Welcome To say that this past year has been eventful at Providence TrinityCare Hospice is an understatement.

Most significantly, Hospice cared for more patients and their families than ever before. In particular, our team provided compassionate services to a greater number of infants and youngsters through TrinityKids Care, our program for children with life-limiting conditions. With our participation in a pilot project to care for children in Orange County whose families otherwise would not be able to afford palliative care services, we expect to serve even more patients.

This past year also saw a dramatic rise in activity at Hospice’s The Gathering Place, our program that offers counseling and support groups to children, teens and adults who are grieving the loss of a loved one.

And, thanks to our partnership with Providence Little Company of Mary Medical Center Torrance, we are now extending a new option to patients and families: inhospital hospice suites in the Transitional Care Unit. The hospice suites have been well-utilized, and we hope to open more suites in the coming year.

This expansion of care, of course, could not have come about without the support of our friends in the community. Despite a challenging economy, you have attended our events, volunteered your time and given of your resources to ensure that thousands of patients and families annually receive the care they need. Each time we walk by our donor wall, we are grateful and humbled to see more of our friends’ names. Thank you for your confidence in us. We will do our utmost to continue to earn your respect and support.

We are also grateful for the confidence that Providence Health & Services has shown us in the past year. Providence awarded our TrinityKids Care program its Mission Leadership Award for outstanding adherence to and advancement of the Providence mission. Following on the heels of this recognition, Providence TrinityCare Hospice, in partnership with Providence Little Company of Mary Medical Center Torrance, earned a prestigious Citation of Honor from the Circle of Life Award committee, which yearly singles out outstanding palliative and end-of-life programs across the country that excel in innovation. The committee specifically recognized us for our pediatric care program and focus on continuity of care across the settings from the hospital to home.

While we are gratified by our accomplishments over the past year, we are always looking ahead, seeking the most effective and compassionate ways to serve our patients and families. Part of this process includes strategic planning. Members of Hospice’s Operating Board of Directors and our Foundation’s Board of Trustees will be working closely with administrators to chart Hospice’s course. What you can be certain of, at this point, is that the well-being of our patients and families will be uppermost in our minds.

A Partnership in Care Benefits America’s Veterans Some swear that they can still hear and smell Viet Nam. Others shudder when you knock at their door. And many, when asked about their families, say that they have none – or haven’t had contact with them in years.

All are war veterans under the care of the palliative care team of Providence TrinityCare Hospice (PTCH), thanks to a unique partnership between Hospice and the West Los Angeles Veterans Affair Medical Center (VAMC). What started as a pilot project in 1999 between PTCH and the veterans hospital has become a full-fledged program that has allowed thousands of patients to receive disease-directed care – including chemotherapy and radiation – along with palliative and hospice care to relieve any discomfort and to ensure their dignity.

“TrinityCare was our primary partner in the project,” recounts Dr. Kenneth Rosenfeld, Assistant Professor of Medicine at the veterans hospital, who was largely instrumental for establishing the collaboration between the two organizations.

“Back when we started it up, we were seeking to change a model that offered patients little more than a terrible choice.”

Patients could either seek continued treatment, he explains, or they could stop altogether. “We wanted them to know that no one was giving up on them.”

Over the past decade, PTCH physicians and staff have worked closely with their peers at West Los Angeles VAMC to provide the best care possible to more than 1,000 patients.

While other hospices now participate in the program, PTCH assists in approximately two-thirds of all cases that fall within its service area in Los Angeles and Orange counties. That’s because Dr. Rosenfeld and his team appreciate PTCH’s approach to collaborative care.

“They have excellent communication, and they have a very good appreciation for the way the VA system works,” the physician notes. “The TrinityCare folks are also very attuned

to the continuity of relationships” that patients already have with their physicians at West Los Angeles VAMC. One of the PTCH staff members responsible for the ongoing success of the program is Sharon Moon, RN, CHPN, who serves as the chief liaison to the West Los Angeles VAMC. For the past eight years, she has been building bridges between the two organizations and making certain that patients find comfort and peace of mind.

While many of the veterans under PTCH’s care enjoy the support of families and live fairly traditional lives, Moon explains, a significant number are estranged from relatives, have little or no support system, and suffer from posttraumatic stress disorder and related conditions. “Meeting their emotional and physical needs can be challenging,” she says, but it’s a challenge worth undertaking. Whether they are living at home with spouses, squatting under freeway overpasses, or surviving on the streets, the goal “is to allow them to feel that they are in control of their environment … to preserve their dignity.”

“It’s a fine line,” she continues. PTCH physicians, nurses, social workers, and other health-care professionals must often balance what they believe is the best course for their patients with what the patients themselves want – all the while never compromising the quality of care. In one case, she says, that meant allowing one veteran to continue living in the makeshift residence in which he felt most comfortable: a garage.

“This work has changed my perspective tremendously,” Moon says, explaining that it has provided her with a broader understanding of human beings’ responsibility to care for each other. That, she says, is an added benefit to the West Los Angeles VAMC – PTCH partnership. But the greatest benefit of all, she concludes, “is that fewer and fewer veterans are dying in pain.”

Dedicated Hospice Suites As a result of Providence TrinityCare Hospice’s continuing efforts to enhance care, many of our patients and their families are benefiting from a new, innovative service: three dedicated hospice suites at Providence Little Company of Mary Medical Center Torrance.

The suites, opened in the summer of 2009, were made possible thanks to the generosity of Jacky Glass, Community Ambassador, and the close collaboration between Hospice and Little Company of Mary administrators and health-care professionals. The suites meet the needs of Hospice patients who require a higher level of care to manage pain and symptoms, and they offer a warm, comfortable and caring environment for both patients and their families. Each suite, a freshly remodeled private room, features a sleeping sofa so that loved ones wishing to remain with patients throughout the night may do so.

2009 A Year at a Glance

Friends of Providence TrinityCare Hospice frequently tell us that they support us because they believe that how we live toward the end of our lives is important… as important as at any other time during our lives. We couldn’t agree more. But what we also want to let them know is that we at PTCH could not do what we do – providing expert, compassionate palliative and end-of-life care – without their ongoing efforts and generosity.

We feel most fortunate that during the past year more friends than ever have stepped forward to lend their support. Throughout Southern California, from the northernmost reaches of Los Angeles County to the southern tip of Orange County, thousands of people, as well as many businesses, have demonstrated their concern for the well-being of our patients and families. They have organized and attended PTCHF events that benefit the programs and services we provide to patients of all ages, faith traditions and income levels.

During a period when so many in our country are coping with a challenging economy, we feel most fortunate for our friends’ confidence in us and in our mission. On behalf of all of the patients and families we serve, we extend our deepest gratitude. Your support is more meaningful – and more needed – as greater numbers of patients in need seek our care.

19th Annual Sunday by the Sea How successful was last year’s Sunday by the Sea? This much-anticipated yearly food and wine gala continues to top the charts. Attendance was up – more than 700 guests – and more than 60 top eateries and wineries participated. The best news was that $200,000 was raised to support PTCH programs and services. We offer heartfelt appreciation to Carolyn and Julian Elliott, who graciously hosted the event at their lovely seaside home. Thanks to friends’ generosity, our 19 Sundays by the Seas have generated more than $4.7 million to help our patients and families.

Night of Art And Hope PTCH’s friends on Los Angeles’ Westside showed just how much they care about our patients at a new, first-time event hosted by the Los Angeles Art Association’s Gallery 825.

A Night of Art and Hope, at which our friends enjoyed hands-on art activities, good food and live entertainment, benefited our young patients who receive services through our TrinityKids Care, our program for children with life-limiting conditions and their families.

In addition to Gallery 825, we want to thank Fox Sports’ Foxy Ladies, our partners in this event, and the many new friends and donors we met at A Night of Art and Hope. We look forward to their continued participation in our activities.

American Girl FUNdraiser at American Girl Place The FUNdraiser at American Girl Place at The Grove in Los Angeles proved at least two things: that children, as well as adults, can find ways to support PTCHF, and that adults can be kids at heart.

Youngsters, parents, grandparents and friends traveled to the doll emporium from near and far to enjoy an array of activities, and American Girl Place donated a portion of our friends’ shopping proceeds to TrinityKids Care. PTCHF is grateful for the generosity of all of our participants, our sponsor, City National Bank, and American Girl Place.

Light Up a Life One of our beloved annual traditions, Light Up a Life, attracted more friends and corporate sponsors this past year and demonstrated that our supporters in Long Beach, where the event was held, are as committed to PTCHF as those in other parts of Southern California. We appreciate the generosity of the Westin Long Beach Hotel, which hosted and underwrote Light Up Life, as well as our exceptionally talented friends in the band, Adam’s Attic, who performed for our many guests.

Far-Reaching Service In 2009, TrinityCare Hospice served more than 1,800 patients and their families, including more than 100 children on hospice.
