By KYLIE WILSON SHE spends most of her life getting to grips with other people’s feet, but for North East woman Brydie Donnelly, it’s all in a day’s work. Now a clinical lead podiatrist working across a number of locations in the North East, Brydie started her podiatry career working in remote communities and towns across the Kimberley in the north of Western Australia. “During this time I developed a great interest in chronic conditions and their impact on the feet, and saw many people who had complications relating to diabetes and chronic kidney disease,” she said. “I also became acutely aware of the inequities in health care that exist because of location, culture, or cost. “These inequities may be more obvious in areas of Australia where remoteness and isolation exist, but these inequities exist all over Australia, particularly for Aboriginal and Torres Strait Islander people.” Brydie’s interest in the broader health care space led her to complete a Masters in Public Health (Primary Health Care) and since her early career, she has worked in a range of settings and locations across Australia, including community health, high risk foot clinics, private practice, aged care, high and low security prisons, and Community Controlled Aboriginal Health Organisations (ACCHO). “My passion is in public health and provision of care that is appropriate and accessible for those most in need of care,” she said. Brydie is now part of a five person podiatry team at Gateway Health Service who provide clinics across the catchment, including in Mansfield, Benalla, Wangaratta, Myrtleford, Bright, and Wodonga, as well as at some aged care services in the region.

Ultrasound for pets ULTRASOUND is a common technology used in human medicine. It also sees widespread use in veterinary medicine as well. There are many situations where it can be employed either rapidly in emergencies, or in a more measured examination looking for a diagnosis in complex medical cases. The benefit of ultrasound is that it is non-invasive and usually can be used with the patient conscious or lightly sedated. It has replaced exploratory surgery in some cases or allows us to go into an abdominal surgery with a lot more information in hand. Ultrasound is used in pregnancy diagnosis in many species. Like in people, you can get an idea of whether an animal is pregnant, whether they are carrying twins (or more) and how the foetuses look like they are developing. It is used extensively in horse breeding where mares need to be prepared to be transported to stal-

lions for breeding at the absolute right moment in their reproductive cycle. In emergency situations, the ultrasound can be used to look for bleeding or fluid accumulation in the chest or abdominal cavities. There are accepted emergency “quick” ultrasound protocols looking rapidly for problems after traumatic accidents, like when an animal is hit by a car. In complex medical cases, the ultrasound is used to examine the organs of the abdomen or chest. It can look for problems in the bladder, liver, stomach, intestines, kidneys, pancreas, heart and many more places. It is an invaluable tool for noninvasive diagnosis in many medical situations.

WINTER is often a time to prune our roses and salvias, clean up leftover leaf litter from autumn, and weed spraying and mulching for this coming spring. However, often lurking amongst that debris can be helpful little critters that are waiting for the first burst of spring to launch into action. Centipedes will eat insects, slugs and snails and are found living in decaying leaf litter around our shrubs and trees. Who doesn’t find watching spiders spin their delicate webs around our gardens fascinating? These creatures come in many shapes and sizes and love munching on flies, moths, mosquitoes and

beetles who wander haplessly into their clutches. The pretty little lady beetle which hibernates over winter in nooks and crannies of bark, twigs, window sills - even cozy spots in sheds - are waiting for their favourite meals of aphids (also known as greenflies). Aphids love to nibble on our budding roses and fresh new growth of many common plants around our gardens and studies have shown that due to the sap sucking nature of these critters they can also pass on viruses from one plant to another. Some varieties of aphids don’t always lay eggs, they instead give birth to live young which are capable of producing a week after birth

resulting in large population explosions when food is in abundance! Talk about the mice of the insect world! Parasitic wasps are very clever mummies who like to lay their eggs into bodies of other insects like aphids and caterpillars which in turn provide free food and then hatch out killing the host insect. Gory! Or there is the all-seeing praying mantis that emerges from eggs in spring and when fully grown love to munch on insects up to half their own body size. So spare a thought when cleaning up for the good guys hiding, waiting for the chance to make a difference in our gardens.

This Day in History, 1930 ON July 31, 1930, the Shell Company officially opened an aircraft refuelling centre at Bowser Airstrip near Wangaratta. It was the first in Victoria, and possibly the first of its kind in Australia. With the inauguration of a scheduled passenger service between Sydney and Melbourne, Bowser was considered by Shell to be an ideal refuelling centre, and an emergency landing ground in case of head winds and bad weather.

Coffee and your health DISCLAIMER: I drink coffee. Yes, I am a naturopath and I drink coffee. I am also a relatively new mum and a business owner (two of the key requirements for coffee consumption) but, before I was any of these I loved my coffee dates. I always have a little internal giggle when doing diet reviews with patients and we get to the inevitable question of “do you drink coffee?” followed by “spill the beans (ha), how many do you really have a day?.” This is because everyone gets this little guilty caught-out look on their face and without giving me a direct answer, they say “I know I shouldn’t have it…but….” And my patients are all pleasantly surprised by my response, which is, “I don’t mind if you keep drinking it,

it’s the dose that’s the issue not the coffee.” We then spend a few moments looking at the effects of caffeine and the appropriate quantities (everyone has a different upper limit for this delicious brew and this limit changes for each person depending on their circumstances at any particular point in time). We then set some the three golden rules about when NOT to consume it: 1. Not when you’re tired Not even the morning after a bad sleep. And definitely not after 12pm. With a half-life of six hours in the average person, caffeine has the potential to hang around and wreak havoc for a long time. 2. Not when you’re stressed Because your stress response has

already ideally given you the appropriate medicinal shot of cortisol to resolve the stress, by adding in a double shot latte, you’re just going to over-rev with no additional benefit. 3. Not when you’re hungry An empty stomach fast tracks caffeine absorption, peaking in 45 minutes. It’s an awful slippery slope to a big crash and further cravings. What’s that you’re saying? That the top two rules are the most common situations when you might reach for it? I know! But this brings out the very worst in coffee and the very worst in us. Never fear, there are tools aplenty to curb caffeine addiction, withdrawal effects and even recharge your energy levels so you can still have a cup or two a day - Hoorah.

“The podiatry team are involved in providing general care to people with chronic conditions, who are at risk of foot complications without this care. “Foot complications can be caused by a number of chronic conditions, most notably diabetes, and can result in pressure areas, callouses, wounds, infections, and if not managed in a timely and appropriate manner, amputation,” Brydie explained. “Podiatry in the community health setting has a role in managing those at risk to assist in providing general care, and support to self manage where appropriate. “Community podiatry can provide care for many conditions, including wounds and biomechanics, and also has a role in referring to more specialist services where appropriate, such as to high risk foot clinics in the case of complex ulceration or infection.” Brydie said she is passionate about helping people care for their health, and loves how diverse her role can be. “I love the challenges of community

How to successfully ask for a pay rise TO ensure your pay rise request is successful, there are certain steps you must take, advises recruiting experts Hays. “Asking for a pay rise can be nerve-racking,” said Nick Deligiannis, managing director of Hays in Australia and New Zealand. “Many people avoid it altogether because they find it too difficult to broach this topic, while others prepare incorrectly and so fail to maximise their worth. “But with the latest data showing that the overall value of pay rises is falling, Australians are increasingly prepared to have this tricky conversation with their boss in order to improve their earnings.” Here’s Hays’s advice on how to successfully prepare for your pay rise request: 1. Gather your supporting evidence “Firstly, for your salary increase request to be successful, you need to show your boss why you deserve a raise,” says Nick. “It’s not enough to say the cost of living is increasing or that you’re generally doing a good job. “You must have specific and quantifiable evidence to present to your boss.”

Nick suggests you ask yourself, “What have I achieved since the last time my pay was increased that warrants a raise today?” To answer this question, prepare a list of your recent achievements that exceed your objectives. It may help to look back at your last review or your original job description. Then list any changed or rising work volumes or duties you’re now undertaking and consider projects you’ve been involved in. For each accomplishment, list the resulting benefit to the organisation. The aim here is to provide strong evidence to justify a pay rise, so focus on outcomes. For example, perhaps you have brought in 22 per cent more business year-on-year, are managing a 25 per cent increase in the overall volume of work or were involved in a project that exceeded objectives. “Whatever evidence you gather, the key point to remember is that it needs to demonstrate the greater value you now bring to your employer,” advises Nick. 2. Research typical market salaries Next, research the salary you feel your performance and results are

worth. This enables you to back up your request with evidence and demonstrate that the salary you are asking for is in line with current market rates. 3. Set the meeting and remain professional throughout You are now ready to ask your manager for a meeting to review your salary. Don’t spring this conversation on your boss though, warns Nick. “She or he could be in the middle of an urgent task or their attention could be required elsewhere. “Instead, book a time with your manager and clearly state that the objective of your meeting request is to present your case for a salary review.” When it comes time for this meeting, Nick suggests you keep it professional. “Take control, but stay calm and focused,” he said. “Do not become emotional and do not talk of how much money you need, such as rising bills or the cost of living. “Instead, clearly present the evidence you’ve gathered to support your pay rise request. “If you’ve gathered appropri-

ate evidence, you’ll have strong grounds for an increase that are hard to ignore.” Nick also suggests that preparing a sheet of paper on which you document your evidence will help keep your boss from altering the trajectory of the meeting. “If you are feeling nervous, it will also provide you with notes to refer to so you don’t forget to present all your proof.” Don’t expect an answer straight away though. “In all likelihood, your boss will need to review their budget, talk to HR and draft the necessary documentation before your pay rise becomes official,” says Nick. “At the conclusion of the meeting, let your boss know that you’ll follow up with an email summing up your request. “Your email should be a clear, concise and accurate summation of the main points you presented and discussed. “This provides you with a written record of your conversation and ensures there’s no room for confusion or misunderstanding.” 4. Be prepared to negotiate Your boss may want to negotiate the value of your salary increase.

THE Federal Government released its final report in the Digital Platforms Inquiry at the end of last week, just days after the US Federal Trade Commision (FTC) fined Facebook $5 billion for violating consumer privacy during the 2016 Cambridge Analytica scandal. The report outlines numerous recommendations by the Australian Competition and Consumer Commission which the Federal Government will consider over the next 12 weeks. It’s then expected that the government will make its final decisions about those recommendations by year’s end. The report itself contains 21 recommendations, with a heavy emphasis on providing local news media organisations some avenues of competitiveness in order that they can continue to provide their communities with localised journalism, consumer data privacy, the dissemination of reliable information on social media, and methods for increasing digital literacy for the broader population. Facebook and Google should fund Australian journalism One of the ACCC’s major concerns was that the tech giants are able to share the work of journalists employed by local media organisations and thereby ultimately remove profits from those organisations. In response, they have recommended the platforms contribute to a $50 million fund for the specific purpose of injecting money back into regional news media. “(It would be) a targeted grants program that supports the production of original local and regional journalism, including that related to local government and local courts,” the ACCC wrote. “The program should be platformneutral and administered at arm’s length from government, with eligibility criteria designed by an independent expert committee.” In its overview, the report stated that such journalism was, “important for the healthy functioning of the democratic process”. The collection of data to have a new set of rules Among the most significant recommendations were those related to data collection and dissemination. “The ACCC is further concerned with the large amount and variety of data

which digital platforms such as Google and Facebook collect on Australian consumers, which go beyond the data which users actively provide when using the digital platform,” the ACCC said in its preliminary report. The recommendations in the report called for social media platforms to be less opaque concerning data collection, while also speaking to the notion of ‘data portability’ - the ability for users to have more control of their data even after it has been collected. Default web browers and search engines to be scrapped At present, Google Chrome is the default, preloaded browser for many devices. The ACCC would see that change under a recommendation similar to that which has been enacted in Europe. The proposed change would allow users to select their default browser upon initial set up. A new watchdog unit and five year inquiry A significant problem over the past decade and a vastly changing digital landscape has been the government’s inability to track changes and respond efficiently. The ACCC proposed that a new ‘spe-

cialist digital platforms branch’ be set up within its own organisation. The new unit will police anti-competitive behaviour by the digital platforms and take action to enforce consumer laws. In addition, it has called for a new independent Ombudsman to be set up to handle complaints from consumers. Alongside this, a new five year inquiry would track changes over the next few years, allowing the government to make informed policy changes as technology develops. One the whole, the ACCC seems to have delivered a positive raft of recommendations - ones which shy away from severely interfering with the platforms technologies or company structure though it remains to be seen which of them will be enacted by the Federal Government. Facebook and Google’s on shore advocacy groups have warned that the measures, if enacted, could impact technology investment in Australia, but the platforms have not responded with more specific criticism of the recommendations at this stage.
