nclusion within the Rockford Park District means individuals with disabilities are welcome to participate in the same recreation programs and activities as their peers. Reasonable accommodations are provided to enable an individual’s successful participation in a program.
The Rockford Park District welcomes and supports the participation of individuals with disabilities in all programs and services. • Consultation on activity modification • Disability awareness training • Behavior modification support • Observation and evaluation • Visual systems and sensory equipment/toys • Sign language communicators • Adapted equipment (sleds, skate walkers, hand cranked bicycles, sport wheelchairs) • Varying levels of disability-trained staff support

Inclusion Services has created successful inclusion opportunities by: • Implementing a positive support plan for a child with a conduct disorder in a full day summer camp setting. • Providing an all-terrain wheelchair for a student with a physical disability for full trail access at Atwood Center. • Trained staff supporting a camper with an attention deficit disorder by providing additional supports such as redirection and positive reinforcements at soccer camp. • Providing a sign language communicator for a teen at horseback riding lessons.

Online www.rockfordparkdistrict.org All you need is your household ID and password. Your household ID can be found on a previous receipt, or you can create one on our web site. If you don’t remember your password to register online, contact our customer service center at 815-987-8800. Following completion, a receipt will be e-mailed to you confirming your registration.

Payment is required at the time of registration. We accept cash, local personal checks, money orders, credit cards (Visa, MasterCard, American Express, and Discover Card only), RPD gift cards, and debit cards on which the VISA or MasterCard logo and cardholder name appear. You must be the cardholder in order to process payment. ID must be provided for on-site registration. Please make checks payable to ROCKFORD PARK DISTRICT. If your check is returned for insufficient (NSF) or uncollected funds, your account may be electronically charged or drafted for the face amount of the check, plus the additional $25 service fee. The service fee added to all returned items is $25 or the maximum allowed by state law. Your payment by check is agreement to this policy and terms. You have the right to revoke this transaction. Rockford Park District offers customers the budget-friendly convenience of an automated payment plan called “EZ Pay,” allowing program and activity fee payments to be spread out. For this “EZ Pay” option, you must: • Register in person at one of our Customer Service locations and sign the EZ Pay Agreement. • Make a deposit payment of at least $5.00 per activity using the same debit or credit card that will be used to automatically deduct payments. • The program or activity total for your transaction must total $100 or more after any discounts or additional fee assistance.

Full refunds will be granted for any registrations cancelled by the participant two weeks prior to the first day of the program. If the program you have registered for is cancelled, a full refund will be issued. Partial refunds may be issued for programs cancelled due to weather or that you are unable to attend due to illness or injury. (To process, you must notify RPD of your situation before the program ends). These regulations apply to recreational and instructional programs throughout the Park District. They do not apply to daily admissions, memberships, permits, leagues, and rentals. These services have specific policies as part of the information provided to the participant at the point of registration. Please note: Any refunds can be applied to your account for future use, or they must be provided in the form of payment in which they were received. Any refunds in cash over $25.00 originally paid in cash will receive a check mailed within 2 weeks of the request to the home address on file. Any refunds under the amount of $10.00 will be applied to your household for future use. Recreation Satisfaction Guaranteed! Our goal is to provide our customers and citizens with recreational programs, events, and activities of the highest quality. When a customer is not satisfied, we sincerely request their comments, suggestions, or ideas for improvements. If a customer is not completely satisfied with their program, event, or activity that RPD directly provides, the customer may tell us and we will arrange a refund as requested.

When can I register? To help ensure a spot, register for your programs early. Registration is accepted one week prior to the start date, unless otherwise noted. Late registrations are accepted on a case-by-case basis by the program’s manager. For more info, contact Customer Service at 815-987-8800 or visit one of our customer service locations; we will arrange a refund as requested. Payment must be recieved at time of registration.
What is a wait list? Some programs are very popular, and fill quickly. You may opt to register for a filled program and be placed on the waiting list. This means that in the event that a spot becomes available, you may be enrolled in the program. To be placed on a waiting list, the program registration process must be fully completed, including payment. Your position on the waiting list is based upon when the registration is processed by RPD. If RPD cannot accommodate your enrollment by the second meeting of the class, your registration fee will be refunded in full. All waiting lists have a limited number of spots, so it is possible that the waiting list may already be filled at the time you attempt to register.
TITLE VI As the Rockford Park District has received financial assistance in the form of Land and Water Conservation assistance, the US Department of the Interior, under Title VI of the Civil Rights Act of 1964, prohibits discrimination on the basis of race, color, national origin, age, or disability. Can I register a person outside of my immediate family? In order to register someone outside of your immediate family, you must bring a completed registration form in which the Waiver and Release of All Claims and Assumption of Risk has been signed by an adult who is legally responsible for the participant.

What is the cancellation and refund policy?
What is your age policy for programs? Unless otherwise noted, lessons and programs require that participants be within the age limits of the class by the first day of the program. Any accommodations made outside of these requirements must be approved by the program’s manager.
Can I transfer from one program to another? Transfers are allowed up until the beginning of the first class. Transfers after a program has started must be requested through the manager of the specific program. If the transfer is to a higher priced program, the additional fee must be paid prior to the transfer. If the transfer is to a lower priced program, a credit will be issued to the household.

FINANCIAL ASSISTANCE Looking for some help to pay for programs? The Rockford Park District has a variety of ways to help you or your family enjoy our programs and facilities.

Option #1 - Help Me Play Fee Assistance Program No youth in our community should be denied access to learning an important lifelong skill, such as swimming due to the inability to pay. The Rockford Park District’s fee assistance program is now called the Help Me Play Fee Assistance Program (HMP). Qualified applicants can receive up to 75% off registration fees for Rockford Park District programs. The 2015 scholarship funds will run from February 23, 2015 through the end of the year. NOTE: Private lessons and season passes are NOT available through Help Me Play.
Help Me Play Ticket Packets Once qualified for fee assistance, you can also purchase one of the Help Me Play ticket packets for individual admissions to a wide variety of Rockford Park District and partnering facilities! We offer a Tots Packet, Youth Packet, and Adults with Disabilities Packet. The packets include admission to facilities such as Magic Waters, Sapora Playworld, Carlson Ice Arena, Nicholas Conservatory, Lockwood Park Children’s Farm, and much more! Only one ticket packet can be purchased each year, per child. • Youth Packet (includes 40 tickets) $25 • Tots Packet (includes 22 tickets) - $10 • Adults with Disabilities Packet (includes 29 tickets) - $25
Ask About Admission to Adventure! The Rockford Park District is thrilled to work with three new adventure providers: Gateway Parks, West Rock Wake Park, and Zip Rockford. Through the “Admission to Adventure” program, reduced rates are offered to qualified Help Me Play participants. Limited quantity available. Only one ticket can be purchased each year, per child.

If you would like to apply or re-apply for assistance, print and complete the HMP application located online at www.rockfordparkdistrict.org, or pick up an application at a Customer Service location. A Spanish-speaking customer service representative is available at the Webbs Norman Center Customer Service location.

For this “EZ Pay” option, you must register in person at either Customer Service location, which will allow the Park District to withdraw regular payments from a bank account and/or credit card; a down payment of at least $5.00 per program session using the same debit or credit card that will be used to automatically deduct payments is required at registration, and program fees must be $100 or more to qualify for the payment plan. This fund was established to provide funding for kids who cannot afford admission fees for activities such as: ziplining, snowboarding, or golfing at Alpine Hills, visiting Magic Waters or Nicholas Conservatory, taking a ride on the Trolley or Forest City Queen, riding horses or taking a hay ride at Lockwood Park, or wakeboarding at the new West Rock Wake Park at Levings Lake.

IMPORTANT INFORMATION The Rockford Park District is committed to conducting its recreation programs and activities in a safe manner and holds the safety of participants in high regard. The District continually strives to reduce such risks and insists that all participants follow safety rules and instructions that are designed to protect the participants’ safety. However, participants and parents/guardians of minors registering for this program/activity must recognize that there is an inherent risk of injury when choosing to participate in recreational activities/programs. You are solely responsible for determining if you or your minor child/ ward are physically fit and/or adequately skilled for the activities contemplated by this agreement. It is always advisable, especially if the participant is pregnant, disabled in any way or has recently suffered an illness, injury or impairment, to consult a physician before undertaking any physical activity. WARNING OF RISK Recreational activities are intended to challenge and engage the physical, mental, and emotional resources of each participant. Despite careful and proper preparation, instruction, medical advice, conditioning, and equipment, there is still a risk of serious injury when participating in any recreational activity. All hazards and dangers cannot be foreseen. Depending on the particular activity, certain risks, dangers and injuries may exist due to inclement weather, slips and falls, poor skill level or conditioning, carelessness, horseplay, unsportsmanlike conduct, premises defects, inadequate or defective equipment, inadequate supervision, instruction or officiating, and other risks inherent to the particular activity. In this regard, it is impossible for the Rockford Park District to guarantee absolute safety.
WAIVER AND RELEASE OF ALL CLAIMS AND ASSUMPTION OF RISK Please read this form carefully and be aware that in signing up and participating in this program/activity, you will be expressly assuming the risk and legal liability and waiving and releasing all claims for injuries, damages or loss which you or your minor child/ward might sustain as a result of participating in any and all activities connected with and associated with this program/activity (including transportation services/vehicle operation, when provided). I recognize and acknowledge that there are certain risks of physical injury to participants in this program/ activity, and I voluntarily agree to assume the full risk of any and all injuries, damages or loss, regardless of severity, that my minor child/ward or I may sustain as a result of said participation. Shelbert’s Swim Academy Levels Level One: Minnows
(Activity# 160503) Students will focus on the following skills in this introductory level of swimming: front and back floats, utilizing kickboards, bobbing in the water, and proper breathing in water. Minnows will also begin combining arm strokes and kicking.

Shelbert’s Swim Academy
S
ince we can’t seem to get Shelbert the sea turtle out of the water, we thought we would invite you to join him in the pool! Shelbert loves the water, and wants his friends to be just as comfortable in his natural habitat. Learn the fundamentals of swimming and ultimately, how to swim by yourself. Shelbert’s Swim Academy classes are taught in the main pool in at least three feet of water. Each level has a specific set of objectives to achieve before moving to the next level. Prerequisite: Children must be able to enter the water and participate on their own.

Youth
Skill level doesn’t matter when it comes to NFL Flag Football. All participants will receive an official NFL Flag Football jersey, a set of flags, trophy, and NFL certificate. NFL Flag consists of a 6-8-game season and playoffs. Practices, at least two per week, are held in various schools and parks. Coaches and parents work as a team on dates and locations. Coach requests will be considered, but not guaranteed. NOTE TO PARENTS/GUARDIANS: • P arents/guardians of participants are required to attend the mandatory parent meeting on September 9, 2015 at the Webbs Norman Center, 401 S. youth basketball Basketball Leagues - Grades Pre-K–4 On the court, right in the middle of the action is the best way for youth to learn the game of basketball. This program emphasizes skill development, sportsmanship, and team spirit. Every participant plays at least half of every 30-minute game per week. There is 20-minute practice prior to the game. Pre-K– Kindergarten grade players use junior size basketballs and 6-foot standard hoops; 1st-2nd graders use junior size basketballs and 8-foot standard hoops; 3rd-4th graders use intermediate basketballs and 10-foot standard hoops. The Chicago Bulls are coming to Rockford! The Bulls/Sox Academy will be hosting a Chicago Bulls FUNdamentals camp with the Rockford Park District for boys and girls ages 5-14 (players will be split by age and ability level). This camp will teach the latest techniques in ball handling, passing, shooting, and rebounding, while also teaching children how to be student athletes and great teammates. Every participant will receive top-flight instruction from our professional Bulls/Sox Academy coaches, who have learned from the Chicago Bulls coaching staff. We provide a fun and safe environment for children to learn the game, as well as teach life lessons such as discipline, respect, sportsmanship, and handling peer pressure. Each participant will receive a replica autographed photo of a current Chicago Bulls player, and a camp t-shirt. Your child will not only have fun, but will be given important tools to become a better basketball player. To become a great basketball player, you need the fundamentals.

This program is designed to help our children and adults achieve their goals whether they pursue any ice sport recreationally or competitively. The skating curriculum that will be followed is known as the Basic Skills program. The curriculum is designed by U.S. Figure Skating, and is publicly endorsed by USA Hockey, U.S. Speedskating, and Special Olympics. Beginning Skating Lessons - Basic 1, 2, 3 These are 30-minute lessons for those interested in learning to skate and progressing through the beginning skating curriculum offered through the Rockford Park District. If your skater chooses to become a competitive figure skater, we have a program for you! Our newly designed Junior Figure Skating program will focus on the necessary skills for our skaters to have the foundation to be successful before joining the Rockford Figure Skating Club. If your skater is already pursuing figure skating, the Rise and Shine clinic is offered to improve their stamina and technique, and provide them with the ability to become a stronger overall athlete.
Junior Figure Skaters - Freeskate 1-6 This 40-minute lesson is for skaters who have passed the Advanced Skills level (page 27). We will focus on skills that are included in Freeskate 1-6. Along with the traditional jumps, spins, and edges, we will also introduce other programs offered by U.S. Figure Skating. Programs that will be featured are Theatre on Ice, Synchronized Skating, Dance, Pairs, etc. Prerequisite: Basic 8.

Hockey Program If your skater would like to learn to play hockey, we have a program for you! Once you learn basic skating skills in Intro to Skating (ages 3–5) or Basic Skating Skills–Basic 1 (ages 6–16), you can begin taking hockey lessons. Our Hockey lessons, clinics, and camps focus on the developing the skills necessary to successfully join the Junior IceHogs Hockey League.

Minor Hawks League This is a 40-minute class that utilizes stations to push skaters to the next level. This class functions as an excellent transition from lessons to the Jr. IceHogs Fundamental Hockey League. Players will receive a jersey while supplies last. • All players must provide their own equipment • No checking allowed • League ages are just guidelines. Players move up or down levels at the Hockey Coordinator’s discretion, with parent/ guardian approval. • A game schedule with locations will be distributed at the beginning of the season • Times and days are subject to change
League Family Discounts • Two enrolled players from the same family will receive 10% off on the second enrolled family member’s registration • Three or more enrolled players from the same family will receive 50% off on the youngest enrolled family member’s registration. The discount qualifies for the third registration plus any additional registrations.

Fur or Feathers; Which One For You? Would you rather be covered in fur or feathers to help stay warm in the winter? Find out what makes the best insulation in keeping our furry and feathery animals warm in the winter by conducting some of your own experiments! Then throw on your winter coats as we head out for an activity! Finally, end your day with a snack as we warm up back inside.
Hibernate, Migrate, Stay When it gets cold outside, we can crawl under a warm blanket, cozy up to a fire, and sip hot chocolate. But what do the animals do? Find out who sleeps the winter away, who looks for warmer places to play, and who toughs it out and stays! We will head outside to seek out dens, nests, scat, and tracks. After our adventure outside, we can head back indoors to warm up as we make a craft and enjoy a snack!
Bird’s Eye View You bring the hiking boots, we’ll supply the binoculars! You’ll get a “bird’s eye view” as we hike the trails of Atwood Park to find which of our fine-feathered friends stick around in winter time. After the hike, save some energy as we play a game, have a snack, and get “beak-to-beak” with our very own Atwood birds of prey! Before you know it, it will be time to take flight and head back to your nest.
Can You See Me? If a squirrel has gray fur, does that mean it’s old? Come find out as we explore how animals use camouflage, even in the winter! After looking for “hidden animals,” we’ll show off our own camouflage abilities, then warm up inside with a snack and a craft. These activities are sure to blend in well with your day!

ewis Lemon Community Center (LLCC) offers a fun, safe, and cooperative program between the Rockford Park District and Rockford Public School District #205. The program provides after-school recreational activities, along with arts and crafts, and the chance to meet new friends.

ashington Park Community Center (WPCC) continues to grow and offer new activities for youth and teens. Enjoy our multi-purpose room, weight room, computer lab, gymnasium, teen center, and skate park.
LLCC - After School Program (grades 1–5) Enrichment program offered to Lewis Lemon students only. Activities include homework help, board games, arts and crafts, gaming systems, and the opportunity to participate in organized basketball and football teams. Daily snack will be provided. Students will also have the opportunity to participate in the 21st Century program, which offers help in reading, math, and science. The students of this program must be teacher recommended. Lewis Lemon’s After School Program runs into the start of the 21st Century program, and then is reinstated once it has ended.
