Choose Providence for Rehab and Get Home

Providence Care Center’s new Homeward Bound Program eases transition from rehab to home

T

o help recovering patients get back to normal as smoothly, safely and permanently as possible following rehabilitation, Providence Care Center, skilled nursing facility, has developed a three-phase rehab-to-home transition program called Homeward Bound. In the first phase of Homeward Bound, patients are evaluated to determine their readiness to resume life in their home community, either independently or with limited outside support. Next, before discharge, a Providence home assessment counselor inspects the patient’s home to determine its livability for that particular patient and to identify conditions, obstacles, hazards or other impediments to a successful return to home life. Finally, based on the findings of the assessment and recommended modifications that would improve the likelihood of a successful transition, Providence doctors, therapists, care givers and nursing staff collaborate to create an individualized transitional care plan which is reviewed with the patient before discharge and given to him or her for reference and review once he or she returns home. In addition to suggested modifications to the home, the plan offers recommendations for diagnostic, therapeutic, dietary, pharmaceutical and practical issues that the patient is likely to encounter when living independently again.

“Providence Care Center’s goal is to provide the most effective services for our clients by developing innovative solutions for rehabilitation and long-term care,” said Providence Care Center director, Raymond DeMarco. “Our Homeward Bound program fulfills the requirements of the new health care regulations. At the same time, it motivates hospitals to provide the most effective procedures for their patients, which, in turn, helps produce the best longterm recovery rates for our rehabilitation clients. We want to make our patients’ lives less complicated. Homeward Bound gives them the tools they need to succeed at that. That’s what we are about.”

Keen-Age News . . . For Beaver County’s Active Seniors Beaver County Office on Aging

By Angela Gentile, Planning Unit Program Monitor Beaver County Office on Aging For Beaver County Senior News

Our group of vegetable gardeners is a fierce group. Each of us knows it all, does it the best, etc., etc. etc. We argue. We bicker. We disagree about the layout of the garden each year. We gripe when there are changes with rules and procedures. We gripe that some of us work harder than others during our Tuesday work nights. The girls insist they are working while all the guys are leaning on their gardening hoes and talking. We tend the garden well under our Taste and Tour event and then end up letting the weeds take over. We gripe a lot about the weeds and wish our vegetable plants would be as hardy and disease and pest resistant as those annoying weeds. We disagree about whether you need to use fertilizer or if using compost is good enough for the plants. I forewarned them that I was going to write about them, and they said okay.

We are not totally grumpy with one another; there is another side. We do socialize. After most work nights, a group gets together at a local establishment in Bridgewater for what has been dubbed the “subcommittee meeting.” What is great about the particular establishment is that we are often hot and dirty from gardening, but other customers and employees do not really mind us being there. Sometimes we will have a corn roast at the garden on a work night and bring a covered dish to enjoy the bounty just minutes after harvesting. After tomatoes have ripened, we will have a tomato tasting among ourselves at one of the gardeners homes. We taste all the tomatoes and rate them for the best tasting. It is a good opportunity to try different tomatoes, which we may want to grow for ourselves the next year. Of course, we all bring a covered dish and enjoy a nice evening together. We share vegetables among one another. One gardener gave me some of his zucchini when the zucchini plants in my parents’ garden died. We will water one another’s gardens when we are away. Some gardeners will dog sit for other gardeners when they are away. We check on one another if someone is not well. Some gardeners have even vacationed together and several even kayaked together this summer.

Despite all of this, we actually produce a lot of vegetables in the demonstration garden. It is great that we can try some

We’ve become more than a group of know it all vegetable gardeners. We have become life-long friends. BCSN

Okay! Okay! I heard that you want to read another gardening article. I am a Master Gardener through Penn State Extension, and I am part of the group that tends the Vegetable Demonstration Garden in Brighton Township. Many of this group also has a senior garden at the same location.

different tomatoes, tomatillos, Swiss chard, peppers, potatoes, corn, onions, beans and more. Each year, we grow dry beans for the Vicary Mansion’s Windows through History program for girls ages seven to 14. They have an Eastern Woodland Indian program in September where the girls learn to shell the beans and use them to make soup over a wood fire. We are happy to help with this program each year. We do take home some of the vegetables to try. I learned that I liked tomatillos, but I did not care for okra or deep red/black tomatoes. With the surplus harvest, we make several trips each year to the Women’s Center, and they are appreciative of the fresh vegetables for their residents to enjoy.

Home Sweet Home housing options event at Senior Day Should I move to a smaller house or apartment? Should I renovate my current home to make it safe to stay? Should I organize and clear out some of my belongings? Should I move to a long-term care facility where there is someone to look after me? Should I stay or should I go? Older adults often need to evaluate their living situation and where to live comfortably and safely. It is not always easy to figure out how to accomplish this. Older adults and their caregivers can find some answers by attending the Home Sweet Home Housing Options event at Senior Day, Wednesday, September 10 from 10 a.m. to noon in the Sears corridor of the Beaver Valley Mall.

Also, the Beaver County Office on Aging will kick off its 28th annual Surprises for Shut-ins program. Information will be available on how you can donate to this program, which provides gifts and a friendly visit at holiday time to hundreds of isolated older adults living in their homes and apartments. Bring a new, practical gift or monetary contribution for Surprises for Shut-ins and be eligible for a chance to win a gift card.

You can talk to vendors about finding a smaller home, organizing and downsizing your belongings, finding someone to do home repairs and home security, locating apartments or long-term care facilities and selecting assistive devices for inside your home.

History Tidbits 2 The Patterson Heights Railway Co. started in 1895 with $6,000 in capital. The incline, as it was known by local residents, was built by John Reeves, an area resident. The 3/8mile car line transported residents between Patterson and Beaver Falls. The bottom terminal was at Bridge Street, and joined the current Seventh Avenue Extension in Beaver Falls. It proceeded northwest along the curb line of Bridge Street to the bottom of Ross Hill. It crossed Bridge Street and went between Ross Hill and Patterson Heights/Country Club Hill on its way to Patterson Heights. 2 The incline required a cable assist. The straightway portion was ¼-mile. The cabled portion consisted of one track the length of the hill or incline for the passenger car and an adjacent tract for the counterweight car, known as the dummy, which traveled just the upper half of the incline. The dummy car was weighted with scrap iron, weighing just less than the passenger car. About 10 to 12 passengers could ride at one time. The 10-minute trip cost six cents. In nearly 30 years, there were just seven incline operators. The final year of operation was 1927, as the automobile was becoming a more favored means of transportation. BCSN

Apps that help to improve memory

Herbed Zucchini Soup

If you are using Android, iPhones or iPads, there are apps available to help older adults improve memory. Here are four that you may want to check out.

As fall begins, it is great to make some soup. Several years ago, a headline flashed on the MSN website on what to do with all your zucchini. There were several recipes listed, including this one, which is quick and easy to make. Plus, it tastes good. I substituted yellow squash for the zucchini, and it works well.

Fit Brains Trainer (iPhone and iPad): The app has 360 games and unique training sessions to enhance memory, processing speed, concentration, problem solving and visual skills. There is a charge for the app, though you can try it for free. Brain Workout (Android): There are four brain training games which analyze memory, concentration, reaction and accuracy abilities. You can track your performance and compare scores with other players worldwide. Music Game (iPhone and iPad): The free app has six colored circles, each emitting its own tone. The app generates a sequence of tones that your have to memorizes and then hit the buttons in the correct sequence. As you progress, the sequences get longer and harder to keep track. BCSN

Columbia Gas And Home Safe Home advise seniors to have home safety checks

C

olumbia Gas of Pennsylvania, Inc. and Valley Care Association’s Home Safe Home program recently met to conduct a home safety check at a local Ambridge resident’s home using the newly published Home Safety Guide. A collaboration of the two organizations, the safety guide heightens homeowners’ awareness of potential risks and provides useful program and resource information to assist them in making their home a safer place to live. “It is important for homeowners to conduct home safety checks regularly,” says Columbia Gas of Pennsylvania vice president Mike Davidson. “Over time, as you become accustomed to your home, it may be hard to notice safety hazards that could result in a devastating incident. The safety guide serves as a reminder that even simple modifications in your home can improve safety.”

“The Home Safe Home program provides home safety related modifications, such as the installation of grab bars and stair railings, wheelchair ramp construction, and bathroom modifications,” explains Home Safe Home coordinator John Seitz.

ColumbiaGasPa.com and click on Contact Us. Submit the “contact us” form indicating the inquiry type as “Home Safety Guide.” The guide will be sent to the address indicated on the inquiry form. You can also request the Home Safety Guide by calling (888) 460-4332.

Columbia Gas actively supports organizations like Home Safe Home that are working every day to improve the quality of life for its customers and the communities it serves. The $15,000 grant contribution will help subsidize funding for safety related modifications that increase home accessibility and prevent hospitalization or premature placement in nursing facilities.

As one of the seven energy distribution companies of NiSource Inc., Columbia Gas of Pennsylvania’s donation to the Valley Care Association is from the NiSource Charitable Foundation providing support to local, regional and national organizations across its various operating companies. BCSN

Drummer problems A musical director was having a lot of trouble with one drummer. He talked and talked with the drummer, but his performance simply didn’t improve. Finally, before the whole orchestra, he said, “When a musician just can’t handle his instrument and doesn’t improve when given help, they take away the instrument and give him two sticks and make him a drummer.” A stage whisper was heard from the percussion section: “And if he can’t handle even that, they take away one of his sticks and make him a conductor.” Must help the wife Smith goes to see his supervisor in the front office. “Boss,” he says, “we’re doing some heavy house-cleaning at home tomorrow, and my wife needs me to help with the attic and the garage, moving and hauling stuff.” “We’re short-handed, Smith” the boss replies. “I can’t give you the day off.” “Thanks, boss,” says Smith “I knew I could count on you!” Taken from www.ahajokes.com.
