The Cygnet Stockport services aim to provide social care services for children and young adults which are focussed on supporting individuals in achieving optimised outcomes, utilising a personalised and structured pathway approach. Based in Heaton Moor, Stockport, Cygnet Stockport offers a range of tailored social care services, specifically;  

A Short Break residential service for children aged 5 – 17 years. A residential transition service for young adults.

Young Adults Residential Transition Service Cygnet Stockport offers residential social care/support for young adults as part of a pathway into supported or independent future living. This service forms part of the pathway of support services at Cygnet Stockport which includes short break services for children and young people and ultimately Cygnet Supported Living (CSL). The aim of the transition service is to provide a pathway service for young people with complex and challenging behaviours who may have emotional and behavioural difficulties, autistic spectrum conditions, learning disability or emerging risk behaviours. The service provides a step-forward pathway for young people moving forward into transition from children’s services, their family home and educational placements; it further provides a step-down service from acute psychiatric services for those who require on-going social care and support, particularly within the development of social and living skills to work towards independent living. The service supports people in developing the skills required to live as independently as possible. This includes;     

Provision of in house ASDAN CoPE qualifications to support the development of independent living skills. Close working with local colleges. Supporting young people in working towards voluntary work and employment. Additional sessional support as required in occupational therapy, psychology and speech and language therapy. Measuring progress and delivery of outcomes using the Spectrum Star®.

Lengths of stays are planned in association with care coordinators, support teams and families, with a view to ultimately transitioning into supported or independent living placements. Referral and Assessment into our residential services We offer a flexible and quick response to all referrals. Following an initial enquiry and discussion with the referrer, we review documentation such as previous reports about the


prospective service user, or notes from their current care workers, depending on where they are currently placed. This is to give us an insight into their history, as well as their current presentation and ongoing recommendations from professionals. If we accept the individual, we need to be sure that we can continue the care pathway which, in some instances, they may have been working towards for some time. We will then arrange to meet with the referred individual and/or their family as part of a pre-service assessment. This is the beginning of a care/support planning process which identifies key aims and objectives of placement, based on the individual’s needs and aspirations, as well as ensuring that the prospective service user would benefit from our service and that their care needs fall within the parameters of our provision. The assessment is also undertaken in order to gather information relating to known risks which may impact on the individual’s wellbeing, their likes and dislikes, current health and social issues, cultural needs, the needs of the family/family dynamics etc. With consent, we will also carry out pre-service assessments with all other relevant parties, such as educationalists, clinicians etc., in order to gain as much information as possible, so that we can plan our care and support in a structured, highly person-centred and informed way. We try to match the skills and knowledge of key-worker staff with the individual service user wherever possible. Following the assessment we complete a written personalised needs assessment document, which will include an initial care plan, activity plan and risk assessments. Why Choose Cygnet?
