Honoring the Past…Building the Future! As we mark our 40th anniversary, we celebrate our past while looking forward to an exciting future! • The need and interest for Polaris programs has increased. We are serving more students than ever before with nearly 1,800 high school students and over 8,000 adult students. • We are making an impact on the local economy with a record number of students earning industry credentials and college credits; over 70% of our high school students are pursuing college degrees. • Property values have declined which has directly impacted the ability to renovate our teaching labs that are nearly 40 years old. • Over $30 million in facility infrastructure repairs have been identified by the Ohio Schools Construction Commission that need to be completed to bring Polaris up to state minimum standards. Our aging facilities and labs make it difficult to meet employer demands and to attract students to productive careers. The following is a partial list of upgrades that will help us meet state facility standards and mandates: $12.5 million to repair our HVAC, roofing, electrical, and plumbing systems $6.6 million to repair our walls, lighting, security and handicapped access $1.4 million to improve technology $6 million to create state-mandated middle school programs For the first time in 31 years, Polaris will be on the May, 2015 ballot seeking passage of a 0.67 mill permanent improvement levy. This millage represents $1.95 per month for every $100,000 of appraised home valuation. The additional revenue will enable us to bring our facility up to state minimum standards while upgrading labs, classrooms and equipment. Cardiographic Technician (230 Hours) The Cardiographic Technician program at Polaris Career Center is designed to prepare students to perform non-invasive cardiac testing procedures such as electrocardiograms, Holter and telemetry monitoring, and stress testing. Cardiographic technicians work in hospitals and cardiology offices to assist physicians in the diagnosis and monitoring of heart disease. Students will receive 130 hours of classroom and lab time. In addition, students will have the opportunity to gain practical experience through a 100-hour unpaid externship in a local clinical setting. A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity in a non-smoking facility. Note: Externship hours must be completed during daytime working hours, Monday through Friday. Healthcare providers require immunizations for students participating in externships. The cost of the immunization(s) will be the student’s responsibility. Prior to the externship, students will be required to have a background check. Upon completion, students will be eligible to take the Cardiographic Technician (CCT) exam through Cardiovascular Credentialing International. Prerequisite: Medical Terminology course or concurrent enrollment in a Medical Terminology course. Proof of registration or successful course completion (transcript or certificate of completion) is required. Polaris Career Center offers the Medical Terminology course, (see page 6). Medical Terminology test-out is available.

Cosmetology Academy (1500 Hours) FINANCIAL AID ELIGIBLE PROGRAM

Starting Fall 2015

The Cosmetology program prepares students for a challenging career in this exciting, cutting-edge field. Gain hands-on experience in the Polaris Career Center New Beginnings Salon while obtaining the industry knowledge necessary for the State Board Cosmetology license. Acquire skills commonly used in cosmetology in the following areas; dispensary operations, sanitation, reception duties, salon management, emphasis will be placed on the handson practice of: scalp care and hair treatments, shaping and cutting hair, styling hair, permanent waving, hair relaxing and coloring, hair highlighting and toning, manicuring and pedicuring, facial treatments, wig and hairpiece care. Career options include (State license required): Cosmetologist, Color technician, Perm specialist, Hair cutting specialist, Esthetician, Nail technician, Platform artist, and Equipment sales. Qualified students will have access to two years of Pell Grant funding consisting of one 900-hour award and one 600-hour award.

Test results and a step-by-step plan will be provided at a follow-up conference. (The cost of this testing will be deducted from tuition when a student registers for a Polaris Job Training program.) Dental Assisting (900 Hours) FINANCIAL AID ELIGIBLE PROGRAM

A career as a dental assistant offers many opportunities and the field is growing faster than average occupations according to the United States Bureau of Labor Statistics. In this course you’ll learn all the skills you need for entry-level employment. Skills taught include: introduction to dentistry/oral health and nutrition, infection control, head and neck anatomy/ physiology, patient care/dental charting/psychology, restorative materials/advanced chairside, laboratory materials, radiology, practice management, computer technology, and dental specialties. Dental Assisting students will have the opportunity to gain practical experience through a 250-hour, unpaid externship (additional days may be required for the externship). A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Students completing the program will be eligible to take the Certified Ohio Dental Assisting examination, and earn the CODA credential (Certified Ohio Dental Assistant). Students will also complete the Ohio Dental Assistant Radiographer initial training course. This initial training course satisfies the requirements of the Ohio State Dental Board and allows the student to earn the Ohio Dental Assistant Radiographer license.

Early Child Care Education/Child Development Associate (930 Hours) FINANCIAL AID ELIGIBLE PROGRAM

The Polaris Career Center CDA program is designed to prepare students to successfully complete the requirements for the nationally recognized CDA credential. The CDA student will gain knowledge of how to put the CDA Competency Standards into practice and an understanding of why those standards help children move with success from one developmental stage to another. With the new Step-Up-to-Quality requirements, earning a CDA credential will enhance a student’s job opportunities. Students will receive 450-hours of classroom training and have the opportunity to gain practical experience in a 480-hour unpaid required field experience/ externship at an area child care center. A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Prior to the field experience/externship, students will be required to have and show proof of a physical exam, complete a background check, obtain necessary immunizations and a TB test. Medical Assisting (946 Hours) FINANCIAL AID ELIGIBLE PROGRAM (for those who qualify) The Medical Assisting curriculum prepares you to perform administrative, clinical and laboratory procedures for employment in a physician’s office, clinic, or hospital. This course will cover, but is not limited to, the following: Medical terminology, anatomy and physiology, Entrylevel clinical skills, Phlebotomy certification, Scheduling appointments, Records management/Electronic medical records, Medical law and ethics, Entry-level ICD-9/CPT coding, Health insurance claim forms, AED/adult/child/infant CPR, First aid/medical emergencies, Computer skills, Psychology of human behavior, Employability skills, OSHA/universal precautions, HIPAA/standards and guidelines. Medical Assisting students will have the opportunity to gain practical experience through a 160-hour unpaid externship which will take place during the program’s 946 hours. A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Students will be required to have a routine physical, and criminal background check prior to externship. Some healthcare systems and physician practices may require proof of current immunizations for students participating in externships. Students must also complete the Hepatitis B series prior to participation in their externships. The Polaris Career Center Medical Assisting Program is accredited by the Commission on Accreditation of Allied Health Education Programs (www. caahep.org) upon the recommendation of the Medical Assisting Education Review Board. Medical Billing & Coding (900 Hours) FINANCIAL AID ELIGIBLE PROGRAM (for those who qualify) This 900-hour program is designed for those who wish to gain employment in medical billing, coding and health information management in physician practices, outpatient clinics, billing companies as well as a variety of other medical facilities. Students will develop a thorough knowledge of medical terminology, anatomy and physiology, diseases and conditions, electronic health records, practice management software, medical office administration practices, diagnostic and procedural coding, hospital billing, reimbursement and claim form procedures, employability skills and healthcare law and ethics. Graduates will be prepared for entry-level positions as medical administrative assistants, electronic health records specialists, and medical billing and coding specialists. Students will complete a 180-hour unpaid externship during daytime working hours towards the end of the program. A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Students will be eligible to take the National HealthCareer Association Certified Medical Billing and Coding Specialist (CBCS), Certified Medical Administrative Assistant (CMAA), and the Certified Electronic Health Records Specialist (CEHRS) exams. Healthcare providers may require immunizations for students participating in externships.

Natural Resources/Ranger/Basic Peace Officer Academy (600 Hours) FINANCIAL AID ELIGIBLE PROGRAM (for those who qualify)

Starting Fall 2015

The Polaris Career Center Natural Resources/Ranger/Basic Peace Officer Academy prepares candidates for police and natural resource law enforcement officer positions and other careers that require peace officer training. This 600-hour program is conducted under the auspices of the Ohio Peace Officer Training Commission. The Academy training includes: administration, legal, human resources, firearms, driving, subject control, first aid, patrol, civil disorders, investigation, physical conditioning, aerosol defense spray, expandable baton and employability skills. Applicants must be 21 years old by the completion of the program, have a valid driver’s license and must complete a background check and a physical examination. All state and federal guidelines dictating the carrying of a firearm and minimum physical fitness standards set by the Ohio Peace Officer Training Commission will be followed.

Transfer of Students from Other Institutions Polaris Career Center does not grant advanced standing to students transferring from other institutions.

Transfer of Students Between Programs Prior to the program start date, registered students may not transfer to another program. Students must withdraw from their current program and re-register for another program. The $100 cancellation fee policy will be in effect and applied.

Nurse Assistant (90 Hours)

Phlebotomy Certification (252 Hours)

State Tested Nursing Assistants gain employment caring for diverse patient populations including the elderly in long-term care settings, assisted living facilities, home care environments and hospitals. The program is state-approved and consists of mandatory attendance for classroom/clinical hours. The STNA exam is included in the tuition and is offered to students after successful completion of the program. Medicare approved Home Health Aide certification and American Heart Association Basic Life Support (CPR) certification is also included. The 2-step Mantoux TB test is required before class begins. Immunizations may be required and the cost is the responsibility of the student. Employers/clinical sites may require a background check. Uniform requirements will be discussed on the first day of class. Uniforms are required and the cost will be the responsibility of the student.

Schedule/Tuition: Monday – Thursday, 8:00 AM – 1:30 PM January 20, 2015 – February 19, 2015 February 23, 2015 – March 24, 2015 April 13, 2015 – May 12, 2015 Tuition Fee: $995 (No Discount – Includes materials fee of $290) Tuition includes textbook, CPR fees and STNA examination fee. Students can expect the following additional costs: immunizations and/or titres, uniform and 2-Step Mantoux TB test.

Pharmacy Technician (600 Hours) FINANCIAL AID ELIGIBLE PROGRAM (for those who qualify) The Pharmacy Technician program prepares students for pharmacy career opportunities in retail, hospital, and longterm care settings. Pharmacy Technician students will have the opportunity to gain practical experience through an unpaid 96-hour externship. The program prepares students to take the Certified Pharmacy Technician (CPhT) Certification exam. Prior to externship participation, students will be required to complete a criminal background check and drug screening. A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Some healthcare systems, physician practices, and/or pharmacies may require immunizations for students participating in externships.

Monday - Thursday, 9:00 AM – 1:00 PM February 17, 2015 – November 23, 2015 (approximate ending date) Tuition Fee: $5,995 (Includes materials fee of $560) Tuition includes textbooks and certification examination fee. Students can expect the following additional costs: immunization and/or titres, drug screening and uniforms.

Students will be trained to perform a variety of blood collection methods on adults using proper techniques and precautions including: vacuum collection devices, syringes, capillary skin puncture, and butterfly needles. Both classroom instruction and clinical experience/externship during a 100hour unpaid externship at a healthcare facility will provide an opportunity for the student to gain the skill and knowledge necessary to take the Certified Phlebotomy Technician (CPT) exam through the National Healthcareer Association. A minimum of 90% attendance with a C average or better is required for students to participate in the externship opportunity. Note: Externship hours will be scheduled during the day and students will be placed as space is available. Students will be required to purchase their own uniforms and, prior to the externship, students will also be required to have a criminal background check. Healthcare providers and labs require immunizations and may require blood titres (at the student’s expense). Prior to starting the externship, students will be required to complete the three-part Hepatitis B vaccination series (at the student’s expense). Students must have the first of three Hepatitis B vaccinations prior to the first day of class. Drug testing and a flu shot may be required (at the student’s expense). Prerequisite: Medical Terminology course, or concurrent enrollment in a Medical Terminology course. Proof of registration in a Medical Terminology course or successful course completion (transcript or certificate of completion) is required at the time of registration. Precision/CNC Machine Technology (600 Hours) FINANCIAL AID ELIGIBLE PROGRAM Starting Fall 2015 (for those who qualify) Be prepared to enter the high-pay, high-tech field of precision machining. Journeyman machinists, die makers, mold makers and CNC programmers/ operators enjoy stable employment in the dynamic environment of manufacturing. All participants will National Institute for Metal Working Skills be given the opportunity to earn Level 1 credentials from the National Institute for Metalworking Skills, Inc. (NIMS). This hands-on curriculum includes: Applied mathematics, blueprint interpretation, geometric dimensioning & tolerancing; Project work on lathes, mills, surface grinders; CNC/CAD/CAM fundamentals; Manufacturing safety and job planning; Hand tools and drilling machines; Inspection instruments and quality control; Metallurgy and heat treating; Web-based instruction in partnership with Tooling U.

Welding (600 Hours) FINANCIAL AID ELIGIBLE PROGRAM (for those who qualify) Become a trained technician in the welding fabricating and metallurgical trades and gain access to a growing and demanding field. The PROGRAM includes: Introduction to Construction and Maintenance (Core Curriculum), welding safety, blueprint fundamentals, reading welding symbols, oxy-acetylene cutting, welding, and brazing, base metal preparations, weld quality, SMAW (Shielded Metal Arc Welding or Stick Welding), SMAW equipment and setup, SMAW electrodes and selection, SMAW groove welds with backing, SMAW open V-groove welds, SMAW open-root pipe welds, SMAW vertical welding, Plasma

arc cutting, Basic GTAW (Gas Tungsten Arc Welding, TIG), Basic GMAW (Gas Metal Arc Welding, MIG), Theory, welding and machine operation, and Introduction to pipe welding. Students are given the opportunity to earn National Center for Construction Education & Research (NCCER) Certification in Core Curriculum and Welding Level 1 and may also be eligible to earn American Welding Society (AWS) certifications. JOB TRAINING REGISTRATION INFORMATION Admission Requirements for Job Training Programs: Students must register in-person and bring the following: • The student’s high school diploma/transcript or GED (General Equivalent Development), or its equivalent. • The student’s valid government issued photo ID (i.e., driver’s license) • Tuition payment (depending on financial aid award) • A Visa or MasterCard if tuition is not paid in full at time of registration. Basic Rider Course - Beginning Rider (BRC) (Approximately 16 hrs.) This class is your first step in learning to ride a motorcycle. It is intended for the true beginner who has never ridden a motorcycle and for the experienced rider looking to refine their skills and obtain a motorcycle endorsement. If successful, you will receive a completion card which you will take to the driver’s license bureau, and after paying the necessary fee, a motorcycle endorsement will be added to your driver’s license. You do not have to take another riding test at the driver’s license bureau. Classes consist of 4 hours of classroom training, followed by approximately 12 hours of actual riding on a controlled course. Courses are offered on a variety of days and times, so make sure you know your class dates and times when registering. For the first class, bring your driver’s license along with either your motorcycle endorsement or a valid temporary riding permit, which you obtain by passing the written test for the permit at the driver’s license bureau. Otherwise, you will not be admitted into the class and your tuition will not be refunded. Also, bring an ink pen, and dress casually. Proper riding attire will be discussed on the first night of class. NOTE: All classes start promptly! You will NOT be admitted once the published class time begins, and your tuition will not be refunded!

Basic Rider Course - 2 Experienced Rider (BRC-2)  (5 hrs.) This course is designed for experienced riders who want to improve both their mental and physical riding skills to lower their risks while riding. The course consists of training and practice on your motorcycle on a controlled course under the knowledgeable and helpful eyes of trained instructors. If you ride two-up, your passenger may also enroll in this course. You must bring: your motorcycle license, proof of current insurance, your street legal motorcycle in good operating condition with proof of ownership, and both rider and passenger must wear full protective clothing as defined on the Motorcycle Ohio website and at the end of this section.

Basic Rider Course – Returning Rider (BRC-RR)  (9 hrs.) The Basic Rider Course-Returning Rider (BRC-RR) is a 9-hour course designed for riders over 18 years of age who have been riding with a temporary permit for longer than a year (over a thousand miles experience) and for experienced riders returning to riding with or without a motorcycle endorsement after several years of not riding. Motorcycles are provided for your use during the course, and if successful, you will receive a completion card which you will take to the driver’s license bureau to receive your endorsement on your license. Canceled Courses The Adult Education Department must cancel courses that do not attain the required minimum enrollment. Berea and Strongsville School Closing When the Berea and Strongsville City School District’s classes are cancelled or dismissed due to inclement weather, evening classes will not be held regardless of weather changes. Classes missed will be added at the end of the session.
