It’s been another busy six months, and it’s hard to believe that we’re now looking forward to the run-up to Christmas once again. Here at Phyllis Tuckwell we have been working hard to make our services more accessible for our patients, and to work more closely with other local care providers.

Page 4 will tell you about changes to our referral process, the introduction of our Rapid Response Team, and the EMIS clinical database, while page 5 outlines our new Living Well activities, which are designed to help patients, families and carers get the most out of life.

The majority of the care which we provide is given outside of our In-Patient Unit, and increasingly in the community – in patients’ own homes or in care homes. Many of our patients tell us how important it is for them to remain at home, and we feel privileged to be able to care for patients and their families in their own special surroundings.

We wouldn’t be able to provide our care without the support of our Fundraising and Retail teams. Find out about our forthcoming events on pages 10-11, and read about our recent activities on pages 12-13. Page 14 will give you the latest from our shops, while page 15 tells you about the support we receive from our fabulous local corporates.

e m o H t a e r a C e if L f o d En Maggie was cared for by our Hospice Care at Home team, who visit patients at home during their last weeks of life. The team visited Maggie daily during the last week of her life, and supported her sister, Chris, and daughter Zoe too, with advice and reassurance.

“Maggie was adamant that she wanted to stay at home,” says her sister, Chris. “She made me and Zoe promise about 40 times one evening!”

“She needed to have her final wish,” says Maggie’s daughter Zoe, “and we had to respect that. And it turned out to be the best decision that she and we made.”

“It was a perfect journey.” Zoe Maggie’s family asked if she could be cared for by Phyllis Tuckwell’s Hospice Care at Home team, and Maggie was visited by several Nurses and Health Care Assistants from the team. The Nurse who spent the most time with her, and who had the most contact with Chris and Zoe, was Jessie.

“She was superb,” smiles Zoe. “I would never have got through it without her. I spoke to her on the phone a lot. She would visit Mum and then she would ring me. She was so calm and caring.”

“The Nurses were professional and relaxed,” says Chris. “They dealt with Maggie’s illness and kept us at ease at the same time. They were incredible - so kind, so respectful, nothing was too much trouble.”

“During that last week, Bryony, one of the Phyllis Tuckwell Chaplains, visited too,”

says Chris. “She was brilliant. She and Maggie had some time alone, and Maggie looked really peaceful after she left. We’re an Irish Catholic family, and it was really important to Maggie to make her peace before she died.”

As Maggie’s condition deteriorated, the Hospice Care at Home Nurses visited more frequently.

“The night before Mum died, it snowed,” says Zoe. “Jessie phoned to see how I was doing and asked if we would like someone to sit with Mum overnight. I said yes. Because the weather was drawing in, we were concerned that the Nurses may not be able to reach us if they needed to, even though we knew they’d move heaven and earth to get to us. We thought it was safer for Mum to have somebody here with her.”

Julie, a Health Care Assistant in the Hospice Care at Home team, came over to sit with Maggie through the night.

“She just settled in and allowed us to be us,” smiles Chris. “She was lovely: very down-to-earth, very natural.”

Maggie died the following morning.

“I didn’t feel scared,” says Zoe. “I just wanted to be with her. The last part of her life, here, it was just right. It wasn’t at all stressful, it was a perfect journey.”

It's not about end of life - it's about rest of life Our Living Well services are designed to help our patients living with an advanced or terminal illness and struggling with everyday life, to manage the impact of their illness, cope with changes and improve their wellbeing – so that they can get the most out of life.

The team is made up of Nurses, Health Care Assistants, Physiotherapists, Occupational Therapists, Complementary Therapists, Psychologists, Counsellors, Social Workers, Welfare Advisors, Dietitians and Chaplains, who together provide a wide range of care to promote physical, emotional and spiritual wellbeing.

Improving Access to Our Care We have recently made improvements to our community care and referral systems, to ensure that those who need our services are able to access them quickly and easily.

The new referral process is being managed our Advice & Referral Team (ART) of Clinical Nurse Specialists and administrators, whom GPs and other healthcare professionals can now contact directly to refer a patient. They are supported by our new Rapid Response team, who can assist patients who require urgent help, avoiding delays and reducing unnecessary hospital admissions. In addition to this, we have moved to a new clinical database, EMIS. As EMIS is also used by the NHS, we will, once appropriate information sharing agreements are in place, be able to access patients’ records quickly and easily once they are referred to our care, and their GPs will be able to see any notes which we have added about the care we have given to their patients.

Our new Community Locality Model will enable us to work even more closely with GPs, care homes, district nurses and our two major hospitals. Around 85% of the care which we offer is given outside of our In-Patient Unit, and increasingly in the community – in patients’ own homes or in care homes. This Locality Model focuses our care into two geographical areas, or localities, enabling our staff to develop partnership working with the Royal Surrey County Hospital in the East of our catchment area, and Frimley Park Hospital in the West, as well as with the GPs, care homes and district nursing teams in each area, to ensure that patients receive more effective and co-ordinated care.

After a holistic assessment by our multi-disciplinary team, patients are directed to the appropriate care, which may include wellbeing and rehabilitative groups, emotional support, complementary therapy treatments or creative therapy sessions. All are held in the relaxed and friendly environment of either the Beacon Centre in Guildford or the Hospice in Farnham, and run for several weeks, enabling patients to build upon each week’s progress and develop friendships within the groups.

Through these sessions, patients can learn to improve their movement and mobility, increase their independence, reduce their stress and anxiety levels, increase their confidence and self-esteem, and develop coping strategies to help them feel more in control of their illness.

In addition to our services for patients, our Living Well team also offers support to families and carers, both during the patient’s illness and into bereavement.

Films show how we help

A makeover day and photoshoot, and a surprise evening out are just two ways in which we have recently helped patients to achieve those things which are really important to them. Both were organised by our Hospice Home Support (HHS) Co-ordinator, Nicola. Nicola arranged for patient Gillian Haworth to come to the Hospice for a makeover and pamper session before having her photo taken by a professional photographer, while Lesley MacManus, another HHS patient, was treated to a night out at G Live, meeting professional dancers Aljaz and Janette from BBC’s Strictly Come Dancing, and ticking an item off her bucket list!

We have created two new short films, which illustrate the range of care which we offer our patients and their families: • Carl Carl and his two children were offered bereavement support after Carl’s wife, Emily, died on our In-Patient Unit.

• Roger Roger was cared for by our Living Well team, who helped him regain strength, mobility and confidence after his cancer diagnosis.

Fun Day helps bereaved families In April, Eagle Radio kindly gave us tickets to take our Storm group to watch the Guildford Flames ice hockey team play, at the Spectrum Leisure Centre. Storm is a social group for anyone of secondary school or 6th Form age, whose loved one has been cared for by Phyllis Tuckwell.

The group meets every six weeks to take part in activities such as ten-pin bowling, donutting, paintballing, mixing music - or watching ice hockey! Eagle Radio gave us the use of their hospitality box and laid on refreshments, and some of the group even had the chance to meet one of the players after the game, which the Flames won!

As well as caring for patients, we also support their families, both before and after bereavement. Our annual Family Fun Day on Saturday 7th July offered our bereaved relatives some time away from the hustle and bustle of everyday life to socialise and build friendships in a welcoming and supportive environment. 15 families came along to the day, which was held at the Explore Activity and Education Centre in Mytchett, taking part in team games, camp fire cooking and an afternoon of raft racing, kayaking, canoeing and garden games.

The day created an opportunity for families to spend some meaningful time together, having fun and creating new memories.

A Day in the Life of...

Volunteer Support for Patients at Home

a HCAH Nurse

Registered Nurse Jessie works in our Hospice Care at Home (HCAH) team, visiting patients who have chosen to spend their last days at home, and supporting their families too.

Our Hospice Home Support (HHS) service provides practical and emotional support for patients at home. The service matches patients with volunteers, who visit that patient for three hours a day, once a week.

“I’ve been a nurse for eight years and I love it!” she smiles. “Helping patients in their last wish to stay at home is really special. You can see the difference you’re making.”

were outstanding, not only to Mum but to the whole family. It was Mum’s dying wish to stay at home and it was a beautiful ending. The Hospice Care at Home team were amazing.”

HHS volunteers help with things such as driving patients to and from hospital appointments, accompanying them to the shops or garden centre, helping around the home, or simply giving them someone to talk to. Patients frequently tell us how much difference this makes to their lives, and it also gives their carers a break too.

“When we first go into a home we can be faced with anything, and quite often we’re faced with panic. People don’t know how they’re going to cope. Our presence helps to calm things. They know that we’re specialists and our being there helps them to relax.

They know that they can always contact us if they need to, which reassures them.”

Our nurses are the beating heart of Phyllis Tuckwell Hospice Care. Every hour of every day, one of our nurses is looking after someone who might be facing the last few weeks, days, or even hours of life. They make sure every person is treated with dignity and respect, and that their final wishes are listened to. For some people, that final wish is to be at home when they take their last breath - surrounded by their family and their memories. That’s why it’s so important that we get donations to help fund our Hospice Care at Home team.

“I’ve known Zul as if forever,” says Clive, one of our patients. “As soon as I met him, I clicked. On a Friday he’ll be here, and we talk, talk, talk, and soon the three hours are up. The conversations we’ve had over the last few months have covered politics, economics, and social issues. We talk as if we could change the world! When he goes I feel rejuvenated and motivated. We share a lot, we have a lot in common, and I think that is excellent.”

“We also talk about sport,” says Zul. “We talk about football a lot, and we don’t support the same team, so that creates a nice little dynamic!”

“As Hospice Home Support volunteers, we get very good training,” he continues. “It’s crucial. It covers all sorts of topics and helps us to support the patient. On an ongoing basis we have regular meetings, which give us the opportunity to run scenarios by the group and check that we’re dealing with things correctly and properly. Listening is a key thing. The patient’s going to have things to say and it’s a case of listening, understanding and giving something back. The training really helps you to take that challenge on.”

As well as providing companionship for Clive, Zul’s visits also enable Clive’s wife Anita to have some time to herself.

“I know that Zul’s good company for Clive,” Anita explains, “so I’m really relaxed when he visits, and I don’t have to worry. I go out - I go shopping, whatever I want to do. It’s nice to get away, just to have a break. I know that Clive’s benefitting from Zul’s companionship and I don’t have to worry about him when I’m out. Before, we were scared of the words ‘Phyllis Tuckwell’, but now we’re not.”

“As they near the end of their life, we visit as much as they need us to. Our visits aren’t appointments, we don’t put a time limit on them, we just give them and their families as much time as they need.”

One of the patients whom Jessie recently cared for recently was Maggie, whose story you can read on page 3. Maggie’s daughter, Zoe, was greatly comforted and reassured by the care and support which Jessie and the rest of the HCAH team provided. “Jessie played a big part in my life over the last stages of mum’s illness.

Fundraising We have to raise over £20,000 every day to be able to provide all of our services - and here are just some of the ways we do it!

Bubbly Fun for Everyone!

On Sunday 17th June, just under 750 people took part in our new fundraising event Bubble Rush – raising £25,000 to help fund our care! Held at Merrist Wood College in Worplesdon, this 4k foam-filled fun run took participants through five special stations along the route, where high-powered cannons sprayed out a sea of coloured foam, creating a 4ft deep bubble bath for them to burst their way through. Thanks go to Nuffield Health in Guildford for the warm-up, Paul Fitchett from Paul Fitchett Photography, and Mike Klein and Bob Russell from the Guildford Photographic y Society, for taking photos on the day, Surre ding provi for ebox Hors ry Ices & The Hung s delicious refreshments, Extreme Adventure the of all les, for providing the bountiful bubb fantastic volunteers who helped on the day, and of course everyone who came along to take part or cheer the runners on!

Don’t Look Down!

On Sunday 3rd June, 22 fearless fundraisers took on our brand new challenge – abseiling down Portsmouth’s Spinnaker Tower - and raised over £12,000 for Hospice Care! The weather was perfect, with clear blue skies and glorious sunshine, which gave the abseillers a magnificent 23-mile view of Portsmouth Harbour and the Solent! Taking part in the 100 metre abseil were our own Chief Executive Sarah Brocklebank and Director of Finance Mark Beale, as well as Physiotherapy Assistant Sandy Seagrove, Louise Pullen, manager of our Quarry Street shop, and two of our wonderful volunteers, Gill Chapman and Carol Choules-Rowe. Well done to everyone who took part!

Open Gardens From May to September, residents across our catchment area of West Surrey and North-East Hampshire opened their gardens to raise money to support our care – in our first annual Open Gardens event. A wide range of gardens were on show, from small cottage and courtyard gardens, with patios, terraces and water features, to larger gardens spanning several acres and incorporating a wide variety of flowers, shrubs and trees, interspersed with ponds, footpaths, pergolas and sculptures. The Hospice also opened its own gardens too, which are tended to all year round by a team of dedicated and knowledgeable volunteers.

The event raised a fantastic total of £15,677 - thank you to everyone who took part.

this Christmas

Tickets are now on sale for our Burlesque Christmas parties, which will be held on Friday 30th November and Saturday 1st December at a NEW VENUE, Best Parties Ever Marquee, on Queens Avenue, Farnborough, GU11 2LQ. Set in the decadent world of 1890’s Paris, the parties will feature impressive trapeze artists, mesmerising aerial hoop acts, Can-Can dancers and the very best of burlesque. Enjoy a fabulous three-course dinner, take a spin on the indoor dodgems, try your hand at the charity casino and then disco dance the night away! The evening will also include a professional photographer, free car parking and cloakroom, a raffle and auctions with fantastic prizes, and a survivors’ breakfast.

Walk for the Wards Over 400 walkers gathered in the picturesque village of Frensham on Sunday 1st July, to take part in our annual sponsored Walk for the Wards, raising just over £16,000 to help fund our care! The walkers set off from More House School, following a path through the stunning Surrey countryside to Shortfield Common and then on to the Great Pond. With hilltop views and waterside walks, the trail really showed off the best of the local countryside, and everyone who took part enjoyed the beautiful scenery and fresh air, before returning to More House School for a well-deserved cream tea! Our thanks go to Baker Law solicitors for their continued support of Walk for the Wards, all those who donated scones and jam for the cream teas, and all of the volunteers who helped to set out the route, put up signs for walkers, directed car parking, registered walkers, marshalled the route and provided refreshments. We couldn’t have done it without you!

Mont Blanc Trek

If you love travelling, relish a challenge and enjoy meeting new people, then this one is for you! In September 2019 we’ll be taking a team of hikers further than ever before on our first exclusive trip - to the Mont Blanc massif region! This area of beautiful and dramatic scenery offers a challenging and varied terrain, and you’ll trek for 6-8 hours a day as you cover part of the circular route around Mont Blanc. You’ll cross the border from Switzerland into Italy and then France, staying in mountain chalets and enjoying the stunning scenery. We have partnered with Vale Furnishers to raise £100,000 in three years. Vale are asking their customers to donate unwanted furniture, which is passed to us to sell in our shops. Vale will also collect the unwanted furniture when they deliver a customer’s new purchases, and can take other items too, such as shoes, clothes, accessories and books, all of which we can sell to raise money. We’d like to HELP say huge thank you to the RAISE staff at Vale Furnishers for £100,000 supporting this partnership.
