Your Support MAKES A DIFFERENCE! The following Providence Little Company of Mary programs and services are supported through the 33rd Annual Golf Classic: THE EDMOND R. AND VIRGINIA G. DOAK CENTER FOR PALLIATIVE CARE supports a team of professionals who provide care and pain management to patients and support to families facing a serious or life-threatening illness. Directed by Glen Komatsu, M.D., this palliative care team of medical, nursing, social services and spiritual care professionals offers assistance with treatment choices and decisions, individual and family counseling, emotional and spiritual support, and end-of-life care both in and out of the hospital. PARTNERS FOR HEALTHY KIDS mobile pediatric van has cared for more than 45,000 children since 1994 within the communities of Gardena, Hawthorne, Lawndale, San Pedro and Wilmington. This traveling clinic provides medical services for eight elementary schools, one middle school and one high school weekly, providing underserved and uninsured children with free diagnosis and treatment of acute medical conditions, immunizations, health education and health screenings. CREATING OPPORTUNITIES FOR PHYSICAL ACTIVITY (COPA) provides an opportunity during the school day and after school for students to practice skills and access support services designed to change their physical activity behaviors. The program serves all children ages 7-14 years and makes special efforts to include those who are overweight or obese. The after school program is an eight-week rotation throughout the school year and includes special events for parents and children to participate together in interactive workshops that promote physical activity. Emphasis is placed on activities that are low cost and easy for the family to do at home. COPA is currently in operation at 14 elementary schools reaching over 5,300 students annually. VASEK POLAK HEALTH CLINIC was created in 2007 to meet the primary care needs of uninsured and underinsured adults. The clinic offers an appropriate alternative to emergency room care for non-urgent conditions and a “medical home� for adults living in and around Hawthorne, particularly those with chronic conditions like diabetes and high blood pressure. Support includes preventative care, health lectures and multisession classes for people experiencing chronic illness. In 2012, the clinic saw close to 8,000 patients.

COMPETITION RULES • Foursomes will play the scramble format; each player will tee off, select the best tee shot, and repeat the process until the ball is holed. • You may move your ball within a club length (but no closer to the hole). Ball must be kept in “like conditions.” If you are in the rough, you must stay in the rough, etc. • All men will play from the White Tees. All women will play from the Gold Tees. • To be eligible for a hole-in-one, all golfers must hit from the White Tee. Golf Pros are not eligible to win. • No minimum drives per golfer required. • To speed up play, once your group has reached “double par,” please pick up. • If your ball is within “putter’s mark”, add one stroke and pick up. • Mulligans must be used for individual putting only. Mulligans may not be used on contest shots. • The maximum handicap for men is 26 and for women is 36. We will add up the total team handicap and divide by 4 and take 75% for your team handicap and that will be subtracted from your team score total. • It is the team captain’s responsibility to turn in the scorecard to the Pro Shop within 10 minutes of completing 18 holes. Failure to do so will result in elimination from the competition.

Hole#16 White Tee: 440 yards Gold Tee: 384 yards A straight away par five. Avoid trap on the right side from tee. Can be reached in two by big hitters. A “fun� hole. Hole#17 White Tee: 322 yards Gold Tee: 207 yards The longest short par four you will play - traps in front left and also right. Hard to hold and severe green. Hole#18 White Tee: 380 yards Gold Tee: 343 yards Slight dogleg right. Keep tee ball on left side for better entry to green. Right side shortens hole.

Laura Ling is a respected journalist, author and human rights advocate. In March 2009, while reporting on the trafficking of North Korean women, Ling was detained by North Korean soldiers along the Chinese-North Korean border. She and colleague Euna Lee were arrested and held captive for 140 days before being granted a special pardon to return to the United States. Laura, along with her sister Lisa Ling, coauthored Somewhere Inside: One Sister’s Captivity in North Korea and the Other’s Fight to Bring Her Home. Currently, Laura serves as the host and reporter on E! Investigates, a documentary series exploring teen suicide, the challenges of military spouses and other timely topics.
