It is time for a new perspective in Brain Hearing TM The brain’s hearing center is comprised of two subsystems. Two subsystems work together inside the brain to help the brain make sense of sound: the orient subsystem and the focus subsystem. The orient subsystem always comes first. When we hear, it scans all surrounding sounds – no matter their nature or direction – to create a full perspective of the sound scene. It then creates an overview of the sound objects around us. The focus subsystem helps people select which sounds to listen to. Once we have an overview of the sound objects around us, we use the focus subsystem to identify the sound we want to focus on, listen to or switch attention to, while filtering out irrelevant sounds.

A silver lining What the future might hold

THE DARK CLOUD OF 2020 got a silver lining for us the other day when Andy and Alli FaceTimed us and said that they were going to have a baby. Kristin teared up and did that hand fluttery thing in front of her face at the thought of a grandchild. I, too, smiled broadly. They sent us pictures of the ultrasound. I thought it (we’re not really sure how to refer to the growing baby at this point) looked a lot like me at that age, but who could say since they didn’t do that in the Middle Ages. This will be the first child of the next generation. All seriousness aside, we expressed to Andy that they would probably be good parents since all their ancestors had managed to have children who lived at least long enough to have children. HOW CAN YOU CREATE some holiday cheer for those isolated because of the pandemic? At one time, holiday greeting cards were the norm. Today, empty mailboxes contribute to isolations’ loneliness. Why not fill your friends’ and families’ mailboxes with unexpected holiday cards. Be creative. Handmade cards and hand-written notes are especially appreciated. Personalize each letter. Include a memory trigger like an old photo gleaned from yearbooks, old albums or even newer digital photographs depicting shared activities, trips or special events. If you choose the prepared letter, consider filling the page with captioned pictures rather than just words. Some people fashion journal entries, newsletters, stories–all enhanced with pictures. Because of isolation, send greetings to local friends as well as distant. Follow up with a phone call or a care package. Bake cookies, make fudge and/or include some family traditional treat along with the recipe. Celebrate your success in creating joy this season.


Celebrate virtually WE EXPECT HOLIDAY GATHERINGS to be smaller this year. Still, we can celebrate with family and friends via video platforms. Select Zoom or any other available virtual program. Suggest one person organize the event and time. Sit at the dining room table with your beverage and dinner. Toast and talk as if you were all enjoying the meal in person. After dinner, recommend that everyone position themselves in front of their favorite decoration and share their gifts. Walk around the inside and outside to reveal your special holiday dÊcor. Remember your friends who are alone this year and contact them through FaceTime or a regular phone call. Set a specific time to call. Surprise your friend with hand-delivered or delivery of appetizer platters or desserts before your scheduled conversation. Choose from businesses like Whole Foods, Sam’s, Costco or local restaurants. Be safe. Enjoy virtual socializing this year.

Share your holiday spirit WITH HOLIDAY ENTERTAINING CURTAILED during this pandemic, share holiday cheer with friends and family via outside decorations. Let your house shine from within. Line the windows with lights on a timer. Place your tree in front of a picture window. On the outside, wrap the garage door with red plastic ribbon and a bow to create the look of a gift. Anchor red bows on the windows. Hang wreaths on the door and/or between windows. Set a small illuminated tree in a large garden pot. Add waterproof wrapped and weighted packages around the base. Line the driveway and sidewalk with luminaries either purchased or handmade. Cover bushes with lights. Surround the front door with garland. Purchase lit snowmen, trees, animals or a nativity scene. Be extra careful if you hang lights from eaves or gutters. Now, sit back and watch your neighbors and friends enjoy your holiday spirit.

Getting back to life with Covid-19

Colorado moved into the “Safer At Home” phase of reopening on April 27th and, as of this writing, that order expires on May 26th, 2020. The next planned phase is titled “Protect Our Neighbors” and is focused on a more normal social life, while still being protective against the spread of Covid-19. What does all of this mean though? Who is still at risk? What does the future look like? First off, we need to understand that these orders and timelines are a general guide and are flexible. If new case numbers begin to rise, we can go back to “Safer At Home” or even “Stay At Home” orders. We have to be ready to make adjustments to our lives as are necessary. We also need to be realistic with what our near future looks like with this pandemic. In reality, we are looking at many more months of precautions. Even with our modest control measures currently in place the medical community is expecting a peak for the pandemic to be around July. Then, its expected that we will continue to see cyclical periods of new cases for the next 12 to 18 months, until we can mass produce an effective and safe vaccine. It is unlikely that we will ever be able to eliminate this virus completely. We may see

small resurgences each fall and winter. We should be ready and willing to have plans for business closures as new waves can be identified. Our most vulnerable populations, those over 65 and people with chronic lung conditions, heart disease, or who are immunocompromised, need to adhere to more strict precautions and should try to practice “Stay At Home” as much as possible. The best way back to normal life is to have readily available testing for anyone who has symptoms or exposures and to be able to isolate and contact trace any positive cases. With many people being asymptomatic or even contagious prior to symptoms, we should expect waves of infections throughout the population. Our lives have been changed due to Covid-19 and the next year or two is going to be vastly different than what we are used to. However, together we will return to normal life. Please do your part in looking out for each other and our communities by following our guidelines for “Safer At Home” and “Protect Our Neighbors”. As a primary care physician, I can truly say that we here for you and we thank you for your resolve during these unique times.

ONE-FOURTH OF OUR population lives alone, yet social connection is crucial for physical and mental health. Some who live alone may already suffer from mental illnesses like anxiety, depression and loneliness, which may increase with isolation during the holidays. The World Health Organization recommends staying connected through: • Snail mail • Phone, text, FaceTime • Email • Social media • Video chat platforms • Virtual classes

Follow these ideas to improve your mental health and that of others. • Contact members of the community who may be at risk during isolation. • Knock on a neighbor’s door, stay a safe distance, and/or communicate as listed above. • Set up regular phone calls or video chats. • Make and/or send care packages, pick up medications or shop for others. • Join virtual exercise programs and online courses. • Enjoy a laugh via YouTube videos like Jeanne Robertson. If more support is needed, contact a doctor or check your local Office on Aging for their resource book of mental health contacts.


Take action with your health resolutions EVERY YEAR PEOPLE PLAN health-related resolutions they rarely fulfill. Rather than resolutions, list your goals as action plans, then answer these questions. Is it a reachable goal? When and how often will I practice it? Choose a confidence level between 1-10 with 10 being the highest. If your level falls below six, consider revising the goal. One example of an action plan is: I will cook dinner at home, at least three nights a week. The confidence level is a nine. Second example: I will limit fast foods to lunch, once a week. What will you do instead? Consider action plans for these health-related goals or others of your choice: • Get enough sleep • Limit screen time • Create more “me” time • Practice self-care • Take walks/exercise • Eat healthier • Cut back on alcohol intake • Enroll in virtual classes/lectures Follow your action plans and enjoy New Year resolution success.

Keep up exercise during COVID COLD WEATHER AND THE pandemic have changed our routines. How can you create a reliable exercise program at home? Designate a time—at least three days a week—and schedule it on your calendar. Try these activities: • Walk outside. • Check to see if your gym or studio presents virtual classes. • Find other online classes–Zumba or yoga. • Create a walk path within your house. • If you have a treadmill, use it! • Keep your workouts interesting. Turn on favorite music and dance. • Walk while talking on the phone. • Do jumping jacks during TV commercials. • Work in the yard or shovel snow. • Smartphones offer free tracking apps. Otherwise, purchase a device like Fitbit to log your steps and set a daily goal. You’re more likely to stick to an exercise plan if you start small, reward/celebrate your successes, and build up gradually.

Scott Titterington SOMETIMES WE JUST FIND OUR MOMENT TO RISE. Bob and Jan Eastin grabbed just such an opportunity as the pandemic was starting to grow. The United Way of Weld County put out a call for people to make masks and the Eastins dove in. Over three months, they made and delivered 900 masks to the agency, helping to prevent the spread of the virus and to keep people safe. Volunteering looks different in these days of COVID-19. “The face of volunteering has changed,” says Craig Secher, president and founder of Realities for Children, “but the needs of our children haven’t changed.” SAFETY FIRST Organizations, including nonprofits, are working hard to continue to support community members in need with new strategies that keep both their volunteers and their beneficiaries safe 22 |

while still serving the growing needs of their communities. “Our distribution drives have had to change so much because of all of the safety mandates,” says Mehgan Castor, community outreach coordinator at Realities for Children, “yet we are not only still running distributions, but doing so in a safe way that volunteers can participate in in critical roles.” Nicole Quinn, donor engagement manager at United Way of Weld County, has found that people want to contribute if there is a way to do it safely. “Volunteers still want to be actively engaged in organizations that they support even if it means doing so from their homes,” Nicole says. “We have been able to engage volunteers through our Mask Up 4 Weld, Hygiene Kit, Diaper Kit and Sleeping Mat projects.” Volunteer Services Program Manager for the City of Fort Collins Sue Schafer

Some like the connection to community, others are focused on the folks that they’re helping, while others like being part of something bigger than themselves. “I love being surrounded by people with wild ideas about changing the world for the better and finding ways to protect and serve these kids,” says Realities for Children Volunteer of


the Year Hayley Carson. “With every Realities event I do, I connect to my community, to leaders, to a cause that speaks to me and to something bigger than myself.” ALL ABOUT CHOICE We all like to do different things and we also bring different experiences and skill sets. Finding a good fit where you feel welcome and believe what you are doing is contributing to the health and welfare of people, animals or our environment seems key to staying engaged in any particular activity. “People are passionate about different causes and we offer many options at the

United Way of Weld County that support many different needs,” Nicole says. THE BOUNCE BACK While your motivation may be to help out where you can, you will also reap many benefits of your own. For starters, you might just feel some satisfaction after you spent your day building that house or sewing those masks or walking a dog. It’s no small thing to take a moment and soak in the feeling that the world is a little better today because of something you did. And maybe when you did whatever it was, you learned something new. Maybe

Downgrading sound input to the brain and failing to treat the hearing loss in the correct way can have a variety of consequences.** • Increased listening effort With less sound information, it is harder for the brain to recognize sounds. It must fill in the gaps, which requires more listening effort. • Increased mental load Having to guess what people are saying and what is happening increases the mental load on the brain and leaves less mental capacity for remembering and performing. Reorganized brain functionality Without enough stimulation in the

hearing center, the visual center and other senses start to compensate, which changes the function of the brain. • Accelerated cognitive decline Increased mental load, lack of stimulation and reorganized brain functionality is linked to accelerated cognitive decline, which affects the ability to remember, learn, concentrate, and make decisions. • Accelerated brain volume shrinkage All human brains reduce in size with age, but the shrinkage process accelerates when the brain must work against the natural way it processes sound. And so, the brain problems can turn into life problems. A limited sound scene without access to the right input can lead to serious problems in life.*** • Social isolation and depression People with untreated hearing loss can end up avoiding social gatherings because they are unable to cope with complex sound environments. • Dementia and Alzheimer’s disease

The risk for dementia increases fivefold for severe-to-profound hearing loss, three-fold for moderate hearing loss and two-fold for mild hearing loss. • Poor balance and fall-related injuries Untreated hearing loss can affect balance, increasing the risk of fallrelated injuries three-fold. Younger and older together The advantages of an intergenerational society READING PARKER PALMER’S latest book, On the Brink of Everything, a paragraph jumped out at me that got me thinking: “When young and old connect, it’s like joining the poles of a battery. Together we generate energy for personal and social change that an age-segregated society cuts off. The social conditions that keep us apart aren’t going to change any time soon. But elders can reach out to the young, many of whom yearn for us to take an interest in them, their fears, their dreams, and their futures.” He goes on to speak of how staying connected to the younger generations enriches our aging years... keeping us vital. Those of us living in and through our second half of life have put much time into our work/career. We have nurtured families, invested in our community. Some of us are still going strong at work, may feel at the peak of careers. Others have reached that point of retirement and on to other pursuits. A few of us still find ourselves putting in time at the office into our 70s. We once stood where teachers taught us and mentors encouraged us. They offered wisdom, knowledge and experiences to help prepare us to be productive adults. We learned the ethics of hard work, how to navigate difficulties, get up after falling down, think outside the box, and be creative. Then at some point we found ourselves in the position of teacher, mentor and then eventually handing off the work and responsibilities to those younger and newer to the task at hand. A great satisfaction for me during the later years of my career has been training younger psychologists and counselors in the art of psychotherapy. 50 |

Again, Parker Palmer: “Age and experience have taught me that mentoring is not a one-way street. It’s a mutuality in which two people evoke the potentials in each other...mentoring is a gift exchange in which we elders receive as much as we give, often more.” The next generation or two coming up behind us can enrich our lives now. We have the opportunity to ‘see’ them, ask them questions, encourage them, show genuine interest in their life path, their dreams, fresh ideas, and hopes for the future...and most of all to LISTEN to what they have to say! In doing so our lives will be enriched. We will see new things, be taught...they can be a source of hopefulness, and keep alive a young spirit within us. A ‘reverse mentoring’ if you will. These past twelve months I have spend significant time in medical offices, hospitals, infusion labs and surgery units. A great gift in these months has been connecting with the wonderful, skilled, compassionate professionals in whom I have placed my life, my well being. I could not help but look them in the eye, thank them, affirm them...appreciate their dedication to do the work, their effort to learn the skills needed to do the job...many, if not most, the age of my children, nieces and nephews. One physician’s assistant I learned double dated with my son to high school prom. Another infusion nurse, I had known since her birth and officiated at her wedding a few years back. My surgeons both in their mid-40s, at the top of their field and skilled in the latest robotic surgery techniques. (During these pandemic times, an additional deep appreciation for all those in the medical field showing up for their jobs, putting their safety on

the line...as they say, cancer and other diseases do not take time off just because there is a pandemic.) And ‘second half of lifers’ can appreciate as well those who are literally doing the heavy lifting we can not now do or shouldn’t be doing... those cleaning our gutters, painting our houses, climbing into our crawl spaces to repair our furnace, delivering our groceries. All those young entrepreneurs having the courage to start their own businesses, sometimes with just a truck, a business card and a lot of drive. And those juggling kids and mortgages who process our taxes, fill our prescriptions, teach our children and grandchildren. This life journey has its seasons. As we age into our 60s, 70’s, and beyond, let us carry gratitude for the young. May we encourage them on their paths. Big challenges lie ahead...We leave them with much work to be done when it comes to the environment, racial justice, economic inequalities, educating the young, and finding a way forward united as a community and country. We still can do our part...joining them where we can in the tasks at hand... offering the next generations hope. May we take the time to really see them, affirm them, and LISTEN.
