As part of your total rewards package, Princeton University offers comprehensive benefits that include various options to meet your healthcare and financial needs. During Open Enrollment—Monday, October 16, through Friday, November 10, 2017—you have the opportunity to review and change your benefits elections to meet your current and/or future needs. For changes to the benefits offerings for 2018, review this open enrollment guide and the enclosed My Benefits 2018 booklet carefully prior to making your selections. You may also attend a Benefits Fair to learn more. See Key Dates for more information. Throughout Open Enrollment, Human Resources staff members are available to meet with faculty and staff to assist with benefits-related questions and help with online enrollment. CHANGES TO PLANS AND PROGRAMS This guide highlights benefit changes going into effect on January 1, 2018. It also provides important reminders regarding the annual open enrollment process and resources available to you. For more details on these benefit changes, new programs, and current benefit plans and programs, refer to the enclosed booklet My Benefits 2018. DUO Effective immediately, to log in to HR Self Service, you will need your netID, password, and be Duo-enabled. Duo is a two-factor authentication system implemented by the Office of Information Technology (OIT) to protect your personal information and Princeton data. If you are not currently Duo-enabled, you may enroll in Duo Self Service at www.princeton.edu/duoportal. If you require assistance with your netID, password, or Duo, contact the OIT Help Desk at helpdesk@princeton.edu or (609) 258-HELP (4357).

Medical Plans

EMPLOYEE CONTRIBUTION CHANGES

TELEMENTAL HEALTH

Your contribution is the amount you pay toward the cost of your healthcare benefits through payroll deductions. Contribution rates will increase by 5% for the Princeton Health Plan (PHP) and J-1 Visa medical plans. The Consumer Directed Health Plan (CDHP) will not have a cost increase in 2018. In addition, the rates for the Aetna DMO and MetLife Dental Plans and the MetLife Vision Plan will not increase.

Telemental Health is included in all our medical plans. It is a convenient option that allows you to video conference with a licensed mental health provider—including psychiatrists, psychologists, and counselors—who can provide both therapy and medication management.

In 2018, the contribution rates for the HMO plan will be structured into salary tiers. The salary tiers are: $75,000 and under, which will have a 2% increase; $75,001 to $150,000, which will have a 3% increase; and $150,001 and above, with a 5% increase. Having salary tiers in the HMO plan helps to ensure more affordability and is similar to the salary tiers in the PHP that limit the out-of-pocket maximum.

Retirement Plans ROTH CONTRIBUTION OPTION Princeton University is introducing a Roth contribution option in the Retirement Savings Plan offered through TIAA. With the Roth option, your contributions are made after-tax and have the potential to accumulate tax-free. Withdrawals after age 59 ½ are tax-free if the distribution occurs no earlier than five years after contributions were first made. Unlike an individual Roth IRA, the Roth option in the Retirement Savings Plan does not have adjusted gross income limits and the maximums are the same as the pretax limits: $18,000 if under age 50 and $24,000 if over age 50. Assets may be passed along to your beneficiaries tax-free. If you contribute to both the pretax and after-tax options, the contributions are aggregated to determine your maximum contribution. Conditions commonly treated through Telemental Health

Visits are covered at the same cost as in-network in-person mental health visits. Individuals enrolled in the PHP or HMO will pay the specialist copay. Individuals enrolled in the CDHP or J-1 Visa Plan will pay the coinsurance after the annual deductible is met. For information on utilizing Telemental Health, refer to the enclosed booklet My Benefits 2018. UTILIZING PREFERRED TIERED SPECIALISTS IN THE PHP AND CDHP Aetna and UHC maintain a list of specialist categories with in-network preferred providers. These physicians have demonstrated higher quality and efficiency of patient care. Therefore, the costs are less. Aetna’s preferred providers are listed as Aexcel with a blue star. UnitedHealthcare’s preferred providers are listed as Premium Tier 1. You are charged a higher amount for utilizing an in-network non-preferred or out-of-network provider in these specialist categories. The Aexcel and Premium Tier 1 provider designations are formally reviewed on a two-year cycle. Therefore, your doctor’s Aexcel or Premium Tier 1 status may have changed. We recommend that you confirm your provider’s status before scheduling an appointment in 2018. For information on utilizing preferred specialists, refer to the enclosed booklet My Benefits 2018.


COVERAGE ADDITIONS AND CHANGES Acupuncture We are expanding our medical plan coverage to cover 20 acupuncture visits per calendar year to treat a full range of conditions. For the HMO, in-network visits will be covered after a $25 copayment. For the PHP, in-network visits will be covered after a $30 copayment and out-of-network visits will be covered after deductible and coinsurance. For the CDHP and J-1 Visa plans, visits will be covered in- or out-of-network after the deductible and coinsurance. Acupuncture is used mainly to relieve discomfort associated with a wide range of diseases and conditions, including pain management for lower back, neck, knee, and postoperative pain; headaches; hypertension; chemotherapy-induced nausea; rheumatoid arthritis; anxiety; and depression.

Physical Therapy Under the Aetna HMO plan, the copay for physical therapy visits will be reduced from $25 to $15 per visit. Under the PHP, in-network services will continue to be covered after deductible and coinsurance and out-of-network coverage will be reduced from 60% to 50%. Hi-Tech Radiology For hi-tech radiology (MRI, CT, PET, and other scans) there is a significant cost difference between services performed in a hospital setting (average cost of $1,800) versus those provided at an independent facility (average cost of $432). In 2018, the Aetna HMO plan will have a copayment of $100 for hi-tech radiology services received in a hospital setting and services received at an independent facility will continue to have no copayment. Under the PHP, the deductible and 10% coinsurance will apply to in-network hi-tech radiology services received in a hospital setting and services received at an independent facility will continue to have no copayment.

NEW INCENTIVE PROGRAM In addition to the Diabetes Management Incentive Program, My Health Coach will offer a Condition Management Incentive Program to help you manage your high blood pressure, high cholesterol, and/or obesity. This program is confidential, voluntary, and offered at no additional cost to eligible employees and their dependents enrolled in a Princeton medical plan. Copayments will be waived for certain prescription drug expenses related to these conditions. An annual allowance of up to $50 for support tools will be provided to those with obesity, unless they are taking a medication eligible for the copayment waiver. Transportation and Parking Transportation and Parking Services has improved the benefits to commuters who choose an alternative commute option—biking, walking, carpool, vanpool, and mass transit—in lieu of a parking permit. To learn more, visit www.princeton.edu/transportation.

You are able to log in to HR Self Service at www.princeton.edu/selfservice as often as you wish from Monday, October 16, through Friday, November 10, to review and/or make changes to your 2018 benefits elections. Instructions for using HR Self Service are in My Benefits 2018.

Annual Benefits Open Enrollment for 2018 begins.

Once you submit your elections for 2018, changes should be accurately reflected in HR Self Service within one to two business days following the election. You should verify your 2018 elections by logging in to HR Self Service and clicking on Benefits Summary under the Benefits menu. On the Benefits Summary screen, to view your 2018 benefits elections, change the current date to 01/01/2018 and click Go.

HEALTH SAVINGS ACCOUNT (HSA)

January 1, 2018

You may be eligible to elect the HSA if you are covered under the CDHP. If you are currently participating in the HSA, your election amount will automatically carry forward in 2018 and monies remaining at the end of 2017 will automatically roll over into 2018. However, you can elect, change, or terminate your contributions at any time during the year by contacting the Benefits Team, and you can manage your HSA at www.payflex.com. HEALTHCARE AND DEPENDENT CARE FLEXIBLE SPENDING ACCOUNTS (FSA) If you are currently contributing to a healthcare or dependent care FSA and would like to continue participating, you must re-enroll during Open Enrollment. Your 2017 elections will not automatically carry over into 2018, and there is no opportunity to enroll after the Open Enrollment period ends unless you experience a qualifying status event. If you contributed to a healthcare FSA in 2017 and elect to participate in 2018, your current debit card will automatically be updated with your 2018 election amount. If you have leftover funds remaining in your 2017 account, balances of $50 up to $500 will automatically roll over into 2018. COMMUTER BENEFIT PROGRAM You can elect to participate or make changes to your account on a monthly basis throughout the year. To participate, register at www.payflex.com. Transit and/or parking orders must be placed by the 10th of the month preceding their use. For example, if you need transit passes for January 2018, be sure to place your order no later than December 10, 2017. The monthly pretax payroll deduction limit for 2017 for transit and parking was $255. The IRS has not yet announced new limits for 2018. If a change occurs, PayFlex’s website will be updated to reflect the new limits. You may purchase transit and/or parking orders over the pretax limit; however, you will be responsible to pay for any expenses that exceed the IRS maximum with your own personal credit card.

The online HR Self Service system is available daily from 8:00 a.m. to midnight. HR assistance is available weekdays 8:30 a.m. to 5:00 p.m. The OIT Help Desk is available 24/7.

This is the final deadline for accessing HR Self Service to make any changes or corrections to 2018 benefits selections. Newly elected benefits become effective.

ID CARD INFORMATION Medical Plans You will receive new ID cards if you are electing a medical plan for the first time or are making changes to your coverage. You can print temporary ID cards from each provider’s website at www.aetna.com or www.myuhc.com. Prescription Plan You will receive new ID cards from OptumRx if you are electing a medical plan for the first time or are making changes to your coverage. You can print temporary ID cards at www.optumrx.com. Dental Plans If you are currently enrolled in the MetLife Basic or High Option Dental Plan, you will not have the option to change your dental coverage until the 2019 Open Enrollment period, unless you experience a qualifying status event. The two-year lock-in period was implemented last Open Enrollment as a strategy to help reduce rate increases in future years. MetLife You will receive new ID cards if you are electing a dental plan for the first time or are making changes to your coverage. You can print temporary ID cards at www.metlife.com/dental. Aetna DMO There are no ID cards issued for this plan because an ID card is not required to receive services or care. However, if you prefer to have an ID card, you can print temporary ID cards at www.aetna.com. Vision Plan All current and new enrollees will receive ID cards. You can print temporary ID cards at www.metlife.com/mybenefits.
