Cardiographic Technician (230 Hours)

Schedule/Tuition: The Cardiographic Technician program at Polaris Fall 2014 Career Center is designed to prepare students to Monday & Wednesday, 6:00 PM – 10:00 PM perform non-invasive cardiac testing procedures September 8, 2014 – January 28, 2015 (approximate ending such as electrocardiograms, Holter and telemetry date) monitoring, and stress testing. Students will also learn employability skills. Cardiographic technicians work in Spring 2015 hospitals and cardiology offices to assist physicians in Monday & Wednesday, 6:00 PM – 10:00 PM the diagnosis and monitoring of heart disease. Dental Assisting (900 Hours) A career as a dental assistant offers many employment opportunities and the field is growing according to the United States Bureau of Labor Statistics. As valuable members of the dental care team, assistants increase the efficiency of the dentist in the delivery of quality oral health care. Dental Assisting students will have the opportunity to gain practical experience through a 250-hour, unpaid externship (additional days may be required for the externship). A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Students completing the program will be eligible to take the Certified Ohio Dental Assisting examination, and earn the credential CODA (Certified Ohio Dental Assistant). Students will also complete the Ohio Dental Assistant Radiographer initial training course. This initial training course satisfies the requirements of the Ohio State Dental Board and allows the student to earn the Ohio Dental Assistant Radiographer license.

Early Child Care Education/Child Development Associate (930 Hours)

Schedule/Tuition: The Early Child Care Education/Child Development Associate (CDA) is the most widely recognized Monday – Friday, 8:00 AM – 1:00 PM credential in early child care education (ECE) and is a key stepping stone on the path of career September 15, 2014 – June 23, 2015 (approximate advancement in ECE. Prior to the field experience/externship, students will be required to have and show proof of a physical exam, complete a background check, obtain necessary immunizations/titres, and a TB test. Note: Field experience hours will vary between 6:00 AM and 6:00 PM and will be determined by the child care center. Upon completion of the CDA program, the student will be eligible to apply for the CDA credential. Heating, Ventilating, Air Conditioning and Refrigeration (HVAC/R) (600 Hours) The highly trained technicians in the Heating, Ventilating, Air Conditioning and Refrigeration (HVAC/R) profession maintain the complex HVAC/R systems used in residential, light industrial, and commercial buildings. Trained technicians have job stability and high earning potential. The HVAC/R program at Polaris Career Center will help the student access this growing field. This program takes place in a comprehensive classroom and practical learning environment. All instructional materials, as well as the CFC Refrigerant Recovery/Recycling and ICE certification examination fees, are included. All students are given the opportunity to earn National Center for Construction Education & Research (NCCER) certification in Core Curriculum. There is no better time than the present to enter the healthcare field. The Medical Assisting program is a great way to gain access to this exciting profession. Recently, Centers for Medicare & Medicaid Services (CMS) have decided credentialed medical assistants can enter orders into patient medical records for the computerized provider order entry (CPOE) – a task previously limited to licensed healthcare professionals. This means that medical assistant students who become credentialed can offer additional workflow efficiency and compliance benefits to their employers. The Medical Assisting curriculum prepares students to perform administrative, clinical and laboratory procedures for employment in a physician’s office, clinic, or hospital. Adult Education Job Training Programs

Medical Assisting (946 Hours)

Medical Assisting students will have the opportunity to gain practical experience through a 160-hour unpaid externship which will take place during the program’s 946 hours. A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Students will be required to have a routine physical, and criminal background check prior to externship. Some healthcare systems and physician practices may require proof of current immunizations for students participating in externships. Students must also complete the Hepatitis B series prior to participation in their externships. The Polaris Career Center Medical Assisting Program is accredited by the Commission on Accreditation of Allied Health Education Programs (www.caahep.org) upon the recommendation of the Medical Assisting Education Review Board. Medical Billing & Coding (900 Hours) This 900-hour program is designed for those who wish to gain employment in medical billing, coding and health information management. Employment opportunities include physician practices, outpatient clinics, billing companies as well as a variety of other medical facilities. Students will develop a thorough knowledge of medical terminology, anatomy and physiology, diseases and conditions, electronic health records, practice management software, medical office administration practices, diagnostic and procedural coding, hospital billing, reimbursement and claim form procedures, employability skills and healthcare law and ethics. Graduates will be prepared for entry-level positions as medical administrative assistants, electronic health records specialists, and medical billing and coding specialists.

Students will complete a 180-hour unpaid externship during daytime working hours towards the end of the program. A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Students will be eligible to take the National HealthCareer Association Certified Medical Billing and Coding Specialist (CBCS), exam, Certified Medical Administrative Assistant (CMAA), and the Certified Electronic Health Records Specialist (CEHRS) exam. Natural Resources/Ranger/Basic Peace Officer Academy (600 Hours) The Polaris Career Center Natural Resources/Ranger/Basic Peace Officer Academy prepares candidates for police and natural resource law enforcement officer positions and other careers that require peace officer training. This 600-hour program is conducted under the auspices of the Ohio Peace Officer Training Commission. The instruction will be provided by those who have experience and expertise in the field of law enforcement, natural resources, and parks and recreation. The Academy Training includes: • Administration • Legal • Human resources • Firearms • Driving • Subject control • First aid

All state and federal guidelines dictating the carrying of a firearm and minimum physical fitness standards set by the Ohio Peace Officer Training Commission will be followed. Nurse Assistant (90 Hours) State Tested Nursing Assistants gain employment caring for diverse patient populations including the elderly in long-term care settings, assisted living facilities, home care environments and hospitals. The program is state-approved and consists of mandatory attendance for classroom/clinical hours. The STNA exam is included in the tuition and is offered to students after successful completion of the program. Medicare approved Home Health Aide certification and American Heart Association Basic Life Support (CPR) certification is also included. The 2-step Mantoux TB test is required before class begins. Immunizations may be required and the cost is the responsibility of the student. Employers/clinical sites may require a background check. FINANCIAL AID INFORMATION Students are responsible for submitting a Free Application for Federal Student Aid (FAFSA). It is highly recommended that the student start the application process well in advance to registering for a program. When student loan disbursements are received from the federal government, the student’s tuition is credited the disbursed amount. If a student has borrowed funds in excess of the tuition amount, remaining funds are refunded in the form of a check and sent to the student’s address of record. All financial aid student loan funds are disbursed through the Treasurer’s office at Polaris Career Center. Providing the student meets

PELL GRANT OVERVIEW • The maximum award for 2014-2015 is $5,730 for a 900+ clock hour program and $3,820 for a 600-clock hour program. • Award amounts are based on the student’s EFC (Expected Family Contribution), calculated from the previous year’s income information submitted on the FAFSA (Free Application for Federal Student Aid). • Pell Grant awards are applied to the student’s tuition account when registering. • Students are required to self-pay the balance of the tuition not covered by the Pell Grant or William D. Ford Student Loan. • All Pell Grant funds are applied directly to tuition. The Pell Grant does not have to be repaid. • To remain eligible for Pell Grant funds, a full-time student must maintain satisfactory academic progress (C average), in addition to a minimum of 90% attendance at all times. Students not in compliance will be responsible for paying the portion of financial aid expected to be received.

APPLYING FOR THE PELL GRANT: Please follow these steps to apply for financial aid: Step 1 – Collect items that will help in completing the FAFSA application, for example: tax returns, bank statements, welfare, assets, child support etc. Step 2 – Go to www.pin.ed.gov and apply for a PIN number. If you are a dependent student, a parent must also apply for a PIN #. You may create your own PIN number and use it the same day to sign your FAFSA. Step 3 – Complete a 2014-2015 FAFSA form on-line at www.fafsa.gov. Registration and Financial Aid Information

Step 4 – Carefully read the information provided regarding the loan process, subsidized and unsubsidized loans, processing fees, interest rates, and repayment. Step 5 – Complete “Entrance Counseling”. This step is dependent upon the loan being disbursed. Step 6 – Complete and submit the “Master Promissory Note” (MPN). This step is also dependent upon the loan being disbursed. Step 7 – Contact Pat Carter (440-891-7668), Financial Aid Coordinator, 3-5 days after the MPN has been submitted, to confirm loan approval and to certify the amount to be borrowed. Step 8 – Once the student has confirmed his/her Pell Grant and/or student loan amount, the student may register at Polaris Career Center in-person. All financial aid recipients are required to register at Polaris Career Center, Adult Education Office, by completing a Tuition Agreement. A valid MasterCard or Visa must be presented at the time of registration and will be kept on file for collateral purposes. Step 9 – Complete “Exit Counseling” before the last loan disbursement. (This step is also dependent upon loan funds being disbursed). Federal student loan funds will be applied first toward outstanding tuition costs for the current loan disbursement period. Once student accounts are made current with loan funds, remaining loan monies due to students will be mailed to the student’s address of record in the form of a check within 2 weeks of Polaris Career Center receiving the funds. The Pell Grant and William D. Ford Federal Student Loans are each disbursed in two separate payments. One half of each award will be disbursed in the Fall and one half will be disbursed in the Spring, depending on the class start date. Polaris Career Center adheres to the policies and procedures established by the U.S. Department of Education, the Auditor of the State of Ohio, and the Treasurer’s Office for all disbursements.

TUITION PAYMENT PLAN • An interest-free tuition payment plan is offered by the Adult Education Department for Job Training Programs. Students may visit Polaris Career Center, Monday thru Friday from 8:30AM – 3:30PM to complete a payment plan and register. Please note – the first payment will be due at time of registration. If the times stated above are not convenient, please contact, Mary Koeka, Administrative Coordinator 440-891-7653, to make arrangements for registration. • The cost of the program will be divided into payments (depending on the length of the class) with an initial payment due at the time of registration and the final payment scheduled for one month prior to the end of the program. • A valid MasterCard or VISA must be kept on file when the Tuition Payment Plan arrangements are made with the Adult Education Office. • Polaris Career Center offers, at no additional cost, an automatic payment option for payments to be deducted on the student’s MasterCard or VISA on payment due dates. • Monthly payments are due the 1st of each month beginning with the month the program begins, except for the following: Nurse Assistant – second payment is due one week after class begins.

Program Cancellation The Adult Education department must cancel programs that do not attain the required minimum enrollment. If a program is cancelled, all tuition agreements are null and void and full refunds are disbursed to the student within 15 business days.

1098-T Tax Forms All job training students will receive via mail a 1098-T form annually.

Transfer of Students/Credits from Other Institutions Polaris Career Center does not grant advanced standing to students transferring from other institutions.

Transfer of Students Between Programs Prior to the program start date, registered students may not transfer to another program. Students must withdraw from their current program and re-register for another program. The $100 cancellation fee policy will be in effect and applied.

Withdrawal/Refunds Policy Official Withdrawal – Withdrawal from a Job Training Program is considered official on the date notification is received in the adult education office from the student. Tuition fees will continue to accrue until notification is received from the student. A $100 cancellation fee will be charged for all job training program withdrawals. Refunds are prorated and based on the hours the program was held until withdrawal notification date. Books and material fees are non-refundable. If a student paid tuition by check or cash, the refund will be disbursed within 15 business days in the form of a Polaris Career Center check. If the student paid tuition using a credit card, the refund will be credited on the credit card initially charged. Unofficial Withdrawal - Withdrawal from a Job Training Program is considered unofficial when a student does not attend, contact the instructor, or the Adult Education office for 10 consecutive days. Tuition fees will continue to accrue until the 10th day at which time a student will be withdrawn automatically. A $100 cancellation fee will be charged for all job training program withdrawals. Refunds are prorated and based on the hours the program has been held until withdrawal date. Books and material fees are non-refundable. If a student paid tuition by check or cash, the refund will be disbursed within 15 business days in the form of a Polaris Career Center check. If the student paid tuition using a credit card, the refund will be credited on the credit card initially charged. TALES FROM THE MORTUARY - A VICTORIAN WAKE At the Warehouse on the Canal in Canal Fulton, Ohio— HISTORICALLY HYSTERICAL (5 hrs.) Step back to an earlier time in America where superstitions about Ghosts ran rampant and to die unprepared or not be mourned properly was the greatest tragedy. You are invited (it was considered a great insult to ignore an invitation to a funeral) to be a mourner and to pay your respects to the dearly departed, SILAS H. ADKINNS. Remember that this is a “full” or “deep” mourning where it is suggested that everyone should wear black (it was believed that a Ghost could not see those dressed in black) where the fabric is to be dull without any sheen to reflect light and no jewelry is allowed and never wear anything new, especially shoes. The veil that can be worn is designed, “to protect family and friends from having the spirits of the deceased hover around relatives and then attach itself should the spirit look into your eyes.” You will participate in the WAKE; provide a testimony to the deceased; learn about the customs, oddities, and superstitions about Ghosts and see authentic items from the Victorian era. Because SILAS H. ADKINNS was from a small family and in order to properly mourn with an adequate number of people, professional mourners (like you) called “mutes” were used in the funeral procession (practice your wailing and crying). When the funeral is over as mourners you will receive traditional gifts as keepsakes and favors. This is a totally customer interactive event where the customs and superstitions in earlier America were so crazy and bizarre that we call it, “Historically Hysterical” where it is difficult to keep a straight face. Note: we will ask for some volunteers of 4-5 people to be the grieving family of Silas that everyone will pay their respects too. Basic Rider Course - Beginning Rider (BRC) (Approximately 16 hrs.) This class is your first step in learning to ride a motorcycle. It is intended for the true beginner who has never ridden a motorcycle and for the experienced rider looking to refine their skills and obtain a motorcycle endorsement. If successful, you will receive a completion card which you will take to the driver’s license bureau, and after paying the necessary fee, a motorcycle endorsement will be added to your driver’s license. You do not have to take another riding test at the driver’s license bureau. Classes consist of 4 hours of classroom training, followed by approximately 12 hours of actual riding on a controlled course. Courses are offered on a variety of days and times, so make sure you know your class dates and times when registering. For the first class, bring your driver’s license along with either your motorcycle endorsement or a valid temporary riding permit, which you obtain by passing the written test for the permit at the driver’s license bureau. Otherwise, you will not be admitted into the class and your tuition will not be refunded. Also, bring an ink pen, and dress casually. Canceled Courses The Adult Education Department must cancel courses that do not attain the required minimum enrollment. The Adult Education Department reserves the right to cancel, discontinue, postpone, or combine courses before or at the first class meeting. Senior Citizens Discount Senior citizens will receive a 10 percent discount on the published tuition fee (discount does not apply to material fees) except where indicated. Note: Senior Citizens include persons over the age of 65 who can produce proof of age and identity.

School Closing Closing of the daytime classes at Polaris Career Center DOES NOT necessarily mean that the evening classes will be cancelled. The decision to cancel evening classes will be made by the Adult Education administration by 3:00 PM. Students should listen to the radio or TV or call the school at 440-8917600 for further information.

Berea and Strongsville School Closing When the Berea and Strongsville City School District’s classes are cancelled or dismissed due to inclement weather, evening classes will not be held regardless of weather changes. Classes missed will be added at the end of the session.
