Chapter 1 - Health and Wellbeing Good health is about the mind as well as the body - feeling physically fit, and feeling good about ourselves means that we can achieve more of the things we want to do in life. Of course, sometimes it’s difficult to find the motivation, or the time and the energy to make a few changes in our lives. We’re all human! However, a bit of effort can pay off in many ways. Even if you already have a health problem or disability there are lots of things you can do to stay as healthy as possible and to remain as independent as possible.

Use the Five Ways to Wellbeing: Connect – connect with the people around you: your family, friends, colleagues and neighbours. Be active – you don’t have to go to the gym. Take a walk, go cycling or play a game of football. Find the activity that you enjoy and make it a part of your life. Keep learning – learning new skills can give you a sense of achievement and a new confidence. So why not sign up for that cooking course, start learning to play a musical instrument or figure out how to fix your bike? Give to others – even the smallest act can count, whether it’s a smile, a thank you or a kind word. Larger acts, such as volunteering in your local community can improve your mental wellbeing and help you to build new social networks. Regularly helping others makes you feel happier. Take notice – be aware of the present moment, including your feelings and thoughts, your body and the world around you.

Getting a Health ‘MOT’ NHS Health Checks are free for anyone between 40 and 74. They only take between 20 and 30 minutes. It is a set of simple tests and questions and is a great way to find out how healthy you are and how to prevent some serious health conditions. Ask your GP about a Health Check. Diet and Exercise Eat well Having a balanced diet with plenty of fluids and an active lifestyle can dramatically improve health and well-being and reduces the risk of getting a large number of diseases. •

Make it five-a-day – aim to eat as least five portions of fruit and vegetables every day. On average, a portion is the amount of fruit or vegetables that fits into the palm of your hand. Trim the fat – choose lower or reduce fat foods, especially saturated fat. Cut fat off meat and grill or bake foods rather than frying them. Pick healthy snacks - choose fruits, carrot sticks, nuts and seeds over sugary or salty alternatives like chocolate or crisps. Increase your fibre – swap white breads, rice and pasta for wholemeal or wholegrain versions. Reduce your sugar – keep an eye out for high sugar foods (high is over 22.5g of sugar per 100g) and think of healthier sugar swaps you can make. Watch your salt – try to look for lower salt foods and don’t add extra salt to your food during cooking and at the dinner table. Think about what you drink – you should be drinking at least 8-10 cups of fluid a day, choose water, sugar-free drinks, tea, coffee or sugar-free squash.

For advice on a good diet, talk to your GP who can refer you to a dietician for specialist, free advice. If you are having trouble being able to afford food, there are organisations that can help – the Trussell Trust can tell where you can obtain food locally from a food bank. Be more active Taking exercise is vital to staying healthy. It can reduce your risk of major illnesses, such as heart disease, stroke, diabetes and cancer by up to 50%, and lower your risk of early death by up to 30%. Exercise is good for mental as well as physical well-being. Regular exercise can help to reduce the effects of anxiety and depression.

Simple ways to get more physically active include: •

Aim to be active every day and make it enjoyable – it isn’t just about being healthy; it is about having fun too. Pick activities that you enjoy and you’ll be more likely to keep doing them. Up on your feet – aim to reduce the time you spend sitting or not moving. Try walking around when you talk on the phone. Walk off calories and weight – aim for 10,000 steps a day. Pedometers are cheap and easy to get so you can count your steps and keep track. Take the stairs – climbing stairs can burn more calories than jogging. Go for 150 – in an average week, adults are recommended to do a total of 150 minutes of moderate activity over at least 5 days. Aim to get your heat beating faster for at least 10 minutes at a time and build up to 30 minutes a day, five days a week.

There are plenty of opportunities for taking exercise, whether it’s walking or playing a sport in your area. Support in Redbridge Here are some activities and programmes in Redbridge that can help. Active4Life is an initiative to help residents become more active by offering activities all year round such as the Healthy Lifestyle Programme, Community Fit Club, Walking for Health, Sport in Parks and much more. Some sessions are pay as you go or free. Exercise referral scheme – Redbridge residents who are 16 years and over, not active and whose medical condition would benefit from physical activity, can get their GP or practice nurse to refer them to this scheme. Staying healthy in other ways It’s important to look after your feet. No matter how mobile you are. Keeping feet healthy means less stress for the rest of your body. Foot problems can lead to other problems like back pain. If you have painful feet or any other foot problem, talk to your GP who can arrange for you to see a podiatrist (a specialist who looks after feet). Stopping smoking is essential if you want to live a healthier life and prevent major diseases. Smoking is the primary cause of preventable illness and death with 100,000 people dying every year in the UK. But we all know it can be difficult to quit – nicotine is a drug. The Redbridge NHS Stop Smoking service will provide free advice, support and encouragement to help you stop smoking for good. Stop smoking medications, including patches and gum are available on prescription. Everyone is different so there are different support options available. For help to quit smoking, call 0800 032 0102 (freephone) Monday to Friday 8.00am to 8.00pm and Saturday 10.00am to 1.00pm. You can leave a message outside of these hours or you can email on stopsmokingservice@nelft.nhs.uk You can also contact your GP for a full range of information and advice on how to stop smoking. Getting a good night’s sleep is very important to overall health. Most people at some point in their lives will have times when they’re not getting as much sleep as they need. Sleep patterns can be affected if you are worried about something.

Worried about your memory? It can be worrying if you or someone you know notices that you are becoming more forgetful or confused. It’s important to understand that, if your memory seems to be getting worse, it does not necessarily mean that you are developing dementia. If you or someone you know are worried about memory, you should first consult your GP. There are things which you can do to reduce the chances that you will develop dementia as you get older: • Exercising your brain Evidence suggests that keeping mentally active can help to prevent or slow down memory problems. Crossword puzzles, Sudoku, word searches and jigsaws are all good ways of keeping the mind active. You could also look into adult education courses at your local college or community centre, or find clubs and organisations that offer activities you enjoy, like board games, book discussions, music or art. • Staying fit and healthy Exercising helps the mind stay alert. You could try going for walks or to your local gym if you can manage it. Plenty of fruit, vegetables and protein in your diet will help to keep your body and brain healthy. A vitamin or mineral supplement might also be useful, particularly for older people. Make sure you have your blood pressure checked regularly by your GP. Support for people with dementia. Dementia describes a number of different conditions that can affect our brains, usually as we get older, and can cause a variety of symptoms including memory loss, confusion and increased problems in looking after ourselves (not everyone with memory loss has dementia though). Having dementia does not mean that you cannot have a say in how you are cared for. There are a number of services that can support people living with dementia and carers who look after them. 10


Support in Redbridge The Redbridge Dementia Partnership has an information pack which outlines specialist local organisations in Redbridge that can provide you with information, advice, practical and emotional support. Information and support can also be provided to individuals with moderate to advanced dementia. The service supports people with dementia to understand what is happening to them, enable them to continue to have a good quality of life and help to plan for the future. It can provide with information about Lasting Power of Attorney, personalisation, care and housing options as well as advanced care plans. It also offers support to carers and families of people with dementia who are adjusting to changes in their loved one and in the level of support they provide. Advisors support individuals and carers to access a wide range of services to meet their needs, as well as help with applications for benefits and transport schemes.

It provides information and support, funds medical and social research, and campaigns for better quality of life for people with dementia and greater understanding of the condition. Through its website, publications and National Dementia Helpline, Alzheimer’s Society provides comprehensive, reliable and up-to-date information about dementia to help people to understand it and empower people to live well with it. Mental Health Good mental health is about feeling well in your own mind. It’s just as important as good physical health. Our mental health can change over time, because of life events which cause distress or unhappiness, or sometimes with no apparent cause. These changes can lead to mental health difficulties or illness. Mental health difficulties are very common and affect one in four of us at some point in our lives. If you feel a bit down, alone, finding it difficult to be motivated or make decisions, this can be a sign that you have some issues that can be helped by talking to someone. Speak to your GP if you have any concerns about your mental health. You can also contact the Access, Assessment and Brief Intervention Service (Redbridge). Talking Therapies in Redbridge is a free, confidential NHS service that will work with you to help you feel better. They can help with a range of problems such as depression, anxiety, stress, bereavement and relationship problems. One Place East provides information and signposting for people experiencing or recovering from mental health problems and run a number of services including drop-in sessions, a befriending scheme for those who have become isolated and a volunteering project to support people into volunteering activities. There is also a mental health information directory and a user group called RUN-UP (Redbridge User Network User Pressure Group) that is open to people experiencing mental health problems and their carers. Learning Disability A Learning Disability affects the way someone understands information and how they communicate. They can have difficulty understanding new or complex information, learning new skills and coping independently. A person with some form of learning disability may find that everyday activities like work, education and jobs around the house can be a challenge. Sometimes they may feel lonely because they don’t have anyone to talk to about how they are feeling. Learning disabilities range from mild to being quite severe.

Support in Redbridge Uniting Friends is a social inclusion project for adults with a learning disability, living in Redbridge and surrounding boroughs. It provides opportunities for people to take part in social activities and provides befriending and support services in the community. Living with a physical disability If you have a physical disability, you can get support and advice that may help you to live more independently. You can register as a disabled person with Redbridge Adult Health and Social Care Services. This may help in getting concessions such as reduced fares for public transport. Your GP will need to refer you to a consultant in the first instance.

Support in Redbridge Redbridge Disabled Women’s Welfare Association offers signposting, information and advice to physically disabled and elderly women. They can visit individuals at home and in hospital and have weekly yoga sessions, awareness seminars and group outings. Support in Redbridge The Sensory Services Centre in Redbridge (part of Action on Hearing Loss) helps those who are deaf, hard of hearing, blind, partially sighted or deafblind. The centre offers advice, information and training on sensory loss, demonstrations on sensory loss equipment and hearing aid maintenance, as well as drop in sessions at three different locations in Redbridge. Redbridge Talking Newspaper Association is a charity which provides a free weekly audio cassette for the blind and partially sighted who live in Redbridge and those who have lived in Redbridge but have moved out of the borough. The tape contains both local news and magazine features. All ages are welcome to apply for the tape, however since the vast majority of listeners are elderly, the content of the tape is more applicable to this age group. Alcohol and Substance Use If you are worried about how much you are drinking, or about your drug use, the best place to first seek help may be your GP. Your GP can discuss your concerns with you, assess the nature of your problems, and help you choose the most appropriate treatment. Your GP may offer to treat you, or may refer you on to another service. R3 (Redbridge, Recovery and Reintegration) is a fully integrated, recoveryfocused service delivered by an organisation called WDP. They offer a free and confidential support service for individuals and their families affected by drug and alcohol problems. The service supports individuals and their families who wish to receive advice, assessment and treatment for their drug/or alcohol problems. The adult service is free and open to all Redbridge residents aged 18 or over, as well as their families and carers. Any professional, such as housing support worker, social worker or nurse, can make the referral on your behalf. Chapter 2 - At Home If you have a health problem or disability, or perhaps just getting a bit older, you might be finding it more difficult to get around your home and safely do everyday tasks such as washing, dressing, cooking, cleaning and going out. Most of us want to stay in our homes and there are many things you can do to maintain your independence and safety in your home. You might be surprised at how many people are able to stay in their own homes for most or all of their lives by taking a few measures to ensure their well-being and safety.

Help with walking and mobility Physiotherapists help people affected by injury, illness or disability by using movement and exercise, manual therapy, education and advice. Trips, slips and falls are among the most common types of accident to occur in the home. As you get older or less mobile, trips and falls can be more dangerous. Falling heavily can lead to serious injuries such as sprains or broken bones and sometimes people find it difficult to get up again. Alarm services at home It’s reassuring to know that by simply pressing a button on a pendant round your neck or wrist, help will quickly arrive if you suddenly become unwell at home, or have had a fall or accident and can’t get up. An alarm alerts a central team who will check that you are alright or contact named relatives or friends to check that you are okay. The central team or your named friend/relative will then come and help you if you trigger the alarm. There are other gadgets that can help, such as falls detectors and other sensors in the home to detect potential risks, burglar alarms and other safety devices etc. You can buy these gadgets online or by phone. Lifeline Residents may have heard of Lifeline as ‘The Community Alarm System’. Should you sign up for the Service, you will be provided with an alarm which is programmed to your home telephone, and a pendant which can be worn on the wrist or around the neck. When pressed, staff at the Redbridge Control Centre will be alerted to the fact that you need help and act appropriately. Immediate support is provided 24 hours a day, and the Service can include text messaging for those who are deaf or hearing impaired. To be eligible for the Redbridge Lifeline Service you must be resident in the Borough and have a land-line telephone. You will also need ideally two people who will be ‘key holders’ who can get to your address quickly to ensure the appropriate assistance can secure entry to your home. Telecare and Assisted Living Technology There is a vast range of technology available for installation in the home, designed to promote and prolong independent living. The use of bed and chair sensors, fall detectors and property exit sensors are of great benefit to many Redbridge residents. Such equipment, alongside monitors which can detect potentially dangerous situations such as smoke, fire, carbon monoxide and gas, offer not only independence for the Service User, but also peace of mind for family members. The various Telecare sensors are linked to the Lifeline alarm service, and are monitored by Redbridge Control Centre staff who will arrange help when the sensors are triggered. Redbridge Adult Health and Social Care Services work in partnership with Millbrook Healthcare, who provide this essential equipment to those who have been assessed and are entitled to Council support. To find out if you are entitled to Council support, your individual needs will have to be assessed. If you are eligible for support from the Council then there is no cost in getting the telecare equipment fitted but there may be a small weekly charge payable to Redbridge Lifeline, currently £2.10, while it is in your home, unless you receive either of the qualifying benefits listed above. Support in your home – ‘home care’ As we get older it can become more difficult to complete some everyday tasks. Staff will be trained in personal care and safety procedures, moving and handling, hygiene, infection control and first aid. If you need nursing care at home, many of these care providers employ registered nurses or these services may be provided by community or district nurses. Home care agencies can also help with shopping, cleaning and preparing your house for you after a stay in hospital. The Care Quality Commission (CQC) monitors care agencies and regularly inspects agencies and produces reports on its findings. Shopmobility Shopmobility lends manual wheelchairs, powered wheelchairs and powered scooters to people to help them shop and enjoy the facilities in Redbridge. It is available for those with injuries and long or short-term disabilities, whether your disability is temporary or permanent. You do not need to be registered disabled to use it, however there is a small fee for this service in the Ilford area.

You may have heard of people who have installed a stair lift, ramp or accessible shower in their home. Of course, these cost money. But there is often money available to help you to pay towards the cost of any work needed.

Adaptations to consider To make adaptations to your home you will need to seek specialist advice from trained health and social care professionals, equipment providers, or qualified builders and tradespeople. The following adaptations could make your home more suitable for you.

Chapter 5 - Staying Safe Everyone has a right to feel safe and to live without fear of abuse, neglect or exploitation. Redbridge Adult Health and Social Care Services and partner organisations are working to make life safer for adults who are most at risk of being harmed. An adult at risk is someone 18 or over who may be in need of care and support because of a disability or illness, experiencing or is at risk of abuse or neglect and as a result unable to protect themselves.

What is abuse? Abuse is something done to another person that harms them in some way. This can be: • •

Physical – hitting, pushing, shaking, over-medicating Domestic violence – psychological, physical, sexual, financial, emotional, ‘honour’ based violence • Sexual – rape and sexual assault that a person does not want or understand • Psychological – shouting, swearing or threats • Financial or material – theft, fraud, internet scamming • Modern slavery - slavery, human trafficking, forced labour • Discriminatory – racist, sexist abuse or abuse based on a person’s age, disability, gender, religion • Organisational e.g. neglect and poor care within an institution or care setting • Neglect and acts of omission e.g. not providing food, drink, or medical care • Self-neglect – neglecting to care for one’s personal hygiene, health or surroundings Abuse can happen to anyone, and the abuser can be someone you know well or a stranger. Chapter 6 - Getting Out and About Being able to get out and about in the local community, whether to catch up with friends, do the shopping, or do things of interest is important for all of us, and vital for our physical and mental well-being. If you have a health problem or disability, or are becoming frailer as you get older, then getting out and about can be a challenge. But you can still lead an active life away from home.

Help with transport Redbridge Mobility Card Scheme The Redbridge Mobility Card Scheme provides a door-to-door transportation service for disabled Redbridge residents. The service is primarily within a ten mile radius of Gants Hill roundabout. It is available every day between 7.00am and 12.00am (midnight) and can be used for all sorts of journeys such as shopping, errands or visiting the dentist. You will be advised of the cost of your journey at the time of booking. The minimum charge is £2.50. Freedom Pass The Freedom Pass lets you travel for free on public transport across London and within the Freedom Pass boundary. This includes National Rail, London Overground and buses where pass holders can travel between 9.30am and 11.00pm. Blue Parking Badges If you have a disability, the Blue Badge scheme helps you park closer to your destination. You can generally park for free in pay-and-display and parking meter spaces with no time restrictions, and on single and double yellow lines for up to 3 hours as long as there isn’t a loading ban and you aren’t creating an obstruction. You don’t need to be able to drive to apply for a Blue Badge – you can specify a ‘nominated driver’ who can be someone other than you. However, eligibility criteria apply, and you will need to supply relevant evidence, and possibly take part in a mobility assessment. Wheelchairs Redbridge wheelchair service provides and maintains wheelchairs for adults or children unable to walk or who have a permanent disability which affects their walking. The service will not treat anyone with a temporary disability which stops them walking. They will support you with your clinical needs and provide you with a wheelchair and postural support seating that’s right for you, as well as support you with maintenance and periodic reviews to see how you are getting on. The service is provided by AJM Healthcare. You must be referred by a healthcare professional, such as a GP, nurse or physiotherapist to access the service. National eligibility criteria There is a minimum threshold for eligible care and support needs that has been set by central government to ensure that all local authorities meet the same minimum level of needs. The aim is to make sure that all decisions on who receives help with care and support are made fairly, openly and consistently. You may be eligible for support from the council if: •

your needs arise from, or are related to, a physical or mental impairment or illness; and • as a result of those needs, you are unable to achieve two more of the specified outcomes (examples of these are managing and maintaining nutrition, maintaining personal hygiene and managing toilet needs); and • as a consequence of being unable to achieve these outcomes there is, or there is likely to be, a significant impact on your wellbeing. For your needs to be considered eligible for support, you must meet all three criteria. In other words, if you are unable to achieve two or more of the above outcomes, but this doesn’t have a significant impact on your wellbeing, then you are not eligible for support. Carers’ eligibility works in a similar way to this. Eligibility is based on the impact of caring on the carer’s wellbeing.

Our services are free, easy to access and confidential. Free Advice - for upholding adults’ Health and Care Act rights

CASCAIDr is a new national advice charity, providing specialist legal advice about everyone’s rights to have their needs for care and support decided lawfully, reasonably and fairly. We aim to empower and support people, their carers and practitioners working in the health and care sector to resolve disagreements about access to services.

Most people only seek support or come to rely on social care services and funding, in times of crisis. Family members and carers, exhausted by providing dedicated care, rarely have the energy required to navigate referral processes or complaints procedures to secure fair processes and lawful packages of care.

WHAT SORT OF PROBLEMS DO WE TAKE ON FOR FREE? • Access to funded independent advocacy rights; • Whether assessments are compliant with the law; • Whether councils are following the statutory Guidance; • Rights to funding for sufficient levels of care to avoid support needs escalating or sacrificing carers’ wellbeing; • The legality of proposed cuts to care plans. For wider problems, complaints, more complex issues, etc, we charge a low cost fee of £125 ph.

WHAT DOES CASCAIDr’s SERVICE INCLUDE? • A telephone helpline - 4 days a week - for a booked half hour free ‘steer’ for people who are not sure if they’ve even got a legal problem. • A referral form for people to fill in online.

• Volunteers to help people to express all the relevant detail of their predicament, if they are struggling. • Expert advisers who will produce the checklist and letters that are the free output. • Support and guidance for those who want to crowdfund to bring formal legal proceedings.

WHAT CAN PEOPLE EXPECT? CASCAIDr ADVISERS WILL: • Analyse a situation for omissions to comply with the Care Act or public law principles. • Give the client a checklist to take back to the council/CCG. • Write a letter in the form of a pre-action protocol letter, setting out what’s not been done correctly • Seek advice on the client’s behalf, from a direct access barrister. • Consider helping the client crowdfund so that the council or CCG knows that the client can, if required, challenge unlawful decisions in Court. Where issues can’t be resolved through dialogue and a barrister has given a positive opinion on the merits of the case, CASCAIDr may support crowdfunding to secure legal challenges. This not only strengthens an individual’s position, but also supports widespread lawful decision-making by clarifying any areas of uncertainty in the current legal framework.
