Serving our communities Presbyterian Healthcare Services is an integrated system of 8 hospitals, 36 clinics, a physician medical group and a health plan. As part of an integrated system, patients in our rural communities have access to highly specialized care, such as open heart surgery or pediatric sub-specialty care and all our facilities in Albuquerque, when needed.

Our 9,400 employees statewide are all dedicated to a single purpose: to improve the health of the patients, members and communities we serve.

More than 400,000 health plan members have access to care provided by more than 9,000 community physicians and providers in our health plan network.

We have three community ambulance systems located in Albuquerque, Española and Ruidoso.

Five Home Healthcare programs serve the Albuquerque metro area, Clovis, Española, Socorro and Tucumcari.

Four Hospice programs serve the Albuquerque metro area, Clovis, Socorro and Tucumcari.

Investments in our communities

We are responding to growing needs in the communities we serve by expanding the access to primary and specialty care services. In rural New Mexico communities, we maintain a valuable healthcare safety net.

RIO RANCHO Presbyterian Rust Medical Center In October 2011, we celebrated the opening of Presbyterian Rust Medical Center, the state’s first 21st century hospital and Rio Rancho’s first full-service hospital. Approximately 6,000 people joined us for a ceremony to officially open the 68-bed (expanding to 92 beds in 2012) hospital. Presbyterian Rust Medical Center was built using design elements that help patients heal faster and help caregivers provide more efficient care. The hospital includes: • 24-hour emergency department to care for the growing and active Sandoval County community • The first Advanced ICU in the state, combining the latest technology with skilled physicians and nurses who are both onsite and connected 24/7. Using sophisticated, real-time video feeds and communication tools, the Advanced ICU provides an extra layer of care to seriously ill patients. • Mother/Baby care to welcome the newest New Mexicans • Neonatal ICU for when the unexpected happens

34 new private patient rooms Scheduled to open in October 2012, the Presbyterian Española Hospital Patient Tower demonstrates our continuing investment in improving the health of people in northern New Mexico.

A new Cardiovascular Lab In 2011, Plains Regional Medical Center opened a new, permanent Cardiac Cath Lab with a $1.5 million investment. The new lab is equipped with sophisticated vascular imaging systems which can show blockages and abnormalities, allowing physicians to evaluate and diagnose patients for heart disease and other vascular problems. Since the lab is available locally, patients who require this testing no longer need to travel to other facilities that are more than 100 miles away.

The first phase of the $22 million investment adds 34 private patient rooms to the hospital in a 55,000 square foot space. Patients and families will be more comfortable in the private rooms, which will also have ample natural light. These rooms will also allow for patients to be treated in the same room from admission to discharge without the need for a room transfer if a patient’s medical condition changes.

Plans for a new Physician Office Building Plans are moving forward to build a 20,000 square foot building to provide room for physician offices and related healthcare providers. Orthopedists will begin offering services at Lincoln County Medical Center in 2012.

Breaking ground for a new Physician Office Building In 2012, Socorro General Hospital will break ground on a new $6.8 million physician office building that will bring patients greater access to primary care and specialty care providers.

Presbyterian Medical Group Primary Care Clinic Opens In May 2012, the Presbyterian Medical Group Primary Care Clinic at Dr. Dan C. Trigg Memorial Hospital held its grand opening. The $660,000 investment adds a 5,800 square foot clinic that provides office space for physicians and support staff and a large waiting area for patients.

Assessing Community Health Needs In 2011, Presbyterian embarked on an assessment of community health needs. Using national, state and county data as well as feedback from all of Presbyterian’s community boards and committees, we selected these system-wide community health priorities: healthy eating, active living and tobacco cessation. In 2012, Presbyterian will conduct forums to gain community and expert feedback that will help drive interventions. Currently Presbyterian supports a number of community-based programs, including: • First Born Program in Española, in which nurses and others make home visits to provide parenting education to first-time families. • Heritage Program, an outpatient behavioral health program for adults age 55 and older in Socorro and Ruidoso, helping people improve their quality of life. • Diabetes Education Programs in Española, Ruidoso, Socorro and Tucumcari, which focus on educating people on self-management and healthy lifestyle changes to manage the disease.

Saving lives Every Day From identifying dangerous blood infections earlier to a faster process to get heart attack patients to life-saving procedures, Presbyterian has made reducing preventable deaths the highest priority. In 2011, through efforts to improve numerous patient care processes in all of our facilities, Presbyterian recorded its lowest mortality rate ever, 1.25 percent. Leading the way in Patient Safety Presbyterian Hospital was one of 65 hospitals nationwide to receive the prestigious Leapfrog Top Hospital designation in 2011. As a Top Hospital, Presbyterian Hospital met standards for safe, high quality care, which includes medication safety, intensive care unit physician staffing and high risk prevention for surgeries and procedures. Sustained excellence in providing childhood immunizations For the eighth straight year, Presbyterian Medical Group Ruidoso-White Mountain Medical has been the state’s leader in immunization rates for children, exceeding 90 percent. New Mexico’s Department of Health and the state’s Immunization Coalition define the goal as children who have received all of their scheduled immunizations by age two. Gold Seal of Approval Both Socorro General Hospital and Lincoln County Medical Center earned The Joint Commission’s Gold Seal of Approval™ for accreditation in 2011 by demonstrating compliance with The Joint Commission’s national standards for healthcare quality and safety in hospitals. The accreditation award recognizes both hospitals’ dedication to continuous compliance with The Joint Commission’s state-of-the-art standards. The Joint Commission’s hospital standards address important functions related to the care of patients and the management of hospitals. The standards are developed in consultation with healthcare experts, providers, measurement experts and patients.

How Presbyterian is transforming healthcare It’s exciting to put an innovation into practice and see a positive change for Presbyterian and our patients and members. These changes are helping us to transform healthcare by lowering costs and enhancing the care we provide.

Patient Centered Medical Home Patients in 10 of our Presbyterian Medical Group primary care clinics now receive care under the Patient Centered Medical Home model, receiving greater access to healthcare through innovations such as group visits and telephone appointments. The patient’s medical care team is led by a Primary Care Provider. Depending on the patient’s needs, the team may also include a pharmacy clinician, behavioral health clinician, diabetes educator or case manager. The entire care team is located at the clinic, the patient’s “medical home.” Pioneer Accountable Care Organization Presbyterian was selected in December 2011 as one of 32 Pioneer Accountable Care Organizations (ACO) nationally by the Centers for Medicare and Medicaid Services (CMS). The Pioneer ACO program is designed to provide Medicare Fee-For-Service beneficiaries with high quality, high value care through enhanced care coordination. As a Pioneer ACO, we serve about 16,000 Medicare beneficiaries across the state.

Hospital at Home Presbyterian partnered with The Johns Hopkins School of Medicine to develop our innovative Hospital at Home program in which doctors, nurses and technicians travel to patient homes to provide hospital-level care. Only a handful of hospitals in the nation offer this service. More than 500 patients have received care through Hospital at Home, and clinical outcomes are the same as for hospitalized patients. Hospital at Home lowers the cost of care by an average of $1,000 to $2,000 per hospitalization, saving more than $780,000 to date. Also important is that Hospital at Home patients are highly satisfied with their care.

Cancer Nurse Navigators While advances in cancer treatment have helped save millions of lives over the last three decades, patients often face more complex treatment decisions and followup options than in the past. Presbyterian’s Nurse Navigators help patients through the maze of cancer-treatment decisions and fears. Discharge Call Center Presbyterian Healthcare Services started this call center to make sure that patients discharged from the hospital understand their doctor’s instructions once at home. By calling each patient discharged from our hospitals, our representatives can provide patients with information, address follow-up care issues and assist with any questions.

Electronic Health Record Presbyterian’s advanced, comprehensive Electronic Health Record system will bring together all of your Presbyterian-based medical information, enhancing communication across our healthcare system. Information about visits to your Presbyterian primary care provider, specialists and emergency department, lab results, and immunization schedules will all be accessible via the electronic health record. Using the patient portal, MyChart, patients will have secure access to view certain test results and will be able to request appointments or prescription refills and view portions of their medical records. Patients will also be able to use secure messaging to communicate with their primary care provider or other care team members. For more information, visit www.phs.org/MyChart.
