in memory of Plastic Surgery pioneer Johann Friedrich Dieffenbach (1792-1847). The future generation of researchers was also awarded in Bremen. The 1500 Euro prize for the best lecture was given to Mustafa Becerikli and his team (Department of Plastic Surgery and Burn Surgery / Hand Surgery Center at BG University Hospital Bergmannsheil in Bochum) for the topic “Silencing of EphB4 tyrosine kinase receptor in synovial sarcoma by RNA-interference inhibits tumor progression and migration”. PD Dr. Andreas Arkudas (Department of Plastic Surgery and Hand Surgery, University Hospital ErlangenNürnberg) was the winner of DGPRÄC Science Award 2012. For his habilitation treatise entitled “Optimising of axially vascularized matrices in tissue engineering” he was awarded 3000 Euro. “Polytech Health & Aesthetics GmbH” donated this year’s Travel Scholarship worth 2500 Euro to Dr. Matthias I. A. Reichenberger (Ethianum Clinic for Plastic, Aesthetic and Preventive Medicine at University Hospital Heidelberg).

Further training of the year For the ﬁfth time the DGPRÄC awarded the best Further Training Hospital of the year. Prof. Dr. Raymund E. Horch (Head of Department of Plastic Surgery and Hand Surgery, University Hospital Erlangen-Nürnberg) won the Award in the category „more than four Assistants in Further Training“. 21st ISAPS Congress

After long and intensive preparations with a big and great team we had a fantastic ISAPS biennial congress in Geneva this year from September 4th until 8th. Due to the outstanding work of our scientific chairman Dr. Alain Fogli, assisted by our Educational Commission Chair Dr. Nazim Cerkes we had an unparalleled faculty. 165 invited speakers, the crème de la crème worldwide in aesthetic plastic surgery, completed by many free papers made this congress scientifically the best I have ever seen or heard. The presentations were completed by a large videothek and lots of master classes all very well frequented. As usual in ISAPS we don’t only look for outstanding scientific programs but also for friendship throughout the world. That means we met a lot of old friends and made a lot of new ones during the magnificent social events. The organization of our local host Dr. Kai Schlaudraff and his lovely wife Annette with the additional inspiration of my wife Christine gave all the events a very positive familiar touch. It started with the Presidential Dinner that took place in the Yacht Club of Geneva, the home of the Alinghi boat. Among many good friends from all around the world, the whole Board of Directors, Catherine Foss and her team were present and many presidents or secretaries of other societies. The Faculty Dinner took place in the wonderful Château de Coppet where we got after a one hour boat ride.

A highlight was certainly the Gala Evening in the Swiss National Circus Knie where we enjoyed food, drinks and a full program of the circus that we had all for ourselves. I opened the program with a short speech from the back of an elephant and ended it brushing the ﬂoor freeing it from all the dirt I had left after the two years of my presidency. Then I handed the besom over to Carlos Uebel, our new president, to keep ISAPS clean for the next two years. All together this congress was a tremendous success in the lovely town of Geneva and its surroundings. It will certainly be unforgettable for the 1300 participating plastic surgeons and the 700 accompanying persons and exhibitors. It’s only left to me to thank all the teams that have been the reason for this success. And there are much more persons involved than one would imagine. You all deserve big, big THANK YOU.

IPRAS report LaserInnsbruck 2012, Advances and Controversies in laser and Aesthetic Surgery, 4th International Conference, took place in the historic building of the faculty of Catholic Theology of the University of Innsbruck from September 12 – 15, 2012. Nicaraguan Association of Plastic Surgery: Volunteer Work Sandra Gutierrez, President ANCP This past June the ANCP conducted its IV annual Volunteer Surgery Plan. The organization began with a meeting in the form of a general assembly where they decided as a group, the type of surgery to perform and which patients to benefit. This year the Association chose blepharoplasties and patients of low income who work in the health sector (nurses, aides, technicians, etc). Subsequently, the necessary surgical groups were organized and the volunteer anesthesiologists were summoned. The board of the Association then exposed the plan to five different private hospitals, which assumed full sponsorship of the campaign. Next, the different surgical teams narrowed the list of potential patients. The whole process was based on team unity, disposition and organization, being one of the activities that joins the ANCP into one body working together for a greater cause. This year sixteen patients were successfully benefited. Our Association thanks all of its members and volunteer anesthesiologists for the effort and work put into this

The 10th annual congress of the Northern Cyprus Turkish Plastic, Reconstructive and Aesthetic Association took place Between the 12-16 September, 2012 at the Merit Crystal Cove Hotel of Girne, in Cyprus. The aim of our association that two years ago became also an afﬁliate of IPRAS, is to bring the criteria of the international knowledge to the island. I believe we have succeeded in this. My colleagues presented their successful work, while remaining loyal to the principle: ‘’The best of all human beings is the one who serves the others most’’. Some of the presentations that we liked very much and that we propose for the IPRAS journal are the following: 1. Dr.Selahattin Ozmen’s “Aesthetics in Facial Transplantation”

We have been very gratiﬁed to receive the support of Dr.Christos Merezas, the chair of Cyprus Plastic and Reconstructive Surgery Association, and his colleagues as well. Their support demonstrates that politics cannot interfere with our friendship.

2nd World Congress of Plastic Surgeons of Lebanese Descent Cancun October 10-13, 2012 Two years ago in Beirut, at the first world congress of Plastic Surgeons of Lebanese descent, we decided to bring the next meeting to Mexico. By that time, I didn’t know exactly the amount of work ahead, but after the completion of the task, I really think it was truly worth it. In Cancun we had some great professors, for which, the guidance of the president of the Society of surgeons of Lebanese descent, Ricardo Baroudi , was essential. The list of Faculty is long, but there were people from Argentina, Belgium, Brazil, Canada, Ecuador, France, Germany, Italy, Lebanon, Nicaragua, Rumania, USA and, of course, Mexico. The scientific program was magnificent, so was the social program enhanced by the presence of Aline Younes, the representative of the Lebanese consulate in Mexico City, who offered an elegant Tequila Tasting in one of Cancun’s most elegant Hotels. I would not like to express my personal thankfulness to all the people involved in this project in order not to omit someone who deserves it. Enough is to say that in my personal point of view it was such a splendid meeting in

all ways, and I am sure that it exceeded the expectations of all the participants. ALL the people who worked on it were very professional and delightful. The topics went from the simple techniques of filling a nasolabial fold, to extremely complicated procedures of microsurgery, but always with the personal “touch” of super experienced surgeons. There is much to say about my beautiful country, Mexico, that served as the frame of this magnificent event. And the story continues…. 12th Congress of the ROAPS & the HAPRAS 9-12 September 2012, SINAIA, ROMANIA

This September between the 9th-12th, the 12th Congress of the Romanian Association of Plastic Surgeons (ROAPS) & the Hungarian Association for Plastic, Reconstructive and Aesthetic Surgery (HAPRAS) took place at the Casino & Palace Hotel. During this time, surgeons could also attend the very interesting IPRAS Course: “State of the Art of Fat Transplantation in Regenerative Plastic Surgery & Microsurgery”. The moderators were: Sydney Coleman, Dana Jianu and Tom Biggs started the IPRAS Course (III). Just after lunch, followed the IPRAS Course (IV). The closing word of IPRAS Course was pronounced by the IPRAS President, Marita Eisenmann Klein. The 12th Congress of ROAPS and HAPRAS started immediately after the IPRAS closing words with SESSION I: AESTHETIC SURGERY (I) – BREAST with the following Chairmen: I. P. Devoted to the 20th Aniversary of the Establishment of Plastic and Reconstructive Surgery Center in Armenia Gagik Stamboltsyan, M.D., Founder of Medical Center “Avangard”, Chief of Clinic

Twenty four years ago, in December 1988, a devastating earthquake struck northern Armenia. Casualties from the quake were overwhelming, with over fifty thousand killed and more than thirteen thousand injured and maimed. At that time Armenia was part of the USSR. And the USSR decided for the first time to allow western aid and medical teams into the borders to help and treat the casualties. In response, numerous Armenian-American charitable organizations mobilized to bring relief to victims and among these was the Armenian General Benevolent Union (AGBU), and its president Louise Manoogian Simone. The AGBU organized the air-lift of more than 60 patients to US. Half of these were plastic and reconstructive patients. Stephan Ariyan, former Chairman of Plastic Surgery at Yale New-Haven Hospital was involved in treatment of these patients. “Unfortunately, it was not possible to bring more seriously injured back because they could not be transported by plane. With the thousand in Armenia still requiring surgery, it only made sense to take the services there”, said Stephan Ariyan. It also made sense to AGBU to undertake training professionals from the Armenian medical community in the vital skills necessary to treat their own people. This thinking brought about the concept of Plastic and Reconstructive Surgery Center (PRSC) in Armenia. Through a Cooperative Agreement with the US Agency for International Development, the AGBU began the process of organizing and staffing the PRSC in Yerevan, Armenia. Prior to the existence of the of Plastic and Reconstructive Surgery Center established by the AGBU in Yerevan, plastic and reconstructive surgery had not existed as a

discipline in the former Soviet Union and the PRSC was planned from the start to operate at current US standards. One of the first steps taken by the AGBU was to develop a training program for the Armenian medical professionals. Therefore an Armenian microsurgeon, Dr. Gagik Stamboltsyan and a vascular surgeon Garegin Babloyan, as well as two anesthesiologists, Armenuhi Kharatyan and Karen Manvelyan and seven nurses were brought from Armenia to Yale New-Haven Hospital for one year to receive hands-on training in their respective fields. Dr. Ariyan together with Paul Barash, MD, Chief of anesthesiology and Karen Camp, RN, Clinical Director of Surgical Nursing, trained the Armenian physicians and nurses from September 1991 to September 1992 under a special waiver. These Yale-trained professionals formed the core group of the PRSC’s Armenian staff. While the training program was underway, the AGBU, helped by physician advisors and numerous consultants, procured almost $2 million worth US manufactured stateof-the-art equipment to furnish the PRSC. The equipment was purchased through a Grant from the US Agency for International Development and the AGBU. In the summer of 1992 a US Air Force C-5 Galaxy military cargo plane 75% filled with equipment carried it from USA to Yerevan to establish the AGBU PRSC. The PRSC formally opened its doors 20 years ago in November 1992. Opening of the PRSC was equivalent to establishment of a new discipline, Plastic and Reconstructive Surgery, in Armenia, as well as establishment of health care with US standards. the PRSC by Dr. Ariyan and Dr. Armenuhi Kharatyan was assigned to be the Chief of anesthesiology by Dr. Paul Barash. The target of the establishment of the PRSC was the treatment of patients who were requiring plastic and reconstructive surgery and not cosmetic surgery. The number of burn patients was increasing at alarming rates because of the severe energy shortages in Armenia as a result of using unsafe kerosene heaters and other makeshift means of heating. One the other hand the amount of patients needing reconstructive surgery was also rising due to the war in Nagorno Karabagh. So, the PRSC was almost always overloaded and thousands of patients were getting healthcare there. As it was planned initially, the education of the PRSC staff was continuing in Armenia. During the first ten years of its activities the PRSC visited more than 12 missions with surgeons, anesthesiologists and nurses who not only performed hundreds of difficult surgical cases but also provided continuing educational program as for the PRSC staff in Yerevan as well as for local physicians and nurses. Other surgeons, like Dr. Earl Scott stayed in Yerevan and worked with the PRSC staff for more than six months and during his three visits traveled to Giumri (the quake epicenter) as well as to Nagorno Karabagh in spring 1995. Hundreds of patients were screened during these visits and forty injured patients from the civil population of Nagorno Karabagh were transported to Yerevan, to the PRSC for their treatment. Forty six surgeries were performed during a onemonth period. The AGBU contributed to the treatment of the patients, their transportation from Nagorno Karabagh and back home and the meals during their stay at the PRSC. Along with the doctors, nurses from America were also visiting the PRSC to continue the education of the PRSC nurses. Among them Carolyn Pritchik and Marianne Hess, who stayed in Yerevan for one year during their first visit. The PRSC has been designated by the Armenian Ministry of Health as a plastic and reconstructive surgery training arm of the Yerevan State Medical University. This prototype center was being viewed by the Armenian Ministry of Health as a model for improved patient care and modern methodology and technology. The PRSC also housed Armenia’s first plastic and reconstructive surgery library. The AGBU sent hundreds of journal issues and textbooks of plastic surgery and anesthesiology and subscriptions of leading American plastic surgery and anesthesiology journals to help keep the PRSC staff up-to-date. In autumn 1995 the PRSC took a new challenge: treatment of brutalized female war victims from Abkhazia and Georgia. When the United Methodist Committee on Relief (UMCOR) turned to the USAID requesting assistance for a group of girls and women maimed in the Abkhazia-Georgia border conflict who were seeking medical treatment in the US, local representatives sent UMCOR to the AGBU PRSC. USAID, which partially funded the Center, was aware of the work being done there and knew the staff was fully capable of handling such trauma cases: gunshot wounds, burns and disfiguring trauma. PRSC Chief Dr. Gagik Stamboltsyan commented this mission “It was not only an honor that USAID singled out our Center, but we and the

AGBU wanted to help in this unfortunate situation. The PRCS is always open for those who seek the medical care that we provide, and the nationality is irrelevant”. Many of the cases being performed at the PRSC were with disfigured victims. It was not only challenging for the surgical staff but also for the anesthesiologists. Difficult airway cases were coming one after the other. The late Dr. Andranik Ovassapian who was a Professor of Department of Anesthesia & Critical Care and the Director of the Airway Study and Training Center, University of Chicago, Chicago, IL, and was known as an expert in difficult airway management not only in US but throughout the world, in 1994 visited Yerevan, the PRSC and trained the anesthesiologist of the PRSC Armenuhi Kharatyan the techniques of difficult airway management. Dr. Ovassapian’s work at the PRSC was focusing on asleep and awake fibrotic intubations not known in Armenia previously. Thus, the PRSC was not only the place where patients could get western standards health care but also a place where plastic surgeons and anesthesiologists were learning new techniques and approaches in their specialties. The Board of the Armenian Association of Plastic, Reconstructive and Aesthetic Surgeons (AAPRAS) consists of seven surgeons. Four of them have worked at the PRSC. Years ago the target of the establishment of the PRSC was the treatment of patients who were requiring plastic and reconstructive surgery and the first ten years of its activities the PRSC were about treating solely patients with burns, contractures, different damages of the body, cleft lips and palates other congenital deformities . Thanks to the AGBU contributions these patients were being treated free of charge. No cosmetic cases were being performed. Since 2000, life started to change in Armenia and the number of patients seeking for cosmetic surgery started to increase. The PRSC started to admit patients for cosmetic surgery and cosmetic and aesthetic surgery also started to develop at the PRSC. Development of life dictates new conditions: today the PRSC does not exist as it was initially established. Some members of the core team of the PRSC established the Medical Center “Avangard” and work together with US standards, which they learned at Yale University. They treat patients requiring plastic and reconstructive surgery as well as those who require cosmetic and aesthetic surgery. “Avangard” Medical Center is the logical extension of the activities of the PRSC. Establishing the PRSC in Yerevan, Armenia has been one of Dr. Stephan Ariyan’s great ideas. It is thanks to Dr. Ariyan’s valuable contribution that today in Armenia we have western standard approach to Plastic and Reconstructive Surgery. Dr. Ariyan’s idea found its financial and organizational support at AGBU. Celebrating the 20th Anniversary of the PRSC in November 2012, the staff addresses its gratefulness to Dr. Ariyan, Dr. Barash, the AGBU and its President Louise Manoogian Simone, Program Director Regina Ohanian, the US International Development Agency and all those physicians and nurses who visited the PRSC to provide patient care and continuous education to the PRSC team, for their efforts and contributions in the establishment of a US standards Plastic and Reconstructive Surgery Center in Armenia.

FRENCH PIONEERS OF COSMETIC SURGERY From the early years of the 20th century, French surgeons have practiced Aesthetic Surgery operations. The first article of Julien Bourguet: “correcting unsightly nose without external scar” in 1909. The first facelift in France was performed by a gynecological surgeon, Samuel Pozzi, circa1910. But it is with the experience

In 1974, Vladimir Mitz and Martine Peyronie published the anatomical description of the SMAS (Superficial Musculo Aponeurotic System) which allowed new lifts techniques. In 1977, Yves Gerard Illouz developed the technique of liposuction, which is a revolutionary advance in the history of Aesthetic Surgery. He was also the source of important studies on fat. Paul Tessier (1917-2007), the designer of the CranioFacial Surgery, devised in 1989, a subperiosteal facelift technique called the mask lift. In breast surgery, one should note the contributions of Georges Henri Arion (1931-2005) who invented the “inflatable” prosthesis in 1965, and Louis Benelli in 1988, who described a technique of periareolar breast reduction. 3. After 1990 In the last decade of the 20th century and the early years of the 21st century, cosmetic surgery takes on a new importance. Jean François Pascal and Claude Le Louarn describe new techniques and abdominoplasty surgery after massive weight loss. Indications of fat injections in the breast are defined by Emmanuel Delay, Mojallal Ali and Eric Auclair. A particular interest is given to the aging of the face and leads to a better knowledge of the aging process. Alain Bonnefon, Bernard Cornette de Saint Cyr, Alain Fogli, Daniel Labbe, Claude Le Louarn, Daniel Marchac, Frank Trepsat are at the origin of important conceptual advances and their surgical implications. Editor’s Note: This essay was written at my request as I have been for a long time highly impressed by the advanced sophistication in aesthetic surgery by my French colleagues. Saudi Scientific Association of Plastic Surgery and Burns (SSAPSB) Saudi Arabia is the largest country in the Arabian Gulf region, and hence, the SSAPSB serves a very large area of the Middle Eastern, Asian and other Foreign Plastic Surgeons in the country, besides the Saudis. We are the closest geographically to Africa, therefore we have very strong ties with the African plastic surgery societies as for example, the Egyptians. In fact, we commonly collaborate in our scientific activities. Greetings from the Saudi plastic surgeons! The history of plastic surgery in Saudi Arabia is a long history of more than 40 years of ambition, hopes and petition. I remember with pride the efforts of my colleagues plastic surgeons, in its early years, to promote the specialty by overcoming the social difficulties and technical needs and to establish safe, successful high standard plastic surgery service in the kingdom of Saudi Arabia, which we successfully achieved. Our success made us dream of creating the first professional Scientific Association of Plastic Surgery and Burns (SSAPSB) in the kingdom, which was founded on 2001 (G) to promote the specialty and to facilitate the exchange of knowledge and experiences locally and internationally with the similar associations and their plastic surgery members. We are an Association starting its second decade but we have proudly achieved a lot in a short time, by providing up-to-date education to our members through local scientific activities, followed by the Annual International Conferences and many Workshops where eminent plastic surgeons been invited from all over the world. We work hard to disseminate accurate and current information to the public and media, and to promote safe plastic surgery nation-wide

1. The beginnings First evidences of documented Spanish plastic surgery come from the first half of the 19th Century, when Diego Argumosa and collaborators, such as Joaquín Hysern, developed original techniques about tissue transplantation and lip and eyelid reconstruction. During the early years of the 20th Century, Professor Cortés Lladó from the University of Seville Surgical Pathology Department, contributed to the development of Spanish plastic surgery after having acquired knowledge and skills with Professor Morestin in France. This author defined Plastic Surgery as “shape preservation or morphological improvement often bound to a physiological objective such as conservation or function recovery”. Regretfully, Spanish Civil War (1936-1939) represented, as happened in many other places in the world, the time when Plastic Surgery truly emerged as a surgical specialization. At that time, warfare defensive systems such as trenches and barricades, and the utilization of fire and stabbing weapons, determined a special kind of head and neck wounds, which challenged both surgeons and anesthesiologists. A New York Times reference dramatized about those facts: “There were so many mutilated faces among the rebel army as there were in the first World War British army”. Wounded soldiers and citizens gathered searching for an urgent solution in order to return to a normal life. Thus, in 1937, General Francisco Franco asked United States surgeon Joseph Eastman Sheehan 62

(1885-1951) to join the surgeon José Soler Roig and create what would become the first Plastic Surgery Service in Spain at the “General Mola Hospital” in San Sebastián. In 1938, Dr. José Sánchez Galindo, who had been sent to New York to learn from Sheehan both reconstructive surgery and orotracheal intubation, assumed the role of Chief of the Plastic Surgery Department. At the end of the war, Dr. Sánchez Galindo remained in charge of the Plastic Surgery Department at the Gómez Ulla Army Hospital in Madrid. At the same time, Spanish Red Cross developed Plastic Surgery Facilities and Departments in Madrid, Barcelona, Sevilla and Valencia. In 1950s, a group of plastic surgeons developed what would become the roots of the Spanish Plastic Surgery Society. In October 15th, 1953, Dr. Sánchez Garrido, Soraluce Goñi, Vilar-Sancho and Álvarez Lowell signed the statutes, which were officially approved by the General Health Department in 1950. Consequently, in May 12th, 1956, the Spanish Plastic and Reconstructive Surgery Society was definitively created. Its main objective was to improve health assistance through learning, ethics, clinical practice and investigation on the field of Plastic Surgery. The first board of directors was constituted by Dr. Sánchez Galindo (President), L. Mir y Mir and J.A. Soraluce Goñi (Vice-presidents), L. Álvarez Lowell (General Secretary), B. Vilar-Sancho (Treasurer), V. Mirabet Hipólito and M- Gras Artero. According to the Spanish General Law for Medical specialties, in 1957 Plastic Surgery was acknowledged as a Surgical Specialization.

2. The development After several national Meetings, on may 25th 1964, the ﬁrst SECPRE National Congress was celebrated in Madrid, serving as Congress President Dr. Vilar Sancho, as Honorary President Dr. Sánchez Galindo, and as President of the Spanish Plastic Surgery Society (SECPRE) Dr. Planas Guash and as General Secretary Dr. Enríquez de Salamanca. Main topics were reconstructive surgery of the hand, trafﬁc trauma sequelae and aesthetic surgery. Seventy members were inscribed as members of the society at that time. The European Society of Plastic, Reconstructive and

Aesthetic Surgery (ESPRAS) was created as a section from IPRAS, during the 4th International IPRAS meeting in Rome, on October 13th 1967. The second Quadriennal congress was held in Madrid, in 1973. The president of the Congress was Dr. Vilar Sancho, Coordinator of the Executive Committee of ESPRAS. In 1965, Argentinian Plastic Surgery Society invited both Spanish and Portuguese societies to the first Iberolatinamerican Congress, presided by Dr. Ernesto Malbec, Norberto Spera and Héctor de la Ruffa. Benito Vilar Sancho and Ulrich Hinderer represented Spain. Dr. Hinderer proposed a second Iberolatinamerican Congress to be held in Spain. It took place in Córdoba, and Dr. José Cabrera and Ulrich Hinderer were the Presidents of that event. Dr. Benito Vilar Sancho and Mario González Ulloa prepared a script expressing the will of all the assistants to create a Federation. During the 12th Congress, under the presidency of Dr. Ricardo Baroudi, Spain and Portugal confirmed their will to join the Latinoamerican group because of the cultural bindings uniting the assistant countries. Finally, in 1974, at the 13th Congress of in Caracas, was founded the Iberolatinoamerican Plastic and Reconstructive Surgery Federation (FILACP). The Spanish Plastic Surgery Journal was first edited in 1967 under the SECPRE presidency of Dr. Lazo. The first issue was published on May 1968 being editors Dr. Quetglas and Dr Hinderer. In 1971, Dr. Quetglas was designated as Editor in Chief of the Journal. Since the Iberolatinoamerican Federation had been already created, both journals were merged into the actual Journal of Ibero-Latinoamerican Plastic Surgery (Cirugía Plástica Iberolatinomericana). During 1973 and 1974, with Dr. Quetglas Moll as President, the general board approved the current name of Spanish Plastic, Reconstructive and Aesthetic Surgery Society (SECPRE). In 1992, the X IPRAS World Congress was held in Madrid

(which was postponed by a year, due to the gulf war). The President of that Congress was Dr. B. Vilar-Sancho and the Congress General Secretary, Dr. M. Bermudez From 1992 to 1999, Dr. Ulrich T. Hinderer was General Secretary of IPRAS The original logotype was mantained until 1998 when the SECPRE decided to switch to a new one, which was chosen at the XXXIII Spanish National Congress.

3. The present Currently, SECPRE stimulates humanitarian activities in different countries where international collaboration is requested. Several programs have been developed. In the present time, 800 plastic surgeons and 100 residents in Plastic Surgery are members of SECPRE. The actual board of directors is formed by Dr. Jaume Masià Ayala (President) and Dr. Gregorio Gómez Bajo (General Secretary) who are committed to promote SECPRE as a world wide distinction of Excellency in Plastic Surgery.
