Cardiographic Technician  (230 Hours)

Schedule/Tuition: The Cardiographic Technician program at Polaris Fall 2013 Career Center is designed to prepare students to Monday & Wednesday, 6:00 PM – 10:00 PM perform non-invasive cardiac testing procedures September 9, 2013 – January 29, 2014 (approximate ending date) such as electrocardiograms, Holter and telemetry Spring 2014 monitoring, and stress testing. Medical Terminology/Introduction to Anatomy & Physiology  (48 hrs.) Your success in the healthcare field requires a working knowledge of the language and medical terminology. Basic information about the structures and functions of each body system is presented to aid in mastering the meaning and spelling of thousands of medical terms and abbreviations.

Consider the tremendous career opportunities for students of cosmetology. Let the Polaris Career Center Cosmetology Academy prepare you for a challenging career in this exciting, cutting-edge field. Gain hands-on experience in the Polaris Career Center New Beginnings Salon while obtaining the industry knowledge necessary for your Cosmetology license. Acquire skills commonly used in cosmetology in the following areas: • Dispensary operations • Sanitation • Reception duties • Salon management Emphasis will be placed on the hands-on practice of: • Scalp care and hair treatments • Shaping and cutting hair • Styling hair • Permanent waving • Hair relaxing and coloring • Hair highlighting and toning • Manicuring and pedicuring • Facial treatments • Wig and hairpiece care

Adult Education Job Training Programs

Cosmetology Academy  (1500 Hours)

Since the Cosmetology Academy is a 2-year program, qualified students will have access to two years of Pell Grant funding consisting of one 900-hour award and one 600-hour award. Call the Financial Aid Coordinator at (440) 891-7668 for additional information.

Dental Assisting  (900 Hours)

FINANCIAL AID ELIGIBILE PROGRAM – for those who qualify A career as a dental assistant offers many opportunities and the field is growing faster than average occupations according to the United States Bureau of Labor Statistics. As valuable members of the dental care team, assistants greatly increase the efficiency of the dentist in the delivery of quality oral health care. In this course you’ll learn all the skills you need for entry-level employment. Skills taught include: • Introduction to dentistry/oral health and nutrition • Infection control • Head and neck anatomy/physiology • Patient care/dental charting/psychology • Restorative materials/advanced chairside

• Laboratory materials • Radiology • Practice management • Computer technology • Dental specialties

Dental Assisting students will have the opportunity to gain practical experience through a 250-hour, unpaid externship (additional days may be required for the externship). Students will be required to have a routine physical, a criminal background check, and purchase their own uniforms. Immunizations will be required prior to participation in the clinical portion of the program. Students who complete the program will be eligible to take the Certified Ohio Dental Assisting Examination, and earn the credential CODA (Certified Ohio Dental Assistant). Students will also complete the Ohio Dental Assistant Radiographer initial training course. This initial training course satisfies the requirements of the Ohio State Dental Board and allows the student to earn the Ohio Dental Assistant Radiographer certification.

Adult Education Job Training Programs

Early Child Care Education/Child Development Associate  (930 Hours) FINANCIAL AID ELIGIBILE PROGRAM – for those who qualify

Schedule/Tuition: Monday- Friday, 8:00 AM - 1:00 PM September 30, 2013 – July 14, 2014 (approximate ending date) Tuition Fee: $6,995 Tuition includes textbooks, supplies, Infant/ Child CPR, Communicable Disease, First Aid and Child Abuse Recognition training and certification fees, and CDA application fee. The Early Child Care Education/Child Development Associate (CDA) is the most widely recognized credential in early child care education (ECE) and is a key stepping stone on the path of career advancement in ECE. At Polaris Career Center, the CDA program is designed to prepare students to successfully complete the requirements for the nationally recognized CDA credential. The CDA credential is based on a core set of competency standards, which guide early care professionals as they work toward becoming qualified teachers of young children. CDA students will learn how to nurture the emotional, physical, intellectual, and social development of children. The CDA student will gain knowledge of how to put the CDA Competency Standards into practice and an understanding of why those standards help children move with success from one developmental stage to another. Completing the CDA program will assist you in meeting current job requirements and advancing your career and salary. The CDA will also increase your knowledge of early childhood education and understand developmentally appropriate practice. In turn, you will increase your confidence and reinforce your commitment to early childhood education. Students will receive 450 hours of classroom training and have the opportunity to gain practical experience in a 480 hour unpaid required field experience student teaching at an area child care center. Note: field experience hours will vary between 6 A.M. and 6 P.M. and will be determined by the child care center offering the student the field experience. Upon completion of the CDA program, the student will be eligible to apply for the CDA credential. A routine physical and a criminal background check are required prior to student participation in the field experience. Immunizations may be required and the cost of these immunizations will be the students’ responsibility. Learn the CDA Observation Method, R-O-R (Review, Observe, & Reflect) and plan for the verification visit.

Graphic & Web Design Certification  (600 Hours)

FINANCIAL AID ELIGIBILE PROGRAM – for those who qualify

Graphic Design – 300 Hours A 300-hour program is designed to prepare the student to perform specific functions in the graphic design/production field. This will be achieved by studying the basics of design, typography, color theory and basic layout; then applying them in the computer disciplines: Adobe InDesign CS6 (page layout); PhotoShop CS6 (photo correction, retouching, etc.) and Illustrator CS6 (creating and modifying vector images). Advanced classes in electronic page layout and print production will round out preparation for jobs in the field. Skills developed include, but are not limited to: Illustrator CS6 PhotoShop CS6 Extended InDesign CS6 • Drawing vector images • Editing and retouching • Principles of page layout photos • Re-drawing bitmap images • Working with text and • Using layers, type and graphics • Using gradients, blends effects and patterns • Color management • Photo Illustration and • Using type and layers • Working with tabs and restoration tables • Saving and exporting • Using images in other images • Preflight and preparation programs for printing

Web Design – 300 Hours A 300-hour course created to prepare the student for the web design field. This will be achieved by studying the basics of: HTML, Cascading Style Sheets (CSS), Content Management Systems, Social Media Networks, Flash animation and more, then application with Adobe Flash CS6 and Dreamweaver CS6. Design skills used from the Graphic Design section will be required before beginning the Web Design section.

Schedule/Tuition Monday - Thursday - 6:00 PM -10:00 PM September 16, 2013 – July 30, 2014 (approximate ending date) Tuition Fee $5,995 Tuition includes textbooks and one Adobe Certification examination fee. Gainful employment information is available at www.polaris.edu/adult-education/programs. *Adobe offers Associate Certification, nationally recognized for the above mentioned Adobe programs. The fee included in the tuition is for one exam. An introduction to the MacIntosh class will be included for those unfamiliar with the MacIntosh OS X v10.7 operating system. Prerequisite: General computer knowledge; keyboarding skills helpful.

The highly trained technicians in the Heating, Ventilating, Air Conditioning and Refrigeration (HVAC/R) profession maintain the complex HVAC/R systems used in residential, light industrial, and commercial buildings. Trained technicians have job stability and high earning potential. The HVAC/R program at Polaris Career Center will help you access this growing field. This program takes place in a comprehensive classroom and practical learning environment. The course includes: • Introduction to Construction and Maintenance (Core Curriculum) • HVAC/R safety related issues • Heating, Ventilating, Air Conditioning & Refrigeration (HVAC/R) • Heating & air conditioning installation procedures • Understanding schematics • Airside systems • Proper venting • CFC Refrigerant Recovery/ Recycling Certification • ICE Certification

All instructional materials, as well as the CFC Refrigerant Recovery/ Recycling Certification and ICE test fees, are included. All students are given the opportunity to earn National Center for Construction Education & Research (NCCER) Certification in Core Curriculum. Students are required to purchase safety glasses.

Medical Assisting  (946 Hours)

FINANCIAL AID ELIGIBILE PROGRAM – for those who qualify There is no better time than the present to enter the healthcare field. The Medical Assisting program is a great way to gain access to this exciting profession. The Medical Assisting curriculum prepares you to perform administrative, clinical and laboratory procedures for employment in a physician’s office, clinic, or hospital. The course will cover, but is not limited to, the following: • Medical terminology, anatomy and physiology • Entry-level clinical skills • Phlebotomy certification • Scheduling appointments • Records management/electronic medical records • Medical law and ethics • Entry-level ICD-9/CPT coding • Health insurance claim forms • AED/adult/child/infant CPR • First aid/medical emergencies • Computer skills • Psychology of human behavior • Employability skills • OSHA/universal precautions • HIPAA/standards and guidelines Medical Assisting students will have the opportunity to gain practical experience through a 160-200 hour unpaid externship. Students will be required to have a routine physical, and criminal background check. Some healthcare systems and physician practices may require proof of current immunizations for students participating in externships. Students must also complete the Hepatitis B series prior to participation in their externships. The cost of the physical and immunizations will be the responsibility of the student. Heating, Ventilating, Air Conditioning and Refrigeration (HVAC/R)  (600 Hours)

Accreditation of Allied Health Education Program, 1361 Park Street, Clearwater FL 33756, 727-210-2350. Upon completion of the program, students are eligible to sit for the Certified Medical Assistant (CMA) certification examination and the International Academy of Phlebotomy Sciences (IAPS) certification examination. Advanced Placement Advanced placement will only be allowed as transfer credit from an accredited institution and will be considered on a case-by-case basis. This must be arranged prior to enrollment. Medical Billing & Coding (with electronic health records)  (900 Hours) FINANCIAL AID ELIGIBILE PROGRAM – for those who qualify

Schedule/Tuition: Fall 2013 Monday – Thursday, 9:00 AM– 3:00 PM September 16, 2013 – June 24, 2014 (approximate ending date) Spring 2014 Monday – Thursday, 5:30 PM – 10:30 PM February 10, 2014 – December 18, 2014 (approximate ending date) Tuition Fee: $8,495 Tuition includes textbooks and examination fees. This 900-hour program is designed for those who wish to gain employment in medical billing and health information management in physician practices, outpatient clinics, billing companies as well as a variety of other medical facilities. Students will develop a thorough knowledge of medical terminology, anatomy and physiology, electronic medical records, practice management software, Microsoft Office, medical office administration procedures, medical procedural coding, medical diagnostic coding, hospital billing, and reimbursement procedures. Graduates will be prepared for entry-level positions as medical records technicians, or billing and coding specialists. Students will be required to complete a 180-hour unpaid externship during daytime working hours at the end of the program. Natural Resources/Ranger/Basic Peace Officer Academy  (600 Hours)

State Tested Nursing Assistants gain employment caring for diverse patient populations including the elderly in long-term care settings, assisted living facilities, home care environments and hospitals. Our program is state-approved and consists of mandatory attendance for classroom/clinical hours. The STNA exam is included in the fee and is offered to our students after successful completion of the program. Medicare approved Home Health Aide Certification and American Heart Association Basic Life Support (CPR) certification is also included. The 2-step Mantoux TB test is required before class begins. Phlebotomy Certification  (252 Hours)

Students will be trained to perform a variety of blood collection methods using proper techniques and precautions including: vacuum collection devices, syringes, capillary skin puncture, and butterfly needles on adults. Both classroom instruction and clinical experience during a 100-hour externship at a healthcare facility will provide an opportunity for the student to gain the skill and knowledge necessary to take the Certified Phlebotomy Technician (CPT) exam through the National Healthcareer Association (exam fee included). Note: Clinical hours will be scheduled during the day, and students will be placed in clinical assignments as space is available. Students will be required to purchase their own uniforms and have a criminal background check. Healthcare providers and labs require immunizations and may require blood titres (at the student’s expense) for students participating in externships. Students will be required to complete the three-part Hepatitis B vaccination series (at the student’s expense) prior to starting their clinical hours. Welding  (600 Hours)

• Pell Grant awards are applied to the student’s tuition account when enrolling in a full-time job training program meeting 600, 900, 930, 946, and 1500 clock hours. • Students are required to self-pay the balance of the tuition not covered by the Pell Grant or other type of financial aid. A payment plan is available (see page 10).

• All Pell Grant funds are applied directly to tuition. The Pell Grant does not have to be repaid.

A variety of financial aid options may exist to cover all or part of your training tuition. Applying for a Pell Grant: Step 1 – Collect items that will help in completing the FAFSA application, for example: tax returns, bank statements, welfare, assets, child support etc. Step 2 – Go to www.pin.ed.gov and apply for a PIN number. If you are a dependent student, a parent must also apply for a PIN #. You may create your own PIN number and use it the same day to sign your FAFSA. Step 3 – Complete a FAFSA application online: www.fafsa.ed.gov/. The Polaris Career Center school code is: 016745 Step 4 – If you provided an email address on your application, you will receive within 3 – 5 days a SAR (Student Aid Report) from the U.S. Department of Education. If you did not provide an email address, the SAR will arrive by mail within 3 weeks. If you would like to check the status of your FAFSA, go to www.fafsa.gov. Step 5 – Once you have received your SAR report, contact: Pat Carter, Financial Aid Coordinator, at 440-891-7668 Your financial aid must be confirmed with Pat Carter before registration can be initiated. William D. Ford Student Loan • Student loans must be repaid. Repayment starts six months after completing school. • Subsidized Loans are available for students with demonstrated financial need, as determined by federal regulations. No interest is charged while you are in school. The maximum amount that can be borrowed is $3,500 (less the processing fee). • Unsubsidized Loans are not based on financial need. Interest is charged beginning with the first disbursement. The maximum amount that can be borrowed is $6,000 (less the processing fee). For further information visit www.direct.ed.gov

Step 8 - Once you have confirmed your Pell Grant and/or student loan amount, you may register at Polaris Career Center in person. All financial aid student recipients are required to register at Polaris Career Center, Adult Education Office, by completing a Tuition Agreement. A valid MasterCard or Visa must be presented at the time of registration and will be kept on file for collateral purposes. Step 9 - Complete “Exit Counseling” before the last loan disbursement. Federal student loan funds will be applied first toward outstanding tuition costs for the current loan disbursement period. Once student accounts are made current with loan funds, remaining loan monies due to students will be mailed within 10 business days in the form of a check to the student’s address of record. The Pell Grant and William D. Ford Federal Student Loans are each disbursed in two separate payments. One half of each award will be disbursed in the Fall and one half will be disbursed in the Spring, depending on the class start date. Ghost Hunt at the Mansfield Reformatory  (6.5 hours) Here is your chance to do your own investigation of one of the most haunted buildings in America. The Ohio State Reformatory (OSR) in Mansfield is claimed to be one of the top haunts in America. It has been featured on TV’s “101 Things To Do Before You Die” on the Travel Channel. This is truly an opportunity not to be missed! Joining us on this ghost hunt will be an expert on the facility, and an experienced ghost hunter. He will give us a personal tour of the prison, pointing out details, and sharing his personal stories about the facility. After our tour, we will then start our own investigations. You are encouraged to bring any “tools of the trade” that you wish to work with: camera (digital or disposable), video camera, tape recorder (with extra tape and batteries), flashlight (with fresh batteries). We will see if we can photograph anything unusual (things not seen by the naked eye - for example: mists, streaks of light, orbs, faces, or body images). We can see if we can tape record sounds, voices, words that cannot be heard by the human ear. This building contains steep steps and uneven surfaces. Proper footwear is necessary for your safety. No heels or “flip-flop” shoes. Bring a warm jacket/ gloves. Motorcycle Safety

MOTORCYCLE SAFETY Polaris Career Center offers Motorcycle Safety Courses for Novice through Experienced Riders. PLEASE NOTE: The tuition for state sponsored classes has been raised to $50. ALL REGISTRATIONS FOR BRC, BRC-2 (ERC) AND BRC-RR ARE ONLINE REGISTRATIONS ONLY at http://motorcycle.ohio.gov

Basic Rider Course - Beginning Rider (BRC)  (Approximately 16 hrs.) This class is your first step in learning to ride a motorcycle. It is intended for the true beginner who has never ridden a motorcycle and for the experienced rider looking to refine their skills and obtain a motorcycle endorsement. If successful, you will receive a completion card which you will take to the driver’s license bureau, and after paying the necessary fee, a motorcycle endorsement will be added to your driver’s license. You do not have to take another riding test at the driver’s license bureau. Classes consist of 4 hours of classroom training, followed by approximately 12 hours of actual riding on a controlled course. Courses are offered on a variety of days and times, so make sure you know your class dates and times when registering. For the first class, bring your driver’s license along with either your motorcycle endorsement or a valid temporary riding permit, which you obtain by passing the written test for the permit at the driver’s license bureau. Otherwise, you will not be admitted into the class and your tuition will not be refunded. Also, bring an ink pen, and dress casually. Proper riding attire will be discussed on the first night of class. NOTE: All classes start promptly! If you ride two-up, your passenger may also enroll in this course. You must bring: your motorcycle license, proof of current insurance, your street legal motorcycle in good operating condition with proof of ownership, and both rider and passenger must wear full protective clothing as defined in the next column. GED & Basic Skills Classes If you need to get your GED so you can move ahead, make Polaris ABLE classes your first stop. In our small, friendly classes, you move at your own pace, with teachers who understand the needs of adult learners. You are in charge here! Canceled Courses The Adult Education Department must cancel courses that do not attain the required minimum enrollment. The Adult Education Department reserves the right to cancel, discontinue, postpone, or combine courses before or at the first class meeting. Attempts are made to contact all students whose courses have been canceled. In the event a class is canceled, a full refund of the registration fee will be given.

Berea and Strongsville School Closing When the Berea and Strongsville City School District’s classes are cancelled or dismissed due to inclement weather, evening classes will not be held regardless of weather changes. Classes missed will be added at the end of the session.

Withdrawals and Refunds • Full refunds will be issued when a class is canceled by the Adult Education Department, or fees may be applied to another class. • No refunds will be issued after the first class meeting. The Motorcycle Basic Rider, Basic Rider-RR Course, and Experienced Rider Course tuition fee is non-refundable. • A $10 cancellation fee will be withheld from all refunds except for Job Training Programs which have a $100 non-refundable fee. • Books and supplies are non-refundable.
