Polaris career-technical training programs don’t end at the middle and high school levels. Polaris offers hundreds of Adult Education programs including G.E.D preparation, personal enrichment and job/career training programs designed to enhance adult students’ job skills and employment prospects. During the past eight years, Polaris expenditures have risen by less than two percent while revenues have declined by more than six percent. We have continued to live within our means and have worked hard to hold the line on spending while more than doubling our student enrollment. Cardiographic Technician (230 Hours) The Cardiographic Technician program at Polaris Career Center is designed to prepare students to perform non-invasive cardiac testing procedures such as electrocardiograms, Holter and telemetry monitoring, and stress testing. Cardiographic technicians work in hospitals and cardiology offices to assist physicians in the diagnosis and monitoring of heart disease. Students will receive 130 hours of classroom and lab time. In addition, students will have the opportunity to gain practical experience through a 100-hour unpaid externship in a local clinical setting. A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity in a non-smoking facility. Note: Externship hours must be completed during daytime working hours, Monday through Friday. Healthcare providers require immunizations for students participating in externships. The cost of the immunization(s) will be the student’s responsibility. Prior to the externship, students will be required to pass a background check. Upon completion, students will be eligible to take the Cardiographic Technician (CCT) exam through Cardiovascular Credentialing International. Prerequisite: Medical Terminology or concurrent enrollment. Proof of successful course completion (transcript or certificate of completion) is required. Adult Education Job & Career Training Programs
The Cosmetology program prepares students for a challenging career in this exciting, cutting-edge field. Gain hands-on experience in the Polaris Career Center New Beginnings Salon while obtaining the industry knowledge necessary for the State Board Cosmetology license. Early Child Care Education/ Child Development Associate (930 Hours)
Dental Assisting (900 Hours) A career as a dental assistant offers many employment opportunities and the field is growing according to the United States Bureau of Labor Statistics. As valuable members of the dental care team, assistants increase the efficiency of the dentist in the delivery of quality oral health care. This course will teach the skills needed to enter the dental assisting field. Skills taught include: • Introduction to dentistry/oral • Laboratory materials health and nutrition • Radiology • Infection control • Practice management • Head and neck anatomy/ • Computer technology physiology • Dental specialties • Patient care/dental charting/ • Employability skills psychology • Restorative materials/advanced chairside Dental Assisting students will have the opportunity to gain practical experience through a 250-hour, unpaid externship (additional days may be required for the externship). A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Students completing the program will be eligible to take the Certified Ohio Dental Assisting examination, and earn the CODA (Certified Ohio Dental Assistant) credential. Students will also complete the Ohio Dental Assistant Radiographer initial training course. This initial training course satisfies the requirements of the Ohio State Dental Board and allows the student to earn the Ohio Dental Assistant Radiographer license. The Early Child Care Education/Child Development Associate (CDA) is the most widely recognized credential in early child care education (ECE) and is a key stepping stone on the path of career advancement in ECE. The Polaris Career Center CDA program is designed to prepare students to successfully complete the requirements for the nationally recognized CDA credential. Employment opportunities include, Pre-K or Infant/Toddler teacher, teacher assistant, or family childcare provider. The CDA credential is based on a core set of competency standards which guide early care professionals as they work toward becoming qualified teachers of young children. CDA students will learn how to nurture the emotional, physical, intellectual, and social development of children. Students will also learn employability skills. The CDA student will gain knowledge of how to put the CDA Competency Standards into practice and an understanding of why those standards help children move with success from one developmental stage to another. With the new Step-Up-to-Quality requirements, earning a CDA credential will enhance a student’s job opportunities in the field. Learn the CDA Observation Method, R-O-R (Review, Observance & Reflect) and plan for the Verification Visit. Students will receive 450-hours of classroom training and have the opportunity to gain practical experience in a 480-hour unpaid required field experience/ externship at an area child care center. A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Prior to the field experience/externship, students will be required to have and show proof of a physical exam, complete and pass a background check, obtain necessary immunizations/titres, and a TB test. Note: Field experience hours will vary between 6:00 AM and 6:00 PM and will be determined by the child care center. Heating, Ventilating, Air Conditioning and Refrigeration (HVAC/R) (600 Hours) The highly trained technicians in the Heating, Ventilating, Air Conditioning and Refrigeration (HVAC/R) profession maintain the complex HVAC/R systems used in residential, light industrial, and commercial buildings. Trained technicians have job stability and high earning potential. The HVAC/R program at Polaris Career Center will help the student access this growing field. This program takes place in a comprehensive classroom and practical learning environment. Medical Assisting (946 Hours) There is no better time than the present to enter the healthcare field. The Medical Assisting program is a great way to gain access to this exciting profession. Recently, Centers for Medicare & Medicaid Services (CMS) have determined credentialed medical assistants can enter orders into patient medical records for the computerized provider order entry (CPOE) – a task previously limited to licensed healthcare professionals. Ultimately, medical assistant students who become credentialed can offer additional workflow efficiency and compliance benefits to their employers. The Medical Assisting curriculum prepares students to perform administrative, clinical and laboratory procedures for employment in a physician’s office, clinic, or hospital. The course will cover, but is not limited to, the following: • Medical terminology, anatomy • Health insurance claim forms and physiology • Adult/Child/Infant CPR/AED • Entry-level clinical skills • First aid/medical emergencies • Phlebotomy certification • Computer skills • Scheduling appointments • Psychology of human behavior • Records management/electronic • Employability skills medical records • OSHA/universal precautions/ • Medical law and ethics personal protective equipment • Entry-level ICD-9/CPT coding • HIPAA/standards and guidelines Medical Assisting students will have the opportunity to gain practical experience through a 160-hour unpaid externship which will take place during the program’s 946 hours. A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Students will be required to have a routine physical, drug screening and pass a criminal background check prior to externship. Some healthcare systems and physician practices may require proof of current immunizations for students participating in externships. Students must also complete the Hepatitis B series prior to participation in their externships. Adult Education Job & Career Training Programs
This 900-hour program is designed for those who wish to gain employment in medical billing, coding and health information management. Employment opportunities include physician practices, outpatient clinics, billing companies as well as a variety of other medical facilities. Students will develop a thorough knowledge of medical terminology, anatomy and physiology, diseases and conditions, electronic health records, practice management software, medical office administration practices, diagnostic and procedural coding, hospital billing, reimbursement and claim form procedures, employability skills and healthcare law and ethics. The program includes instructor lectures, knowledge based assessments, practical applications, and self-directed study. Graduates will be prepared for entry-level positions as medical administrative assistants, electronic health records specialists, and medical billing and coding specialists. Students will complete a 180-hour unpaid externship during daytime working hours towards the end of the program. A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Students will be eligible to take the National Healthcareer Association Certified Medical Billing and Coding Specialist (CBCS), Certified Medical Administrative Assistant (CMAA), and the Certified Electronic Health Records Specialist (CEHRS) certifications exams. Healthcare providers may require immunizations for students participating in externships. It is highly recommended the student have keyboarding skills with some experience in Microsoft Word & Excel. Natural Resources/Ranger/ Basic Peace Officer Academy (600 Hours) This program will adhere to all State of Ohio OPOTA/C standards. The Polaris Career Center Natural Resources/Ranger/Basic Peace Officer Academy prepares candidates for police and natural resource law enforcement officer positions and other careers that require peace officer training. This 600-hour program is conducted under the auspices of the Ohio Peace Officer Training Commission. The instruction will be provided by those who have experience and expertise in the field of law enforcement, natural resources, and parks and recreation. The Academy training includes: • Academy uniforms (except pants • First aid and shoes) • Patrol • Administration • Civil disorders • Legal • Investigation • Human resources • Physical conditioning • Firearms • Aerosol defense spray • Driving • Expandable baton • Subject control • Employability skills As an all-inclusive academy, the following are provided for use during the program: • Use of duty belt with accessories • Expandable baton (holster, magazine pouch, • Chemical repellent spray handcuff case etc.) (OC/Pepper Spray) • Handgun • Taser • Shotgun Applicants must be 21 years old by the completion of the program, have a valid driver’s license, valid automobile insurance, and must complete and pass a background check, pass the physical assessment test and physical examination. All state and federal guidelines dictating the carrying of a firearm and minimum physical fitness standards set by the Ohio Peace Officer Training Commission will be followed. The Pharmacy Technician program prepares students for pharmacy career opportunities in retail, hospital, and long-term care settings. Pharmacy Technicians assist licensed pharmacists to provide medication and other health care products. Pharmacy Technician students will have the opportunity to gain practical experience through an unpaid 96-hour externship. The program prepares students to take the Certified Pharmacy Technician (CPhT) Certification exam. Students will be trained to perform a variety of blood collection methods on adults using proper techniques and precautions including: vacuum collection devices, syringes, capillary skin puncture, and butterfly needles. Students will also learn employability skills. Both classroom instruction and clinical experience/externship during a 100-hour unpaid externship at a healthcare facility will provide an opportunity for the student to gain the skill and knowledge necessary to take the Certified Phlebotomy Technician (CPT) exam through the National Healthcareer Association. A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Note: Externship hours will be scheduled during the day and students will be placed as space is available. Prior to externship, students will be required to complete and pass a criminal background check and complete the three-part Hepatitis B vaccination series (at the student’s expense). Students must have the first of three Hepatitis B vaccinations prior to the first day of class. Drug testing and a flu shot may be required (at the student’s expense). FINANCIAL AID INFORMATION Students are responsible for submitting a Free Application for Federal Student Aid (FAFSA). It is highly recommended the student start the application process for all financial aid options well in advance of registering for a program. Federal financial aid including loans and grants will first be applied to student tuition accounts. Funds remaining above current charges will be refunded and mailed, in the form of a check, and sent to the student’s address of record within 2 weeks of receipt of the funds. All financial aid student funds are disbursed through the Treasurer’s office at Polaris Career Center. Providing the student meets satisfactory progress standards and has completed all necessary financial aid documentation, the financial aid student refunds are disbursed to the student as scheduled and stated in the Student Policies and Financial Aid Consumer Handbook found on the Polaris Career Center website www.polaris.edu. PELL GRANT
APPLYING FOR THE PELL GRANT:
• The maximum award for 2015-2016 is $5,775 for a 900+ clock hour program and $3,850 for a 600-clock hour program. • Award amounts are based on the student’s EFC (Expected Family Contribution), calculated from the student’s previous year’s income information submitted on the FAFSA (Free Application for Federal Student Aid). • Students are required to self-pay the balance of tuition not covered by the Pell Grant, and/or the student may apply for a student loan. • All Pell grant funds, when received, are applied directly to the student’s tuition balance. The Pell grant does not need to be repaid. • To remain eligible for Pell grant funds, the student must maintain a minimum of 90% attendance at all times, as well as maintain satisfactory academic progress (C average). • Pell grant funds are meant to assist students with tuition and will not cover tuition costs completely. Students may consider applying for a William D. Ford student loan to pay the additional tuition costs not covered by the Pell grant. It is suggested that a student wishing to complete a student loan application (www.studentloans.gov) do so at the same time they complete their Pell grant (FAFSA) application. A student loan is only activated after a student registers for a program and accepts the award.

The student will receive communication from an assigned servicing agency the U.S. Department of Education chooses to manage the loan repayments. • Subsidized Loans are available for students with demonstrated financial need, as determined by federal regulations. No interest is charged while the student is attending school. The maximum amount that may be borrowed is $3,500 (less agency processing fee). • Unsubsidized Loans are not based on financial need. Interest is charged beginning with the first disbursement. Unofficial Withdrawal – Withdrawal from a Job & Career Training Program is considered unofficial when a student does not attend, contact the instructor, or the Adult Education office for 10 consecutive business days. Tuition fees will continue to accrue until the 10th day at which time a student will be withdrawn automatically. A $100 cancellation fee will be charged for all Job & Career Training program withdrawals. Refunds are prorated and based on the hours the program has been held until withdrawal date. Books and material fees are non-refundable. If a student paid tuition by check or cash, the refund will be disbursed within 15 business days in the form of a Polaris Career Center check. If the student paid tuition using a credit card, the refund will be credited on the credit card initially charged. Students who officially withdraw or are unofficially withdrawn and have a balance due will be invoiced by Polaris Career Center. All balance due amounts must be paid-in-full by the end of the fiscal year (June 30th). Students may contact the Adult Education office if a payment plan is needed. If the balance is not paid-in-full by the end of the fiscal year (June 30th), the debt will be reported to the Ohio Attorney General’s office for collection. In the event a student does not qualify, or is unable to fulfill the program requirements to participate in the externship portion of a program, the student will be ineligible to complete the program and, therefore, the withdrawal policy will be applied.
Students who have not satisfied grade and attendance requirements or have not provided all required student loan documentation may not receive financial aid awards as listed per the schedule in the Adult Education Student Policies & Financial Aid Consumer Handbook.

• An interest-free tuition payment plan is offered by the Polaris Career Center Adult Education Department for Job & Career Training programs. Students may visit Polaris Career Center, Monday through Friday from 8:30 AM – 3:30 PM (except during the 2 week winter/holiday break) to complete a Tuition Agreement plan and register. Please note – the first payment will be due at time of registration. If the times stated above are not convenient, please contact, the Administrative Coordinator at 440-891-7653 to make arrangements for registration. • The cost of the program will be divided into payments (depending on the length of the class) with an initial payment due at the time of registration and the final payment scheduled for one month prior to the end of the program. • The student will have the option to give Polaris Career Center authorization to automatically make the scheduled payments using the credit card listed on their Tuition Agreement. If the student does not authorize this option, it is the student’s responsibility to pay the scheduled payments within the 5 day grace period to avoid a $50 late fee. • If any payment is not made within 5 days of the listed due date, a $50 late fee will automatically be added to the current monthly payment and the total current monthly payment will be charged automatically to the student’s VISA or MasterCard on file. • Monthly payments are due on the 1st of each month beginning with the month the program begins. • Any unpaid balances not paid by the end of the fiscal year will be referred to the Ohio Attorney General’s office for collection.

If a program is cancelled, all tuition agreements are null and void and full refunds are disbursed to the student within 15 business days. 1098-T Tax Forms All Job & Career Training program students will receive, via postal mail, a 1098-T form annually. Transfer of Students/Credits from Other Institutions Polaris Career Center does not grant advanced standing to students transferring from other institutions. If successful, you will receive a completion card which you will take to the driver’s license bureau, and after paying the necessary fee, a motorcycle endorsement will be added to your driver’s license. You do not have to take another riding test at the driver’s license bureau. Classes consist of 4 hours of classroom training, followed by approximately 12 hours of actual riding on a controlled course. Courses are offered on a variety of days and times, so make sure you know your class dates and times when registering. For the first class, bring your driver’s license along with either your motorcycle endorsement or a valid temporary riding permit, which you obtain by passing the written test for the permit at the driver’s license bureau. Otherwise, you will not be admitted into the class and your tuition will not be refunded. Also, bring an ink pen, and dress casually. Proper riding attire will be discussed on the first night of class. NOTE: All classes start promptly! You will NOT be admitted once the published class time begins, and your tuition will not be refunded!

Maintain Your Motorcycle (8 hrs.) Keep your bike safe, reliable, and enjoyable by learning how to maintain your motorcycle. This 8-hour course is a combination of theory and hands‑on instruction using Polaris Career Center’s motorcycles. Bring your owner’s manual to class. Valuable handout materials are provided. Going on to a Training Program or College?
This course is designed for experienced riders who want to improve both their mental and physical riding skills to lower their risks while riding. The course consists of training and practice on your motorcycle on a controlled course under the knowledgeable and helpful eyes of trained instructors. If you ride two-up, your passenger may also enroll in this course. You must bring: your motorcycle license, proof of current insurance, your street legal motorcycle in good operating condition with proof of ownership, and both rider and passenger must wear full protective clothing as defined on the Motorcycle Ohio website.

The Basic Rider Course-Returning Rider (BRC-RR) is a 9-hour course designed for riders over 18 years of age who have been riding with a temporary permit for longer than a year (over a thousand miles experience) and for experienced riders returning to riding with or without a motorcycle endorsement after several years of not riding. Motorcycles are provided for your use during the course, and if successful, you will receive a completion card which you will take to the driver’s license bureau to receive your endorsement on your license. Withdrawals and Refunds • Full refunds will be issued when a class is cancelled by the Adult Education Department, or fees may be applied to another class. • No refunds will be issued after the first class meeting. The Motorcycle Basic Rider, Basic Rider-RR Course, and Experienced Rider Course tuition fee is non-refundable. • A $10 cancellation fee will be withheld from all refunds except for Job & Career Training Programs which have a $100 non-refundable fee. • Books and supplies are non-refundable.
