Application for Certificate of Special Recognition for Community Service and Volunteerism Qualifying for the Society’s Certificate of Special Recognition for Community Service and Volunteerism Members of The American Society for Aesthetic Plastic Surgery (ASAPS) participate in a wide range of volunteer activities that contribute substantially to the welfare of their communities and, in many cases, the world. To recognize these outstanding efforts, ASAPS is awarding a Certificate of Special Recognition for Community Service and Volunteerism. Members qualifying for this certificate will also be recognized in a special area on the Society’s web site at www.surgery.org, with a link to their Enhanced Practice Profile (EPP). To qualify for this special recognition, the recipient must meet the following criteria: 1. Be an Active, Life or International Active Member of The American Society for Aesthetic Plastic Surgery 2. Commit 25 or more hours to community service or volunteer activities over a 12-month period 3. Requisites for Certificate of Advanced Education in Cosmetic Surgery The American Society for Aesthetic Plastic Surgery (ASAPS) has created an award program to recognize your special commitment to cosmetic surgery continuing education. The ASAPS Certificate of Advanced Education in Cosmetic Surgery (ASAPS CAE) is available to every ASAPS member who earns 150 CME credits (Categories 1 and 2) in cosmetic surgery over a three-year period.

Applicable Category 1 Credit ASAPS members may be awarded Category 1 credit toward the ASAPS CAE for activities designated for Category 1 credit by Accreditation Council for Continuing Medical Education (ACCME) accredited sponsors, provided that the content of the specific educational activity is focused on cosmetic surgery.

ASAPS has structured the program to make it accessible through a variety of educational options. Applying for the certificate is simple. Just complete and sign the enclosed form, attaching any applicable transcripts of educational activities provided by medical societies, hospitals and other organizations, and send it to the ASAPS Central Office. As the plastic surgery organization dedicated to your cosmetic surgery education, ASAPS invites you to apply now for this important award.

A New Recognition Award Exclusively for ASAPS Members The American Society for Aesthetic Plastic Surgery Certificate of Advanced Education in Cosmetic Surgery (ASAPS CAE) is designed to recognize members who demonstrate an exceptional level of commitment to their continuing education in cosmetic surgery. The requirements for the certificate are fulfilled by earning a specified number of Continuing Medical Education (CME) credits in cosmetic surgery over a three-year period (for example, certificates issued in January 2007 were for credits earned December 2004 through December 2006). The details of how credits are calculated and the certificate application process are outlined in this brochure. The basic model for the ASAPS CAE is the American Medical Association Physician’s Recognition Award (AMA PRA). The types of eligible activities are traditional CME, except that activities earning credit toward the ASAPS CAE must be related exclusively to continuing education in cosmetic surgery. Those ASAPS members who qualify will receive a handsome, engraved certificate suitable for display in their office, exam room or patient reception area. Duplicate certificates may be ordered. Your patients will appreciate knowing that you have achieved a recognized level of continuing education in cosmetic surgery and that this award is updated regularly to reflect your current knowledge in the field. 150 CME Credit Hours in Cosmetic Surgery The ASAPS CAE is awarded only to qualifying ASAPS members. A total of 150 CME credits in cosmetic surgery over a three-year period is required. Sixty (60) of these credits must be in Category 1. Ninety (90) credits may be in Category 1 or Category 2. All categories are as defined by the AMA PRA, except that the activities must be focused on cosmetic surgery. Definition of CME All activities and credits reported to fulfill the requirements for the ASAPS CAE must comply with: • AMA definition of CME • AMA ethical opinions on Gifts to Physicians from Industry and on Ethical Issues in CME. CME consists of educational activities that serve to maintain, develop or increase the knowledge, skills and professional performance and relationships a physician uses to provide services for patients, the public, or the profession. The content of CME is that body of knowledge and skills generally recognized and accepted by the profession as within the basic medical sciences, the discipline of clinical medicine, and the provision of health care to the public (AMA House of Delegates definition of CME). If you have questions about AMA opinions on ethical issues, you may request information from the AMA Council on Ethical and Judicial Affairs at 312-464-5223.

Those applying for the certificate should claim only the number or hours commensurate with actual participation in the activity or studying the materials, regardless of the maximum number of credits for the activity designated by the sponsor. The types of activities qualifying for Category 1 credit include:

• CME activities in journals • CME enduring materials including printed educational materials, audiotapes, videotapes, computer-assisted instruction, DVDs and CD-ROM products • Other meritorious learning experiences of exceptional benefit with prior approval by ASAPS ASAPS provides plastic surgeons with a variety of ways to earn Category 1 CME credits. For example, the ASAPS/ASERF Annual Meeting each spring typically provides attendees with the opportunity to earn more than 50 of the 60 Category 1 credits required for the ASAPS CAE. In addition, the CME articles and exams in ASAPS’ bimonthly Aesthetic Surgery Journal allow readers to earn one Category 1 CME credit for each successfully completed exam. ASAPS symposia, as well as meetings with cosmetic surgery content sponsored by the American Society of Plastic Surgeons (ASPS), regional plastic surgery societies and other ACCME accredited sponsors provide many avenues for fulfillment of the ASAPS CAE requirements. U.S. and Canadian members who participate in accredited continuing medical education activities sponsored by Canadian medical school can report that participation for Category 1 credit. In general, physicians must maintain their own records of CME credits based on Certificates of Category 1 credit, printed programs or conference brochures. Include the name of the accredited sponsor designating Category 1 credit, the maximum number of credits that may be earned (physicians may claim credits only for their actual participation), and the title, location, and exact dates of the educational activity. Applicable Category 2 Credit The types of activities qualifying for Category 2 credit toward the ASAPS CAE include: • Learning experiences including consultation with peers and medical experts; developing and reviewing quality assessment data; use of enduring materials; small group discussions; self-assessment activities; teaching; medical writing; teleconferences; preceptorships and lectures; seminars and workshops not designated for credit • Up to 10 hours may be claimed for publication of each article or book chapter related to cosmetic surgery, or learning for the purpose of a scientific exhibit preparation. Documentation of Category 2 education is not always possible, and the physician’s report of such activities is acceptable (continued on next page)
