Disability Resource Center  Student self-advocacy is an important aspect of the disability accommodation agreement.  It is the student’s responsibility to deliver a Memo of Accommodations to each instructor

for each course in which they are enrolled. Delivery can be hard copy and/or email attachment.  This Memo of Accommodations will list all accommodations for which the student is

approved. Each instructor will receive the same memo.  The student is instructed to work collaboratively with each instructor to determine which

approved accommodation(s) they will be utilizing.  We no longer require instructor and chair signatures.

Memorandum of Accommodations
