SUMMER DAY CAMP INFORMATION AND POLICIES
Swimming Trips Policy – SWIMMER DESIGNATIONS Please read the following information carefully prior to selecting your child’s swimming designation during registration. To help ensure the safety of your child while he or she is participating in swimming pool trips this summer, you will be asked to designate him or her as a “Non-Swimmer” or “Swimmer” during registration. Use the following descriptions as a guide for selecting your child’s appropriate swimming designation. NON-SWIMMER: Non-swimmers have limited or no previous experience with swimming pools or other aquatic environments. They are unable to independently swim distances of 20 yards or more and/or cannot swim or play comfortably in water that is above their heads.
If you designate your child as a “Non-Swimmer” he or she will be restricted to chest-deep water on swim trips. SWIMMER: Swimmers have previous experience with swimming pools or other aquatic environments. They can swim a minimum of 20-yards and can swim or play comfortably in water that is above their heads.
If you designate your child as a “Swimmer” he or she will be required to pass a swim test each time he or she visits a pool. The swim test requires participants to swim a minimum of 20 yards in water more than five feet deep and continuously tread water for a minimum of 20 seconds. Upon successful completion of the required swim test, children designated as “Swimmers” will have full access to both shallow and deep-water areas of the pool during swimming trips. Designated “Swimmers” who do not pass the required swim test will be re-designated as “Non-Swimmers” and restricted to chest-deep water. If your child’s swimming abilities change between the time you register for a program and the start of the program, please contact staff, you will need to sign a new form.

To comply with the state of Maryland’s Youth Camp Standards, The Department of Parks and Recreation has instituted a new, streamlined process for completing and collecting required forms. DURING REGISTRATION… Once you’ve selected the program you want for your child, a pop-up screen will appear listing 8 prompts which you are required to answer in order to complete a registration. The prompts are: • Emergency Contact Name Please provide the full name of the person to contact in case of an emergency. • Emergency Contact Phone Number Please provide the phone number of the emergency contact. • Physician’s Name Please provide the name of your child’s physician. • Physician’s Phone Number Please provide the physician’s phone number. • Is the participant a swimmer/non-swimmer? Your options are: (a) Yes – student in a Maryland public or private school ; (b) Yes – student is home-schooled or was in a school other than in the state of Maryland school; (c) No.
• Are there any health issues? Please answer Yes or No. Participants who have health issues such as allergies or other illness, take medication (prescription or over-thecounter), or use emergency medical devices such as inhalers or Epi-pens should answer Yes. The Health-Medication Form will be printed with your Day Camp Confirmation Receipt at the end of the registration. This form must be completed and returned to staff prior to your child’s participation in the day camp program. If you answer No, you will not need to complete any additional health forms. • Date of last Tetanus or DPT vaccination Please fill-in the month/year (example: 01/2012) when the child last received a Tetanus or DPT vaccination. This is a requirement of the State of Maryland. Once your registration and transaction are complete, the following items will print out for your convenience: • A Day Camp Confirmation Receipt • A Summer Day Camp Authorization Form (This form authorizes who may pick up your child during a camp session, and must be returned to camp staff prior to participation in the program.) • And, depending upon your answers to the prompts, the Health-Medication Form or Maryland DHM Immunization Certificate. PLEASE NOTE: For 2012, the streamlined forms system is not available via SMARTlink automated, touch-tone telephone registration. If you choose to register via SMARTlink automated, touch-tone registration, Customer Service Help Desk staff will contact you about completion of required forms.
If you answer Yes – and the student was in a Maryland public or private school, you will not need to complete a Maryland DHM Immunization Certificate. If you answer No or Yes – the student is home-schooled or was in a school other than the state of Maryland, then the Maryland DHM Immunization Certificate will print out with your Day Camp Confirmation Receipt. This Certificate must be completed and returned to staff prior to your child’s participation in the day camp program.

THERAPEUTIC RECREATION AND INCLUSION SERVICES
WHAT IS INCLUSION? Inclusion within The Maryland-National Capital Park and Planning Commission’s Prince George’s County Department of Parks and Recreation means that individuals with disabilities are welcome to participate in the same recreation programs and activities as their nondisabled peers. Reasonable accommodations may be provided to enable an individual’s successful participation in a program. Certain minimum eligibility requirements, such as age and level of participation, must be met in order to participate. WHAT ARE EXAMPLES OF DISABILITY ACCOMMODATIONS? Accommodations that may be available to promote successful participation in programs include: the provision of an enhanced staff/participant ratio; use of companions; sign interpretation for the deaf and hard of hearing; adaptive activity equipment and/or supplies; accessible transportation when transportation is provided as part of an activity or program; and assistance with health services that do not require medical training. HOW DO I REQUEST A DISABILITY ACCOMMODATION? All disability accommodation requests for camps, including extended care, must be indicated on the Health-Medication Form, noted on the individual’s SMARTlink account, and requested at least two weeks prior to the start date of the camp. WHAT ARE THE MINIMUM REQUIREMENTS FOR SUCCESSFUL INCLUSION? The same minimum requirements that apply to a person without a disability also apply to a person with a disability. These include: • Meeting the program’s age and registration requirements; • Following the rules of conduct (with or without reasonable accommodation); • Voluntary participation (Note: Recreation programs and activities are voluntary in nature. Participation will be encouraged and aided, but not forced.); • Meeting necessary level of participation with or without reasonable accommodations (participant will engage in scheduled activities for the majority of the program time); • Ability to use a consistent form of communication to indicate basic needs and follow simple directions; and • Ability to function, with assistance, as a member of a group of 12 or more people.
46
WHAT SERVICES ARE NOT PART OF AN INCLUSIVE PROGRAM? Inclusion services do not include the following: • Provision of separate program activities • Exception (with or without accommodations) to minimum compliance with program rules and regulations established for safety purposes. • Hiring, selection, or guaranteed assignment of a specific staff member; • Individualized therapy within a program; and • Provision and/or purchase of personal custom devices. IS AN INCLUSIVE RECREATION PROGRAM THE RIGHT CHOICE AND/OR EXPERIENCE? Observe the program first-hand and ask yourself… • If school-aged, do your child’s current classroom activities or goals prepare him/her for an inclusive recreation experience? • Has the individual shown an interest in the program activity or subject matter? • Is he/she comfortable and able to function appropriately, with or without assistance, in inclusive environments (malls, restaurants, parks, etc.)? WHAT ARE THERAPEUTIC RECREATION PROGRAMS FOR INDIVIDUALS WITH DISABILITIES? The Prince George’s County Department of Parks and Recreation offers alternative choices (for Prince George’s County residents only) through its provision of specialized therapeutic recreation activities for individuals with disabilities. Patrons have a wide range of abilities and special needs; some prefer and benefit from participating in specialized programs designed specifically for participants with disabilities. For program inquiries, please call the Special Programs Division at 301-446-3400 (voice) or TTY 301-446-3402. Program spaces are limited and available on a first-come, first-served basis. For appropriate referrals related to eligibility requirements for inclusion services, contact the Park and Planning Help Desk at 301-699-CALL (2255), TTY 301-699-2544 or e-mail help4smartlink@pgparks.com.
