Out in the cold

Enjoy outdoor Colorado’s winter activities

C

olorado is a great place for families to enjoy year-round outdoor activities. The colder weather is swiftly approaching and being prepared and staying safe is the best way to experience all our great state has to offer. Here are six quick and easy safety tips for your family. Layers, layers, layers. The sun keeps us nice and warm during the day but as the sun goes down, it can quickly cool down. Bring extra layers in case of quick weather changes or if you know you will be staying out past sundown. When strapping a child into a 5-point harness car seat, keep on only the lightest layer to help you gauge the tightness of the harness. You should not be able to pinch the shoulder strap webbing at the top of the shoulders. Puffy jackets can make it seem tight, but the force of a crash could compress the material and cause the child to be looser than you thought. • Apply sunscreen. With our higher altitude and strong sun, it is still possible to get sunburned in the winter. Especially if there is snow on the ground as it reflects the sun.

6 | RMPARENT

| FAMILY HEALTH GUIDE

• Wear your helmet. When skiing and snowboarding, wearing a helmet can decrease the risk of injuries by up to 50 percent. Kids are always striving to be like us so it is important to be a good role model. Make it a family outing where everyone picks out her own helmet. Also, if a child is just learning how to ski or snowboard, lessons are a great way for them to build confidence and reduce their risk of injury. • Stay hydrated. Drier winters plus higher altitudes equals a higher output of fluids. This is because moisture from your skin evaporates quicker plus we exhale and perspire twice as much moisture. Bring plenty of water for the entire family to stay hydrated. Coffee, tea, soda and juice can take fluids from your body so water is best. • Install and check smoke and carbon monoxide detectors. Both smoke detectors and carbon monoxide detectors should be installed on every level of the home and in all sleeping areas. Check monthly to make sure each detector works and that they aren’t expired. They expire 10 years after the manufacturing

date on the alarm. And, while it is still warm enough, create and practice a fire escape plan with the entire family. This way everyone will feel prepared in case of a true emergency. • Watch for frostbite and hypothermia. If the family has been outside for a long time or any clothing has gotten wet, it is much easier to get frostbite and/or hypothermia. Coming inside to warm up and put on dry clothes is a great way to prevent both. If you see pale, grey or blistered skin, especially on the ears, nose, fingers and toes, these are signs of frostbite. Run the affected area under warm (not hot) water and visit an urgent care if color does not return to the area. As for hypothermia, signs include slurred speech, shivering and unusual clumsiness. Call 9-1-1 immediately if you believe your child has hypothermia. Safe Kids Larimer County works to prevent accidental childhood injury, the number one cause of death for children in the United State. For more information, visit safekids.org or sklarimer.org.

Avoid colds and flu this season

Follow these five tips from webmd.com reduce your risk of contracting illness. 1. Wash your hands. Use soap and warm water for 20 seconds. Wash after going to the bathroom, coming in from outside, after using a tissue, when hands look dirty, and before eating. 2. Use separate cups and utensils. Remember people are contagious before they start sniffling or coughing. 3. Cough into your arm. Teach children to cough or sneeze into the inside of her elbow.

No nasal spray

The flu vaccine will be by injection only this year KATIE HARRIS

S

queamish kids and adults alike will have to tough it out this flu season, as for the first time in over a decade, an injectable vaccine will be the only available option for fighting the flu. For 13 years the nasal spray flu vaccine (LAIV ) has offered an alternate route to prevention for the nearly 20 percent of all patients with needle aversions, but in an unprecedented move last spring the Center for Disease Control (CDC) recommended that LAIV not be circulated this season. According to www.cdc.gov, data from the 2015-2016 flu season found the nasal spray to be only 3 percent effective in patients ages 2 to 17. “This 3-percent estimate means no protective benefit could be measured,” says a media release from the CDC on June 22nd. “The data from 2015-2016 follows two previous seasons (20132014 and 2014-2015) showing poor and/or lower than expected vaccine effectiveness (VE) for LAIV.” In comparison, the injectable version of the vaccine had an effectiveness of 63 percent for the same age group. Senior Director of Infection Prevention for Banner Health, Joan

8 | RMPARENT

| FAMILY HEALTH GUIDE

Ivaska, says initial studies back in the early 2000s found the nasal spray to be as effective or even more effective than the injectable version, but with the outbreak of H1N1 a few years ago the effectiveness plummeted, especially in 2- to 8-year-old children. Despite the discontinuation of LAIV, the CDC does not expect a shortage of vaccines this season. “The nasal spray only made up about 8 percent of available vaccines each season, so it was a relatively small amount of what was circulated,” she says. “Manufacturers along with the CDC have spent the summer making sure that enough injectable vaccines will be available for everyone.” Ivaska says that, though the most commonly used vaccine is now considered safe for everyone except those suffering from true, anaphylactic egg allergies, there are also egg-free and preservative-free versions available upon request, making the vaccine appropriate for virtually everyone these days.” “The nasal spray was only recommended for ages 2 to 49, in good health,” says Ivaska. “The injectable vaccine is recommended for everyone over 6 months.”

4. Get enough rest. Kids need 10-11 hours a night for school-age children (ages 5-10) and more for younger children. This helps keep their immune system strong. 5. It’s okay to postpone a playdate. Help your child understand that when he is sick, he needs to say home.

The CDC and Banner Health recommend that everyone over 6 months of age be vaccinated against the flu this season. In addition, recommendations for staying healthy include proper hand washing, teaching children to cover their mouths properly when they cough, and keeping them home when they have a fever or respiratory illness. Banner health offers vaccinations at all of their medical office locations and retail pharmacies. The organization is also hosting flu vaccine drop-in clinics this season, which are open to the public. Clinic dates and locations are available at www.bannerhealth. com/nocowellness. “The injectable vaccine is still the best way to ward off the flu, and has been shown to reduce symptoms in those patients who contract the flu after being vaccinated,” says Ivaska. As for the shot itself, Ivaska suggested that parents talk to their children ahead of time about the importance of getting vaccinated, and keep their children as calm and distracted as possible. “The good news is that the flu vaccine injection isn’t one of the painful ones,” she says. “This one’s just a poke.”

Are your kids ready for a pet? Carefully weigh pros and cons before making your decision LY N N U. N I C H O L S

P

ets are great companions and offer the chance for kids to learn important life lessons, but they have to be ready to handle the responsibilities of pet ownership. How do you know if your kids are ready to care for a pet? And which pet is right for them? Each pet comes with its own pros and cons list. A dog or cat, for instance, will provide the most affection and interaction but will demand the most care. Is your child ready to commit to picking up after your dog in the yard or taking the dog for a daily walk? If not, this responsibility will fall on you. Dander associated with fur—and how your child might react—is also something to consider before getting a dog or cat. Reptiles and amphibians including lizards, turtles, frogs, iguanas, snakes, toads, chameleons and salamanders are interesting to watch, but carry salmonella —a bacteria that can cause stomach upset. If your child is good at personal hygiene, or willing to wash his or her hands each time after holding the pet—and also willing to clean the cage regularly—reptiles might be a good choice. However, they often demand live food like meal worms or crickets which means weekly trips to the pet store. Consider how that fits into your schedule. Some people think guinea pigs are a good way to go, and they certainly can be. It’s a pleasure to feed them lettuce

12 | RMPARENT

| FAMILY HEALTH GUIDE

scraps and carrot peels and watch them squeal in delight. They can be calm and allow some cuddling and holding, but sometimes remain skittish. Cage upkeep can be a challenge as it’s a large cage that demands regular cleaning. If you are not quite ready for a guinea pig, try a gerbil or hamster. After all, the smaller the pet, the less mess they make. Hamsters can be quite entertaining, even when they get out in the house and you have to move furniture to find them. There’s even a mini version— robo hamsters—that are amazingly fast and fun to watch. Rats can also be great pets as they are more intelligent and often like to be held. The first question to ask before choosing a pet is if your kids are developmentally ready to take on the care and needs of that particular pet. Don’t expect a 4-year-old to be able to clean a guinea pig cage on her own. Test the waters with small, less demanding pets like goldfish and hermit crabs to reinforce the daily habit of pet care. Once your kids master that you can move up the ladder of the pet world. Parents often ask at what age

kids are ready for a pet. That depends more on your particular child than anything else, but here are a few guidelines. Is your child able to take care of his own basic needs? Can he dress himself, use the toilet alone and get a snack if needed? Then he might be ready. Toddlers and preschoolers don’t know how to control their strength or emotions and can hurt a small pet inadvertently. School age is often a good time to get a pet. Getting a pet that your child can keep in her room might also be something to consider—if you are not keen on having a pet underfoot. Teenagers can usually handle pets but may not have the time for them. Research has proven that pets are good for our wellbeing. Our blood pressure drops when we pet them, and they provide an open, non-judgmental source of comfort. Kids who have a close pet are often less anxious and withdrawn and pets can bring families together, giving everyone something to rally over and laugh about. Finally there’s the obvious—pets teach kids how to care for someone other than themselves.

Lower your cervical cancer risk Why you should get a regular Pap test

LY N N U N I C H O L S

L

owering your risk for cervical cancer is pretty straightforward: get regular Pap tests. Cervical cancer is preventable if detected and treated early. But it’s not the only reason to get a regular Pap. Pap smears check for the presence of abnormal cells in the cervix. Besides cancer, changes in the cells of the uterus can also indicate reproductive problems, STDs and infections.

About Pap smears

Regular Pap tests detect cervical cancer before it develops. In fact, most women who get cervical cancer have not had a Pap in the last five years, or ever. That’s reassuring news if you are faithful about getting regular Pap tests. It means you most likely will never get cervical cancer. Many physicians see Pap tests as a true public health victory in the last century. Cervical cancer used to be very common. Now, it’s fairly rare. Women without a family history of cervical cancer or known HPV infection should get a Pap every three years, starting at age 21. If you do have a family history or have HPV, talk with your doctor about a schedule that best fits for you. If your doctor deems a Pap is needed more regularly, your insurance will likely cover it. The American College of Obstetricians and Gynecologists used to recommend Pap smears annually but that changed in recent years. One reason is because experts know much more about how pre-cancers behave today.

Causes of an abnormal Pap

So, what does it mean to get have an abnormal Pap? Abnormal cells are rated for their severity. Precancerous cells, known as dysplasia, can be mild, moderate or severe (CIN 1-3, respectively). Depending on the rating, your doctor will either recommend a retest in a few months, want to perform a colposcopy to take a closer look, or opt to remove more serious cells during a biopsy. Cervical cancer starts out as abnormal cells. A Pap test is the easiest way to monitor changes in these cells. It is

14 | RMPARENT

| FAMILY HEALTH GUIDE

often combined with a test for human papillomavirus virus (HPV), which can lead to cervical cancer.

The role of HPV

HPV is the most common reason for an abnormal Pap test, but an abnormal result may also be caused by a bacterial infection or yeast infection in the vagina, other sexually transmitted diseases (including chlamydia, gonorrhea or trichomonas), cancer (uterine, vulval or cervical) and even simply having the test done too close to the start of your period. HPV is a big player in cervical and other reproductive cancers. HPV is a nasty set of viruses—about 150 to be exact. At least 12 are labeled high risk, contributing to the development of cervical, vaginal, vulval, penile and anal cancer, not to mention uncomfortable genital warts. Our bodies usually fight off the mild forms of HPV, but have a harder time fighting off high-risk forms. It can only be passed by direct skin-toskin contact during sexual activity. Over half of all sexually active men

and women will be infected with HPV at some point in their lives. The virus has been linked to about 70 percent of all cervical cancers. That’s why if your Pap test comes up abnormal, your doctor will likely check for HPV. An HPV vaccine is available, and is recommended for teens starting age 11 or 12.

Annual exams are important

Even if you don’t get a Pap every year, it’s still a good idea to get an annual exam—even if it’s simply to review your health history for the year—keeping both you and your doctor up to speed. It can also be used to discuss more immediate needs and receive breast and skin examinations, review family risk factors, and complete lab tests to determine how well you are maintaining your overall health. To keep it regular, attach it to a season or your birthday. If it’s that time of year to schedule your exam, go ahead and make the call. Keeping on top of any potential health problems takes mere minutes, and can save lives.

Outside play is perfect for the whole family Kim Sharpe

O

ne of the best things you can do for your kids is to kick them out of the house. With more and more research pointing to nature deficit (i.e., kids spending less time outdoors) as the cause of many modern childhood social, emotional, mental and physical issues, kicking kids outside to play is an act of kindness from which your whole family will benefit…because you should join them! “Spending time outside with my kids combines two things I love more than anything: seeing their sweet smiles against the backdrop of the amazing Colorado landscape and promoting healthy living,” says Deirdre Sullivan, parent of three and community health advocate. “I love

16 | RMPARENT

teaching them lifelong habits to promote their health and well-being. Doing it with them not only serves as an example, but allows me to spend time with them when we’re all crazy busy. From being in the garden to walking the dog to mountain biking on Colorado’s trails, it’s a win-win for all of us.”

Fresh air flashbacks

Another good reason to kick your kids out of the house is that they’ll remember it forever…in a good way! According to Nancy Blakey, parent educator and author of the Mudpies Activity Book series, “90 percent of our best childhood memories take place outdoors.”

Jessica and Mike Hinterberg have fond childhood memories “...of playing outside and being able to explore” which is why they’re adamant about making sure their 2-year-old son Jamison gets plenty of outdoor time and has a fun backyard where he can explore nature to his heart’s content. They believe “it’s important to have a simple, safe introductory home base of nature where a child can explore and create independently on a daily basis.”

Natural classrooms

Some schools place an emphasis on providing natural play spaces for children, too, because research shows that children who spend time outdoors and interact with nature concentrate better, score higher on tests and have fewer behavior issues than their peers who are nature deprived. For example, the River Song Waldorf School in Fort Collins, for students 2 1/2 to 5 years, take walks to natural areas every day so they can “…feel the seasons’ changes, know the diversity of life, and experience varying terrain and obstacles for their physical, social and emotional development,” says Aurora Romine, River Song board of directors member.

LYG Executive Director Alyssa McConkey says, “In all of our programs, students are outdoors,

connecting with nature and each other while learning, growing, and being active. We have seen time and again that being outdoors, and especially working in the garden with others, can provide personal, social and emotional benefits.” In Estes Park, the YMCA of the Rockies (www.ymcarockies.org) offers programs and classes to kids and families that promote year-round outside fun. The Poudre River Trail (www. poudretrail.org), which winds through Greeley, is a great place to explore the great outdoors all year long.

Track down outdoor fun

Let your kids come up with their own ways to play outside, but be available with a few ideas if they need help getting their creative juices flowing, like: • Ride a bike • Go for a walk • Explore a field • Play hopscotch • Climb a tree • Fly a kite • Play hide-and-seek • Toss a ball around • Walk the dog • Dig in the dirt

Ways to play

While there are endless ways to enjoy the outdoors, one of the best things kids can do—outdoors or in—is to have unstructured playtime, allowing them to use their imagination and naturally stay active. At first, kids who are kicked out may protest and tell you they’re bored and there’s nothing to do. Don’t let their whining win them a pass back inside, however. “Boredom is good for kids. It forces them to entertain themselves, which ignites their creative intelligence. From this, they learn that they can solve their own problems. This is huge!” says Lenore Skenazy, author of Free Range Kids (http://freerangekids.wordpress. com/ ).
