SPENDING six months last year in medical limbo before finally receiving a diagnosis of a rare paediatric form of multiple sclerosis (MS), Wangaratta teenager Katelyn Papakostas is passionate about raising funds for research into the condition, as well as raising awareness. She first went to the doctor in February 2018 after experiencing tingling sensations in her leg, along with weakness and even falling asleep in class, but said doctors initially believed it was related to her anxiety. After having an MRI, doctors believed she may have had a tumour, and she soon had further tests done at Melbourne’s Royal Children’s Hospital. While the tumour was ruled out, medical teams then thought she had demyelination (loss of protective covering) of a nerve in her back, but when symptoms recurred, she was finally diagnosed with MS in August last year. Multiple sclerosis in young people is rare, with only about five per cent of people with MS experiencing disease onset before they reach the age of 18, but Katelyn is philosophical about her situation. “The main thing now is just living with it everyday,” She said. “I have a different outlook on things now.” Balancing her medical needs with her Year 12 studies, Katelyn has plenty of big plans for this year.

Cherry eye A CHERRY eye is a colloquial name for a prolapse of the third eyelid gland in the dog. It presents as a pink bulge at the inner corner of one or both of the eyes and looks somewhat like a cherry, especially if it becomes red and inamed with exposure over time. The gland is part of the immune system and can become swollen if the dog encounters allergens in its eyes, which is quite common in curious young puppies. Swelling of the gland alone is not sufficient to cause a prolapse of the gland. The gland must have poor anatomical anchoring within the eye socket as well for the prolapse to occur. This can happen in lots of breeds but seems to be most common in shorter nosed breeds like bulldogs and boxers.

The gland may prolapse intermittently or become permanently prolapsed over time. If it is prolapsed for a longer period it usually dries out and gets red and sore. Correction of this problem usually requires surgery, as the underlying issue is a lack of good anchoring of the gland that should hold it in its normal position even if it swells up. There are several techniques that can be employed surgically depending on the severity of the prolapse. This condition tends to crop up in early life and rarely resolves on its own or with medical management alone.

The rain we needed

RAIN, glorious rain. It was truly amazing to once again hear the sound of rain on the roof last week - but what does decent rainfall at this time of the year mean for us fishermen? Well first off, one of my favourite yabbying dams was bone dry last week, however it now has 20cm of water in it. One of the biggest benefits of decent rain at this time of the year is that it increases stream flows, making it easier for trout to swim upstream, find a partner and spawn. Brown trout usually start spawning around late May/early June, and usually start to pair up about now. Rainbow trout usually start around six weeks later, in the depths of winter. Thankfully now they have enough water to support a productive spawning season this year. The trout fishing has been excellent of late, but over the next few weeks as they prepare to spawn the trout fishing may become quite inconsistent from stream to stream. Tim was fishing as the water was rising when he caught the fish. I have not had any further reports since the dirty water arrived. Once again, Lake Mulwala is leading the way with Murray cod fishing reports. I have had several reports of these favoured natives being caught down there and have seen photos on social media of some absolute monsters. My friend Christian Charnstrom caught a few down there last weekend while participating in a competition, including a magnificent 73cm Murray cod.

She has begun a fundraising page under the Kiss Goodbye to MS campaign, while relatives in Sydney have also set up fundraising in her name under the May 50K campaign, where participants aim to walk or run 50km during the Month of May. Katelyn, her Mum Tina, dad Arthur, sister Jennifer and brother James, are also planning to participate in the MS Walk and Fun Run on June 2 in Melbourne, and Katelyn is looking forward to celebrating her 18th birthday on June 15 by participating in a Sydney Harbour Bridge climb. She has also worked together with Galen College to organise a bake sale fundraiser on May 30, which is World Multiple Sclerosis Day. While her symptoms mainly consist of fatigue, shooting pains and occasional tremors, Katelyn said MS symptoms often range between individuals. “I want people to know that adolescents and kids can get it as well, and every single case is

completely different,” she said. She added that her coping methods for dealing with her multiple sclerosis were sleeping when necessary, going to the gym regularly, and maintaining a healthy diet. Katelyn said the best way people can help those with MS is to treat them like everyone else, while still being sympathetic to their needs. “Don’t treat anyone any different,” she said. “Everyone’s the same, it doesn’t matter.” Katelyn said she was lucky to have the support of her close friends and her school - Galen College in Wangaratta, as well as her family. When she’s not busy raising awareness, Katelyn is also a committed musician, particularly with drums and violin. NOVARTIS recently announced that the Therapeutic Goods Administration (TGA) has approved Gilenya® (fingolimod) for the treatment of children and adolescents 10-17 years old with relapsing forms of multiple sclerosis. The registration makes Gilenya the first licenced disease-modifying treatment for children and adolescents in Australia. MS is the most commonly acquired disease of the central nervous system among young adults in Australia. “The debilitating symptoms of MS can severely limit children and adolescents’ ability to participate in normal day-to-day activities such as going to school,” said Paediatric Neurologist Associate Professor Andrew Kornberg. “This milestone is welcome news to both the medical community and those affected by the disease.” The TGA approval of Gilenya is based on the results of the PARADIGMS Phase III study, published in the New England Journal of Medicine (NEJM) in 2018.

A balanced approach to food A VERY worrying trend we are seeing in clinic is the misguided thought that food is classified as either bad or good and the shame and guilt that people are displaying if they, or their family consume that “bad” food. In 1998 the term orthorexia was coined, and a new kind of disordered eating pattern has been emerging ever since. Orthorexia is an obsession/constant thought about if a food is healthy enough. We all know someone who is obsessed with quitting sugar, no carbs, paleo, or cutting out food groups for no reason other than it being “bad”.

They will research if there will be anything for them to eat at a restaurant and spend hours planning their meals and contemplating if they are eating purely enough. They also obsessively follow socalled “health experts” on blogs and social media and take their uneducated word as gospel, leading them down a slippery slope of nutritional deficiencies and unhealthy thought patterns. This belief and pattern is being role modeled to the next generation, and I worry about what that may develop into. Using words like “sometimes” food or “everyday” food can be helpful

when educating children. Showing them that food can be enjoyable and provides you with nutrients, energy and fuel to not only get through the day but thrive is important. Food should be mostly living, that is, something that has been picked, pulled out or was once walking this earth, not numbers and preservatives, that is for sure. Keeping these foods to a minimum is of course optimal, but when this becomes an everyday/minute thought then maybe some self-reflection and seeking some help might be in order. We are here to listen and help.

6 ways to move past employee engagement to employee experience EMPLOYEE experience is following employee engagement as the next big thing in getting the most out of staff and that requires a more holistic view, according to recruiting experts Hays. Businesses are increasingly moving away from a focus on engagement to a more holistic model, known as employee experience. “It’s an approach that still includes engagement at its core, but also covers culture and performance management,â€? explains Nick Deligiannis, Managing Director of Hays in Australia and New Zealand. Hays spoke to several HR experts and shared the following advice in its Hays Journal on how this more comprehensive strategy can be put in place: 1. Focus on individuals ďŹ rst: Presenting a holistic experience, from the moment an employee arrives (or even before that) to the moment they leave, can be complex. “Experience is a sum of all interactions – but connected interactions, which means organisations have to be more thoughtful about how they set up their workplaces,â€? argues Ben Whitter, Founder of the World Employee Experience Institute. “The key is that experience has to start by ďŹ rms focusing on the individual ďŹ rst, rather than them being the last port of call, but this is easier said than done.â€? 2. Take a ‘vitality audit’: According to Jackie Jones, Living Business Design Director at Fjord, the Accenture-owned innovation consultancy, ďŹ rms really must start from scratch to

redeďŹ ne all their processes. “Most companies have policies that now seem very clunky,â€? she says. “Given employees now demand greater personalisation – where employees expect companies to know about ‘them’ and what their skills path needs to be – it’s easy to see that an end-to-end approach is needed.â€? To do this, Jones says ďŹ rms need to take a ‘vitality audit’ – to understand all the things that affect the personality of a business 3. Put responsibility on team members: Mathew Paine, Director of Human Resources at the International Convention Centre Sydney (ICC Sydney), says that putting responsibility on team members allows businesses to better match employee expectations of growth. 4. Offer an individual experience to each employee: HR can’t just pick a policy and think it’s got experience covered. Janelle Reiko Sasaki, Executive Director, Diversity & Inclusion Services, EY Advisory & Consulting says that in Japan a shrinking, ageing population and a low birth rate means that focusing on employee experience is a must if they wish to attract and retain talent. She says that trying to offer a more individual experience to each employee is key. 5. Ask staff what they want: Making everyone responsible allows for what Simon Fanshawe, co-founder, Diversity By Design, calls the ‘experience deďŹ cits’ in businesses to be addressed – everything from what people feel are blockers to advancement to how staff feel. The history of the keyboard’s strange layout ON occasion, when you’ve had reason to use a computer, or have been typing away on your phone, you might have wondered just why it is that the letters on the keyboard are arranged in such a seemingly strange way. Most of the time, the thought has likely never crossed your mind as we’ve all simply become conditioned to the layout. But there is a story behind this seemingly random arrangement of keys - the QWERTY keyboard - and it’s probably not the one you’ve come across if you’ve ever been curious enough to look into it. The world’s love–hate relationship with the QWERTY keyboard began in a small workshop in Milwaukee in 1866. It was at this time and in this place a publisher called Christopher Latham Sholes began work on an invention he hoped

would make him wealthy - a machine to automatically number the pages of books. Sholes was joined by an inventor friend called Carlos Glidden. In July 1867, he happened to read a short description of a “type-writing machineâ€? in ScientiďŹ c American. It appeared to have inspired them to change course and create “a machine by which ‌ a man may print his thoughts twice as fast as he can write them.â€? A year later they were in possession of three patents. You’d be hard pressed to recognise their creation as a typewriter, though, as it looked like a miniature piano crossed with a clock and/or a phonograph and/or a kitchen table. Sholes did, in fact, design the prototype out of his kitchen table. As you can imagine, it didn’t

boast what today’s designers would call “intuitive UX.â€? The machine was prone to jamming and the lines of type tended to drift off course, but Sholes used it to write to potential investors. One of them, James Densmore, immediately bought a quarter share of the patents, sight unseen. But when he got to Milwaukee to cast an eye over his investment, he was less than impressed, declaring it to be ‘useless’. Nonetheless, Densmore believed in the general idea and urged Sholes to continue. What happened next is a little unclear. Sholes ďŹ led another patent in 1872 which shows the piano keyboard had been dropped in favour of rows of circular keys, but it did not specify which letter was where. Then, almost out of the blue,

QWERTY (almost) appeared. In August 1872 ScientiďŹ c American published a glowing article about the “‘Sholes’ Type Writerâ€?, illustrated with an engraving of the machine showing a four-row keyboard with a second row starting ‘QWETY’. It is believed that Sholes was intent on winning over the stenographer demographic - he assumed such workers were the likeliest customers, and that they would do the most to improve the product - so he did the natural entrepreneurial thing and hired a consultant. He sent James Ogilvie Clephane, a renowned court reporter in Washington, D.C., an early model and told him to go to town on it. Clephane did; so unforgiving was his ďŹ eld-testing that he destroyed a succession of costly prototypes, and he told Sholes in no uncertain terms

that his typewriter was garbage. But his brutal feedback may have began the long process that culminated in the QWERTY design - a design intended to slow down the process of typing in order to stop the early machines from jamming and breaking. Densmore demonstrated the typewriter to engineers at E. Remington & Sons, a gunmaker based in New York that had branched out into home appliances. Remington signed a contract to manufacture the machine, and produced a prototype with another slightly different keyboard: QWERTUIOPY. Sholes was apparently unhappy and demanded that the Y be reinstated between the T and the U. Remington agreed, and for the ďŹ rst time QWERTY came together.

Remington put its No. 1 Type Writer to the market in 1874 and it quickly became the world’s ďŹ rst commercially successful writing machine. Nowdays, the issue of our computer keyboards jamming up due to the speed of typing is no longer an issue, but the QWERTY layout has become so ingrained in our technological devices that to change it would cause more problems than it solves. Additionally, while it was always common wisdom that QWERTY slowed typists down, studies have shown that the QWERTY layout still allows the fastest touch typists to reach 90 per cent of theorectical maximum typing speed - so the intention to slow people down was never all that effective to begin with. So, whether to love it or hate it QWERTY is here to stay.
