I don’t have to tell you how the recession is touching America, Georgia or Atlanta. We hear about it every day in the news. A cure for our ailing economy can’t come soon enough. At Piedmont Healthcare, we’ve had to tighten budgets to keep the hospitals in our not-for-profit system strong. But rest assured, we have not sacrificed quality of care – that remains our No. 1 priority at Piedmont. In our ongoing mission to provide you with the perfect balance between health and care and to empower you to take charge of your healthcare, we’re sharing some important information in this issue of Piedmont Profiles that can help keep you and your wallet healthy in these tough economic times. Not everyone has extra money to spend on healthcare today, but there are simple, cost-effective and sometimes free options you have to maintain your health. You can read more in the article on page 24. In addition to your health, the health of Georgia hospitals is a concern when money is tight. I am fortunate and honored to serve as chairman of the Board of Trustees of the Georgia Hospital Association (GHA) this year. GHA is the state’s largest trade organization of hospitals and health systems providing education, research and risk management services to its 174 hospital and health system members. It also represents and advocates health policy issues benefiting Georgia’s citizens before the state legislature and U.S. Congress, as well as before regulatory bodies. I look forward to continuing GHA’s influential work to strengthen Georgia hospitals and improve healthcare in our state, despite the immense challenges we face. It is my passion to work toward improving quality and access to care for all Georgians. My new GHA role gives me a wonderful opportunity to make strides in this area. Lithonia resident Michael Gilmore became the first heart transplant patient in the world to receive simultaneous treatment with two ventricular assist devices, according to the device manufacturers. The procedure took place at Piedmont Hospital in December 2008 when 36-yearold Gilmore was admitted for allograft (heart) rejection and cardiac shock following a September 2006 heart transplant. Piedmont’s dedicated cardiovascular team provided a unique bridge to recovery using ventricular assist devices to temporarily reduce the workload of Gilmore’s heart. The procedure was led by transplant cardiologist Nirav Raval, M.D., and assisted by Vivek Rajagopal, M.D. “The team concept at Piedmont Heart Institute is what led to our success,” Dr. Raval says. “We drew upon the talents and expertise of everyone to save this patient’s life. I have the transplant knowledge. Dr. Rajagopal is an expert on inserting the devices. Our physician assistant, Gregg Steahr, and his critical care service provided the 24/7 attention Mr. Gilmore needed.”

Dr. Rajagopal first inserted an Abiomed Impella 2.5 partial ventricular assist device, known as “the world’s smallest heart pump,” to divert blood from the left ventricle to the aorta and the rest of the body, allowing the ventricle to rest. However, because of additional right

ventricle failure, he also inserted a second device, a CardiacAssist TandemHeart ventricular assist system, to redirect blood to an external pump and back to the pulmonary artery. Together, the two devices rested the patient’s left and right ventricles for approximately two weeks while life-saving anti-rejection medicine targeted lymphocytes that were causing the rejection of his transplanted heart. “Advances in technology have given our patients and cardiologists the precious gift of time,” notes Dr. Rajagopal. “When the heart is completely failing, we can bring our patients back from the brink of death and give them a chance to recover.” Speaking of the medical significance of Gilmore’s

experience, Dr. Raval adds, “This procedure clearly saved his life and should be considered a viable option for others who suffer from the same condition.” Gilmore has high praise for the entire Piedmont team, but singles out the nursing staff in particular. “When I was down and didn’t feel like talking, they would come in and ask me about my family and my kids,” he says. “They reminded me that I had a life to get back home to. Everyone worked so hard to keep me going and get me up and out of there.” In February, he went back to visit the nurses who had cared for him. “They were so happy and surprised to see me come walking in. I’m doing well and feeling good. I wouldn’t want to wish my experience on anyone. But, looking back on it, I feel like I had a purpose.”

Changes in color distribution, especially the spread of color from the edge of a mole into the surrounding skin, also are an early sign of melanoma. D is for diameter: the mole or skin growth is larger than 6mm or about the size of a pencil eraser. Any growth of a mole should be of concern.

PIEDMONT HOSPITAL CELEBRATES 2,000 TH TRANSPLANT RUTH JACKSON HAD SUFFERED FROM POLYCYSTIC KIDNEY DISEASE SINCE SHE WAS A YOUNG WOMAN . B ORN AT P IEDMONT IN 1949, THE ATLANTA WOMAN RETURNED THIS PAST T HANKSGIVING FOR A SECOND CHANCE AT LIFE WITH A KIDNEY TRANSPLANT. Jackson’s was the 2,000th transplant performed at Piedmont Hospital, which celebrated the milestone on November 28, 2008. “While these might seem like just numbers and dates, they represent lives saved and new beginnings through the miracle of transplantation,” says John Whelchel, M.D., who performed Jackson’s transplant. “And, we can’t forget that every one of the transplants performed by the Piedmont Hospital Transplant Services team is linked to a gift of compassion and generosity by an organ donor and their family.” Jackson received a kidney from her husband’s sister, Jane Barringer, who lives in Dunwoody. When Barringer found out that her sister-in-law might need a kidney transplant, she immediately signed up to be tested. She was a match from the beginning. “I wanted to be tested and donate my kidney, because it was a way that I could help,” says Barringer. “It was the right thing to do, and I wanted to do something to help the whole family.” Barringer also was worried about Jackson’s 18-year-old son who has autism. Barringer says she wanted to make sure that Jackson was around for a long time to care for her son. “Jane made up her mind she was going through with the donation no matter what,” says Jackson. “I was

afraid for Jane, but the testing was so thorough that it set my mind at ease.” Jackson was diagnosed with polycystic kidney disease 30 years ago by Piedmont nephrologist Jerry Cooper, M.D., and knew she would have to have a kidney transplant at some point. She credits Dr. Cooper with keeping her healthy for so long. “For many years, it was a matter of maintaining a healthy weight and exercise regimen, and I did everything I could through a strict diet and exercising,” says Jackson. “Dr. Cooper says my disease would manifest itself in my 50s, and he was right. The disease started taking its toll beginning in 2006.” “Ruth Jackson was the ideal patient, because she was an informed patient and really took good care of herself,” according to Dr. Cooper. “It has been an honor and a privilege to care for Ruth and to help make her life better. Courageous patients like her are an inspiration to all of us.” Because of the progression of her kidney disease, Jackson required dialysis beginning in 2008. However, due to her family and medical circumstances, her best option was to receive a kidney transplant. While

Jackson didn’t have to wait long since she had a family donor willing to donate, many throughout the country do have to wait. Currently, there are more than 2,700 Georgians on the transplant waiting list; nearly 2,370 are awaiting a kidney transplant and more than 200 are awaiting a liver transplant. Nationally, more than 100,000 people are waiting for an organ transplant. “The need for donors far outweighs the supply, so we encourage all individuals to discuss donation with their family members and friends,” says Mark Johnson, M.D., who performed Barringer’s kidney

donor surgery. “We are pleased that more people are considering being living donors themselves because it is a very safe option and results in great outcomes for many individuals awaiting a transplant.” Piedmont Hospital has one of the shortest kidney transplant wait times in the region with an average of 26.7 months, while the national average wait time is 41.6 months. Piedmont’s one-year patient survival rate after kidney transplant is 98 percent and remains better than the national average and the majority of programs in Georgia and the surrounding states. “The Piedmont Hospital transplant team is fabulous,” says Jackson.

“Everyone has been positive, informative, professional and caring.” Jackson says when she woke up from the surgery, she felt different from the very start. Within 24 hours her toxin levels had been reduced by half, and within 48 hours they were normal. “Life is 180 degrees different, and it is wonderful. I feel now like I felt about five to six years ago. It’s as if the last few years have been taken away,” says Jackson. “Kidney disease

is very gradual, and you don’t realize that you and your family are increasingly compensating for the illness.” Jackson and her husband, who have been married for 37 years, want to celebrate their anniversary in October by resuming one of their favorite family activities, hiking. They have hiked the Grand Canyon and Yosemite with their son. For now, Jackson is slowly building up her strength and stamina. “I am so grateful and fortunate. I will be here for my son who is making progress. But, it will take longer for him to function in the world,” she says. “This gives us more time together. I’m here for him because of Jane’s wonderful gift.”

TRANSPLANT PATIENT IN TOURNAMENT OF ROSES PARADE Alpharetta resident and double liver transplant recipient, Lauren Donkar, had the “ride of a lifetime” in the New Year’s Day Tournament of Roses Parade in Pasadena, California. Lauren was chosen to ride on the “Stars of Life” float, along with four other winners of a national essay contest, after telling the story of how her life has been transformed by transplantation. “I wouldn’t be alive today if I didn’t have a liver transplant,” says Lauren, whose cover story was featured in the Fall 2008 issue of Piedmont Profiles magazine. “It’s a shocking reality, but that’s why I look at life differently now.” The fall of 2006 was supposed to be the happiest time in her life – she was expecting her first child. But as her due date edged closer, Lauren developed preeclampsia and needed an emergency c-section. Her daughter, Allie, was born healthy, but 12 hours later, Lauren’s liver ruptured. She spent a month in a coma and ultimately needed two liver transplants to survive. Today, Lauren is an active mom and works part time as a pediatric nurse practitioner. This past summer, she participated in the National Kidney Foundation’s 2008 U.S. Transplant Games in Pittsburgh, Penn., winning a silver medal in women’s tennis and competing in the 5K race. In addition, Lauren, her husband, Nick, and their friends help others in need of transplants through the Chipping in for Life golf tournament benefitting Transplant Services at Piedmont Hospital and the Georgia Transplant Foundation. WHAT IS DIABETES?

Before we can define diabetes, we must talk about glucose and insulin. Glucose is the sugar that your body uses as its primary source of energy to perform all of its functions like moving, breathing, heart contractions and even thinking. But first, glucose must be allowed to enter the cells. For that to happen, your body needs insulin. Diabetes occurs when your body doesn’t make enough insulin or can’t use it properly. It is a disease in which blood sugar (glucose) levels are higher than normal. High blood sugar (diabetes) is the leading cause of blindness, heart attacks, strokes, kidney failure and amputations. There are two types of diabetes. Type 1 diabetes occurs when the body doesn’t produce any insulin. Type 2 diabetes occurs when the body doesn’t make enough insulin to meet your daily needs, combined with the fact that the body’s cells don’t use insulin correctly. Type 2 diabetes occurs in nearly 95 percent of people with diabetes. Before people develop Type 2 diabetes, they usually go through a pre-diabetic stage. Impaired glucose tolerance (IGT) and impaired fasting glucose (IFG) are the two kinds of pre-diabetes. In people with IGT and IFG, blood sugar levels are higher than normal, but not high enough to be diabetes. People with pre-diabetes have a high risk of getting diabetes. They also are more likely to have a heart attack. A blood test can determine if you have diabetes or pre-diabetes.

Diabetes Risk Factors • Being overweight or obese • Having a parent or sibling with diabetes • Being diagnosed with diabetes during pregnancy or having a baby that weighed more than nine pounds at birth • Being African American, Native American, Latin American or Asian/Pacific Islander • Having high blood pressure (above 140/90 mm hg) • Having high-density lipoprotein (HDL) cholesterol level (good cholesterol) less than 40 mg per dl (for men) or less than 50 mg per dl (for women), or your triglyceride level is higher than 150 mg per dl Preventing Diabetes Making changes in your lifestyle, according to the Diabetes Prevention Program research study, can lower your risk of developing diabetes. If you are overweight, losing 5 to 7 percent of your total body weight can help. Losing weight also will lower your blood pressure and cholesterol levels. Exercise of any kind also can lower your risk. Your exercise routine should include 30 minutes of moderate physical activity at least five times a week. Be sure to stay at an exercise level that your doctor says is safe for you. Following a healthy diet also can help. Eat foods like salads, vegetables, fruits, whole grains, fish, beans, poultry and other meats. Don’t eat a lot of white sugar, honey or molasses. Eat foods made with whole grains instead of white flour. Less than 30 percent of your total daily calories should come from fat. Less than 10 percent of your daily calories should come from saturated fat. Carbohydrates should make up 50 to 60 percent of your total daily calories. Your diet should include at least 20 grams of fiber per day. Your doctor might refer you to a dietitian or diabetic educator for help in changing your eating habits. If the online evaluation indicates significant risk for cardiovascular disease, participants may choose to be contacted by a Piedmont Heart Institute cardiac nurse who will offer a free consultation and screenings to further assess cardiac risk. Free screenings include blood pressure measurement in arms and ankles for peripheral vascular disease; height and weight measurement for body mass index; and cholesterol screening. The cardiac nurse may recommend a follow-up visit with a physician or cardiologist and can provide assistance in referring to a physician if necessary. Information about CT coronary calcium scoring also may be made available to individuals deemed at high risk. CT coronary calcium scoring is a non-invasive test that uses computed tomography (CT) to check for calcium buildup in the coronary arteries of the heart. It can detect early-stage heart disease. ONCOLOGY SERVICES MOVING FORWARD Despite the economic challenges facing Georgia’s healthcare providers, Piedmont Fayette Hospital continues to move forward with plans to enhance its oncology services and develop a comprehensive cancer center to serve the local communities. The hospital will purchase a linear accelerator for radiation therapy that would be the centerpiece for centralized oncology services. A linear accelerator uses microwave technology to provide external beam radiation treatments for cancer patients, targeting a uniform dose of high-energy X-ray at the patient’s tumor. In addition, Piedmont Fayette is poised to begin offering seed implants for prostate cancer patients. Seed implants are a form of radiation therapy that are placed in the body and allow a high dose of radiation to be delivered to the prostate with less damage to surrounding tissues. The procedure can be more convenient than external radiation, which can require weeks of daily treatments. The hospital also plans to develop a cancer wellness program that will include nutritional counseling and support groups. The hospital is in the process of surveying cancer survivors for input on the services most needed through a cancer wellness program. The wellness program will help link cancer patients to other services in the community as well.

CORONARY CTA PART OF ADVANCED CARE A s other hospitals around Georgia are just beginning to offer advanced coronary CTA (computed tomography angiogram), Piedmont Fayette Hospital’s expertise in the high-tech heart imaging test helps explain the hospital’s reputation for bringing advanced medicine to Fayette County. Coronary CTA is a non-invasive heart imaging test that can detect fatty or calcium deposits that have built up in the coronary arteries. It also can help prevent possible heart attacks by detecting heart muscle disease early. The test is actually upgraded software for Piedmont Fayette’s 64slice CT (computed tomography) scanner. Piedmont Fayette was the first in Georgia to use the scanner. Less invasive than other procedures, coronary CTA requires less recovery time and can be covered by Medicare for qualifying patients. AUXILIARY SCHOLARSHIP HONORS SPIRIT OF ‘MISS GERALDINE’ No one’s roots reached deeper into Piedmont Fayette Hospital’s history than those of Geraldine Adams Stinchcomb. As a small girl growing up in Fayette County, Stinchcomb played in the very fields where Piedmont Fayette Hospital now stands. She entered healthcare at a time when Fayette County had only one doctor. She was the county’s first public health nurse. And, when demand for a hospital grew, Stinchcomb was volunteering before the doors were open. “The people of Fayette County felt a great need for a hospital in this area,” she once reminisced before a

group of community leaders and hospital staff. “Our hospital is one of the best things to happen for Fayette County; it’s our pride and joy.” Stinchcomb died in 2008, leaving a legacy of tireless service not only for Piedmont Fayette, but also for her community. To honor her dedication, many generous donations were made to the Piedmont Fayette Hospital Auxiliary Scholarship Program in memory of Geraldine Adams Stinchcomb or “Miss Geraldine” as she was known. The scholarship is for Fayette County residents pursuing health-related degrees at a Georgia university or college. Most of Stinchcomb’s life was dedicated to serving the healthcare needs of Fayette County residents. She graduated from the Piedmont

Hospital School of Nursing in 1940. After starting her nursing career at Piedmont Hospital, she returned to Fayette County to work for the area’s local doctor. World War II had been over just a few years when Fayette experienced an outbreak of diphtheria. Stinchcomb helped set up clinics across the county as part of a countywide immunization program. In 1949, she opened the Fayette County Health Department and became the county’s first public health nurse. In those days before disposable equipment, Stinchcomb often found herself sterilizing her medical instruments in boiling water atop old wood stoves in one-room schoolhouses. As she made her rounds from school to school, she soon earned the nickname “Shot Lady.”

Stinchcomb was involved at Piedmont Fayette from the beginning. She joined the Auxiliary before the hospital was opened. On opening day, September 17, 1997, Stinchcomb was greeting people at the front information desk. In all, she logged over 1,500 hours as a Piedmont Fayette Hospital volunteer and received the Auxiliary’s Angel Award for her dedication. The work of “Miss Geraldine,” and her dear friend “Miss Janice” Cole, on a project with Piedmont Fayette’s blood lab, was vital to the hospital receiving a Quality Patient Safety Award from the State of Georgia. Stinchcomb’s many years of service are only part of her family’s healthcare legacy in Fayette County. Stinchcomb’s daughter, Harriett Hazelton, followed in her mother’s footsteps. Hazelton just recently retired after 22 years with the Fayette County Health Department as a public health supervisor. An outpouring of memorial gifts in Stinchcomb’s honor will ensure that the next generation of students in the medical field will benefit from a life that was spent ensuring the health of Fayette County. “There was no one like Geraldine,” says Diane Sartini, president of the Piedmont Fayette Hospital Auxiliary. “She gave tirelessly to this hospital from the first day. First Paired Donation in Georgia Takes Place at Piedmont Hospital ORGAN SWAPPING

BARBARA A LLEN OF NEWNAN. BARBARA HAD BEEN ON THE ORGAN DONOR WAITING LIST FOR TWO YEARS, AND MARIE SIMPLY WANTED TO HELP. The two met as teenagers when their military families were stationed in Turkey. Then, years later in 1993, they discovered each other again, this time at a local recreation center where their sons happened to be on the same wrestling team. They have been best friends since. Barbara had been in a car accident in 1984 and her kidneys were damaged. She had a kidney transplant in 1990 from her sister that lasted 16 years. Unfortunately, she developed antibodies rejecting the kidney, and none of her family members could donate. So her best friend was tested in hopes of matching. After Piedmont Hospital transplant coordinator Cheryl Manley told Barbara and Marie, both 48 years old, that they were not a match, she asked if they would be interested in a paired donation. While most living kidney donors donate to people they know, paired donation allows them to swap organs with unknown donors to create a compatible match. Creating a domino effect, one donor swaps with someone else who also wants to be a donor, allowing two individuals to receive a transplant. In this case, it took less than three months for the two of them to find a match. On Friday, October 10, 2008, two kidney transplants occurred. At Piedmont Hospital, Barbara Allen received a kidney from 43year-old Juan Ramirez, who had attempted unsuccessfully to

At a hospital in Philadelphia, Daisy Ramirez, whose kidneys started failing in 1992 and who had been on the waiting list for more than a year, received a kidney from Marie Johnson. In both cases, neither recipient had met or known her donor previously. All four later met via webcam. “The United Network of Organ Sharing reported this year that a new threshold was exceeded. For the first time, 100,000 people are on the organ donor waiting list and the majority, over 76,000, are waiting for a new kidney from a deceased donor,” says John D. Whelchel, M.D., former chairman of Piedmont Hospital Transplant Services, which experienced phenomenal growth during the past fiscal year ending June 30, 2008. Transplants increased 15.3 percent over the prior fiscal year with 218 total transplants performed in FY2008. Liver transplantation increased 37.5 percent over the prior fiscal year. The program’s outcomes (graft and patient survival) continue to be among the best in the nation, and Piedmont’s percentage of living donor transplants continues to exceed those of the region. “The new paired donation option allows us to help more transplant candidates across the country,” Dr. Whelchel adds. “In addition, a kidney from a live donor has a longer life than that from a cadaver donor. On a Saturday morning in March 2008, 20-year-old Trae Cagle was shot in the neck and rushed to Piedmont Mountainside Hospital, where he was declared brain dead. “I remember thinking that this young man’s life was just starting, but now he was beyond our help,” says Barry Weaver, the hospital’s house supervisor. “His mom and dad were my age, and I have a son about his age. It was very emotional. The kindness and professionalism shown by the Piedmont staff to the family was extraordinary. We are like a family, and we strive to treat others like family, too.” “If it hadn’t been so foggy that morning, Trae might have been transported to another hospital,” says his mother, Terri Cagle. “I know God put that fog there so we would be at Piedmont Mountainside. This was the worst day of my life, but I will never forget the awesome care and kindness we received. The staff was wonderful. The Emergency Department doctor on duty, Nicole Higgs, cried with me. I remember a nurse – I found out her name is Josie Thomas – giving me a big hug and telling me, ‘Don’t worry, I’m going to take care of your baby all night long.’” The Cagle family decided to donate Trae’s organs through LifeLink, a non-profit organization that recovers organs and tissue for transplants. Piedmont Mountainside and the crew on duty had never participated in a procurement of organs. Weaver met with the LifeLink representatives and the medical professionals on duty to prepare for the procedure. The team consisted of three surgeons, two surgical technologists, a clinical nurse coordinator, operating charge nurse Allura Allen and anesthesiologist Rosemarie Spillane, M.D. Dr. Spillane’s job was not to administer anesthesia, but instead to assist the surgeons with lung ventilation and collapse as necessary. “The LifeLink people couldn’t believe that this was our first time,” says Allen. “They said we went out of our way to accommodate them and the donor’s family. We were just acting the way we always do.” While the surgeons recovered the patient’s organs, the recipients were notified and prepped for surgery. The organs were transported to the airport five minutes away where a helicopter was waiting. Timing was critical since

organs need to be transplanted anywhere from four to 12 hours after being recovered. One kidney and the pancreas went to a 38-year-old Georgia woman with diabetes. Another kidney went to a 15-year-old student from Georgia who had suffered kidney failure. The liver was transplanted into a 50-year-old Florida man after the first transplant failed. A 35-year-old man from Georgia with heart disease received the heart. All patients are doing well. Allen was overcome with so many mixed emotions that night. “I felt tremendous sadness for the young man and his family, and such joy knowing so many recipients would live because of this tragedy. As a nurse, I pledge to help people and increase the quality of their lives. In the operating room, we often perform surgeries to prolong life and, in this case, I still did that. Except this time, it wasn’t for the patient on the table. It was for people I’ve never met. I’ve always been an organ donation advocate. My husband had been against it, for him and for me. This experience changed his mind.” “There was nothing I could have done to save my son,” Cagle says. “I know his death was not in vain. I wrote to all the recipients last year. I wanted them to know my son, that my first born was a hero. I received a letter back from the man who received Trae’s heart. He’s married with two toddlers and had been sick for some time. He wrote, in part, ‘Because of your kindness, I was given my life back and even more. Because now I appreciate each and every moment I have. I can hug my wife, kiss my girls, hang out with my friends and family, and attend church. I love you without even knowing you.’” “Trae was a very loving boy. He played football and baseball and was very popular. He had so many friends and he always hugged his friends. When JOHN COUEY turned 59, he took stock of his life. He lost his job after 12 years and started a new career as a business owner and consultant. On New Year’s Eve, the Stockbridge resident made a resolution that December 31 would be the perfect time to get his annual physical – every year. 24

“It’s an unusual way to spend the holiday, but I’ll never forget to have my annual checkup,” he says. Couey, an avid golfer, is relatively healthy, though at his age, he is concerned about heart disease and prostate and colon cancer. “I have 14-year-old and 16-year-old daughters, and I’m more concerned about them than myself,” Couey says. “I love them so much, and I want to be there for them as long as I can.” Couey is not alone. For many people, staying healthy remains not only the best way to avoid costly healthcare bills, but also to enjoy a healthy, active lifestyle. While the economy is causing a strain on many families, with some people forced to choose between buying groceries or buying medicines, it is important to keep in mind that your health is one of your most important assets. Once you lose it, not only is it hard to get back, it’s also more expensive to get back. According to a recent survey, 55 percent of people say they are taking better care of their health because of the economy, compared to 41 percent who say they are taking worse care. Of those taking better care, 60 percent say they are exercising more and eating healthier.

Commitment to Prevention

Couey has been a patient of Alan Perry, M.D., of Piedmont Physicians in Atlantic Station, for the past 10 years and has a strong allegiance to Piedmont Hospital. Both his daughters were born at Piedmont, and both he and his wife have been Piedmont patients. Dr. Perry said Couey is a great example of a patient who is proactive and understands the importance of prevention. “The old adage that ‘prevention is worth a pound of cure’ still applies today,” Dr. Perry says. “People can make simple changes to stay healthy, such as eating right, exercising regularly, getting annual physicals and getting screened for preventable diseases.” Dr. Perry suggests simple, no-cost ways to stay healthy, including hand-washing to prevent infection; monthly blood pressure checks, which can be done for free at places such as supermarkets or pharmacies; monitoring changes in bowel habits such as checking for blood in the stool; watching for unusual weight gain or weight loss; keeping track of non-prescription and prescription medications, and monthly breast self-exams for women.

Dr. Perry also recommends the following preventive measures:

• Women over 40 should get a mammogram every one to two years and a Pap smear every two to three years if they have had three normal Pap tests for three years in a row. Those over 50 should undergo a colonoscopy, unless there is a family history, which would require earlier testing.

• Men 40 and over need a prostate-specific antigen (PSA) test for prostate cancer and also should undergo a colonoscopy at age 50.

• Men and women should have a baseline electrocardiogram (EKG) at 40, unless they have a family history of heart disease.

Importance of Annual Check-Ups

At the annual physical, the doctor will listen to the heart and lungs, check blood pressure and pulse, and check the ears, nose and throat, the carotid arteries and abdomen. It’s also a good idea to have the thyroid checked and to have a complete body check for moles to prevent skin cancer. Dr. Perry also recommends a daily multi-vitamin and to check with your physician if you are taking other vitamins or dietary supplements, as they could interfere with prescription medications. Additionally, he recommends a coated 81-milligram aspirin for men and women over 40 to prevent heart disease. While some doctors say you can stretch some medical visits, there are three areas of healthcare you must attend to, not only for your well-being, but to avoid more costly chronic care later. Following are three must-do habits and suggestions on how to get help paying for care.

1. Take Your Medicine – More than one third of people in a recent survey reported that they were splitting pills, skipping doses of medicine or not filling prescriptions. “If you have a chronic disease such as high blood pressure, diabetes or asthma, it is critical to take your medicines,” Dr. Perry says. “It’s important to continue taking your medicines so you can avoid more costly health problems.” Even minor illnesses such as strep throat or an ear infection can turn into a serious problem if left untreated. Tip: If you’re having trouble paying for your medicine, talk to your physician immediately. He or she may be able to switch you to a less expensive alternative or a generic version of the drug you’re now taking. Your doctor also may be able to help you enroll in a prescription drug aid program such as the Partnership for Prescription Assistance, a coalition of pharmaceutical companies and healthcare advocates, according to a national consumer advocacy group. Another good source for help with finding affordable prescriptions (and all types of medical financial assistance) is MedlinePlus, a Web site backed by the National Library of Medicine and the National Institutes of Health.

2. Pay Attention to Your Symptoms – When you have symptoms or start to feel sick, it’s time to get checked, says Dr. Perry. Skipping tests and treatments won’t solve the problem. If you end up needing a diagnostic test, like a CT scan or MRI, there may be ways to get it done at a reduced cost. Tip: Ask your doctor about free screening programs and low-cost clinics in your community that can provide an affordable test. If your test or treatment is related to a specific disease, search the Web site for the association or foundation that specializes in that illness. These organizations often provide resources for financial assistance to patients in need.

3. Be Aware of Your Mood and Stress Level – Feeling out of control financially can lead to unhealthy habits, but this is not the time to smoke or drink more, Dr. Perry advises.

Tip: Continue to eat healthy foods, exercise regularly and get enough sleep. “All three of these low-cost activities have proven health benefits,” Dr. Perry says. “Living a healthy life may help you relieve some of the stress you’re feeling about your finances.”

John Couey already has his goals in mind for the coming year for a healthy lifestyle. In addition to daily swimming, he is committed to eating healthier and losing weight. “I want to maintain my blood pressure, get my cholesterol down to 180 and get my weight down to 190 pounds,” he says. “I can do it, as long as I cut down on my fast food and soda.”

PALLIATIVE CARE HELPS FAMILIES SAY GOODBYE By Judy Stanton

When you’re in your 30s and married for eight years with a new baby boy, end-of-life planning is not high on your priority list. So, when cancer strikes, you fight back. That’s what Cathleen and Tim Lavelle did for nearly 18 months. They did surgery, radiation therapy, chemotherapy, PET scans, chemo cocktails and adjunctive radiochemotherapy. “We were in curative mode,” says Tim. “I was Cathy’s cheerleader. Her champion. I gave her hope. We agreed to do whatever it took to give her more time.” But, sadly, Cathy’s time was running out. The cancer that started in her uterus was spreading to her lymph nodes, chest, abdomen and pelvis. After 17 months, doctors finally advised them to stop fighting and help Cathy be at peace.

When Deanna Richardson, RN, family nurse practitioner and manager of palliative care at Piedmont Hospital, talked to Tim, he didn’t know what palliative care was. He was emotionally jarred and drained. He wasn’t thinking clearly. But he really needed help. Cathy’s parents were minding the couple’s two-year-old son, Jace. And Tim’s employer allowed him to work a very flexible schedule. But nothing prepared him for this major life change. Deanna helped educate Tim about the process. She supported him in making important decisions, such as how to give Cathy meaningful time with Jace and how to choose a hospice facility. She made sure that medications were ordered to manage Cathy’s pain and keep her comfortable. When Cathy wanted to go home, Deanna

arranged for the necessary medical equipment. She made sure that advanced directives and a living will were in place. Cathy never made it home. Now, looking back, Tim says that palliative care helped make that end-of-life experience beautiful. “Our families were here and everyone spent time with her. Cathy was at peace with her life. With the support of palliative care, I was not bogged down with details and paper work. The transition to hospice was transparent,” Tim says. “I was able to focus on spending quality time with Cathy.” “And they didn’t forget about me. Deanna’s periodic calls in the months following Cathy’s death meant so much to me. There aren’t a lot of people my age with whom I could share this experience and truly connect. Deanna assured me that my feelings were normal. She supported me on my healing journey. Her care and compassion were very comforting,” Tim says. “Palliative care is often equated with hospice or endof-life care, because it is aimed at aggressively managing symptoms and assuring comfort versus treatment toward a cure,” says Deanna. She has helped families deal with their loved one’s new physical limitations, pain control, and chronic disease, whether from cancer, heart disease, stroke, kidney failure, a neurological disorder or chronic obstructive pulmonary disease. She also has helped resolve family conflicts and legal issues stemming from the patient’s plan of care. “Our goal is to minimize suffering and discomfort for patients and their loved ones dealing with actively deteriorating disease,” adds William Jonas, M.D., oncologist and medical director of palliative care. “For patients who have conditions that will potentially decline, the goal is to decrease their stressors by preparing them with advanced care discussions to help determine and document endof-life care choices, and to achieve their best possible quality of life.”

HELP US MEET THE CHALLENGE Piedmont Hospital has been issued a challenge. An anonymous Atlanta Foundation has awarded the hospital a challenge grant of $150,000 to help fund additional staff and educational materials for palliative care. However, the money is contingent on the hospital raising matching funds. In the two years since its implementation, more than 900 patients have been referred for palliative care; 558 of those patients have received services. Adding a second palliative care nurse practitioner would double the number of patients who could receive palliative care; increase education for Piedmont clinical staff so that patients would be referred earlier; and would enable more intensive training of consulting staff. A fully-staffed palliative care program can result in healthcare cost savings. ATLANTA FAMILY ESTABLISHES GALAMBOS LIVER DISEASE AND TRANSPLANT FUND FOR PATIENTS By Judy Stanton

L

Liver transplant patients at Piedmont Hospital will benefit from a donation from the Scott Hudgens Family Foundation which makes possible new technology and financial support needed to secure matching funds from the Georgia Transplant Fund. The Hudgens family is native to Atlanta. The late Scott Hudgens developed homes; parks, including Centennial Olympic Park; and shopping centers, including malls at Greenbriar, Town Center, Gwinnett Place and the Mall of Georgia. His philanthropy is recognized by buildings named in his honor, including a Center for Skilled Nursing in Suwanee, the Early Education Center at Gwinnett Technical College and a park in Duluth. The Scott Hudgens Family Foundation learned about liver disease when their daughter, Marcia, suffered a severe liver ailment. Although it took some time to get her illness under control, in many ways, she was fortunate. Her condition did not require a liver transplant. Her family had good insurance and could afford treatment. And she was treated by a specialist in liver disease, Michael R. Galambos, M.D., a gastroenterologist who practices at Piedmont Hospital in Atlanta and Piedmont Mountainside Hospital in Jasper. Even though it has been more than a decade since her illness, Marcia Hudgens Dugan has not forgotten what a difference Dr. Galambos’ treatment has made in her life.

The Hudgens Foundation made a sizable donation to the American Liver Foundation, but she wanted to support Piedmont Hospital in promoting community awareness and help others suffering from liver disease. The Galambos Liver Disease and Transplant Fund for Patients established by the Hudgens Family Foundation honors the trust and devotion that Michael R. Galambos, M.D., and his father, John T. Galambos, M.D., embody with their

David Peardon of Talking Rock, outside Jasper in Pickens County, is just one. He had end-stage liver disease which could only be treated by a liver transplant. His illness forced him to quit work, so he didn’t have insurance. He was able to secure coverage from Medicaid, but before he could have a transplant, he needed to have money in the bank to help cover the cost of very expensive anti-rejection medications which would be required for the rest of his life.

Dr. John Galambos, 87, is a retired physician who pioneered liver transplant services in Atlanta. The gift will be used to secure muchneeded technology to meet the needs of an expanding patient population at Piedmont Hospital and to help individuals who do not have the financial means to secure matching grants for expensive post-surgical medications, which is required before being placed on Piedmont Hospital’s liver transplant waiting list. Dr. Galambos is acutely aware of the need. He sees many patients in advanced stages of the disease who have to postpone surgery until they can raise funds needed to get a matching donation from the Georgia Transplant Foundation.

So, fundraising became his job. “It took about a year and involved a lot of paperwork to get city, county and state permits; make contacts; and put plans into action,” Peardon says. “I did it all on my own.” Peardon organized his own bake sales, spaghetti dinners, raffles and neighborhood coin banks, and raised close to $10,000 in about a year. Money he raised qualified him to receive matching transplant assistance funds and got his name on the list for a liver transplant at Piedmont Hospital. On October 22, 2007, Peardon got a call at 3 a.m. that a liver was available for him. The transplant has given him a new lease on life. In fact, at age 52, he’s reconnected with his high school sweetheart, whom he plans to marry in June.

According to Arietta Maney, LMSW, one of Piedmont Hospital’s transplant social workers, the medications required to keep the liver functioning after the transplant can cost $3,000 to $4,000 per month. So, even insured patients whose plan requires a co-payment for medications may still have to cover a $500 to $600 monthly bill. “Patients requiring a liver transplant are often in a life-or-death situation. Their entire family is impacted. I evaluate their ability to support the patient’s needs, both before and after the transplant. The struggle to raise money before they can be placed on the transplant list adds to everyone’s stress,” Maney says. “This fund will be incredibly helpful for many patients who don’t have the resources to apply for Georgia Transplant Foundation matching funds.” Since performing the first liver transplant at Piedmont Hospital in 2005, more than 200 people have had the life-saving procedure at the hospital. Piedmont is one of only two liver transplant programs in Georgia, enabling more Georgians to stay closer to home for transplant services. While the numbers of patients served has grown, so has the number of people waiting for a transplant. In fact, over the past decade, the waiting list has increased nearly nine-fold. Mark E. Silverman, M.D., left a lasting legacy to Piedmont Hospital when he passed away on November 12, 2008. His leadership provided the foundation for the Fuqua Heart Center of Atlanta at Piedmont Hospital. His standards of excellence and passion for teaching live on in the Piedmont Heart Institute. He touched the lives of hundreds of patients, nursing and physician colleagues, hospital leaders and employees, as well as students, residents and cardiology fellows. He invested in his vision for Piedmont Hospital by establishing the Medical Scholars Fund in 1993. Dr. and Mrs. Silverman established the fund from their own resources, with matching support from his patients, friends and colleagues. Shortly before his death, Dr. Silverman, as well as his wife, Diana, and the Piedmont Hospital Board, agreed that a chair in cardiology and education would be the most appropriate use of the fund. Recently, Piedmont announced that the Medical Scholars Fund will provide a $2 million foundation toward a $5 million goal to create Piedmont Hospital’s first Distinguished Endowed Chair. It will be named The Mark E. Silverman Distinguished Endowed Chair in Cardiology and Education in his honor. This effort has been endorsed by a committee of friends of Dr. Silverman and Piedmont, including: Stephen M.

He had a unique role as a member of the Emory University School of Medicine faculty based at Piedmont Hospital. He was a prolific author and well-known medical historian; a respected teacher and beloved mentor; a passionate supporter of medical, patient and community education; and a generous philanthropist. He was inspired by the late William Osler, M.D., who wrote, “No bubble is so iridescent or floats longer than that blown by the successful teacher.” That was Dr. Mark Silverman.
