Now that it’s spring, the kids are out playing T-ball, Little League and soccer and trying new tricks on their skateboard. Providence Tarzana Medical Center has an expert team of pediatric orthopedists should your child suffer serious injuries—on the field or off. It’s important to work with pediatric specialists to treat injuries and other orthopaedic conditions because a child’s physiology is different from an adult’s. Women’s health is a well-respected specialty at our Medical Centers. Our goal is to provide comprehensive care for all stages of life, from adolescence through menopause and beyond. Providence Tarzana, Providence Holy Cross and Providence Saint Joseph Medical Centers each provide award-winning prenatal and maternity care. Check the calendar toward the back of the magazine for the schedule of classes offered for new parents. For very different health care needs, the Providence Rehabilitation Institute treats a range of conditions from injuries to the effects of illness such as stroke, heart disease and cancer. The institute programs are available at Providence Saint Joseph and Providence Holy Cross, and are coming soon to Providence Tarzana. Also in this edition, you’ll see the dramatic stories from patients who have lost more than 100 pounds through weight-loss surgery offered by our Bariatric Wellness Center at Providence Saint Joseph Medical Center. These patients come to us facing serious illness, such as heart disease and diabetes, and through weight loss regain their health. Lastly, we’re very proud to announce that the Roy and Patricia Disney Family Cancer Center opened in February at Providence Saint Joseph Medical Center to rave reviews from our patients. Children aren’t simply little adults, especially when it comes to orthopedic care. That is why Providence Tarzana Medical Center has expanded its pediatric orthopedic Deirdre Ryan, M.D. program by welcoming Nina Lightdale, M.D.; Karen Myung, M.D.; Deirdre Ryan, M.D.; and Jennifer Weiss, M.D.; to the medical staff. All fellowship trained, and members of the Childrens Hospital Los Angeles faculty, these physicians specialize in general fracture care, sports medicine, scoliosis, hip and foot disorders, hand reconstruction, upper extremity traumas and disorders, neuromuscular diseases and more. Comprehensive Care, Close to Home People living in the North San Fernando Valley and Simi Valley will soon have a wide range of health care services available in Porter Ranch—staffed by physicians associated with Providence Holy Cross Medical Center. Opening June 1 Providence Holy Cross will bring its expertise to an outpatient facility less than seven miles west of the hospital. An independent urgent care clinic and other health care providers are already in place. “We’re looking forward to being part of the local community by offering convenient access to the services and programs people need,” says Jonathan Emerson, associate administrator for Providence Holy Cross. Outpatient Rehab Near You Providence offers a full array of advanced services The Providence Rehabilitation Institute provides a complete range of outpatient rehabilitation programs at Providence Holy Cross Medical Center, Providence Saint Joseph Medical Center and, starting early this year, Providence Tarzana Medical Center. Across Providence’s three Outpatient Rehabilitation Centers, specially trained and certified rehabilitation therapists can address the full spectrum of patient needs. Services include orthopedic and sports injury, neurological, oncology and pediatric rehabilitation, and speech and language therapy. Providence also offers lymphedema treatment, a hand clinic and vestibular rehabilitation for balance and dizziness disorders. “These are specialty services you can’t find in traditional outpatient clinics,” says Patrice M. Hallak, P.T., NCS, service area director for Neurological, Orthopedic and Rehabilitation Services. “We really pride ourselves in the specialized skill and attention our therapists provide to get patients back to their lives as soon as possible.” Providence Tarzana Medical Center offers cardiac rehabilitation by certified therapists for those who have been directed there by their physicians. The Cardiac Rehab and Fitness Center at Providence Saint Joseph, which is open to the public, offers medically supervised fitness and nutrition classes. A team of exercise physiologists, nurses and dietitians provides individualized exercise and nutrition plans for people with heart disease and other health conditions, and also welcomes those without medical conditions. Several classes are offered each day. “Our patients tell us this program is the best-kept secret in the Burbank area,” says Kristen Coblentz, supervisor of the Cardiac Rehab and Fitness Center. “Coming here to exercise is like coming home to a big, fun and supportive family.” Turn to page 18 for class listings.

Bariatric Wellness Center helps restart lives

F

or many people, losing weight isn’t simply about looking and feeling good. It could actually be a matter of life and death. Consider 42-year-old Gena Downey of Burbank, who used to weigh 230 pounds. At that weight, she had diabetes and serious sleep apnea, and she needed medication to protect her heart. But after shedding more than 100 pounds following bariatric weight-loss surgery performed at Providence Saint Joseph Medical Center, those health issues are a thing of the past. For Helen Kanataryan of Van Nuys, weighing 341 pounds was preventing her from achieving her dream of being a mother. She’d tried dieting, exercising and weight-loss pills, but it took bariatric surgery to help her reach a healthy weight—and give birth to her first child.

“I tried to get pregnant for five years,” says the 28-year-old Kanataryan. “I finally did, and it’s all because of the surgery.” Mark Farnsworth, a 48-year-old from Newhall, is another of the many success stories to come out of Providence Saint Joseph’s Bariatric Wellness Center. He lost more than 200 pounds and, last year, walked 29 miles in the Avon Walk for Breast Cancer, something he never would have dreamed of in the days when his weight exceeded 450 pounds.

coordinator. “They still have to make all the right food choices, maintain their activity levels and take the proper vitamins." In fact, the center's director, Philippe J. Quilici, M.D., a bariatric surgeon who performs minimally invasive procedures, won’t perform a procedure on a patient who isn’t serious about making lifestyle changes. “My job is to take them through surgery safely, but 90 percent of their success comes from changing their lifestyle,” he explains. “If patients are committed to losing weight, they do well.” From the very beginning, the center's expert team assesses whether a patient is a good candidate, says Sharon Sperling, R.D., bariatric coordinator. Patients are given detailed information on the available procedures and the lifestyle changes they’ll be required to make. “We give them a very accurate picture of everything they’re going to encounter,” she explains. For example, patients must follow a strict meal plan for the first six weeks after surgery. They also have to attend at least one support group session following their procedure. “We let our patients know we’re available for a lifetime of support and follow-up,” Sperling says. “Even after they reach their weight-loss goals, we’re here to answer their questions.”

Extensive Expertise Providence Saint Joseph’s award-winning Bariatric Wellness Center performs more than 1,000 procedures a year, which means patients can feel confident about the level of experience of its surgeons and other team members. What’s more, it’s one of only about 15 programs in the country certified as a Center of Excellence by both the American College of Surgeons and the American Society for Metabolic and Bariatric Surgery. The program was also five-star rated by the HealthGrades rating agency for two years in a row. The program offers all currently approved bariatric surgical procedures. These include techniques such as laparoscopic gastric banding or sleeve gastrectomies, which restrict the amount of food a patient can eat at any one time. The center also offers minimally invasive gastric bypass procedures that reduce calorie absorption to aid in weight loss. In addition, the Providence Saint Joseph program provides more aggressive procedures for extremely obese patients, as well as techniques to revise flawed or nonperforming gastric bypass procedures. “Our outcomes have proven to be very good,” Dr. Quilici says, “and we’re getting great results.”

Finding Health and Happiness To qualify for bariatric surgery, a patient must have a body mass index (BMI) of 40, or a BMI of 35-39 combined with significant health issues such as diabetes or high blood pressure. If left unchecked, obesity could turn these conditions into lifethreatening problems. The surgery and resulting weight loss can often reverse these health issues, reducing or eliminating the need for medication. “We often see type 2 diabetes go into remission almost immediately,” Subido explains. Equally important, program participants can get more out of their lives. “I just feel like I have more confidence,” Kanataryan says. “When I walk up the street, I don’t feel like everyone’s looking at how big I am. I feel so proud.”

Gynecologic Services Beginning in adolescence, regular gynecologic exams are an important aspect of preventive care. Routine screenings such as Pap smears can detect cancer or precancerous abnormalities that can be treated more effectively when found early. The experienced gynecologists at Providence Holy Cross, Providence Saint Joseph and Providence Tarzana Medical Centers are prepared to handle everything from basic annual exams to advanced surgical procedures. In fact, Providence Saint Joseph performs the latest in single-incision laparoscopic surgeries for hysterectomies and gallbladder removals. These minimally invasive procedures typically involve less pain, faster recovery and minimal scarring. Richard L. Friedman, M.D., a gynecologic oncologist, performed the first laparoscopic hysterectomy at Providence Saint Joseph last year, cutting a half-inch single incision in his 50-year-old patient’s bellybutton for surgery to detach her uterus. Aided the entire time by a high-tech device and a computer screen image, Dr. Friedman then removed the woman’s uterus, which is a pear-sized organ, through her vagina. “Hysterectomy patients can go back to work in a week or two with this technology, compared to a six-week recovery time after traditional surgery,” he says. “There’s much less pain and you don’t have a visible scar.”

Breast Health Largely due to early detection and treatment, deaths from breast cancer have been on the decline since the 1990s. And Providence provides the most advanced diagnostic technology available, including digital mammography, which offers numerous advantages over traditional mammography. “The image resolution is better and can show a lot more detail,” says Carrie Sayeski-Reid, manager of the outpatient diagnostic center at Providence Holy

Cross, which recently upgraded to all-digital mammography and added digital stereotactic biopsy capabilities to its updated women’s imaging suite. “It’s especially good for women with very dense, hard-to-image breasts, because the higher image quality delivers a much more thorough study.” Many women also find digital mammography more comfortable because the equipment is warm and the entire process is faster (since there’s no film to be developed). Digital mammography is also available at Providence Tarzana and at the Providence Saint Joseph Breast Health Center. At the Breast Health Center, every minimally invasive breast diagnostic procedure can be performed in a single location, making the process more convenient and less stressful. The center’s services include biopsies, fine-needle aspiration, ductograms and cyst aspiration. Noninvasive imaging such as breast ultrasounds and PET scans are also available.

Obstetrics Care Having a baby is a life-changing event and should be a joyous and comfortable one. At Providence, highly skilled obstetricians and care providers work in a setting that combines the latest medical technology with all the comforts of home. This includes Labor/Delivery/Recovery (LDR) suites equipped with state-of-the-art monitoring and delivery capabilities at Providence Tarzana and Providence Saint Joseph Medical Centers, which also feature amenities such as flat-screen TVs and other conveniences for mothers and their families. In addition, Providence Holy Cross has been designated a “BabyFriendly” hospital by Baby-Friendly USA, the authority for the implementation of the Baby-Friendly Hospital Initiative, a global program sponsored by the World Health Organization and UNICEF. Providence Holy Cross will have LDR suites when the new Women’s Pavilion opens later this year. In addition to support for breastfeeding moms, all of the Providence locations advocate keeping mother and baby together in the hospital to promote enhanced bonding and

continuity of care. Mothers also have choices about the type of delivery they would like to have, including traditional, natural and doula-guided childbirth, which uses a nonmedical assistant, such as birth assistant or labor support specialist. These are among the many reasons why babies born at Providence hospitals are now returning as adults to have their own babies. “Everything is high quality, including our equipment and our staff,” says Tanaz Kahen, M.D., chair of obstetrics at Providence Tarzana. “Everybody is board-certified, and we’re very strict in our selection criteria.” This same level of excellence is in place at the other two Providence Health & Services’ Valley Service Area locations. What’s more, Providence Tarzana and Providence Saint Joseph each have a Neonatal Intensive Care Unit (NICU) that can address the needs of critically ill babies, including those born prematurely. Providence Holy Cross is opening a 12-bed NICU later this year.

Incontinence Treatment Urinary incontinence, or the involuntary loss of urine, is a debilitating diagnosis, affecting 15 percent to 50 percent of women of all ages. It can severely affect a woman’s quality of life, preventing her from enjoying activities with family and friends for fear of embarrassment. The risk of developing incontinence increases with aging, obesity, diabetes and childbirth. Fortunately, many advances in the treatment of incontinence have been made in the past 10 years to offer women relief. “When patients come to my office, my goal is to get them completely dry,” says Jennifer Mehdizadeh, M.D., a urologist at Providence Tarzana Medical Center. At her office, Dr. Mehdizadeh can conduct a comprehensive evaluation, including diagnostic tests to determine the etiology of the incontinence. There are many forms of urinary leakage including stress, urge, overflow, functional and mixed incontinence; each requires a different kind of treatment.

For stress incontinence, when urine is lost during physical exertions such as sneezing or coughing, there are exercises to strengthen the pelvic floor that can be taught in the doctor’s office. For women who do not improve with the exercises, simple in-office or outpatient procedures are available. The injection of a bulking material around the urethra to thicken the wall and help keep the urethra closed can be performed in the office. Another option is known as a “suburethral sling,” which is a mesh hammock placed under the urethra near the bladder neck to add support and help close the sphincter muscles. These options are minimally invasive. Urge incontinence is the leakage of urine following a sense of urgency. Women with urge incontinence often have their daily activities disrupted by the urge to urinate at frequent intervals and will lose bladder control before reaching the toilet. Conservative treatments such as bladder retraining, diet modification and medication are very effective. Unfortunately, some patients don't get relief from these therapies, but Dr. Mehdizadeh is able to provide other options including neuromodulation that can help get bladder urgency under control. The two methods for modifying bladder nerve activity and reducing urgency are via the sacral nerves near the tailbone or the tibial nerve in the ankle. The sacral stimulator involves the placement of a small lead into the tailbone attached to a battery placed in the buttocks which can be programmed to optimize stimulation. The tibial nerve stimulator involves weekly office visits for six to 12 weeks, during which the bladder nerves are stimulated via the nerves in the ankle. “The neuromodulation treatments have drastically improved the lives of many patients who thought they had no other options. Patients should seek out specialists like me who are specially trained to provide these treatments when conservative therapies have failed,” says Dr. Mehdizadeh. “There are more options available today than ever before, so there’s no reason for women to endure incontinence as a way of life.”

Osteoporosis Care Osteoporosis is a very common disorder in women—and it is a disease, instead of being part of the normal aging process, as experts used to believe, says Hafeez Parray, M.D., an internist at Providence Holy Cross Medical Center. When women develop osteoporosis, which usually begins after menopause, their bones become more fragile and at risk for fracture. But when detected early, osteoporosis can be treated before it advances to a painful or potentially debilitating state. An easy 10-minute test, which Dr. Parray performs in his office, measures bone density to diagnose osteoporosis. If a woman has osteoporosis, he recommends a three-pronged treatment approach—and the first two elements of treatment are also important preventive measures. First, women need regular weight-bearing exercise such as walking, jogging or gentle weightlifting. The second aspect of treatment is getting enough vitamin D and calcium. Dr. Parray recommends that women get between 1,500 and 1,600 milligrams of calcium and 800 IU of vitamin D daily. If they can’t tolerate milk products, calcium-fortified foods or supplements are an option. The third part of treatment is medication, most of which is taken in pill form once a month. Treating a Pain in the Leg

Peripheral arterial disease can be serious, but it’s easily diagnosed and treated Anyone can get a little sore after exercise. But if a short walk causes you leg pain or you’re awakened in the middle of the night by foot pain, you might be one of the millions of Americans who suffer from peripheral arterial disease (PAD). The leg arteries in people with PAD are clogged with fatty deposits that can limit or block blood flow. This can cause pain that may become debilitating, says Sasan Najibi, M.D., a vascular surgeon and medical director of the Vascular Diagnostic Center at Providence Saint Joseph Medical Center. “It’s a chronic problem that slowly progresses and limits how much a person can walk,” he explains. “Then it becomes a vicious circle because as people walk less, they become less fit and the problem gets worse.”

Expert Treatment If left untreated, PAD can lead to painful lower-limb sores or gangrene, both of which can become severe enough to require amputation. What’s more, PAD is a risk factor for heart attack and stroke. Fortunately, treatment is available at Providence Holy Medical Center, Providence Saint Joseph Medical Center and Providence Tarzana Medical Center. The first course of action is usually nonsurgical, Dr. Najibi says. The treatment involves exercise therapy—a strict regimen of walking for 30 minutes a day uninterrupted—in conjunction with medication. In more serious cases, or if medical therapy doesn’t work, surgical options can restore blood flow. The minimally invasive procedures available include the insertion of stents and the removal of clots or blockages. Bypass surgery is another option; however, it is usually reserved for people whose PAD is limbor life-threatening. “PAD shouldn’t be ignored,” Dr. Najibi says. “We have a number of options for treating it successfully that can improve mobility and decrease the risk of heart attack and stroke.”

Checking for PAD There are several screening tests to detect peripheral arterial disease (PAD) that are simple and only take a few minutes. Providence Saint Joseph Medical Center offers low-cost vascular screenings, including: Carotid Artery Ultrasound—to detect plaque and/or arterial narrowing that may increase the risk for stroke. Abdominal Aortic Ultrasound—to determine the size of the abdominal aorta. A nkle-Brachial Index (ABI)—to compare the blood pressure at the ankle with that in your arm to determine if there is significant lower extremity arterial narrowing or blockage. All screenings are provided by registered vascular technologists and Board-Certified Vascular Surgeons on staff at Providence Saint Joseph Medical Center. • The Care Can’t Wait campaign is in progress at Providence Holy Cross Medical Center, which expands the hospital to nearly 400 beds and upgrades technology. It also enhances patient services throughout the facility such as cardiology, operating rooms, women’s services, GI lab and more. The medical center also is building a new 12-bed neonatal intensive care unit (NICU). The new chapel will feature an original Lalo Garcia mural as a tribute to Our Lady of Guadalupe. “If you have an affinity for a certain area of care or a particular medical center, contact that foundation directly to find out what campaigns you can give to,” Meyers says. “Funds are always needed and any support is appreciated.”

All in the Family As Providence Health & Services, California, continues to grow to serve the community, so does the need for funding. “While other hospitals are closing or cutting back on their services, we’re moving ahead and building,” says Theresa Meyers, vice president, Providence Health & Services Foundation. “The only way we can do that is through the philanthropic support of the communities at each of our medical centers.” Projects currently under way include: • At Providence Tarzana Medical Center, a campaign is under way to raise $8 million to establish a Pediatric Intensive Care Unit (PICU). Through a collaboration with Childrens Hospital Los Angeles, Providence Tarzana will be equipped to treat critically ill children with state-of-the-art pediatric practices and will be the first certified PICU in the San Fernando Valley. • At Providence Saint Joseph Medical Center, about $3 million in contributions are needed to complete the $11 million campaign for the Hycy and Howard Hill Neuroscience Institute. The institute has already received numerous awards and was recently designated an Approved Stroke Center (ASC), recognizing Providence Saint Joseph’s commitment to providing advanced neurological care.

The campaign for Providence Saint Joseph Medical Center’s Roy and Patricia Disney Family Cancer Center was a huge success, raising $38 million in less than three years. But there will always be a need for funding—and the campaign demonstrated that everyone can help.
