1 Objective This guide will help students acquire skills required to function within the Virtual Environment and, to complete the virtual pandemic flu triage exercise.

2 The Viewer Interface

Figure 1 gives an overview of the viewer interface. In the Resources section, at the end of this manual you will find a link to the Linden Lab Second Life Quickstart Guide. It will help you 6 Students’ Manual


with your Second Life experience. Download and print a copy for easy reference. The Quickstart Guide also includes a keyboard shortcuts section.

3 The Orientation Stations The training stations will help you complete some of the common tasks, such as walking, talking, viewing and interacting to facilitate the sessions/exercises in the virtual environment.

3.1 Communicating with other Avatars around you 3.1.1 Text Chat The chat bar is located at the bottom of the screen.

Type in what you want to say, and press [Enter] key at the end of your line. To close the Chat Bar, press the [Esc] key. Everyone within 20M of you will be able to see what you typed. 3.1.2 Voice Chat – Push to Talk Hold down your mouse wheel and speak. Release to deactivate the mic.

When you speak green waves signals around the white dot will indicate that your microphone is active. 3.1.3 Adjusting general sound volume To adjust the general level of sound, use the slider near the talk button panel.

3.1.4 In World Practice We have set up a “Communication” orientation station to familiarize you with basic communication skills in Second Life. Complete this course as many times as needed until you are satisfied with your skills. Once you have mastered these skills you can move on to the next section in this work book.

3.2 Avatar Movement: How does my avatar walk and run To move your avatar use the arrow keys on your keyboard, and release the key to stop.

3.2.7 In World Practice We have created an obstacle course to practice walking, turning, and running. Complete the obstacle course as many times as needed till you are comfortable with walking, turning, and running in the environment. Once you have mastered these skills you can move on to the next tutorial in this work book.

3.3 Interacting with objects: Moving hospital transfer and transport devices 3.3.1 How to move gurneys and wheel chairs

There are two ways to move a gurney i)Left click on the black rectangle to get into the push position and use arrow keys to move gurney around.

3.3.3 Lying down on a gurney Two ways to lie on the gurney i) Left click on bed OR ii) right click on bed, and on the pie menu click on “use”.

To get up from gurney, click on “stand up” button at bottom of SL window and use arrow keys to move. 3.3.4 Wheel Chairs : pushing and sitting You may not practice with the wheel chairs during the session but it is good to know that moving them is similar to moving the gurneys. There are two types of wheel chairs manual and motorized. 3.3.5 Pushing a manual wheel chair i) Left click on the black rectangle to get into the push position and use arrow keys to move wheelchair around.

OR ii) you can right click on the black rectangle to bring the pie menu up, and click on “push” or “use”. 3.3.6 Stop pushing and releasing a wheelchair To stop pushing and release wheelchair, click on “stand up” button at the bottom of your SL window. 3.3.7 Sit on a motorized wheelchair To sit on the wheel chair , (i) left click on the seat

To get off the wheelchair, click on “stand up” button at bottom of SL window. 3.3.8 In World Practice We have set up a this station to allow you to interacting with objects in SL. Moving hospital transfer and transport devices will be important skills for the coming sessions. Complete this course as many times as needed until you learn how to move and lie on gurneys, and sit and push wheel chairs. Once you have acquired these skills you can move on to the next tutorial in this work book.

3.4 Looking around and Viewing Objects 3.4.1 Camera Controls You view things in Second Life through a camera. Normally, this camera is pointed at the back of your avatar. You can move this camera around to look at objects, or zoom in for a closer look. During this session you will be able to access and view this video which explains camera controls. To play press on the play button in the bottom right of your SL window.

3.4.2 Focus on an object Position cursor on the object of interest. 13  Students’ Manual


Press and hold the [Alt] key: your cursor will change to a magnifying glass with a "plus" sign in the middle.

Left-click and hold on the object (or your own avatar) you want to look at.

3.5 Get a closer look: zoom in Place cursor on object, hold both the [Alt] key and left mouse button simultaneously and drag the mouse forward to zoom into the object. Drag the mouse back to zoom away from the 14  Students’ Manual


object. You can also use your mouse’s scroll wheel to zoom in and out. Note that zooming in/out is just moving your camera forward/backward.

3.5.1 Look left and right around object To view an object’s right and left sides, place cursor on the object of interest, hold both the [Alt] key and left mouse button simultaneously(A), and move your mouse left (B)then move it to the right (C).

Additionally you can combine zooming (Step 3.3) and rotating (Step 3.4).

3.5.2 Roll your view up and down around object To move your camera up/down around the object: hold down [Alt] key and [Ctrl] key (A), then left click and hold on object of interest, then move your mouse down (B) and up (C).

3.5.3 Return to regular view/Reset view

To go back to the regular view at any time, just press the [Esc] key.

3.5.4 In World Practice We have set up a this station to familiarize you with the some camera control tools. The skill to view an object from a particular angle and distance is particularly useful. It takes some practice. Complete this course as many times as needed until you learn how to successfully focus on an object, and easily zoom and pan to get a better view of the object. Once you have acquired these skills you can move on to the next stage or to the Feedback section.

3.6 Communicating with Other Avatars – Advanced (This section is optional) Note that this section is optional and we will not go through this station during this week’s session. You can move on to the next stage and come back to these exercises at your own leisure to learn about more advanced communication features. 3.6.1 Enabling Voice chat The default settings should have automatically activated Voice Chat when you registered. If not it can be activated in the Edit / Preferences ->Voice Chat, and Enable Voice Chat and go through the set up.

Once “enabled” you will see a white dot above your avatar’s head.

3.6.2 Push to Talk in Toggle mode If you have checked Use Push-to-Talk in toggle mode in the Voice Chat preferences, you can use the key or button as a mic toggle. Press and release the key/button trigger to switch your mic on and off. 3.6.3 Hands Free Voice Chat To enable hands-free voice chat, click on the lock icon on the left side of the Talk button. To switch off your mic click on the lock again.

3.6.4 Adjusting voice volume To adjust voice volume click on the arrow highligted below and use the voice slider.

3.6.5 Adjusting the volume of individual active speakers To adjust the volume of each active speaker individually click on the button showing two voice bubbles on the bottom right hand corner.

You can select a speaker from the list of active speakers’ pane that gets displayed and use the slider to adjust the sound level.

3.6.6 Direct Voice Calls (think Cell phone) Direct voice call allows you to voice chat with the specific avatars you choose to talk to. Click on Communicate button.

Select Contact and click on “IM/Call”.

Click on Call Button.

On connection press mouse button or wheel to talk.

To disconnect click on “End call”. 3.6.7 In World Practice We have set up an optional advanced “Communication” station to familiarize you with using your mic, talking, adjusting voice volume and making Direct voice calls in Second life. You can move on to the next stage and come back to learn and practice your advanced communication skills at your own leisure. 3.6.8 HICS Training and Quiz You will utilize these learned skills to follow the HICS lesson, and to take a quiz. You will find a link to the user manual for the quiz engine is in the Resources section, at the end of this manual.

4 Triage Training Area In this lesson you will learn how to use the virtual resources in the triaging area and, how to wear uniforms and clothing and change appearances within the virtual environment. It also gives an overview of the Inventory.

4.1 Using Virtual Resources in Triaging Area 4.1.1 Inventory The lesson will start in the conference room where some content from the previous lesson will be revisited and an overview of ESI, START, and utilization of Triage Tags presented. We will then proceed to the triage training area followed by the uniform and clothing stores. This guide will provide you with instructions, and give you an overview of the Second Life inventory. This will not be an exhaustive exploration of the Inventory, but rather an overall look at how we are going to use the Inventory during the current session. Almost everything you own in SL is kept in your Inventory. 20  Students’ Manual


The Inventory tab is located in the bottom right corner.

Click on it to open the Inventory window.

You will find many folders, and we will be working mainly with the Objects, Clothing and Notecards folders in this tutorial. If there is time at the end of this lesson, we will explore Landmarks. Interacting with the mannequins or Avatars in the triage area 4.1.2 Moving the mannequins You can use the skills you acquired in previous lessons to push the wheel chairs that have mannequins sitting on them. To lead the standing mannequins right click on the rectangular object in front of them and walk to required location. The mannequin will follow you.

To stop, click on stand up at the bottom of the screen. If required use trolleys or wheel chairs to move the avatar patients. 4.1.3 Listening to the mannequins Stand close to a mannequin. Left click on its face use your camera skills to zoom on it – so you can hear their symptoms clearly and loudly.

4.1.4 Triaging: ESI categories and Start Triage Tags The patients will have the START Triage Tag and an ESI category indicator attached to them.

To tear off the appropriate sections from the tag to label the victim, left click on the tag and the bottom section will disappear.

With each left click the next section will disappear.

If you go beyond the last section of the label it will reset to a full tag again.

An ESI indicator tag is also attached to the mannequin, to assign ESI category levels left click on the circular tag. It will go through 1-5 as you continue left clicking on it.

4.1.5 Triage Helper The Triage Helper can be used as a memory aid. You can left click on the Triage Helper box.

Click on [Keep] on the message that appears.

Go to the Objects folder in your inventory. Right click on “Triage helper” and left click on “wear”. You will see the triage helper on the top left of your interface.

4.1.6 Interact with Triage helper Use the top menu to explore the triage helper.

Click on the empty glass icon

on the menu to make the Triage helper transparent so you can

see through it. And revert back to the opaque helper by clicking on the full glass icon

4.1.7 Note Cards – Taking notes If you want to take notes during the lesson click on the notes icon

. You will receive a

message, click on [Keep]. A note card will open up.

You can write your notes, click on [Keep] and on the “Save changes?” prompt, save it and click on [Keep] again to send it to your inventory. You can also close it at any time and you will be prompted to save/cancel changes if you have not done so.

It will be saved in the Notecard folder in the inventory. You can double click on it to open, or right click and choose “Open”. You can change the name of your note card by right clicking on it and choosing “Rename”. 26  Students’ Manual


4.1.8 Go through the Triage Algorithms by clicking on the back and forward buttons.

4.1.9 Minimize, maximize, close or detach Triage Helper You also can minimize the triage helper, maximize it or close it using these buttons

You can detach it by closing it or right clicking on the triage helper in the inventory and choosing “detach”. If you want to reopen or wear the triage helper, it is in the Object folder. You can also search for the triage helper, by typing the name in the search box at the top of the inventory and wear it again. Tips: In the Inventory window you can click on “recent items” the top menu and you will find all the items you recently acquired. You can also search for the items, by typing the name or any keyword in the search box at the top of the inventory. 4.1.10 Wearing Personal Protective Equipment, and Changing Appearance Donning Personal Protective Equipment (PPE) At the virtual work station you will find Personal Protective Equipment on a trolley. 27  Students’ Manual


Left click on each box. Click on [Keep] when the following message appears.

Go to the Objects folder in your inventory.

Right click on a PPE item e.g the “Shield goggles”, and left click on “wear”. It will now show “worn” next to the item. To detach it from your avatar right click again on the item and on the menu left click to “detach” or “take off”.

4.1.11 Changing clothing and appearance Next you will be directed to the clothing and uniform stores. For wearing/taking off clothing the same principle as for objects applies. Go to the clothing folder and wear the clothing or “take them off”. But first … save your current outfit Before you start changing your appearance, and experimenting with different looks, it is recommended that you save an outfit to which you can revert back, in case you are not satisfied or need a quick change over. To save your appearance to a folder just right click on your avatar and then left click on "Appearance" in the pie menu.

The appearance changing window will appear. In today’s session this window will be used to save your current outfit. Note: The window allows you to change your avatar appearance (shape, size, color etc.) too, but we will not be exploring this at today’s session. You are encouraged to experiment with it at your own leisure.

At the bottom of the Appearance window, click on the button [Make Outfit]. Another window will come up indicating all the items that make up your appearance.

Click on every small white box in this window so that each has a tick inside it. These are the items that will be saved to a folder in your inventory. Click the box at the bottom-left marked "Rename Clothing to Folder Name", and then type a suitable name into the box at the top of the window (e.g., 'Conference Outfit’). You can make outfits of the your preferred outfits. Click the [Save] button and you will then have your current appearance saved to this new folder in your clothing folder in the inventory. You can then drag this folder onto you avatar at any time or right click on the folder and choose “replace outfit” to restore your avatar to this saved appearance.

4.1.12 You are ready to experiment with your appearance and looks! You will be directed to the clothing stores where posters will guide you. You will be given you the choice to wear the clothes when you left click on the outfit to select it.

Just check on the wear now option before clicking on “buy”. Please be patient it may take a few seconds before you see the new outfit on your avatar. While your clothing will appear on you, you may need to get other objects such as shoes from the inventory and wear them. For clothing items, the new item selected will replace the one you are wearing e.g. if you select a new shirt to wear, it will replace the current one automatically on your avatar. You may choose not to wear the clothing now. You can find the items you bought in the Inventory. The folder will bear the ID # of the outfit you bought (e.g. 07a) on the store panel, and prefixed with an “F” (e.g. the folder name will be “F07a”) for female outfits. For male outfits the folders will be prefixed by “M” followed by the ID# of the outfit as labeled in the store. Tip: You can always go to the “Recent Items” in the menu of the Inventory to see all the new things you acquire during this session.

4.2 Landmarks Folder 4.2.1 Landmarks for Lesson 3 You may want to create landmarks of locations you have visited. Landmarks allow you to teleport to that location from somewhere else. Landmarks are stored, by default, in the Landmarks folder in the Inventory. The three main landmarks for this session will be as follow: Conference Room, Obelix (150, 131, 22) Triage Training Area, Obelix (220, 71, 33) PPE &Clothing Store, Asterix (143, 112, 59) 31  Students’ Manual


To create your own Landmarks, click on the "World" option from the top menu bar, and select "Create Landmark Here".

You will see a little Landmark window appear. Just close the window. That’s it! You’ve created a Landmark.

Click on the "Inventory" button at the bottom of your screen. Open the "Landmark" folder and look for the Landmark you created. Double-click on the Landmark and it will open that little Landmark window.

If you have gone to somewhere else, and wish to return to this location , go to the landmark in the Landmarks folder and click on "Teleport". Tip: Click on the “Rename” and give the landmark a more meaningful name.

Pandemic Flu Triage Exercise in Play2TrainVirtual Environment

In this lesson you will learn how to use the virtual resources in the triaging area outside the hospital. The Scenario will start in the conference room, where the Incident Commander will address you. 5.1.1 Wearing Scrubs and PPE You will then be asked to proceed to the Triage area set up outside the hospital. Wear your scrubs and PPE. Further team leaders will be assigned and will need to wear appropriate ICS vests. You will be using the skills you learnt in the previous lessons to participate in the Pandemic Flu drill such as camera skills, pushing cart or wheelchairs, wearing clothing and objects and interacting with your inventory. Your previous manual will be particularly helpful. 5.1.2 Patient Triaging You will take on your emergency role within the Hospital Incident Command System (HICS) and then figure out how to appropriately and correctly use the triage systems available to you to manage the increasing numbers of patients that are now presenting for care. There are two types of patients the mannequin ones and a few avatars. They are all recognizable by their triage Tags. Mannequin Patient

5.1.3 Move mannequin patients If they are on wheel chairs, right click and push their wheel chairs .

If they are standing right click on the black platform and lead them.

5.1.4 Listen to mannequin patient symptoms Stand close to a mannequin. Left click on its face use your camera skills to zoom on it – so you can hear their symptoms clearly and loudly.

5.1.5 Make Mannequin patient wear respiratory mask Left click on their lip to make them wear a mask

5.1.6 Vitals Notecard A few dummies have a note card with their vitals on them. Left click on belly of Mannequin and you will get a notecard with vitals. [Not all dummies have these cards]

Click on keep and the note card will open. Once you have read it, click on [keep] and it will be stored in your inventory under notecard. You can always access it by clicking on it in the inventory.

5.1.7 Interact with Avatar Patients They will come to you at the triage area. If they can’t walk carry them on stretchers. You can talk to the avatar patient. S/he will respond to you by voice or text. If necessary you can request her/him to put on a mask.

5.1.8 Triage Tents You can change the signs on the tents by clicking on them and direct the triaged patients to appropriate tents.

5.1.9 Feedback and Debrief At the end of triaging, the IC will reconvene a meeting in the triaging area for feedback and debrief. You will find a link to the scenario narrative in the Resources section, at the end of this manual.

6 Pandemic Flu Triage Exercise in the ED of the Virtual Hospital 6.1.1 Communicate with an avatar who is far from you e.g. in another room You will revisit the voice call orientation station to review direct voice calls and private IM to communicate with another avatar that is far away from you on a one-on-one basis. Please use this only for remote communication when the person you are addressing is not in view. Talking to someone one-on-one means you won't be a participant in group voice chats. Please use the usual proximity voice chat if you are in the same room or space to allow other team members to hear you and to allow interaction within the team.

You will then proceed to the conference room for more instructions and information on the exercise scenario. The Scenario will start in the conference room, where the Incident Commander will address you. 6.1.2 Previously Learned skills You will be using the skills you learned in the previous lessons to participate in the Pandemic Flu drill such as wearing PPE, vests pushing cart or wheelchairs, interacting with your inventory, and reading note cards with the patient’s vitals. Your camera control and navigation skills will be particularly helpful for indoor navigation in the hospital. Camera controls Besides the zooming in and out, and panning and orbiting around an object, Mouse scroll is another way to use the scroll wheel on your mouse to zoom in or out the view. You can therefore see a close view from right behind your avatar or from quite a distance away with yourself as simply one tiny figure. 6.1.3 Two types of Patients: Please recall that there are two types of patients the mannequin ones and the avatars driven by people. They are all recognizable by their triage Tags. 6.1.4 Mannequins Move Mannequins: If they are on wheel chairs, right click and push their wheel chairs. If they are standing right click on the black platform and lead them. Listen to mannequin patient symptoms: Stand close to a mannequin. Left click on its upper face use your camera skills to zoom on it – so you can hear their symptoms clearly and loudly. Make Mannequin patient wear respiratory mask: Left click on its lip to make it wear a mask Get Mannequin Vitals Note card: The dummies have a note card with their vitals on them. Left click on chest or upper stomach of the mannequin and you will get a notecard with vitals. Click on keep and the note card will open. Once you have read it, click on [keep] and it will be stored in your inventory under “notecard” folder. You can always access it by clicking on it in the inventory. 6.1.5 Avatar Patients Talk to avatar patients: They will come to you at the triage area. If they can’t walk carry them on stretchers. You can talk to the avatar patient. S/he will respond to you by voice or text. Ask them to wear masks: If necessary you can request her/him to put on a mask 39  Students’ Manual


Request them for their vitals: They will give you a note card with vitals. 6.1.6 Interact with new resources: 6.1.7 Oxygen tank Move oxygen tank: Right click and select move to carry oxygen tank.

6.1.8 Heart Monitor Move the heart Monitor: Right click and select move to carry it around.

6.1.9 Hand Sanitizer: Gel in Gel Out

To use the gel, left click on the sanitizer pump bottle. You will get a message saying “ your name just used me.”

6.1.10 Wash your hands Left click on the wash basin or faucet and you will get a message saying “your name just washed her/his hands.”
