Help for hearing problems Hearing is important Do you hear but not always understand? The probability that you will answer yes to this question could be more common than you may have realized. In North America an estimated 31 million people have some degree of hearing loss. Approximately 11 million of these people are 65 years of age and over and another 10 million are between the ages of 48 & 64 years of age. Why is hearing so important? Hearing occurs in our brains and keeping a clear signal from our ears helps not just our ability to communicate effectively but can affect our overall health.
Symptoms of hearing loss Fear, loneliness, isolation, social withdrawal, anxiety, confusion, depression and paranoia are symptoms of hearing loss. You should be concerned about leaving hearing loss untreated. It may affect your job, your marriage, bring frustration to your family and friends without you realizing it. Denial of the possibility you may suffer from some form of hearing loss can easily be overcome by seeking help and finding out for sure.
Help for hearing problems Modern technology makes better hearing possible. Just as a person with a vision impairment understands that glasses or surgery will correct their vision problem,

help for hearing impairment begins when a person is willing to invest the time and effort to have their problem tested and treated. Most hearing problems can be helped, either medically or with amplification. The quality of help today far surpasses what was available even a few years ago! Which hearing instrument is right for me? There are many different approaches to hearing instrumentation. Some instruments are very small and cosmetically appealing, however the smaller the hearing instrument, the smaller the battery which makes changing them more challenging as well as more frequent. Some styles are more appropriate for certain hearing losses and matching the device to the hearing needs is of great importance in the results of the application. Home Health Definitions: Cardiac Care refers to services including patient assessment, specific diet information, and management of the disease process, medication management and education. Cardiac rehabilitation is a program designed to help a person with heart disease return to health. Cardiac rehabilitation may consist of exercise, modifying risk factors to prevent further heart damage, or psychological rehabilitation.
Diabetic Management refers to services including patient assessment, diet information, management of the disease process, medication management and education on all of the above. Services are provided by a Registered Nurse under the supervision of an M.D. Some agencies may employ certified diabetic nurse instructors or cardiac care specialty RN’s. Home Health Aides provide support services such as personal care which do not require a licensed nurse. Medicare does not cover home health aides unless the person is also receiving skilled nursing care or other therapy. Certified Nursing Assistants (CNAs) provide personal care such as bathing, dressing, transporting and other essential activities.
Hospice is a comprehensive program of care to patients and families facing a life threatening illness. Hospice emphasizes palliative rather than curative treatment. The dying are comforted. Professional medical care is given, and sophisticated symptom relief provided. Very few Home Health Agencies are also licensed for Hospice care. In-Home Care/Personal Care is care related to activities of daily living, including: mobility, eating, toileting, bathing and personal hygiene, dressing and grooming or cognition support. These services are usually provided on a private pay basis and do not require a physician’s order. Intravenous Therapy is any medical treatments/medications that need to be introduced to the body via a patient’s veins. Numerous IV Therapies can be provided inhome under supervision of a MD. Some of these include: inter-infective/antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance. Psychiatric Services refers to the agencies ability to offer services related to psychiatric support with the prevention, diagnosis, and treatment of mental illness. Services may include counseling or administering of medications. Registry Services refer, but do not employ, independent caregivers. Clients control all aspects of the arrangement, including compensation, directly with their caregivers. Registries may include free non-profit listing services or companies that require training, bonding and insurance of their caregivers. Clients are charged a referral fee.

Skilled Nursing is a higher level of care where services are preformed by a licensed Registered Nurse or Licensed Practical Nurse. Skilled Nursing includes, but is not limited to patient assessments, performance of prescribed medical treatments, administration of prescribed medications, education for the patient and the caregiver (if available) related to the disease process, treatment, health measures and medication management. Skilled Nurses provide medical care as prescribed by physician.
Care & Health
Respite Care is short-term or temporary. It is designed to give families a break from the stress of daily caregiving. Respite care can be for a few hours, overnight, or for days. The amount of care changes with the needs of the family and the ill or disabled person.
Social Worker refers to a clinical social worker whose purpose in health care is to enhance, promote, maintain, and restore the best possible social abilities of the patient or family. Services provided may be preventive, developmental, or remedial in nature.
24-hour care is “round the clock treatment� in a home environment. The care may be shared by multiple caregivers or by a single caregiver. Therapy Services (ST, PT, OT, RT) Occupational Therapy, (OT) refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Therapists also help them to develop, recover, or maintain daily living and work skills. Occupational therapists help clients not only to improve their basic motor functions and reasoning abilities, but also to compensate for permanent loss of function. The goal is to help patients become as independent, productive, and satisfied as possible. Physical Therapy (PT) refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills. Exercise techniques are only part of the special skills used by therapists to treat patients to help lessen disability, pain and improve overall function and quality of life. Respiratory Therapy (RT) refers to services for care of patients with deficiencies and abnormalities associated with the cardiopulmonary system. It may include the therapeutic use of medical gases and their equipment, environmental control systems, humidification, aerosols, ventilator support, bronchi pulmonary drainage and exercise, respiratory rehabilitation, assistance with cardiopulmonary resuscitation, and maintenance of natural, artificial, and mechanical airways. Speech Therapy (ST) refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing. A Therapist provides individually designed treatment programs to maximize communication and swallowing.

Raising the Kids’ Kids Are you a grandparent raising a grandchild? If you are, you’re one of more than 2.4 million other grandparents nationwide doing the same. When the children’s families needed help, the grandparents stepped up and volunteered to take care of grandchildren. Sometimes, a parent has died; in other cases, substance abuse or mental health disorders have afflicted parents. Some parents are incarcerated; and some are serving in the military. According to the 2000 census, about 4.5 million children across the country (6.3% of all children under the age of 18) lived in households headed by grandparents. That number increased by 30% since 1990. In Oregon, 37,500 children lived in grandparent headed households. Those grandparents are probably a lot like you. Most are younger than 60; many feel like they’re all alone; and many don’t know where to turn for the help they need. There is so much to learn when you take over the raising of a grandchild, so much to think about, so many problems to solve. By stepping in to care for their grandchildren when parents are absent, these caregivers provide stability for children and keep children out of the formal foster care system. They have made great sacrifices to help care for and protect these children, and would have it no other way. They hope to be able to continue care for the children and keep them out of foster care, but they need help.

These family caregivers have unique needs which are not adequately addressed by existing laws or policies governing non-parental care, which are often misunderstood, or which simply don’t exist. The burdens can be immense. When can you make decisions for the children? What does it take to get them enrolled in school? Is there financial aid from the government? Can we qualify for food stamps? What do we do if the house isn’t big enough? Currently, a few states have enacted legislation and created programs to ease some of these issues and to provide assistance. Oregon has not yet done so. However, there are numerous other resources. Your grandchild may be eligible for Social Security or Medicaid, or able to get money form the state’s Temporary Assistance for Needy Families program. Aid and Attendance Pension The Aid and Attendance pension benefit may be available to wartime veterans and surviving spouses who have in-home care or who live in nursing-homes or assistedliving facilities. Although this is not a new program, not everyone is aware of his or her potential eligibility. “Veterans have earned this benefit by their service to our nation,” said Secretary of Veterans Affairs Jim Nicholson. “We want to ensure that every veteran or surviving spouse who qualifies has the chance to apply.” Many elderly veterans and surviving spouses whose incomes are above the congressionally mandated legal limit for a VA pension may still be eligible for the special monthly Aid and Attendance benefit if they have large medical expenses, including nursing home expenses, for which they do not receive reimbursement. To qualify, claimants must be incapable of self support and in need of regular personal assistance. The basic criteria for the Aid and Attendance benefit include, but not limited to the inability to feed oneself, to dress and undress without assistance, or to take care of one’s own bodily needs. People who are bedridden or need help to adjust special prosthetic or orthopedic devices may also be eligible, as well as those who have a physical or mental injury or illness that requires regular assistance to protect them from hazards or dangers in their daily environment.

For a wartime veteran or surviving spouse to qualify, the veteran must have served at least 90 days of active military service, one day of which was during a period of war, and be discharged under conditions other than dishonorable. Wartime veterans who entered active duty on or after September 8, 1980, (October 16, 1981, for officers) must have completed at least 24 continuous months of military service or the period for which they were ordered to active duty. If all requirements are met, VA determines eligibility for the Aid and Attendance benefit by adjusting for un-reimbursed medical expenses from the veteran’s or surviving spouse’s total household income. If the remaining amount falls below the annual income threshold for the Aid and Attendance benefit, VA pays the difference between the claimant’s income and the Aid and Attendance threshold. The Aid and Attendance income threshold for a veteran without dependents is now $19,736 annually. The threshold increases to $23,396 if a veteran has one dependent, and by $2020 for each additional dependent. The annual Aid and Attendance threshold for a surviving spouse alone is $12,681. This threshold increases to $15,128 if there is one dependent child, and by $2020 for each additional child. Housing Definitions: Adult Day Care Programs offer a protective setting during the day, for those who cannot be left alone due to health care and social needs. The structured program within a community setting offers: socialization, planned activities, care services, companionship and meals. Adult Day Care provides daytime respite for the caregiver who typically works, needs time for other responsibilities, to simply to take a break. Some programs are designed for older adults with dementia and/or physical limitations. Programs through senior housing communities that are listed in this guide must be able accept non-residents to be included. Adult Care Home or Family Care Home are private residences that provides a home-like setting, and serves 5 or less residents. The caregiver may perform several functions, such as personal care, housekeeping and activities and group meals. There are three levels of Adult Foster Care Home licenses. The classification system is based on level of care the Adult Foster Home may provide to residents who live in the home as well as the experience and training of the providers and their ability to assist residents with: personal hygiene, mobility, eating, dressing, toileting, and behavior management. Residents may require very light assistance or may be dependent with several care needs; it is best to ask individual locations for their specific policies and license. Assisted Living Communities are State-regulated rental properties where six or more residents may reside. Care assistance is available, including: medication management, bathing, dressing, toileting and eating. Apartments must be fully self-contained private living units with a lockable door, private bathroom, and kitchenette facilities. The fee schedule is regular monthly rent along with additional fees for specific services and amenities. Assisted living communities are best suited for individuals who want to remain as independent as possible and who are able to direct their own care. CCRCs (Continuing Care Retirement Communities) are communities that include a continuum of care from independent living, assisted living, residential care and skilled nursing services all on one campus, allowing individuals to live within the same community as their needs progress through the spectrum of care. They typically offer the full selection of amenities associated with retirement living. An endowment fee in addition to a monthly maintenance fee can be expected. Independent Living Communities provide residents an independent living setting without the burden of home ownership. Typically, residency is established on a monthly rental basis. Residents live in fully equipped private apartments or cottages from studios to large two-bedroom units that may be rental-assisted or market-rate depending on the community. Amenities and hospitality services such as housekeeping, linen service, transportation and social and recreational activities may be included, provided for an additional charge, or may not be available at all. These communities do not provide assistance with activities of daily living or personal care.

Memory Care or Dementia Communities offer or provide care for residents with Alzheimer’s disease or other dementia in a home like environment. These communities, must receive an endorsement and is governed by additional regulations that are specifically intended to support individuals with dementia, including: a secure building that alerts staff if a resident has exited, a secure outdoor area that provides outdoor freedom safely, interior finishes that are non-glare and well lit, and visual contrasts between floors, walls and doorways. Alzheimer’s units must also have programs, which include: gross motor, self care, social, craft, sensory enhancement and outdoor activities. Rehabilitation & Therapy is treatment for an injury, illness, or pain with the goal of restoring function, including nursing and therapy services. Rehab is ordered by a physician and services are provided by nurses and physical, occupational, and speech therapists. Examples include working with a physical therapist to help you walk and with an occupational therapist to help you get dressed. Residential Care Facilities provide housing and supportive services for six or more people who do not require 24 hour nursing care. Pricing can vary greatly depending on the level of care provided and the size and amenities of each apartment. Accommodations are typically in a home like setting and may range from a shared bedroom with common bathroom, to private apartments with kitchenette and private bathroom. Monthly fees vary greatly based on unit amenities and care services. Many residential care facilities specialize in caring for individuals with Alzheimer’s or dementia. Respite Care is short term relief for family caregivers to take a break from the caregiving responsibilities and allows for a healthier and better quality of life for both the caregiver and care receiver.. Respite services include: companionship, personal care, recreational activities and security. Respite care can be provided as short stay or for a longer period that allows the family caregiver to go on vacation or a business trip. Longer duration respite care is generally found at select senior housing communities although some Companion services may provide this service in the home. Skilled Nursing Facility (SNF) or Nursing Homes are Statelicensed facilities that provide a safe, therapeutic environment for individuals who require rehabilitative care or can no longer live independently due to functional or cognitive impairment. Nursing Homes offer 24-hour skilled nursing care and medical services by registered nurses and support professionals. This is the highest level of care that can be provided that is not hospitalization. Additionally, nursing facilities offer residents planned social, recreational and spiritual activities. Nursing facilities are most appropriate for people who need 24-hour medical oversight and a protective/structured setting. Most residents must share their room, but residents are allowed to bring personal items to encourage a more home-like atmosphere. Choosing A Community A good match between a community anda resident’s needs depends as much on the philosophy and services as it does on the quality of care. The following suggestions can help you get started in your search for a safe, comfortable and appropriate assisted living community: What are your future needs and how will the community meet those needs? Is the community close to family and friends? Are there any shopping centers or other businesses nearby (within walking distance)? Visit each community more than once, sometimes unannounced. Visit at meal times, sample the food, and observe the quality. Observe interactions among residents and staff and ask questions. Ask about social, recreational, and spiritual activities. Ask about the type and frequency of staff trainings. Review state licensing reports.

In many states, the ombudsman checks on conditions at assisted living units and nursing homes. If the assisted living community is connected to a nursing home, ask for information about it, too. Share The Joy During the Holidays Tips to Help Caregiver’s and Those Living With Alzheimer’s Disease The hustle and bustle of the holidays comes to every household. When you are caring for someone with Alzheimer’s it’s not all “joy”. The shift in routine, decorations, and influx of visitors can put a damper on everyone’s holiday spirit. How to welcome holiday guests who do not understand the situation is often a dilemma for caregivers. “Concerns will arise from those who are surprised of the progression of the disease, and some visitors will not understand why Grandpa has changed,” said Jo Gosselin, Executive Director of the Alzheimer’s Network.
Consider sending out a note to anyone planning on dropping by. Lay out simple guidelines and explain the progression of disease and how it will affect the visit. “For a smooth visit, avoid loud noises, long stays and drawn out questions that only cause more confusion. Don’t hesitate to schedule morning and afternoon visits, things may go smoother when starting out rested,” Gosselin said. Being prepared will help everyone feel like the visit was a success and there will be fewer surprises. Focus on the “can do” and not the “we used to,” said Gosselin. Don’t concentrate on what holiday traditions you can’t enjoy together, instead make new ones or scale down the old ones. Make treats together in the kitchen. Measuring, mixing and a little soft music can be quality time together.

The holiday’s can be rather animated, dancing Santa’s, blinking lights and talking reindeer don’t bring a calm environment and can be confusing for someone suffering from Alzheimer’s. Keeping decorations simple and not making a lot of changes can help with confusion. Rest and Relaxation make for a real nice holiday. Make sure everyone including the caregiver is getting enough rest. “Less is more, having a blessed time is more important that rushing through twelve events and feeling harried and frazzled in the end,” said Gosselin. Don’t ignore family members who have offered to help. Take time to enjoy the festivities with friends or other family members. The holiday may have changed, but embrace the warmth of the season and take what works for you, and resist pressure from those events that overwhelm you. Questions about Alzheimer’s? Talking to Your Family About Aging and Incapacity The next time you have a family gathering, one of the best things you can do for yourself and for your family is to engage everyone in an honest and open discussion about aging and disability planning. Many adult children and their parents think about the potential for incapacity, but the majority of them never bring up the matter with each other. Top of the list of worries are money, financial management and health care decisions:
1. Your Financial Situation One of the biggest worries for parents and their children is money. Parents and children each worry about whether the parents will have enough money so they can enjoy their retirement and, if necessary, pay the costs of long term care. Maybe this is a conversation you also want to have with a financial planner to make sure you have covered all the bases. A family discussion about this matter can take the fear and emotion out of this topic for both parents and children.
2. Who Will Handle Your Finances If You Become Ill? Decide who you want as your agent to handle your finances if you are incapacitated and how you want your finances managed, When you have made these decisions, share this information with your loved ones. And, seek out an

experienced elder law attorney who can prepare a Durable Power of Attorney, a document that will specify your wishes and can be used by your agent for bill paying, managing your retirement and insurance benefits, and anything else that may be necessary.
3. Who Do You Want To Help You Make Medical Decisions And What Types Of Life Support Do You Want If You Are Seriously Ill? Discuss these matters with your loved ones and put them in writing in an Advance Directive for Health Care. This document names the person you want to make and communicate your wishes and addresses your choices concerning what type of medical intervention and life support you want if you are seriously ill. Make this discussion a family affair. Get everyone in on the conversation and on board with your wishes. By openly discussing and sharing your thoughts and concerns with your family, both you and your loved ones can learn how to face and plan for the challenges of aging together, without creating undue stress or burdens. Every Home Needs a Little Attention Even a dream home will turn into a nightmare if regular maintenance is not part of your standard routine.
In your home, as in life, few things are maintenance free. Despite the time and money it consumes, preventive home maintenance is far more cost effective than the crisis management approach of waiting until something breaks and then scrambling to have it repaired. So, maintain your home for the same reasons that you maintain your health, because without regular attention both you and your home are headed for trouble and costly repairs down the road. Like a visit to the dentist, your home preventive maintenance plan really needs two components. Routine upkeep is similar to the cleaning your hygienist provides. And a monitoring plan that covers structural, mechanical, plumbing, and electrical systems, as well as appliances is like the diagnostic exam the dentist provides. There are so many home maintenance and repair items that are important it can be hard to decide which are most critical. At Hey Handyman we emphasize preventive maintenance as a factor in a safe home environment. We recommend that you start by testing or installing smoke and carbon monoxide detectors and at least two multipurpose fire extinguishers. Find and correct electrical hazards, remove or correct trip hazards, and finally locate and mark the shut offs for gas, electric, plumbing and heating systems.

Next, establish a maintenance schedule. Some routine upkeep chores like testing ground fault interrupters should be carried out monthly. Others, like servicing the furnace or cleaning out gutters, might be performed semiannually. Ideally, preventive maintenance inspections should be performed semiannually in the spring and fall, but some areas are easier to monitor in the spring, some in summer, and others in the fall or winter. Assign each area for a particular month to break the job into manageable size and ensure regular inspection of each area. Everything wears out eventually. Thatâ&#x20AC;&#x2122;s why housing experts recommend budgeting between one and three percent of the market value of the house each year for maintenance and repair costs. But the news isnâ&#x20AC;&#x2122;t all bad. An investment in a scheduled home maintenance program will reduce your total cost of home ownership.
