We have organized this annual report to highlight how we grew and changed in FY2011 in our services to just a small selection of those populations: families, the LGBTQ community, adolescents, communities of color, young children, homeless people and women.

In addition to telling the stories of the people we serve—from Christina and her family to broad populations—this annual report introduces the mosaic of those who make it possible for us to do this work. Take a moment to look at our lengthy list of partners, including government agencies, foundations, trusts and other nonprofit organizations, and corporations. Visit the online list of individual donors and consider the impressive financial position we enjoy because of these critical collaborations. We are an intricately linked community of people and entities that quite simply care enough to come together to get the important work of public health done.

In another FY2011 family-centered program, Health Promotion Council (HPC), a PHMC affiliate, collaborated with then-affiliate* Best Nest to provide nutrition workshops for families. When Best Nest received a grant to work with families around health and nutrition, they called on HPC, whose mission is to promote health and prevent and manage chronic disease—especially among vulnerable populations—through community-based outreach, education and advocacy. “Nutrition is a key aspect of the work HPC does,” says Kerri Barthel, program director for in-home protective services at Best Nest. “We recognized there was a need to educate about nutrition that HPC could address.”

Health Intervention Program provides needed resources

The four-month nutrition education series covered a different topic each month. “Build a Healthy Base: Aim for a Healthy Weight and Make Healthier Food Choices” discussed basic nutrition, including tips for healthy eating and the importance of water.

“Protecting Your Heart: Choose Sensibility and Aim for Fitness” focused primarily on fitness, exercise and the benefits of a low-fat, low-cholesterol and low-salt diet. “Reading Food Labels and Stretching

Shortly after the birth of her daughter Giavannah, Christina realized something was very wrong. “My daughter was born early and kept in the neonatal intensive care unit for 52 days,” she says. “I kept telling the nurses, ‘There’s something wrong with her legs.’” Christina’s instincts were correct. Her days-old daughter was soon diagnosed with cerebral palsy and referred to the Health Intervention Program (HIP), provided by PHMC affiliate Resources for Children’s Health.

The home visitation and case management program offers support and resources to families caring for children with special healthcare needs. The HIP staff immediately initiated intensive home visiting services to assess the entire family situation, created a comprehensive service plan to address the needs of each family member, and assisted Christina and her family in locating and accessing the appropriate resources. “I love my caseworker, Jane Prusso, to death,” says Christina. “The HIP team has helped me with Giavannah by coming over and giving me resources.”

Your Food Dollars” centered on reading and understanding food labels and offered tips for getting the most from a food budget. “Building a Healthy Plate” reviewed the US Department of Agriculture’s MyPlate guidelines and the benefits of each food group.

REG’s assessment of the Get REAL program began in fall 2005 and ended in FY2011. PHMC collaborated with Mazzoni Center to study how drugs and alcohol affect sexual risk among black and white substance-using MSM be-

tween the ages of 15 and 29. PHMC developed, implemented and evaluated a community-level intervention for this population using persuasive media communication and interpersonal networking.

When the project ended, PHMC had developed and distributed 25 diverse role model stories based on interviews with MSM in the community, as well as an additional four stories featuring community allies. “We found overall that reading a Get REAL story had a positive impact on men’s sexual behavior patterns over time,” says Lauby. The project’s outreach team, supervised by Mazzoni Center, actively distributed more than 39,000 copies of these stories, along with condom packs and other materials, directly to young men in various neighborhoods, as well as through businesses, organizations and community events.

In 2008 REG began work on Project Reclaiming Inner Strength Everyday (RISE), formally titled “A Randomized Controlled Trial of the Efficacy of an Individual-Level HIV Prevention Intervention for Black Men Who Have Sex with Men and Women (MSMW).” In response to the lack of tested interventions for bisexual men, PHMC is one of three sites in the United States funded to develop an intervention that, if effective, could be disseminated to other areas of the country. Building on PHMC’s extensive experience with HIV prevention, the study is assessing the efficacy of a behavioral intervention versus standard risk reduction counseling for preventing acquisition and transmission of HIV among black MSMW. Conducted in partnership with The COLOURS Organization, the study is recruiting 250 black adult MSMW, including both HIV-positive and -negative men in Philadelphia.

management. Our previous work has made us aware that programs must address the needs of the whole person and not focus solely on risk behaviors. In addition to providing HIV information and prevention strategies, RISE counselors work with participants on coping skills, decision-making strategies, and integration of health-seeking behaviors into real-life situations.

In January 2011 REG began work on Assessment of Health Needs of LGBT Older Adults in Philadelphia, a project that explores the health needs and barriers faced by older sexual minority adults in the Philadelphia region. The ongoing study collects information from LGBT persons age 55 and older using focus groups and a structured survey. The project aims to examine healthcare services accessed by older LGBT adults in Philadelphia; document barriers to care, including homophobia, racial discrimination, financial concerns, legal obstacles and lack of appropriate healthcare providers; and explore older LGBT adults’ concerns and plans for the future, including health care, long-term care, housing and end-oflife decisions. The project also assesses the differences in these experiences and concerns across ages, races/ethnicities and genders. It employs an anonymous, self-utilized survey and aims to reach 200 to 250 older adults. The project’s community advisory board is essential to this work, having assisted with participant recruitment, consultation on survey content and dissemination of results to ensure that the findings will help improve policy and services for this population.

The teens attend three times each week for three-hour blocks, and parents are required to participate in at least two hours of programming per month.

The core mission is to prevent children from entering the foster care and juvenile justice systems and to increase their likelihood of graduating high school and becoming productive, law-abiding citizens.

“We go to court hearings with the families, we go to school meetings with the families...we’re very accessible to the families,” Wood says. “Our philosophy is that children thrive and do best when they are raised in their own homes with their biological parents.”

Wood used to work with adults and relishes the opportunity to counsel teens, whose minds are still malleable and whose mistakes are less scarring. As he puts it, “Being able to work with those same adults, but wind back the clock before they got there.”

Focus on Fathers, a program administered by PHMC affiliate Resources for Children’s Health, provides resources to fathers around the city. In FY2011, the program—now called Focus on Families—came to The Bridge substance abuse program in Fox Chase—also an affiliate of PHMC—to serve teenage fathers.

At SALT meetings, youth prepare a healthy meal to share with other teenage activists and convene forums on pressing nutrition issues such as equitable food access.

The Bridge empowers youth by letting them share frustrations and aspirations on their own terms.

During the once-weekly, 12-week program, Focus on Fathers participants at The Bridge get a safe space to talk about the challenges of fatherhood.

“They feel that they are able to connect with the other guys in the program and have a deeper understanding of what they’re facing,” says Michael Ogden, director of The Bridge.

Ogden says that most parenting initiatives target women, leaving men—particularly those without their own father figures to consult—feeling overwhelmed and underappreciated. Focus on Fathers fills that gap by teaching parenting skills in an environment that promotes healthy group dynamics and encourages youth input.

“The kids are steering the conversation,” Ogden says.

That is a mandate embraced with equal fervor at SALT and PUFFA, a peer-driven healthy eating education group created in FY2011 for teenagers in South and West Philadelphia. It is part of a program known as Philadelphia Urban Food and Fitness Alliance (PUFFA).

Members of PUFFA are equal parts students, organizers and advocates. Through the program they learn the many benefits of a healthy and sustainable diet, create educational materials and social media messages to share with their peers and lobby public bodies like the School District of Philadelphia for healthier food options.

SALT is the acronym for the once-monthly meet-ups the teens hold at alternating community sites. It stands for Students Advocating for Lifestyle Transformations. For each meeting, the youth prepare a healthy meal to share with other teenage activists and convene forums on pressing nutrition issues such as equitable food access.

From month to month and meeting to meeting, the youth drive the content of the programming. “It works,” says program director Diane-Louise Wormley. “They feel more comfortable, they participate, they don’t feel as if they’re on a stage. It’s their space and their event.”

PUFFA isn’t a classroom and its objectives aren’t mapped out like lesson plans in a curriculum. There is no chalkboard or headmaster. PUFFA doesn’t claim to teach students about nutrition in a scholastic sense. Rather, it provides them with the tools to learn about nutrition and compels them to explore.

In groups of 10 to 12, Pierre says, women talk about “self-pride and self-love” as a path toward empowerment and healthy sexual choices. He believes that by using shared heritage as a starting point, SIHLE can broach the kinds of topics many women typically feel uncomfortable tackling. “These are conversations that don’t occur in clinics. These are conversations that don’t occur in African-American communities, period,” Pierre says.

So far 48 women have passed through the program, and Pierre hopes to take the message further.

According to PHMC’s Community Health Data Base, Latinas are the least likely ethnic demographic to get regular breast exams. A full 40.9% have not had a mammogram in the past year.

Maria Vega, the NCS Project’s patient navigator, tackles this troubling trend by reaching out to Latinas directly through churches, community centers and even family picnics with information about the dangers of cancer and an offer of free cancer screenings.

With that sort of zeal, it’s no surprise that the NCS Project—run by Health Promotion Council, a PHMC affiliate—has in its first year already eclipsed its three-year goal of reaching 2,400 women.

Vega’s personal, streetwise approach helps allay many of the fears that keep women from seeking diagnostic intervention. “We talk a little bit about our country back home and then we get people comfortable...they feel open to talk. They talk to me about personal stuff, not only breast cancer,” Vega says.

Once she has their ear she can also dispel some of the breast cancer myths that circulate through the Latina community, including the notion that some interventions proliferate the spread of the disease. “We try to empower the women, teaching them how to do the breast self-examination. [We tell them] how important it is to have regular breast exams,” Vega explains.

It’s a cause that keeps Vega busy well beyond the nine-to-five strictures of her job. As a leading crusader for women’s health in the Latina community, Vega rarely passes on an opportunity to spread her gospel.

“I do outreach in the streets, in the bus, in the train, on my way to work, and on my way back home,” Vega says.

PHMC Care Clinic offers a healthcare home The Health Intervention Program (HIP) staff soon realized that Christina’s family faced additional barriers. “I wound up not having insurance, so I went to the PHMC Care Clinic for a checkup,” says Christina. HIP staff had referred both mother and father there for medical services. A two-time cervical cancer survivor, Christina needed help obtaining health insurance coverage and follow-up care. “I lost my job when Giavannah was born,” she says, “because when she came home she was on a heart monitor and I had to miss a lot of days of work to care for her.” Christina also suffered from depression after her baby’s birth. She received testing to monitor her cancer status, assistance in securing full medical coverage, a psychological evaluation and referrals to an oncology specialist and to mental health services.

“I have seen parents and their children thrive in the Mabel Morris program,” says home visitor Maria Mazzocchi. “One young Ecuadorian mother from South Philadelphia with two children came into the program with little confidence. She went on to study English as a Second Language at Community College of Philadelphia and took classes to get her GED at Community Women’s Education Project. She’s not afraid to ask questions anymore.”

In addition to working directly with parents and their children, home visitors refer their clients to other resources, including the PHMC-managed ChildLink program. Since 1992, ChildLink has provided service coordination, under contract with the City of Philadelphia’s Department of Behavioral Health and Intellectual disAbility Services, to Philadelphia County families of children ages zero to three with developmental delays. “Roughly 22% of the children in Mabel Morris are referred to ChildLink,” says Kinsey.

ChildLink service coordinators work in partnership with parents and early intervention specialists and therapists to evaluate each child’s needs, identify outcomes, explore options and develop an Individualized Family Service Plan composed of supports and services for each eligible child and his or her family.

ChildLink also provides tracking for children at risk of developmental delay. Its at-risk service coordinators complete the Ages and Stages Questionnaire with families to monitor ongoing development and determine the need for further evaluation. In FY2011, the program provided tracking services to 853 families in Philadelphia.

PHMC expanded on its tradition of providing care to the homeless with a new offering in Montgomery County in FY2011. In its 2010 Household Health Survey, PHMC’s Community Health Data Base found that 21,250 adults in Montgomery County were at risk for homelessness, defined as living below 100% of the Federal Poverty Level and having difficulty paying housing costs in the last year. PHMC took action by assuming the operation of the Montgomery County Homeless Prevention Center (the Center) in December 2010 with funding from Montgomery County Department of Housing and Community Development, North Penn Community Health Foundation and Montgomery County Foundation.

PHMC Connect: Piecing Together the PHMC Mosaic In FY2011, PHMC and its affiliate programs served more than 10,000 children under 18 years of age. While the majority (approximately 70%) of clients resided in Philadelphia County, a sizable number were from surrounding counties. Where did we get these data? PHMC Connect.

Created in FY2011, our new data warehouse offers insightful and useful facts like these that help tell the PHMC story, putting demographic and geographic data behind the work we do every day in our communities. The information in the PHMC Connect database illustrates the synergies across the organization, helping us make the connections that let us serve our clients even better.

The Center is a centralized screening service that assists county residents who are homeless or at risk for homelessness. Consumers and community providers can access the Montgomery County Homeless Prevention Center by calling a toll-free telephone number. Qualified service assessment specialists at the Center conduct initial screenings and refer consumers to appropriate housing or homeless provider agencies or resources. Through the Montgomery County Homeless Prevention Center, individuals and families can access referrals for shelters, food pantries, substance abuse and behavioral health resources, mental health and intellectual disabilities resources, developmental disabilities resources, and rental and utility assistance.

Service assessment specialists at the Center also screened potential clients for homelessness prevention and Rapid Re-Housing Program financial assistance from the US Department of Housing and Urban Development. The program, which was active in FY2011, provided rapid housing options to those at risk of homelessness or recently experiencing homelessness.

Additionally, the Montgomery County Homeless Prevention Center stores and analyzes data collected from callers to aid funders, housing and homeless providers, and government agencies that seek to improve the quality of housing and homelessness programs and services.

On June 21, 2011, PHMC commemorated 25 years of providing health care for the homeless in Philadelphia with a ribbon-cutting ceremony to mark the expansion of its Mary Howard Health Center facilities and services.

an entire city block, stood empty. Since 2007, however, it’s been home to a growing substance recovery community known as New Pathways for Women.

Appraising the industrial relic from its exterior alone, Argires says people “can’t believe what lives inside.” Thanks to extensive expansion, the program space for New Pathways for Women almost doubled in size in FY2011.

As if that architectural transformation wasn’t miraculous enough, the project itself has met with tremendous success. It is on target to reach its goal of enrolling 540 women by September 2012. Of the more than 400 women struggling with addiction who passed through its doors in its first four and a half years, a remarkable 40% had sustained their recovery at their six-month follow-ups.

New Pathways for Women reaches the neediest users: women who’ve used within the last month and aren’t enrolled in any other type of recovery program. Argires calls it “the front porch to recovery,” the first step for those seeking help with their drug and alcohol problems.

The project evolved organically from the pilot New Pathways Project, a mixed-gender program of similar ilk. Argires and her staff noticed that nine out of 10 individuals enrolled in New Pathways Project were male and wondered why more women weren’t seeking support for their recovery. The women at New Pathways told her that they often felt intimidated or excluded in a mixed-gender setting, which discouraged them from participating. Sometimes women unwittingly found themselves in common waiting areas with men with whom they previously shared drugs or exchanged sex.

Hence New Pathways for Women, a safe space where women can come together to talk about their exposure to traumatic loss and other intimate details related to their substance control issues.

It’s challenging work—dealing with people in the wobbly first steps of recovery is never easy—but Argires relishes her role on the front lines of recovery. She calls her involvement at New Pathways for Women “absolutely the most moving work I’ve ever done.” Additionally, in FY2011 new funding from the Philadelphia Department of Behavioral Health and Intellectual disAbility Services allowed New Pathways for Women to begin enhancing its services for women further along in their recovery, including plans for additional peer recovery groups.

“These are women who are just disregarded,” Argires says of this recovery community. New Pathways for Women’s first four years have proved to Argires that those still in the throes of addiction deserve treatment.

ChildLink makes critical connections Giavannah’s Health Intervention Program (HIP) team, which included her family, agreed that health education, incorporating occupational and physical therapy, would best address her needs and the family’s priorities for her development. “I noticed my daughter wasn’t picking up toys,” says Christina. “She was only pushing them around with her hands.” With occupational therapy coordinated through PHMC-managed ChildLink, Christina hopes her daughter will be able to stay on target with her fine motor skill development. “I just want her to be healthy and I want to make sure she’s not far behind when it comes to learning,” says Christina. Both of Giavannah’s parents receive homework from ChildLink in the form of activities to do with their daughter to encourage her to develop to her fullest potential.

“My daughter comes first for me,” says Christina. “I’m so thankful to my HIP caseworker for helping me with Giavannah.”

“I don’t just believe that, I know it,” Argires says.

“Anyone can recover.”

For four decades, the first steps taken at programs like New Pathways for Women have led women to Interim House in Mount Airy. Interim House’s backstory reads much like New Pathways for Women’s. In 1971, recovering alcoholic Clara Baxter Synigal felt the undue pressures of a treatment facility dominated by men and took matters into her own hands by founding what is now the oldest residential and outpatient substance abuse treatment program for women on the East Coast.

Women arriving at Interim House enter a residential program that lasts three to six months and features extensive outpatient therapy lasting up to seven years after the residency. Women

throughout the Philadelphia area are referred to Interim House, which in FY2011 expanded its clientele through a new referral partnership with Chester County.

The list of programs and supports hosted at Interim House runs long. Women have access to everything from psychiatric treatment to art therapy to a program where they can build their own bikes. In FY2011, Interim House expanded its peer-mentoring program that gives women struggling with addiction the opportunity to receive support from other recovering women. And funding from the Pew Charitable Trusts helped Interim House continue to offer a job-training program. “We have everything women need not only to stay clean but to find purpose and meaning in their lives,” explains longtime director Kathy Wellbank.

At Interim House the residents have 10 to 12 hours of structured time every day, developing the kind of routine they need to stay productive and substance-free. On a programmatic level, such rigorous, structured activity and licensed clinical services distinguish Interim House from the more come-as-you-are, community-based outreach approach at New Pathways for Women.

At their core, though, Interim House and New Pathways for Women share the same sustaining force: an ability to make women feel welcome and whole. As Wellbank puts it, “They really feel like they’re loved here.”
