Local author and blogger offers support for dementia patients and their caregivers
I
t’s true that life changes dramatically for an entire family when the decision is made to move a loved one who has dementia from home to community care. Many questions and concerns arise, such as: How do I choose the best place for my loved one? What happens once they are living there and what should I say or do when I visit? How do I say good-bye when I leave from a visit? What if my loved one doesn’t recognize me? A local gerontologist and dementia care consultant tackles these questions and much more in her book, When Someone You Know Is Living in a Dementia Care Community: Words to Say and Things to Do [Johns Hopkins University Press, 2016]. The book guides readers through a challenging situation by helping them cope with the difficult behaviors, emotions and anxieties that both they and their loved one may experience. “Do not let anyone make you feel like you have taken the ‘easy way out’ by choosing a dementia care community,” Rachael writes. “You are still going to deal with a lot of challenging behaviors, concerns, and questions regarding your loved one’s care.”

Reminiscing about communication in the “Good Old Days”

C
omputers and smartphones provide us many options in instant communication today. It’s fun to reminisce about how we communicated with one another 30 to 40 years ago before we had advanced technology. When you wanted to privately get a message to a friend in school, you wrote your note on a piece of tablet paper, folded it up in a square or a triangle and passed it to your friend. This was our low-tech text message. Little handheld black boards with chalk to draw or do assignments are today’s iPad. When I was growing up, the majority of families had one landline rotary telephone in the home, which may have been located in a kitchen or dining room. You had no idea who was calling as there was no caller ID. If no one answered the telephone, you had to try calling later, as there were no answering machines. If you were expecting a friend to call to make plans to attend an event, you needed to stay home in order to receive the call. Many mothers were home and took messages so you knew to call someone back. Of course, everyone in the family generally knew who was calling once you started talking to the caller. If my parents did not know who I was talking to, they would keep asking who I was talking to until I answered
I feel that having just landline phones required everyone to be more responsible in planning. At least by the night before an event, you called everyone on the phone about what you were going to do, where you were going to meet and who was going to drive. Doing this pretty much required everyone to show up and be on time. If someone was late, you did not know why until they arrived. You then found out if they got stuck in traffic or had to get gas in the car. When I was in college and we wanted to find out what fun activities were happening on campus, I would either need to look for advertisements in the student newspaper or walk to the student union to see what posters were hanging on the bulletin board for information on events. There was no Internet in the 1980s. Another way people communicated was by sending letters. Telephone rates were more expensive years ago, so many people would write and mail letters. It was exciting to go to the mailbox and find a handwritten letter from a relative or friend. Many letters documented family history, especially if someone was serving in the military during the war. I have kept some of the letters my grandparents wrote to me while I was in college. Communication also happened as many people had a subscription to their daily newspaper for not only national and state news, but also for local news and sports. You could find out what events were happening through stories or paid advertisements. Many people planned their weekly shopping trip by reviewing items on sale in the grocery store advertisements. Many civic organizations kept in touch with their members either by calling members and sending monthly newsletters through the mail.
Many of us enjoyed those times. However, technology can help us in today’s world, and it can help you do some things that you could not do 30 years ago. Several years ago, my family had lunch at a winery in Ohio. Afterward, we decided to try to find an Italian supermarket that friends recommended us to visit. Before we left the restaurant, we talked with the staff about the location of the store. I took out my smartphone and an online search showed the address. Using the restaurant staff guidance and GPS, we were able to get to the site; however, we were surprised to find that the store was empty. Well, a second Internet search showed another address at their new location, and we were able to correct the situation. In days gone by, we may have had to stop in another store to get guidance or would have just given up. Our instant technology can be beneficial for older adults living at home who may have complex health problems and poor balance. A PERS (Personal Emergency Response System) unit can help an older adult continue to live at home but be able to summons for help if experiencing a medical emergency. PERS are home devices that connect older adults to a 24-hour call center with the push of a button. The transmitter is typically worn on a neck pendant or wristband, and it sends a signal to a receiver that is connected to the telephone line. When a person pushes the button, the staff at the call center evaluates the situation, deciding whether to call an ambulance or a designated friend or family member. With most PERS setups, a person can talk with the call center staff from anywhere in the house. These units can be purchased, leased or rented. People qualifying for in-home services through the Beaver County Office on Aging can be considered for a PERS unit by talking to their care manager. This new communication technology is a great way to help keep people at home longer—where they want to be.

them. There was only so much privacy you could have while talking on the phone, and it was nearly impossible to get into another room to talk because the phone either was attached to the wall or a desk top model, and the phone cords were not very long.

In 1816, Ohioville lost some of its territory to Brighton Township. With the establishment of Glasgow Borough (1854), Industry Township (1856) and Midland (1906), even more land was lost. R
esidents of nursing facilities have the right to participate and attend their care plan meetings. They have the right to have their representatives attend, as well. During the care plan meeting the interdisciplinary team meets to set goals and outcomes for the resident. The care plan also includes the services needed for the resident’s highest practicable well being, the resident’s preferences and potential for future discharge. The team is supposed to include appropriate staff based on the resident’s need or as requested by the resident. Nursing facilities are mandated to provide person-centered care, which focuses on the resident making their own choices and having control over their daily lives. Facilities are to support the resident in achieving person-centered care and make reasonable accommodations for the resident in doing so unless it would endanger the health or safety of the individual or that of others living in the home. Care plan meetings are an essential element in achieving the highest quality of care. Residents and their representatives should make every effort to attend and participate in these meetings. If you are a resident in a nursing facility and you have not been invited to your care plan meeting, then there is a problem. The care plan meeting is about the resident and the resident and their representatives need to be part of the process. If you have questions about your care plan or that of your family

member, contact your social worker or the director of nursing at the facility. One resource for information on the care plan process and how it plays a key role in receiving individualized care is the book titled “Nursing Homes, Getting Good Care There,” written by Sarah Greene Burger, Virginia Fraser, Sara Hunt and Barbara Frank. This excellent publication is a consumer action manual prepared by The National Citizens’ Coalition for Nursing Home Reform, now called the National Consumer Voice for Quality Long-Term Care. Those interested can find more information on this publication by visiting consumervoice.org. Residents, family members and advocates for resident rights will find a wealth of valuable information related to nursing home care. If you still need assistance, contact the Ombudsman Department at the Beaver County Office on Aging at (724) 8472262 or (888) 548-2262. They can provide information and direction. When requested, an Ombudsman can attend the care plan meeting with the resident and their representatives. BCSN

BCOA announces caregiver training The Beaver County Office on Aging’s Caregiver Support program and Beaver County Behavioral Health are bringing author and presenter Karen Stobbe to Beaver County. She will conduct a seminar titled, “Being in the Moment: Tips for Caregiving and Living.” Karen is the author of Sometimes Ya Gotta Laugh: Caregiving, Laughter, Stress and Alzheimer’s Disease. Karen is a family caregiver and has worked in a care community. This training is for family caregivers of individuals with dementia, as well as for paid caregivers who may be providing assistance to this population. Social service staff and other professionals who assist, counsel and provide support and direction to families in caregiver situations also will benefit from attending. At the end of the presentation, participants will be able to:
