Expert health care in your home When a health condition makes it difficult for you to leave home, Centura Health at Home brings our advanced health care services right to you. Our highly trained medical team provides many health services, including wound care and medication management, as well as physical, occupational, and speech therapy. We are proactive, treating issues before they become more complex, which helps prevent hospital readmissions and additional health problems. Every time we visit, we will make sure to explain our treatment process and address all of your questions and concerns before we leave. A registered nurse is available by phone 24 hours a day.
Centura Health at Home is available in your home, nursing home, or retirement community. We work with your doctor to develop a treatment plan that helps you recover as quickly and fully as possible.

We take the time to care A complete care team Once you qualify, Centura Health at Home providers will visit your home two to three times per week, or more if ordered by your physician. Depending on your needs, you may be seen by: Nurses and nursing aides Physical, occupational, and speech therapists Social workers Chaplains These professionals can provide the following services: Skilled nursing care, including assessment of symptoms and medical care that needs to be provided by a trained professional Pain management Wound care Administering medications, including injections and infusions Nutrition therapy (tube or IV feeding) Patient and caregiver education Physical, occupational, and speech therapy Help with bathing, toileting, and personal care Assistance finding community resources for help with meals, advance directives, and financial counseling Spiritual care

Am I eligible for home care? When you are ready to leave the hospital, a discharge planner, home care coordinator, or your physician will speak to you about your needs for follow-up care. If you qualify for home care services, you will need to obtain a physician referral. In addition to a physician’s referral, patients will also need to meet the following criteria: Patients must require skilled nursing care, physical therapy, or speech language pathology services Patients must be homebound Patients has a safe home environment
