Has national OH&S created harmony? Harmonisation of safety laws across Australia has increased focus on OH&S, writes Rob Stummer.
I
N the past few years the interest in Occupational Health & Safety (OH&S) has increased considerably within the resources sector. Harmonised Work Health and Safety (WHS) laws commenced in New South Wales, Queensland, the Commonwealth, the ACT and the Northern Territory on 1 January 2012, and companies have also placed increased importance on OH&S. Large Australian resources companies generally have OH&S policies and procedures in place which are well in excess of minimum legislative requirements. However, companies in the Engineering, Procurement & Construction (EPC) industry often find it challenging to demonstrate OH&S compliance and the lack of integrated information systems is a key contributing factor.
EPC contractors The trend is for EPC contractors to offer turn-key installations. This shifts all the risks to the EPC contractor which designs and constructs the installation and manages all purchasing activities and subcontracting. Also, the contractor is responsible for every aspect of safety, although the contracting company must also ensure that the contractor demonstrates OH&S compliance.

Contractor management Clients measure the performance of subcontractors, suppliers and other parties involved, based on agreements on safety and working methods. In general, parties will document in detail agreements made with regard to roles and responsibilities, in order for organisations to properly control them and create an accident free workplace on a day-to-day basis. For a company that outsources work to a contractor or subcontractor, it is far more efficient to work with an integrated system which records and can report on health and safety information, insurance data, training programs and specific contractor and owner/client documents.
Risk and control
subcontractors deal with OH&S policies. While this insight is increasingly easy to realise by deploying modern automation tools, not all companies have the systems to efficiently capture and report the information. This can be an issue for EPC companies when they are gearing up for a new project. Many lack the systems to efficiently coordinate the right resources and demonstrate OH&S compliance.
Automating processes For many organisations, monitoring incidents and reporting on them is a complex matter. Often, incidents are registered in stand-alone information systems that support only a part of the entire OH&S policy, and do not provide a complete overview, making it impossible to coordinate with other business processes. The choice now lies between an integrated system that supports all processes, or a best-of-breed approach where separate specialised applications are deployed for different types of processes. While efforts to harmonise different OH&S regimes in Australia are welcome, there will never be a one-size-fits-all solution. Systems which can be easily configured to meet different OH&S policies are far more valuable and costeffective than those requiring expensive or time-consuming customisation. How to effectively manage contractors SOFTWARE can assist in properly managing safety obligations when it comes to dealing with contractors. Many organisations, including big miners, use contractors for flexibility: they can quickly scale their workforce up and down to meet changing operational requirements. However, while most firms conduct initial insurance and certification checks at the point of employment, many find tracking expiration and renewal unwieldy, and fail to effectively manage their contractors. “When a contractor is uninsured or does not have up to date training or certificates contracting firms can be left exposed financially and have their reputations significantly damaged,” says Adrian Manessis, from safety management software firm, myosh. “Companies have lost large contracts as a result of the mismanagement of contractors. “Under the new OH&S harmonisation laws the term ‘employee’ is changed to ‘worker’, making it even clearer contractors and volunteers need to be included in safety plans. Cool and tough shirt HEAT stress is a serious issue with Australian mining sites often reaching above 45 degrees. Heat stress can cause heat rash, muscle cramps, dehydration, naeusea, and heat stroke. This is exacerbated by clothes that are heavy and lack ventilation, which traps heat and acts as insulation, further raising the body temperature. Failsafe valves ROTORK has unveiled its new range of manually energised spring-return actuators for failsafe valves. The new Rotork ManPower range is built for failsafe valves that are infrequently operated. According to the company, these new spring-return actuators reduce installation time and expense since no external electrical, hydraulic or pneumatic power supply is required. Suitable for a wide range of ball, butterfly, and plug valves, the scotchyoke actuators are equipped with a compact, self contained manually operated hydraulic power pack. A hydraulic hand pump on the power pack is used to operate the actuator and compress the failsafe spring, holding the valve in the open or closed position until a failsafe signal is received, at which point the spring immediately drives the valve to the safe position. If safety ain’t productive, it ain’t safe Frank Schrever, principal consultant at Machine Safety By Design, writes that safety applied without reference to the productivity and operability of the workplace can lead to dangerous outcomes, regardless of the technical and legal compliance of the solution.
T
HE law has long been exhorting us to eliminate risk and if that is not possible to minimise it by engineering means. Those engineering means are not described in legislation, except to the extent of expressing a guarding hierarchy which defines the guarding types and the order in which they should be examined and employed. In order of the degree of access required for the machine in question, these proceed from a welded physical guard which is never removed from the machine, to an interlocked guard which prevents access to the machine until hazardous machine functions have stopped, to guards which can only be removed with the use of tools, to bolted guards and then presence sensing systems. The law points us via the codes of practice to Australian standards, more specifically AS 4024.1-2006 Safety of
Machinery, with its suite of technical and machine specific standards, when designing and implementing guards. For machines needing frequent worker access, oftentimes the guards are ones which can be removed by the operator. While workers may be required to comply with certain procedures in order to remove the guards, sadly, over time diligence wanes, complacency creeps in and terrible injuries can result. Humans are relatively unreliable when it comes to performing repetitive tasks and accidental, occasionally deliberate departures from procedure are made. If we simply follow the guarding and protective requirements, we could be ignoring the most dangerous and often subtle issue. That is, the tendency of humans to want to do things quickly and easily, and maximise production output. A safety system which has not taken

Often, safety designers have only safety in mind, and try to make things extraordinarily difficult to defeat, only to find that the defeater has been even more highly motivated to find even cleverer ways to get around it. We must keep the productivity and operation of the plant we are trying to make safe at the forefront of our thinking. We should discuss possible solutions with the people who operate, maintain and clean the machines, solicit their input and arrive at solutions which not only make the machine safe by design, but do not make them harder to use and do not reduce the productivity of the process. Online safety course for transport sector PEOPLE and Quality Solutions (PaQS) has launched a new online safety course tailored to the transport sector. According to Safe Work Australia, in 2009 to 2010, the transport and storage industry accounted for eight percent of all serious workers’ compensation claims (10,400 claims). This equates to 29 employees each day requiring one or more weeks off work because of work-related injury or disease. PaQS says its course fits into the transport sector’s safety induction programs. It is designed to develop workers’ attitudes towards safety as well as their confidence and self esteem through self awareness. It also provides tips about communication skills to deal with issues such as conflict and bullying. According to PaQS, its courses target human error by developing and enhancing rational judgement, perception, safety thinking and responsibility. This makes it possible to increase adherence to safety systems, improve awareness, and reduce workplace incidents. slowing down reel rewinds REEL-Tech has released the Safe-R-Reel system for spring rewind reels, reducing rewind speed and risk of injury and equipment damage. Spring rewind hose reels are commonly used in industry for hose storage. In some hose handling applications, excessive spring rewind speed due to misuse can cause the hose to whip out of control, resulting in significant safety issues: for example, the rapidly moving hose can strike users or objects, resulting in injury or damage. Such issues can be significantly reduced with the Safe-R-Reel Rewind Speed Control System. By reducing the rewind speed, it also reduces the risks of injury, equipment damage, down time and hose wear associated with excessive rewind speed. The system is designed to be added to most common brands of spring rewind reels and can be easily fitted on to an existing reel without major irreversible modifications. It is most readily adaptable to the Hannay range of industrial spring reels. Depending on the reel capacity, the speed control system simply bolts on to the hose reel canister and it can be tuned to achieve 30-50 percent speed reduction compared to a standard spring rewind reel. Research calls for focus on eye safety equipment THE Centre for Eye Research Australia says more than half the people who undergo emergency treatment eye damage due to metal fragments did not wear safety glasses during the accident. Severe eye damage can occur due to high-velocity impact machining, such as metal grinding, welding, repair work and drilling. Work-related eye injuries account for about 3,500 emergency presentations and cost the Victorian community $39 million each year. Injuries caused by metal fragments can result in corneal scarring and permanent visual impairment, as well as infection. The report from the Centre for Eye Research Australia looked at 100 patients who attended the Royal Victorian Eye and Ear Hospital Emergency Department over a two month period in 2009. 97 percent of the patients were male. 25 patients had suffered a previous eye injury caused by metal fragments, yet more than half of those were not wearing eye protection at the time of their most recent injury. The Centre maintains that the study raises the issue of low compliance in terms of use of safety apparatus when undertaking high-risk machining activities. This may be due to the attitude of the individuals, lack of appropriate safety apparatus, inadequate training or workplace management issues such as the company not complying with safety standards. Another issue raised by the study is the design of the safety eyewear, since 45 percent of the injuries occurred while some form of eye protection was worn. Safety eyewear may fail due to design flaws where gaps are present between the eyewear and the face, the pattern of particle emission, or the alignment of the work operation. Latex rubber insulating gloves HONEYWELL Safety Products has launched the Electrosoft AS2225 Approved Natural Latex Rubber Insulating Gloves, providing protection for electrical workers. Every year numerous workers across Australia suffer the effects of electric shock, which can include burns, muscle contractions and pain, respiratory paralysis, heart paralysis and sometimes, even death. According to Safe Work Australia (2010), 71 people were fatally injured at work between 2003 and 2008 – deaths that could possibly have been prevented with the use of properly fitted personal protective gear. Honeywell says the insulating gloves are the first line of defence for contact with any energised components or lines. With approval to the AS/NZS standard AS2225: 1994, the 100% latex gloves are suited for use in a range of electrical conditions, including low voltage to high voltage environments and have glove ratings from 650V to 4000V. With a natural latex base, the gloves have a straight cuff for easy wear and a non-flocked lining. The gloves should be used in conjunction with leather over-gloves to provide mechanical protection of the electrical glove against cutting and tearing. They are available in multiple sizes from size 8 through to size 11. Hierarchy of control The five-level hierarchy of control will be the major change in NSW, Queensland and South Australia. The system considers human behaviour and cost-effectively deals with risk reduction. It calls for higher order controls like walkways and guard rails, as opposed to more documentation-heavy lower order ones like administrative controls or procedures.

Penalties
The model code details the consequences of being suspended in a harness, including death. Self-rescue is no longer acceptable. At least one trained person must be on standby at the site to execute the rescue without relying on assistance from the suspended person. This significantly increases the labour cost of working in a harness.
The penalties for those held responsible for falls will be much more serious. OH&S lawyer Michael Tooma says court-imposed penalties are only part of the cost faced by employers. “A serious injury or fatality will usually result in quite an extensive investigation,” he said. This could then lead to a coronial inquest, increased scrutiny of the organisation,

onecranes has installed lifting solutions for austal, boosting safety when building very large ships. austal is a Defence Prime contractor and a leading designer and constructor of commercial and defence vessels. austal typically uses 16 overhead hoists working simultaneously to lift and move each assembled ship section before positioning into the main structure. Previously each hoist required its own operator, so 16 operators were needed. each operator had a control button and had to work together to lift the section. now, its shipyard’s building halls in Henderson, Wa, utilises 32 Konecranes cXT monorail hoists, each with a capacity of 3.2 tonnes. They travel east-west along the 110 metre x 28-metre wide building halls on
