Qualifications: Fourth-year medical students who set up and have formal interviews with at least four residency programs prior to the date selections are made. Any visits made after selections are made will not qualify for reimbursement. To Apply: Visit www.wafp.org and roll over the "Students and Residents" tab to complete the online application form. Once received by the WAFP, you will be notified whether or not reimbursement is available (funds are available on a first-come, first-served basis). Reimbursement Process: When you are accepted into the PassPort Program, you will receive information about how to access the PassPort Reimbursement Form from our website. After you complete all of your visits, submit your travel receipts along with your reimbursement form to WAFP. Once the receipts and reimbursement form are received and your visits are verified, the Academy will send you a check in the amount stipulated according to the schedule above. Please review the program descriptions on the next pages. A Rural Training Track (RTT) is an accredited three-year program located in a rural area that is linked to a traditional program. Residents spend the first year at the core program.


Mercy Health System Family Medicine Residency JANESVILLE Program mission and goals:  Our residency is a community-based, unopposed program located in a small city that allows us to take advantage of the best aspects of rural and suburban practice. Program emphasis:  We provide a full spectrum family medicine program with obstetrics environment, a strong family medicine teaching service, outpatient medicine, procedures, practice management and partnership-oriented practice with resident

physicians. Residents and Faculty volunteer their time at the local free clinic to provide care to the underserved population. Curriculum components:  Competency based and emphasis on evidencebased medicine. Specialties:  Sports Medicine Fellowship and we offer an opportunity to participate in medical missions to Honduras and Ecuador for those interested in international medicine.

Faculty composition:  Mercy has six full-time faculty including the program director, one parttime faculty, one behavior medicine coordinator and a mid-level provider. The faculty includes CAQs in geriatrics, sports medicine and acupuncture. Accreditation:  ACGME Significant features:

Program mission and goals:  The La Crosse-Mayo Family Medicine Residency is built upon the Mayo tradition of excellence in patient care, education, and research. Curriculum components:  Our curriculum reflects our vision of preparing residents for full-spectrum rural practice, with emphasis on patient-centered officebased care, obstetrical care, procedural experience, and hospital care. The biopsychosocial model is emphasized and a family systems approach is utilized. We also have an innovative, international medicine curriculum. Significant features:  Our 18 residents are the only residents at Franciscan Medical Center, a 250bed community hospital featuring state-of-the-art technology and services. Program mission and goals:  St. Joseph Family Medicine Residency Program’s ongoing mission is to “improve the health of communities one Family Medicine Graduate at a time.” Our residency program also offers opportunities, instruction, and guidance to instill in our graduates the mastery of progressive delivery systems and a caring philosophy in order to serve the communities in which they live and work. Program emphasis:  Our program is designed to provide training that is as close to a real life setting as possible. Therefore, upon graduation, our residents are at a significant advantage to succeed in their new careers. Curriculum components:  State-of-the-art outpatient clinic with electronic medical records that is successful in providing patient quality care and financially sound. Wellrounded didactic lectures given by program faculty, local specialists, and our

Community medicine opportunities:  A full-time community nurse coordinates the following programs for resident involvement: teen maternity care program, daycare health clinics, “Ask the Doctor” program at local elementary and high schools, diabetic and immunization projects, home visits and much more!

Diverse procedural experience including obstetrical ultrasound, colposcopy, flexible sigmoidoscopy, vasectomy and flexible nasopharyngoscopy training all done in our Family Medicine center. Strong community support (Family Medicine is a kingpin of health care delivery in the county) with a solid financial backing. Population served:  We serve patients who live throughout Waukesha County, and serve a diversity of ages and ethnic backgrounds with our two leading ethnic groups being Caucasian and Latino. Location/community:  The only residency program in Waukesha County, we are located in downtown Waukesha which is minutes from sparkling lakes, lush State and County Parks, and the amenities of downtown Milwaukee.

Urban/rural opportunities:  Our population is a true mix of urban and suburban populations. Spouse/family opportunities:  Both Waukesha and Milwaukee offer ample employment and social activities for residents and spouses of all ages. The Medical College runs a spouses social network and we work directly with spouses to help them find what they are seeking in the community. Program mission and goals:  Your education is our primary mission. Located on Milwaukee’s beautiful east side overlooking the shores of Lake Michigan, the Columbia St. Mary’s Residency is built on decades of excellent residency education. At Columbia St. Mary’s, your service commitments are built around your education. Program Emphasis:  Not only will you be trained as a skilled generalist, you will have the opportunity to specialize, if desired, by participating in one of our four curricular tracks. This will further enable you to be fully prepared to practice in any setting anywhere. Curriculum components:  The Columbia St. Mary’s Family Medicine Residency pioneered track-based curriculum design. Aside

from our general Family Medicine curriculum, residents can choose to pursue one of four curricular tracks. Highlights of each track include: Maternity Care: Residents pursuing this track have the flexibililty to advance their maternity care training to match the depth of their desired learning. Whether you choose our Standard Maternity Track or pursue the Advanced option, you can expect 100-150 deliveries over three years with experience in vacuum extractions and vaginal repairs. Behavioral Medicine: This track’s focus enables you to implement a proactive approach with a wide variety of psychiatric,

psychosocial and family problems presented in a primary care setting. Residents will leave this track with advanced skills in psychopharmacology, psychotherapeutic methods, psychiatric intervention, and a patient-centered approach with individuals and families. International Health: This track is for the resident who wants to enrich their education by helping change the health of populations in developing countries. You will learn, firsthand, how health can be affected by the lack of vital public health services and infrastructure in impoverished communities. Your experience will

be supported by a curriculum that will augment the immersion field experience within an international setting. Community Medicine: While participating in this track, you will work at the grassroots level, where local health change occurs. You will become well versed in the interplay between an individual’s health and the community where they reside, becoming an active participant in helping the community work towards solutions. Specialties:  Our program’s track-based curriculum revolves around our strengths in maternity care, behavioral medicine and international and community medicine. Several past graduates have completed OB fellowships. One is

now a faculty member with our program. In addition, two of our 2013 graduates will be entering Sports Medicine Fellowships. Faculty composition:  11 full time faculty; two part time faculty. Two of our full time faculty have advanced training in Obstetrics and one full time faculty is a PhD in Psychology, and one full time faculty has recently been awarded the Fulbright Scholarship. Accreditation:  Dual accreditation with the ACGME (full five years) and with the AOA. Significant features:  Other significant features include experiencing an evidencebased, ambulatory focus practice including strong emphasis on continuity of care, preventative medicine, quality improvement, and patient-centered care and education. There are ample procedural experiences including colposcopy, OB ultrasounds, vasectomy, flexible sigmoidoscopy, IUD insertion, joint injections and many more. In addition to the traditional office visit, we offer prenatal group visits, musculoskeletal clinics, refugee clinics, and osteopathic manipulation clinics.

Program mission and goals:  The Madison program is committed to the personal and professional growth of each of its residents. That growth is fostered by a dynamic curriculum and nurtured by a supportive community of residents, faculty, and staff. Curriculum components:  Our curriculum offers robust experiences in obstetrics, critical care medicine, and pediatrics, as well as numerous electives. Diverse continuity clinics provide residents with a dedicated patient panel for three full years. Specialties:  Madison faculty have expertise in a variety of areas, including: integrative medicine, sports medicine, geriatrics, research, global health, developmental disabilities, and more.

Faculty composition:  Our residency faculty is a talented and dynamic group of academic Family Medicine educators and clinicians, complemented by fellowship recipients and community faculty physicians. The Madison program also has behavioral science faculty with extensive experience working with Family Medicine residents. Accreditation:  Accredited by the Accreditation Council for Graduate Medical Education (ACGME), and The American Osteopathic Association (AOA) for all three years of residency. Significant features:  Partnership System: First-year residents are paired for their inpatient rotations, community health

rotation, and in their continuity clinics. Continuity of Care: Residents establish a continuity practice at one of four Family Medicine centers. Each center has patients of all ages with a wide range of medical problems. All centers are staffed with at least six Family Medicine faculty, plus nurse practitioners and a behavioral scientist. Maternity Care: Residents can expect to be involved in 115-130 deliveries on our OB rotations. All residents manage their own obstetric patients from their continuity practices. Critical Care Medicine: Robust experiences in critical care medicine.

Teaching: Opportunities to teach medical students in a variety of settings. Wide Array of Electives: Residents can choose from integrative medicine, international health, palliative care, teaching nursing home, addictive disorders, sports medicine, practice styles, and more. Research: Opportunities to write for publication and do research with NIH funded researchers. Community Health Rotation: Programming designed to enhance understanding of how to practice communitybased medicine and work with community resources important to patients.

Pathways: A preestablished yet flexible set of experiences within the standard residency curriculum that allows a resident to gain further experience in a specific topic area. These topics include community and global health, maternity care, developmental disabilites, geriatrics, integrative medicine, sports medicine, and women’s health. Professional Development: Competency-based evaluation and close mentor/mentee relationships. Resident Health and Well-Being: The program supports many activities to foster resident personal growth and collegial relationships including a yearly residency retreat, monthly check-in group for first-year residents, and bi-weekly chief resident rounds. Our integrative medicine faculty also spearheads

a curriculum that teaches residents a “constellation of practices that awaken the heart, mind, and more.” Population served:  Residents establish their continuity practice at one of four Family Medicine centers. Two centers are located in small towns, and two are in urban neighborhoods with ethnically diverse populations. Inpatient care is at the community-based St. Mary’s Hospital and the UW Hospital. At St. Mary’s, our residents are the only house staff, and are involved in all levels of care. The Family Medicine service at UW Hospital gives residents experience in a tertiary care setting. Location/community:  A vibrant, picturesque city of 236,000, Madison is the Wisconsin state capital and the home of the University of Wisconsin. Madison boasts many big-city amenities, but is only 20 minutes from the countryside. The city consistently

ranks as one of the best places in the country to live, work and play. Urban/rural opportunities:  Residents in the Madison program gain experience in both rural and urban settings. The four Family Medicine centers offer residents a large and varied patient population. While each center has its own distinct characteristics, all provide residents with excellent learning opportunities and with a full range of experience and skills. Focused Rural Experiences:  First-year residents complete a rural surgery preceptorship as well as a rural community emergency medicine rotation. Residents are also required to complete a six-week rural rotation in their second or third year. Residents gain exposure to small town practice and get an intense “hands-on” experience that enhances self-confidence, maturity, and clinical competence. Residents can also choose from focused electives in rural or urban settings.

Spouse/family opportunities:  Madison has an affordable cost of living, a stable economy, and plentiful employment opportunities. Major employers include UW-Madison, the state government, American Family Insurance, Kraft/ Oscar Mayer Foods, and Epic Systems Corporation in Verona, WI. Madison is a very family-friendly city, offering numerous outdoor recreational opportunities (paddling, sailing, swimming, cycling, cross-country skiing), and many community-oriented events (farmer’s market, concerts on the square, festivals, sporting events).

Program mission and goals:  Mission: Train high quality family physicians for rural practice in Wisconsin and in the United States. Goals: Educate residents in a learning laboratory similar to their future practice as they interact with their patients as neighbors and citizens. Match learner needs with preceptor strengths. Address the growing shortfall of rural maternity care access by providing unopposed, high quality obstetrical training. Provide full continuity

patient care within the clinic, nursing home and hospital settings Offer a flexible “hybrid longitudinal curriculum” in an apprenticeship model allowing residents to gain full-spectrum family medicine experience by participating in: hospital rounds, clinic, urgent care/ER call, hospital admissions, and obstetrics. Program emphasis:  Rural practice. Curriculum components:  Our curriculum was created to fit a “1-2” Rural Training Track model, in which residents are placed on inpatient hospital service rotations with fellow Madison residents for their 1st year. During the 2nd

and 3rd years, the residents follow a longitudinal curriculum, in Baraboo, which allows for scheduling flexibility to meet curriculum requirements. Specialties:  Our residents have access to all medical specialties, with onsite faculty who have expertise in Women’s Health, Geriatrics, Pediatrics, Behavioral Health and Osteopathic Manipulation Treatment. Faculty composition:  We have 13 full and parttime Family Medicine physicians, as well a variety of community preceptors who embrace the opportunity to teach. In addition, Baraboo residents have access to the Madisonbased faculty, for additional learning opportunities,

such as Global Health, Integrative Medicine, Sports Medicine and Research. Accreditation:  We are dually accredited by the Accreditation Council for Graduate Medical Education (ACGME) and the American Osteopathic Association. Significant features:  The Baraboo Rural Training Track Residency, based at Dean Clinic-Medical Associates Baraboo, offers the following: Flexible curriculum with an emphasis on life-long learning. Strong maternity care training, from prenatal to postnatal, taught by family physicians in an unopposed setting with a supportive consulting OB/GYN from Madison.

We excel in training residents in high volume OB, ambulatory care, procedures, community medicine, geriatrics and behavorial medicine. The program is unopposed with no competing residencies within 100 miles and is dually accredited in allopathic and osteopathic medicine. Curriculum components:  The Fox Valley currriculum is individualized to residents’ interests and passions. Robust OB experience where residents obtain 125-150 or more

deliveries in addition to 10 or more continuity OB patients per resident. Electives in fertility, high-risk and operative obstetrics. Location/community:  Appleton is the largest of the Fox Cities, a cluster of 18 communities located along the Fox River. With a combined population of over 220,000, the Fox Cities area is the thirdlargest metropolitan area in Wisconsin, yet combines the tranquility of smalltown America with urban

The clinic is located in vibrant downtown Appleton within easy walking distance to a variety of ethnic restaurants, coffee shops, local clubs, and one of five area YMCAs.
