Porter Hospice Residence Porter Hospice Residence provides 24-hour care in a nurturing and peaceful homelike environment. Our facility provides: • 17 private, residential rooms • 24-hour staffing by a registered nurse on-site, with 24-hour physician supervision • An individualized plan of care from our board-certified hospice medical director • 24-hour, secured access visiting hours • Overnight stays for guests • Made-to-order, chef-prepared meals • Healing Arts programs, including pet, massage and music therapy

Home Hospice Care

We also provide hospice care to patients in their own homes, or in nursing homes and assisted living facilities. Our care team will work closely with your physician and design a customized visit plan to assess, monitor, and manage symptoms, while helping loved ones and other caregivers provide patient care. Plus, nursing staff are on call 24/7 for phone consultations or emergency visits.

A Dedicated Care Team Every Porter Hospice patient receives care for body, mind, and spirit from a multidisciplinary team: • Medical director • Skilled registered nurses provide wound care, medication administration, symptom management, and patient/family education • Certified nursing assistants help with clinical needs, as well as providing personal care • Licensed clinical social workers offer counseling and referrals to community resources • Nondenominational chaplains • Trained volunteers provide a break for family members, help with running errands, and provide companionship • Bereavement counselors are available up to 13 months after the death of a patient
