WAIVER AND RELEASE OF LIABILITY

I, the undersigned, acknowledge the inherent risks involved in any sporting activity, particularly the sporting activity that I intend to participate in at the Robert Morris University. Accordingly, in consideration of being allowed to participate in Ice Hockey and intending to be legally bound, I, the undersigned, do hereby, for myself, my heirs, executors and/or administrators agree to the following: 1.

I attest that I am physically fit for the Activity. I will assume my own medical and emergency expenses in the event of an accident or other incapacity or injury resulting from or occurring in my participation I agree that prior to participating the Activity, I will inspect the training/competition area and all equipment to be used, and if, through my inspection, I determine that anything related to the Activity is unsafe, I will immediately notify an official of Robert Morris University of this unsafe condition and will not participate in the Activity until this condition is corrected.

I hereby waive, release, discharge and covenant not to sue, Robert Morris University , its affiliates, agents, associates, officers, directors, owners, employees and representatives (collectively “releasees”) from demands, losses or damages, including without limitation, any injury, death, or damage to property that may be suffered by me whether caused or alleged to be caused in whole or in part by releasees or any other party’s actions or inactions, or otherwise, and agree to indemnify releasees from any and all third party claims caused in whole or in part by my actions.

PAYMENT/REGISTRATION A $200 deposit

MISSION STATEMENT We will be the most organized and

will confirm registration. Your cancelled check will serve as your receipt. Balance of payment is due by the first day of camp,. Applications received within one month of the camp must be accompanied by the TOTAL registration fee. Per NCAA rules, positions will be filled on a first come, first serve basis. There is not a registration deadline. However, registration may close earlier if groups are filled. The first skate will be on July 16 2012. Players should check-in 60 minutes prior to their first scheduled skate to get last minute instructions and receive their jersey.

PROGRAM CANCELLATION The number of

openings in any Program is limited. Consequently, it is not possible for us to offer an open refund policy. In the event that it is necessary to cancel for medical reasons only, REFUNDS will be issued within six weeks of cancellation minus a 10% administration charge.

HOCKEY CAMP JERSEY & EQUIPMENT

Each student receives one souvenir hockey jersey. Jerseys are provided for identification purposes only, and are not exchangeable for cash or credit. The students will receive their jersey during registration. Each player will be given a Skills & Small Games jersey that they will be able to keep. Players must provide their own equipment and sticks. Players are required to supply all protective equipment recommended by USA Hockey including facemask & mouth guard. Students should be prepared for vigorous off-ice activities. Off-ice equipment should include running shoes and proper workout attire.

DISCOUNTS EARLY BIRD- Take $25 off if registered by April 30, 2012 RMU ISC-Take an additional $10 if you play hockey out of RMU ISC (Island Colonials, Arctic Foxes or Pittsburgh Hornets) or you are an RMU employee .

best staffed hockey camp in the region. We will have NCAA Division One Hockey coaches and players on the ice on every session. We will push every hockey player to the limit to be the best they can be and improve on a daily basis. This camp will work on all areas of skill development and then allow you to test the skill improvements out in small game settings. The staff at the Robert Morris University Summer Hockey Camp wants to stress the importance of academics and good citizenship to all school participants. We strive to mold solid character along with mental and physical strength. We’ll introduce students to the idea that being an athlete is only a small part of being a well-rounded individual.
