Dear Prospective Board/Commission member: Congratulations on your nomination to a board or commission for the City of Meriden. Your willingness to volunteer your time is greatly appreciated. On behalf of the Mayor and City Council, I am writing to request that you provide answers to the following questions for purposes of the nomination process. We respect your right to privacy and will hold your answers in the strictest of confidences. However, failure to provide complete and accurate answers may result in a decision by the Mayor and the City Council not to confirm you for this position. Please fill out and answer the questions in the attached questionnaire and return it to the Mayor's Office, c/o Jacqueline Fordiani, Constituent Caseworker, 142 East Main Street, Meriden, CT 06450, by the close of business on Thursday, March 26, 2009. ALL nominees are required to comply, regardless of whether you have served before. To reiterate, failure to complete and return this questionnaire by the deadline may result in your not being confirmed for a board or commission position. This questionnaire is in addition to any letter you might have received regarding delinquent taxes.
