As a part of the service to seniors, PMMA has always offered care after hospitalizations through skilled nursing and physical, occupational and speech therapies with the objective of supporting a return to prior levels of independence and overall health. In 2012, PMMA launched a program called PATH® or “PostAcute-to-Home” to strengthen service to older adults. This program serves patients who have been discharged from a hospital, but still require therapy and special skilled care in order to make the transition from hospital to home. By incorporating the PMMA team of medical professionals to develop individualized programs, PATH helps patients regain functionality and promotes a safe return to their home. PATH sets the standard with team members who are committed to making the admissions process more efficient and features prompt admissions decisions, extended admission hours, continual communication with physicians and families, proactive planning and post-discharge follow-up, and a

Through a partnership with Aegis Therapies, patients set unique goals upon admission to PATH. These goals are discussed throughout the PATH stay as a way to motivate and honor progress. Examples of unique goals that have been set include: · Playing on the floor with grandkids again · Getting back to my garden · Visiting my sister in Texas · Getting strong enough to go home and take care of myself and my husband PATH - continued on page 4

Works by local winners are automatically entered into a masterpiece level competition with winning art from 16 other PMMA communities. More than 190 entries were received for this year’s masterpiece competition. The winners are featured in the Art is Ageless calendar with pull-out postcards and greeting cards. “We are blown away by the artistic talent in our community,” said Crystal Stock, marketing director at Emporia Presbyterian Manor. “Residents and friends of PMMA’s Art is Ageless program are proving that art, in any form, is an ageless ambition.” Art is Ageless, open exclusively to people age 65 and older, is a copyrighted program of PMMA. For the competition, works must have been completed in the past five years.

PMMA’s Art is Ageless program encourages Emporia Presbyterian Manor residents and other area seniors to express their creativity through its annual competition, as well as activities throughout the year. Art is Ageless is an extension of PMMA’s wellness programs. Learn more at ArtIsAgeless.org. ◆

Staff spotlight: Sarah Funck While Sarah Funck has been working as a CMA in assisted living since January 2018, being on the job is nothing like “work” at all. “I love the residents at Emporia Presbyterian Manor,” said Sarah. “I never feel like I am going to work, I feel like they are family.” Sarah enjoys being a part of residents’ lives. “I love being able to build relationships with them, care for their needs, be there on their good days and their bad days. I love seeing them get stronger and reach a goal, and also being there when they aren’t feeling well. I love getting to know their families and hearing their stories,” said Sarah. Sarah credits her team of colleagues as a great network of support, too. “This job is very rewarding, and I learn more about myself every day. I also have some amazing nurses on my team that I couldn’t imagine doing this without,” said Sarah. When she’s not on campus, Sarah enjoys hanging out with her “other” family and kids. She has two children—Haley, who is almost two, and Carter, who is five. Sarah is involved with her church and enjoys photography and singing. ◆

COMMUNITY MATTERS | September 2019
PATH - continued from page 2
Upon discharge, goals are celebrated with a photo, the recovery team and then shared with others taking advantage of PATH services. “The ‘I Took the PATH to …’ programming sets PMMA communities apart from other providers by demonstrating our commitment to whole-person wellness. The physical aspects of recovery from an illness or injury are typically the focus, but our talented teams focus on the emotional and spiritual aspects of recovery as well. I’m not aware of another program where patients have the opportunity to serve as inspiration to others experiencing similar obstacles of health,” said Jennifer McClanahand, regional director of sales and marketing for PMMA.
