Welcome

Much has happened at Phyllis Tuckwell over the last few months and we have a great deal of exciting and encouraging news to update you on. As you will know, our primary focus is always our patients and families, and we work hard to offer them the best care. You can read about how Vickie’s husband Nigel was looked after on our In-Patient Unit, on page 3, and how we have helped Day Hospice patient Adrienne, on page 8. To ensure that our care is the best it can be, we are continually reviewing and developing our services. Our recent focus has been on the introduction of ‘locality working’, the formation of our ‘Living Well’ team and the work which we have completed in partnership with SECAmb (South East Coast Ambulance Service), all of which help us offer appropriate and accessible care for patients, and reduce unnecessary hospital admissions. Page 6 has more details. We have received a great deal of external recognition of our work recently, with two members of staff winning awards at the Eagle Radio Biz Awards, one of our doctors being appointed as an honorary teaching fellow at the University of Surrey, and a CQC inspection of the services based at our Beacon Centre awarding us an ‘outstanding’ rating! Read more on pages 4-5. Hand-in-hand with our expanding services are our fundraising events, which help to support this improved care. Pages 10-11 tell you more about what we are up to and how you can get involved, with pages 12-13 containing detailed information on both recent and forthcoming events. Once again, thank you for your continued support of PTHC.

Making the Most of Life “He was fun, full of life and irrepressible, with a wicked sense of humour,” says Vickie, whose husband Nigel died on our In-Patient Unit (IPU). “And he was terribly naughty! I think there are quite a few people here who’d remember him. He was a wonderful man and I loved him to bits.” Nigel was admitted to the Hospice eight months after his first symptoms of pancreatic cancer. “When he was diagnosed, his doctor referred us to Phyllis Tuckwell straight away,” says Vickie. “Within 24 hours Phyllis Tuckwell’s Medical Director, Dr Cate, was on the phone, offering to come and see us.” “One of the most valuable things they did for us was pain control,” says Vickie. “Nigel had serious pain issues, so Cate suggested that he come to the Hospice for a week to see what they could do. He wasn’t terribly keen like a lot of people he thought that once you went into a hospice that was it, you never came out again! But he came in for a week, and the difference it made to our lives was phenomenal. It meant we could travel, we could have weekends away, we could go and see people - we had some fantastic times, all because the pain was under control.” “It was a marvellous six months, but towards the end the pain came back. I remember the weekend before he came in for the second time. I phoned the Hospice at about 3am and spoke to one of the IPU nurses. She was

“If he couldn't be at home, then he wanted to be here.”

fantastic. She supported me and advised me on what medicines he could have. He went to sleep around six o’clock, but she said to phone her back if I needed to. By 10am on the Monday he was in the hospice again. They just said ‘come and stay with us, and let’s get your pain sorted’.” In the summer of 2014, Nigel came to the Hospice for the final time. “We were in a lovely room, with the gardens outside,” Vickie remembers. “The moment we arrived, Nigel rallied, and for a couple of days he was feeling very comfortable. We were made so welcome. It made it much easier for us to spend time with him.” “If there’s got to be an end of life, it’s the best place to be. It allowed Nigel to die with dignity, which I shall be forever grateful for. If you can’t die at home, I don’t know anywhere better. Nigel felt the same. If he couldn’t be at home, then he wanted to be here.”

Celebrating our Success We’re thrilled that our vital work has been recognised and rewarded both locally and nationally.

Two New Team Members and an 'Outstanding' Rating!

As two of our senior team take retirement, we welcome two new faces - Mark Beale, who replaces Pat Bowyer as Director of Finance, and Jayne Holland who takes over from Clodagh Sowton as Director of Patient Services. Jayne’s expertise has already been proven in her work establishing our Community team at the Beacon Centre in Guildford, where a recent Care Quality Commission (CQC) inspection has awarded us an ‘outstanding’ rating. The CQC is the independent regulator of health and social care in England, and one of their inspectors visited the Beacon Centre to evaluate our Hospice Care at Home team, Clinical Nurse Specialists and Day Services there. Her report was packed with glowing references to the care which we offer, including comments such as: ‘managers showed outstanding leadership and recognised, promoted and implemented innovative ways of working in order to provide a high-quality service’; ‘staff went out of their way to ensure people were involved in decisions about their care’ and ‘people and their families were supported by kind and compassionate staff who went the extra mile to ensure people continued to have life-enhancing experiences’. We are delighted with this ‘outstanding’ rating and would like to thank all of our staff and volunteers for their commitment and dedication.

Page 4

Double Win at Biz Awards We won twice at this year’s Eagle Radio Biz Awards, which were held at Mercedez-Benz World in Weybridge, with our Chief Executive Sarah Brocklebank securing first place in the Leadership category and one of our Occupational Therapists, Lisi Pilgrem, taking Employee of the Year. Since 2005, Sarah has increased the number of people we care for every day from 100 to 250, managed the transfer of the Beacon Service to Phyllis Tuckwell, and helped us care for more people at home through our Hospice Care at Home team. Lisi has introduced Social and Therapeutic Horticulture (STH) sessions to our patients, enabling them to access nature and gardening, which can greatly reduce stress levels and improve quality of life. Her work has been recognised by Hospice UK and she now also advises other hospices around the UK on STH. Congratulations to them both for this welldeserved recognition of their hard work!

PTHC Doctor Appointed as University Teaching Fellow Dr Nick Dando has recently been appointed as an Honorary Teaching Fellow with the Faculty of Health and Medical Sciences at the University of Surrey. Nick has a longstanding interest in medical education alongside his clinical practice, and has completed a Postgraduate Diploma in Medical Education, achieved Fellowship of the Higher Education Academy during Registrar training, and was previously an Associate Clinical Sub Dean for the University of Southampton Medical School. In this new role, Nick will be responsible for clinical teaching in the second year of the University’s new Physician Associate training programme, and is also contributing to the design and delivery of a cancer and palliative care case-based learning module.

Recent Developments We are committed to delivering flexible, responsive and patient-centred end of life care, and are continually developing our services to ensure that they are the best they can be.

Working with Other Care Providers Our new ‘locality working’, which we are introducing this year, will enable us to work more closely with other local care providers, such as GPs, district nurses, care homes and our two local hospitals – the Royal Surrey County Hospital (RSCH) and Frimley Park Hospital (FPH). Instead of one large team which covers our whole catchment area, our Community Services will be split into two locality teams, one covering the Guildford & Waverley area and linking with the RSCH, the other covering Farnham & NE Hampshire and Surrey Heath, and linking to FPH. Within this will sit our new ‘Living Well’ team, which will offer treatment and advice through our Day Services – at the Hospice in Farnham, the Beacon Centre in Guildford and at outpatient appointments - and will include wellbeing and rehabilitative groups, emotional support and complementary therapy. Our Nurse Specialist and Hospice Care at Home teams accept referrals seven days a week, responding to patients who need urgent assistance, to ensure that they receive the best care and helping to reduce unnecessary hospital admissions.

Page 6

Staying out of A&E Many of our patients don’t want or need to be admitted to hospital. However, if they do call an ambulance for any reason, they are often then taken to A&E. At Phyllis Tuckwell we provide care at home in conjunction with patients’ GPs and District Nursing Teams, as well as at the Hospice. By working with SECAmb (South East Coast Ambulance Service), we have been able to reduce the number of PTHC patients conveyed to hospital. SECAmb’s computer system enables clinicians to record information about PTHC patients, so that on receiving a 999 call, ambulance staff can check each individual’s care plan and patient wishes, and can then provide advice over the phone, treat the patient in their home, or contact us to request that the patient is admitted to our IPU. As well as significantly reducing the number of PTHC patients who are taken to hospital, this new system also saves the NHS much-needed funds.

Extending our Care We’re always looking for ways to develop our services, and offer a range of sessions and workshops for everyone in our care.

Hospice Doors Open to All Every Tuesday afternoon (2-4pm) we open the doors of our Hospice in Farnham to anyone who would like to find out more about our care. These ‘Open House’ sessions are an informal way for people to see the Hospice and its facilities, with staff and volunteers on hand to answer any questions. As they coincide with the last hour of Day Hospice (10am-3pm), visitors are also able to see how these sessions run. An increasing number of people are taking this opportunity to see the Hospice, and we now welcome up to five groups every week. Many are patients who are considering attending Day Hospice in the future, and this informal visit helps ease any anxieties they may have about coming to a hospice. Others may be relatives wanting to find out more about Phyllis Tuckwell, or donors wishing to see what their money has helped to fund. If you would like to come to an Open House session, drop in to our Hospice in Farnham any Tuesday between 2pm and 4pm.

Keepsake Books Hold Special Memories

Tai Chi Benefits Mind and Body

We have recently hosted a series of workshops, run by Farnham Maltings, for patients to create a keepsake book or box to hold their special memories. Each patient, accompanied by a relative or friend, has brought along items such as letters, photos, ticket stubs, coins, medals and fabrics which trigger special memories. By talking about and capturing these in a personalised book or box, they have been able to make new memories whilst reliving and sharing old ones. “I love chatting with the others here about our memories, and the creativity which the project has inspired in me,” said one patient.

We have recently introduced a six-week seated Tai Chi course for patients and carers, which runs on a Tuesday morning at the Hospice. Feedback from those who took part in our first six sessions was so positive that we have already started another six! This adapted Tai Chi is based on normal efficient movement and is gentle and enjoyable, yet has many health benefits, working both the mind and the body. It can be performed anywhere, so techniques taught through the course can easily be practised outside of the sessions.

Helping Patients Feel Positive “I didn’t know anything about the Hospice, and I was a little bit apprehensive at first, because you think of people dying don’t you? But it isn’t - it’s wonderful.” Retired junior school teacher Adrienne has a neurological condition which means that she is at high risk of falling. She was referred to Phyllis Tuckwell last year and started coming to Day Hospice in January. “I love it,” she says. “I love the companionship and the people. I’ve seen a speech therapist and a physiotherapist – the physiotherapist came out to visit me at home, which was brilliant, and so did the occupational therapist. She was very good; she gave us advice on what sort of equipment would be suitable for me. She’s been discussing a wet room with Mike and it’s just about ready.” Mike is Adrienne’s husband and full time carer. “He does everything,” she nods. “He showers me and everything. We have a daughter, Susie, who lives nearby. She always brings me here to Day Hospice on a Wednesday, and Mike picks me up.” Adrienne has also had complementary therapies at Day Hospice. “I’ve had arm and leg massages – they are lovely, particularly the leg massages. I also go to hypnotherapy because my arms feel heavy,

“I love the nd companionship a the people.” Page 8

like I can’t lift them, and my leg hurts too. The hypnotherapy has helped - it’s been very good - and so has the relaxation. The occupational therapist who runs it has a lovely, gentle voice. She gives you tips and advice to help you get off to sleep and stay asleep. I’ve started doing the breathing exercises she recommends and they’re very good. I often go the church service afterwards and I speak to the chaplain; she’s very nice too.” Adrienne was initially referred to Day Hospice for twelve weekly sessions, and only has two more left. She feels that the support we have offered her has helped her to feel more positive about the future. “You’ve helped, very much so. I’m less frightened about it all than I was. It’s been super, the twelve sessions. I’m only sorry I can’t carry straight on and do another twelve!”

A Day in the Life of...

t is p a r e h t io s y h P a Our team of Physiotherapists work with our patients to improve their quality of life, helping them to remain as independent as possible throughout all stages of their illness. Lead Physiotherapist Sue Cullum tells us more about how they do this. There are five of us in the team, four Physiotherapists and a Physiotherapy Assistant. We also have a team of ten volunteers who support us, two of whom are qualified Physiotherapists themselves. Our focus is on rehabilitative palliative care – helping patients to remain as independent and functional as they can, within the bounds of their illness. We encourage patients to decide what is important to them and set realistic goals to enable them to achieve this, working with them to help them manage everyday activities, increase their mobility and muscle strength, reduce any pain or stiffness which they are experiencing, and teach them techniques to conserve energy and manage breathlessness. We see patients on our In-Patient Unit, in Day Services and at outpatient appointments at both the Hospice in Farnham and the Beacon Centre in Guildford, as well as in the community across the whole of our catchment area. We attend daily meetings with our colleagues and also liaise individually with staff both at PTHC

and in our local hospitals, all of which help to ensure that there is a continual conversation between all teams, and that everyone involved is kept up-to-date with each patient’s current situation. We also visit patients at home or in care homes, run exercise classes at both the Hospice and Beacon Centre, and have recently introduced seated Tai Chi sessions, which are proving very popular. We each have a caseload of patients, some of whom we may see just once or twice, others whom we will work with on an ongoing basis. After assessing each of them individually, we provide care and advice to suit their specific needs. We may set individual exercise programmes for them, suggest a series of acupuncture sessions, or provide walking equipment to help them stay mobile, or TENS machines to help control their pain. Helping patients achieve their goals is very rewarding, and the feedback we receive confirms how important physiotherapy is. Our exercise groups are also a great way for patients to meet each other, and seeing them form strong friendships is really heart-warming.

Fundraising We have to raise over £20,000 every day to be able to provide all of our services - and here are just some of the ways we do it!

Pedal Power 20 keen cyclists took part in this year’s Prudential RideLondon in aid of PTHC, raising over £14,600! The ride took place on Sunday 30th July, starting at the Queen Elizabeth Olympic Park in London and following a 100-mile route on closed roads through the capital and into Surrey’s countryside, before returning to London to finish on The Mall. It was our biggest team ever for Ride and many of our cyclists had a personal reason for taking part, such as David Moore, who said “I’m proud to ride for PTHC. They’re supporting a relative of mine and it’s important to say thank you and help in a small way”. Thank you to all of our cyclists and everyone who sponsored them. If you would like to join our 2018 team, email fundraising@pth.org.uk or call 01252 729446. (Places are subject to a £50 registration fee and a minimum sponsorship of £399.)

Walking the Walk Over 500 walkers took part in this year’s Walk for the Wards, taking on either a five or ten mile route through the stunning countryside around Tilford village, and raising over £20,000 for us!

The Quick Way Down! 14 brave supporters took to the skies for our June skydive, raising over £10,000 between them! The weather just held out for them and they all landed with a big smile. A big thank you to all who took part and everyone who sponsored them.

Marathon Success This year saw 14 runners take part in the Brighton Marathon in aid of PTHC, and 19 complete the London Marathon for us. Congratulations to them all, and many thanks for their fantastic fundraising efforts which raised just under £85,000! If you’re looking to secure a place in the 2018 London or Brighton Marathon, please contact us on 01252 729446 as we have 10 places available for Brighton, and 13 available for London. If you receive a place in the ballot, do get in touch, as we’d love you to join our team.
