NCLUSION WITHIN THE ROCKFORD PARK DISTRICT MEANS INDIVIDUALS WITH DISABILITIES ARE WELCOME TO PARTICIPATE IN THE SAME RECREATION PROGRAMS AND ACTIVITIES AS THEIR PEERS. REASONABLE ACCOMMODATIONS ARE PROVIDED TO ENABLE AN INDIVIDUAL’S SUCCESSFUL PARTICIPATION IN A PROGRAM.

The Rockford Park District welcomes and supports the participation of individuals with disabilities in all programs and services. • Consultation on activity modification • Disability awareness training • Behavior modification support • Observation and evaluation • Visual systems and sensory equipment/toys • Sign language communicators • Adapted equipment (sleds, skate walkers, hand cranked bicycles, sport wheelchairs) • Varying levels of disability-trained staff support

Popular programs in which individuals with disabilities have been included: • Lessons (swimming, hockey, ice skating, horseback riding, tennis, golf) • Day and overnight summer camps (Summer Blast, Nature Quest, Camp Lone Oak, Pony Camp) • Neighborhood programs (Playgrounds, Lewis Lemon and Washington Park community centers) • Environmental education and recreation • Youth sports • Rec nights • After school programs

Inclusion Services has created successful inclusion opportunities by: • Implementing a positive support plan for a child with a conduct disorder in a full day summer camp setting. • Providing an all-terrain wheelchair for a student with a physical disability for full trail access at Atwood Center. • Creating and training staff on an enhanced visual system for a camper with a visual impairment. • Trained staff supporting a camper with an attention deficit disorder by providing additional supports such as redirection and positive reinforcements at soccer camp. • Providing a sign language communicator for a teen at horseback riding lessons.

A parent’s guide to inclusion INCLUSION SERVICES ROLES 1. Contact family/guardian to complete full assessment. 2. Obtain and organize any additional info, equipment, and additional support staff (if applicable).

HOW INCLUSION SUPPORT WORKS: 1. Register for desired Rockford Park District program at Webbs Norman Center, Carlson Ice Arena, or rockfordparkdistrict.org. 2. Check “yes” where the registration form states, “…does the participant have a disability or medical conditions that may require assistance or accommodation?” 3. A team member from Inclusion Services will contact you within 48 hours to assess needs, level of support, and an appropriate start date.

3. Notify program staff of participants’ needs and the staff’s role in supporting them in their program. 4. Communicate with family any updates, changes, or any information that they need to be aware of before the start of the program. 5. Maintain communication with parents/ guardians, support staff, and program staff throughout the course of the program. 6. Recommendations will be addressed based on the participant’s progress or level of success in the program.

PARENT/GUARDIAN ROLES 1. Verify the participant meets minimum eligibility requirements of program (i.e., age, registration, and level of participation). 2. Keep in mind, recreation programs and activities are voluntary in nature. Participation will be encouraged and aided, but not forced. 3. Where can I register? Online, Mail, Fax, or In Person

How can I pay?

Online www.rockfordparkdistrict.org All you need is your household ID and password. Your household ID can be found on a previous receipt, or you can create one on our web site. If you don’t remember your password to register online, contact our customer service center at 815-987-8800. Following completion, a receipt will be e-mailed to you confirming your registration.

Payment is required at the time of registration. We accept cash, local personal checks, money orders, credit cards (Visa, MasterCard, American Express, and Discover Card only) and debit cards on which the VISA or MasterCard logo and cardholder name appear. You must be the cardholder in order to process payment. ID must be provided for on-site registration. Please make checks payable to ROCKFORD PARK DISTRICT. If your check is returned for insufficient (NSF) or uncollected funds, your account may be electronically charged or drafted for the face amount of the check, plus the additional $25 service fee. The service fee added to all returned items is $25 or the maximum allowed by state law. Your payment by check is agreement to this policy and terms. You have the right to revoke this transaction. Rockford Park District offers customers the budget-friendly convenience of an automated payment plan called “EZ Pay,” allowing program and activity fee payments to be spread out. For this “EZ Pay” option, you must: • Register in person at one of our Customer Service locations and sign the EZ Pay Agreement. • Make a deposit payment of at least $5.00 per activity using the same debit or credit card that will be used to automatically deduct payments. • The program or activity total for your transaction must total $100 or more after any discounts or additional fee assistance.

What is the cancellation and refund policy?

When can I register?

Full refunds will be granted for any registrations cancelled by the participant two weeks prior to the first day of the program. If the program you have registered for is cancelled, a full refund will be issued. Partial refunds may be issued for programs cancelled due to weather that you are unable to attend due to illness or injury. (To process, you must notify RPD of your situation before the program ends). These regulations apply to recreational and instructional programs throughout the Park District. They do not apply to daily admissions, memberships, permits, leagues and rentals. These services have specific policies as part of the information provided to the participant at the point of registration. Please note: Any refunds can be applied to your account for future use, or they must be provided in the form of payment in which they were received. Any refunds in cash over $25.00 originally paid in cash will receive a check mailed within 2 weeks of the request to the home address on file. Any refunds under the amount of $10.00 will be applied to your household for future use. Recreation Satisfaction Guaranteed! Our goal is to provide our customers and citizens with recreational programs, events, and activities of the highest quality. When a customer is not satisfied, we sincerely request their comments, suggestions, or ideas for improvements. If a customer is not completely satisfied with their program, event, or activity that RPD directly provides, the customer may tell us and we will arrange a refund as requested.

To help ensure a spot, register for your programs early. Registration is accepted one week prior to the start date, unless otherwise noted. Late registrations are accepted on a case-by-case basis by the program’s manager. For more information, contact Customer Service at 815-987-8800 or visit one of our customer service locations; we will arrange a refund as requested.

What is a wait list? Some programs are very popular, and fill quickly. You may opt to register for a filled program and be placed on the waiting list. This means that in the event that a spot becomes available, you may be enrolled in the program. To be placed on a waiting list, the program registration process must be fully completed, including payment. Your position on the waiting list is based upon when the registration is processed by RPD. If RPD cannot accommodate your enrollment by the second meeting of the class, your registration fee will be refunded in full. All waiting lists have a limited number of spots, so it is possible that the waiting list may already be filled at the time you attempt to register.

TITLE VI As the Rockford Park District has received financial assistance in the form of Land and Water Conservation assistance, the US Department of the Interior, under Title VI of the Civil Rights Act of 1964, prohibits discrimination on the basis of race, color, national origin, age, or disability. Can I register a friend or someone outside of my immediate family? In order to register someone outside of your immediate family, you must bring a completed registration form in which the Waiver and Release of All Claims and Assumption of Risk has been signed by an adult who is legally responsible for the participant.

What is your age policy for programs? Unless otherwise noted, recreation programs require participants to reach the minimum age by January 1, and not reach the maximum age before the conclusion of the program. Lesson programs require that participants be within the age limits of the class by the first day of the program. Any accommodations made outside of these requirements must be approved by the program’s manager.

Can I transfer from one program to another? Transfers are allowed up until the beginning of the first class. Transfers after a program has started must be requested through the manager of the specific program. If the transfer is to a higher priced program, the additional fee must be paid prior to the transfer. If the transfer is to a lower priced program, a credit will be issued to the household.

FFAK Ticket Packets You must qualify for FFAK in order to be eligible for FFAK ticket packets for tots (ages 1-2), youth (3-18), and adults with disabilities (18+). These packets provide individual admissions to a wide variety of Park District programs and facilities. Each ticket packet contains $190 in value to do fun recreational activities such as: • M agic Waters Waterpark admission • Nicholas Conservatory & Gardens admission • Forest City Queen boat and Trolley rides • Sapora Playworld admission • Swimming • Golfing • Ice skating • Pony rides

Only one ticket packet may be purchased per child per year. Each ticket packet is assigned to the qualified child or adult with a disability to use as admission to the provided Park District facilities. Coupons are valid for use in and during the year listed on the coupon during the facilities’ operating seasons. Ticket packets can be purchased at one of the Customer Service locations: the Webbs Norman Center Customer Service office, Carlson Ice Arena, and Washington Park Community Center.

FINANCIAL ASSISTANCE Looking for financial assistance or additional help to pay for programs? The Rockford Park District and partners have a variety of ways to help you and your family enjoy our programs and facilities.

Option #1 - Fun For All Kids Program No child or person with a disability should be denied access to learning an important ability, such as swimming, or developing skills for a lifetime of enjoyment, due to their inability to pay. The Rockford Park District offers a fee assistance program called Fun For All Kids (FFAK). Qualified applicants can receive up to 75% off registration fees for Rockford Park District programs. The 2014 scholarship funds will run from February 3, 2014 through the end of the year.

FFAK GUIDELINES: • Must be a resident of the Rockford area with proof of residency such as a state ID or driver’s license (or foster parent). • Must provide one of the options as requested on the FFAK Review and Verification of Eligibility form. • Must apply in person at one of our Customer Service locations for approval based upon the information and documentation provided.

NOTE: Daily admissions, private lessons, and season passes are NOT available through Fun for All Kids. If you would like to apply or re-apply for assistance, please print and complete the FFAK application located online at www.rockfordparkdistrict.org, or pick up an application at a Customer Service location. A Spanish-speaking customer service representative is available at the Webbs Norman Center Customer Service location.

Option #2 - EZ Pay Rockford Park District offers customers the budget-friendly convenience of an automated payment plan called “EZ Pay,” allowing program and activity fee payments to be spread out. For this “EZ Pay” option, you must register in person at either Customer Service location, which will allow the Park District to withdraw regular payments from a bank account and/or credit card; a down payment of at least $5.00 per program session is required at registration, and program fees must be $100 or more to qualify for the payment plan. Customer Service locations are: Webbs Norman Center, 401 S. Main St., Rockford, and Carlson Ice Arena, 4150 N. Perryville Road (Riverside and Perryville), Loves Park. For more information about “EZ Pay,” call 815-987-8800.

This fund was established to provide funding for kids who cannot afford admission fees for activities such as: zip lining, snowboarding, or golfing at Alpine Hills, visiting Magic Waters or Nicholas Conservatory, taking a ride on the Trolley or Forest City Queen, riding horses or taking a hay ride at Lockwood Park, or wakeboarding at the new West Rock Wake Park at Levings Lake.

I recognize and acknowledge that there are certain risks of physical injury to participants in this program/activity, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. Learn To Swim–Rolling Green Elementary School

Since we can’t seem to get Shelbert the Sea Turtle out of the water, we thought we would invite you to join him in the pool! Shelbert loves the water, and wants his friends to be just as comfortable in his natural habitat. Learn the fundamentals of swimming and ultimately, how to swim by yourself. Shelbert’s Swim Academy classes are taught in the main pool in at least three feet of water. On the first day of class, students are placed in the most appropriate of five levels. Each level has a specific set of objectives to achieve before moving to the next level. Prerequisite: Children must be able to enter the water and participate on their own. LEVELS 1-4 INCLUDE: • Water exploration • Stroke readiness • Primary skills • Stroke development

LEVEL 5 INCLUDES: • Emphasize on stroke refinement • Preparation for higher levels of swimming

nfl flag & cheer NFL Flag Football

Skill level doesn’t matter when it comes to NFL Flag Football. All participants will receive an official NFL Flag Football jersey, a set of flags, trophy, NFL certificate, and a field trip. NFL Flag consists of a 6-8-game season and playoffs. Practices, at least two per week, are held in various schools and parks. Coaches and parents work as a team on dates and locations. NOTE TO PARENTS/GUARDIANS: • Parents/guardians of participants are required to attend the Parent Meeting at Webbs Norman Center (401 S. (AGES 8-22) PROGRAM STARTS IN FEBRUARY Buddy Basketball is a non-competitive recreational league for individuals with disabilities. Each player will be on a team with 6 players and 4 "buddies" to assist in creating a fun and successful experience. The program focuses on the experience of team play, exposure to a lifelong leisure skill, and participation in basketball. N THE COURT, RIGHT IN THE MIDDLE OF THE ACTION, IS THE BEST WAY FOR YOUTH TO LEARN THE GAME OF BASKETBALL. THIS PROGRAM EMPHASIZES

skill development, sportsmanship, and team spirit. Every participant plays at least half of every 50-minute game per week. There is a 20-minute practice prior to the game. About our program:

• O ur programs use the nationally recognized US Figure Skating Basic Skills Program. Each participant receives two free passes for Public Skate, or 30-minute freestyle sessions. • Classes meet once a week, and skates are provided for all levels. • The minimum class size is six skaters. We reserve the right to combine classes with fewer than two skaters. • Skaters progress at their own rate, moving on to the next level only after required elements are mastered. All levels may be repeated until the requirements are met. The Rockford Park District reserves the right to move any skater between levels. • If you need to miss your regular class, you can make it up if it is offered another time during the week. If not, you can use one of your public or freestyle session passes to practice.

The Bridge to Private Lessons

This 45-minute class introduces skaters to synchronized skating and teaches the fundamental skating skills and teamwork, which become the building blocks for future success in the growing sport of synchronized skating. Skate with your friends, learn something new, and have fun! Opportunities to perform in the winter and spring shows.

At any time during the Basic or Freestyle levels of the Learn to Skate program, skaters may choose to implement private lessons in addition to group lessons, or to switch from group instruction entirely to one-on-one instruction with a coach. Fees are set by each individual coach. All private lessons take place during freestyle skating sessions (additional cost). Located adjacent to Riverview Ice House, the Junior IceHogs Off-Ice Training Facility offers hockey players the chance to improve their stickhandling and shooting skills. Players can also improve their strength and endurance using the workout equipment—weights, treadmills, and elliptical machines.

Monday Madness

Don’t wait to sign up - limited spots are available! This one-of-a-kind camp includes a tour of the BMO Harris Bank Center including the IceHogs locker room, ticket to the game, and lunch. A light off-ice workout will be followed by an on-ice practice, all at the BMO Harris Bank Center. Drop-off and pick-up will be at the box office at the BMO Harris Bank Center. This will be a great experience for the kids; don’t miss out! Hold on, it just keeps getting better; it also includes skating with the IceHogs after the game! Drop-off 8:45 am, pick-up 4:30 pm. This date is tentative, and will be confirmed once the Rockford IceHogs schedule has been posted.

This patented system can help players improve their hockey shooting. RapidShot uses real pucks, requiring the shooter to receive passes, and shoot a series of 16 pucks in under two minutes.

Stewardship Saturdays

Seth B. Atwood Park is a diverse 334 acres complete with prairie, woodland, river, and wetlands. Each of these areas needs special attention, and you can help! Join Environmental Recreation and Education (ERE) staff on select Saturdays and learn a little about one of these areas. After a brief lesson, we will put on our gloves and see how we can help maintain the second largest park in the Rockford Park District. Snacks will be provided. All ages welcome. Atwood Wanderings HOW TO FIND A YETI - Did you know that there were several Yeti sightings reported in Atwood? Trace the path of the reported sightings using a map and compass to see if you can find the Yeti. Afterwards, warm up to the cozy fire and have a s'more for a snack. Snowshoeing is optional, based upon weather conditions. A BIRD'S LIFE - Discover how birds brave the cold northern Illinois winter while taking a hike through Atwood park. Then, warm up indoors with a snack of hot chocolate and a cookie. Participants will also be part of a bird encounter with our on-site birds of prey, and dissect an owl pellet. SCAT & TRACKS - Animals are abundant in Atwood Park all year long. We'll take a hike to look for animals and their tracks, then warm up inside with a snack of hot chocolate and a cookie. Afterwards, get a close-up look at the turtles and snakes that reside in Atwood Center, and make an animal craft. WINTER SURVIVAL - Do you think you could survive outdoors in the winter? Explore the basics of outdoor living by using a compass, creating shelters, learning the signs of hypothermia, and creating a first aid kit. Enjoy a s'more snack created over a fire. O28

Stewardship Saturdays

Seth B. Atwood Park is a diverse 334 acres complete with prairie, woodland, river, and wetlands. Each of these areas needs special attention, and you can help! Join Environmental Recreation and Education (ERE) staff on select Saturdays and learn a little about one of these areas. After a brief lesson, we will put on our gloves and see how we can help maintain the second largest park in the Rockford Park District. Snacks will be provided. All ages welcome. It’s one thing to run a 5K, but it’s a whole different race when the course is infested by a horde of zombies! Register as a runner/ walker or a zombie in an event that promises to be other-worldly. This semi-competitive race will travel through Atwood Park, where peril lurks around every corner, and no one is safe from the lumbering undead. Whether you survive the attack or not, there will be an after party complete with prizes and finger food! Ages 8 and up; youth under the age of 16 must be accompanied by an adult.
