Dear Friends, I’ve been wearing glasses since I was in 6th grade. Oh, I fought having to wear them. I would hide them from myself and just hated being seen in them. Nowadays – older and wiser but still nearsighted – wearing my glasses is second nature. It’s the first thing I do every morning. I have embraced the preciousness of eyesight and acquiesce to the fact that if I want to see clearly now, I have to wear my glasses. But now and then I ponder the possibility of having LASIK surgery. I’ll have to look into it. This month’s issue of Health&Wellness explores other aspects of eye health, from age-related eye problems to the effect our tiny phone and tablet screens may have on our vision. Our good health is in our hands. Wouldn’t it be great to combine heart health and eye health? Check out the article about the upcoming Tri-For-Sight triathlon. The run, bike and swim will do your heart good and you’ll be raising funds for eye research for the University of Kentucky Department of Ophthalmology and Visual Sciences. Tiny Screens Can Cause Big Vision Problems PRACTICE SAFE HABITS TO PROTECT YOUR EYES By Angela S. Hoover, Staff Writer For many people, their SMART phone has replaced their computer. Checking the weather, reading the news and emails, keeping up with social media and even typing documents are all done on a SMART phone these days. Using any electronic screen can cause eye issues, but staring at these tiny screens can make problems worse because the font is smaller. Here are a couple of difficulties you may encounter and ways to safeguard your vision. Computer Vision Syndrome Nearly one in four people complain of eyestrain from reading text on screens. Eyestrain can result from a variety of factors. One is how frequently you blink your eyes. The average blink rate is
about 15 times per minute, but the rate is reduced by half when looking at a screen. Eye fatigue is another cause of eyestrain. Eyes can become fatigued if you are constantly reading small fonts on a small screen, squinting and straining for long periods. The combination of blurred vision, headaches, tired eyes, muscle strain and dry eye is called Computer Vision Syndrome. The Dark Side of Blue Light The body uses the cycle of the sun’s light to guide its internal circadian rhythm – the biological clock. Important bodily functions happen during sleep, but light at night can throw the circadian rhythm out of whack. Sleep suffers and then health suffers, even increasing risks for cancer, diabetes, heart disease and obesity. Blue wavelengths, which are beneficial during daylight hours because they boost attention, reaction times and mood, are more disruptive at night. The intense blue light of SMART phones, tablets, computer screens and even LED lights can affect sleep patterns. Blue light can decrease levels of melatonin twice as much as other wavelength colors of light, according to Harvard Medical School researchers. A reduction in melatonin levels makes it difficult to fall asleep. This is why using any device with a blue screen in bed, especially with the lights off, is not advised. A University of Toronto study found melatonin levels were comparably the same in individuals who wore blue-light-blocking goggles in a room brightly lit with blue light and individuals in a
dimly lit room not wearing the goggles. This further validates blue light as a more potent suppressor of melatonin than other forms of artificial light. Safe Screen Practices Changing practices when using SMART phones and other electronic devices with blue backlighting usage may eliminate these issues. Follow the 20-20-20 rule: During every 20 minutes of usage, look away and stare at something at least 20 feet away for at least 20 seconds. This exercise will help rest the eyes and prevent the fatigue and strain that lead to headaches and blurred vision. It is especially important for children to practice the 20-20-20 rule since their eyes are still developing. Don’t use any devices in bed, close to bedtime or in the dark for extended lengths of time. Avoid using a SMART phone or other blue-lit electronic device at least 20 minutes prior to going to sleep – ideally two to three hours beforehand. If your blue-lit screen usage is high, especially at night, wear blue-light-blocking glasses or goggles. Inexpensive sunglasses with orange-tinted lenses block blue light, but they also block other colors. Glasses that block only blue light can cost up to $80. Apps that filter the blue/green wavelength at night are also available. If you notice your vision is not returning to normal, especially after enacting good, safe screen practices, see an eye doctor.

Retinal Detachment A HEALTHY INTACT RETINA IS IMPORTANT FOR CLEAR VISION By Jean Jeffers, Staff Writer The eye is a marvelous organ. Sight occurs when light rays enter the eye through the cornea. The cornea bends the light, which passes through the pupil, a size-changing hole in the iris. The iris works somewhat like a shutter in a camera, allowing light to pass through. After it passes through, the light hits the eye’s crystalline lens. The light rays are then focused onto the retina. The retina converts the light rays into impulses. These impulses travel through the optic nerve to the brain, where they are interpreted as images we see. According to the American Academy of Ophthalmology, a healthy intact retina is the key to clear vision. A thick gel called the vitreous is attached to the retina. On occasion, clumps of gel in the vitreous cast a shadow on the retina and the individual sees small dots, specks, strings or clouds. As we age, the vitreous may shrink and pull on the retina. When this occurs, the patient may notice flashing lights or have the sensation of seeing stars. While usually there is no problem with the vitreous as it moves away from the retina, once in a
while the vitreous makes a forceful pull, tugging so hard it may cause one or more tears. If a tear in the retina happens, fluid may pass through, lifting the retina off the back of the eye. This process has been described as similar to wallpaper being peeled off a wall. When the retina is pulled from the back of the eye, it is called a retinal detachment. Symptoms include blurry vision and a shadow or blindness in part of the visual field. Retinal detachment is a very serious problem that usually results in blindness unless surgically treated. With retinal detachment, bleeding from nearby blood vessels adds to the problem, making vision that much more cloudy. Generally, the patient cannot see at all. Treatment, which occurs soon after diagnosis, includes (for milder problems) a laser to seal the tears in the retina before the detachment occurs or a gas bubble that helps the retina float back in place. Treatment for severe detachment includes a scleral buckle used to push the eye wall up against the retina and removal of vitreous or scar tissue pulling on the retina (this method is used for the largest tears). Success depends on the location and extent of the detachment and how early the detachment is detected. If treated early, the results are usually better. If the macula is left undamaged, the prognosis is excellent. Retinal detachment occurs more commonly with age. Cataract surgery may be a factor in the formation of the condition. Myopic patients are also more susceptible. Prevention includes protective eye wear to prevent eye trauma. If you have diabetes, control your blood sugar levels.

Age-Related Eye Problems Are Normal SOME CAN BE CORRECTED WITH GLASSES OR SURGERY By Harleena Singh, Staff Writer Once you’re in your 40s, your vision starts to change. You may need glasses to see up close; you may have more trouble adjusting to glare or distinguishing some colors. These changes are a normal part of aging. But you are also at higher risk for developing eye diseases and conditions such as age-related macular degeneration, cataracts, diabetic eye disease, glaucoma, low vision and dry eye. Take a look at some of these problems: Presbyopia. Presbyopia is when you can’t see close objects or small print clearly. You may not notice any changes until around age 40 years. It’s easy to correct with reading glasses and contacts. Age-Related Macular Degeneration (AMD). AMD is a disease that slowly destroys
sharp, central vision, which is needed for seeing
objects clearly and for common daily tasks such as reading and driving. Some studies suggest regular exercise, such as walking, can reduce the risk of AMD by up to 70 percent. Cataract. A cataract is a clouding of the lens in the eye. Vision with cataract can appear cloudy or blurry, colors may seem faded and you may notice a lot of glare. Cataracts often form slowly without pain, redness or tearing. Glaucoma. Glaucoma is a group of diseases that can damage the optic nerve. Usually associated with high pressure in the eye, it affects the side or peripheral vision. If you don’t catch glaucoma early, it can lead to permanent vision loss. Diabetic Eye Disease. Diabetic eye disease is a leading cause of blindness. The most common form is diabetic retinopathy, which occurs when diabetes damages the tiny blood vessels inside the retina. Low Vision. People with low vision find everyday tasks – reading mail, shopping, cooking, watching TV, writing – difficult to do, even with regular glasses, contact lenses, medicine or surgery. Dry Eye. Dry eye occurs when the eye does not produce tears properly or tears that are not of the correct consistency and evaporate too quickly. Dry eye can make it hard to perform some activities, such as using a computer or reading for an extended period of time. Tearing. This occurs when your eyes make too many tears, which can happen if you’re sensitive to light, wind or temperature changes. You can take steps to prevent damage to your
eyes. When you’re outdoors in the daytime, always wear sunglasses that shield your eyes from 100 percent of the sun’s harmful ultraviolet rays. This may help reduce your risk of cataracts, pinguecula (a yellowish, slightly raised thickening of the conjunctiva on the white part of the eye) and other eye problems. Be sure to wear safety glasses when working with tools or participating in active sports to help prevent eye injuries that potentially could cause permanent vision loss. People who smoke are at greater risk of developing AMD, cataracts, uveitis (an inflammation of the uvea, the middle layer of the eye) and other eye problems. Regular eye exams become more important as you reach your senior years. The American Optometric Association recommends annual eye examinations for everyone over age 60 years. A large percentage of those with type 2 diabetes have sleep apnea, and obesity increases a person’s risk for both disorders. There is an increased risk for pancreatic, renal and skin cancers, autoimmune disorders, hypothyroidism, glaucoma and pregnancy complications such as gestational diabetes or high blood pressure and delivery of an underweight baby. Sleep apnea can also worsen asthma and increase body inflammation. The cognitive and behavioral disorders from sleep apnea can result in memory loss or decreases in attention, vigilance, concentration, motor skills and verbal and visuospatial memory. It can lead to dementia in older adults and learning disabilities in children. There is also an increase in motor vehicle accidents as a direct result of sleep disorders. Sleep apnea can be caused by a person’s physical structure or medical conditions. These include obesity, large tonsils, tumors in the upper airway, endocrine disorders, neuromuscular disorders, heart or kidney failure, certain genetic syndromes and premature birth. Sleep apnea is characterized by frequent breaks or pauses in breathing during sleep, causing interruptions in the sleep pattern. Often the sufferer does not fully wake up and is unaware of the severity of their sleep disturbance. These interruptions to restful sleep cause powerful and dangerous effects within the body. Sleep apnea disrupts the way the body takes in oxygen and can affect circulation, which makes it hard for the brain to control how blood flows in the arteries and the brain itself. The death rate from all causes is higher in patients with moderate to severe sleep apnea. Sleep studies for apnea are usually conducted at hospitals or sleep labs. Of the roughly 3.5 million sleep tests performed each year, about 80 percent can be done at home. Home sleep tests have become increasingly popular with patients, physicians and insurance companies over the past decade.

LEARN BRAILLE OR FIND OUT WHAT IT’S LIKE TO HAVE A VISION PROBLEM By Dr. Tom Miller, Staff Writer What does it mean to be visually impaired? The Centers for Disease Control and Prevention says a visually impaired person’s eyesight cannot be corrected to a “normal” level. Visual impairment is the functional limitation of the eyes or visual system. Like the term legally blind, visual impairment is not a functional definition; it is a classification system. It does not tells us much about what a person can and cannot see. Eye disease causes low vision. This is when visual acuity is 20/70 or poorer in the better-seeing eye and cannot be corrected or improved with regular eyeglasses (Whittaker and Scheiman, 2015). Visual acuity is a number that indicates the sharpness or clarity of vision. A visual acuity measurement of 20/70 means someone with 20/70 vision who is 20 feet from an eye chart sees what a person with unimpaired (20/20) vision can see from 70 feet away. A series of apps have been developed that are available for vision impairment. None can substitute for the professional examination of an ophthalmologist or optometrist, but SMART technology provides options for the layperson to review their visual acuity. EyeXam is available free for both Android and Apple devises. It lets you gain an estimate of your vision against the 20/20 benchmark and also includes tests for color perception, astigmatism and eye dominance. Self-monitoring is an important ingredient in the future of health care. An Amsler Grid app can test and monitor vision changes related to macular degeneration
or other distortion in a person’s vision field resulting from damage to the macula. For individuals who are losing some ability to see things normally, some SMART apps may offer assistance. One is the Big Clock HD. This app will provide a larger figure of a clock for telling time. It also displays the date in regional format and has a programmable timer-based alarm. Other SMART apps assist in magnifying what you are reading. EyeCBest is a Web browser that increases the readability of Web sites and is fully customizable – the user can change the font, size and boldness of any Web site text. The iRead app, a magnifier designed by an ophthalmologist, can enlarge and brighten reading material such as a menu in a dim restaurant. For people who need a magnifying glass but cannot take it everywhere, there is a Magnifying Glass app – just point your phone to the object being viewed and the app will magnify it. The application has a light as well and can also freeze-frame what you’re reading. The Magnifying Glass with Light for Apple devices is great for magnifying small font on menus, medicine bottles, receipts and more. Some SMART apps are designed for those who are seriously visually impaired or blind. Be My Eyes assists blind persons with important everyday tasks such as reading labels and receipts. The Braille app provides a Braille table and a simple quiz for reading and writing. Braille Tutor teaches Braille and the Braille Writer translates text into Braille. The NantMobile Money Reader is for blind or visually impaired people who have difficulty recognizing currency. It tells the denomination and helps make sure you’re paying the proper amount at the cash register. It can also check that you receive the right amount of change. This app supports more
than 20 currencies and several languages. If you want to know what visually impaired people experience, the Braille Institute developed VisionSim to give people with healthy vision a chance to see what the world looks like to someone with macular degeneration, diabetic retinopathy, glaucoma, cataracts or other eye diseases. It applies special filters to simulate the symptoms of these diseases. Sources

At a minimum, update your child’s emergency medical plan every year before school starts—but you can also update it when your child receives any sort of medical care.
What’s on your back-to-school to-do list? If you’re like most parents, that list probably includes shopping for school supplies, buying new clothes to replace what your kids have outgrown and planning out after-school activities and childcare. But you may be overlooking one vitally important back-to-school to-do item: creating or updating an emergency medical plan for your child or children. You know your child better than anyone, and you know where you’d take them in an emergency—but what if an urgent medical situation occurs when you’re not there? Creating an emergency medical plan and providing it to your child’s school, sports team, babysitter or other caretaker can help you rest easier, knowing that no matter what, your child will receive the best care possible thanks to your planning. Here are five recommendations for crafting an emergency medical plan that works for your family. It only takes a few minutes, and it can make a world of difference when it matters most.
1
Choose a Children’s Hospital
When it comes to caring for kids, all hospitals are not created equal. Most hospitals have some pediatric specialists available, but children’s hospitals offer dedicated facilities, specialists and services just for kids—available around the clock. With teams of doctors, nurses, therapists and surgeons who have dedicated their careers to caring for kids, children’s hospitals offer the kind of care kids deserve. Write it Down

You can always specify your preferred hospital, whether or not your school or daycare’s forms include a place for it—your child’s teachers and caretakers won’t complain about having more detailed care information available in case of an emergency. In addition to any required forms or paperwork, it’s always a good idea to have a written medical emergency plan handy

Having this information readily available will help ensure that your child receives the right care if a situation occurs when they’re away from you.

Include Important Info
Every child is different—and you know your child better than anyone. You know their likes and dislikes, their fears and strengths, their history and needs. If a medical situation occurred and your child required care when you weren’t available, what sort of information would be important for their healthcare provider to know to give them the best care possible? This vital additional information can include medical details, like allergies to medications, foods, or other materials, as well as relevant medical history like surgeries or chronic conditions that could affect your child’s care. But there’s other information that’s also important for your child’s care. If they have a comfort item like a treasured blanket or toy that would make their experience a little easier, that’s important for teachers, doctors and nurses to know. If they’re scared of the dark or claustrophobic, that’s also important to put in their medical emergency plan. At a children’s hospital like KCH, staff can use these details about your child to improve their care and their experience, whether it’s a quick visit to the emergency department or a longer stay. Your child’s emergency medical plan is far from the only place that will list your contact information— but it may be the first place a teacher, coach or babysitter checks in case of an emergency. It’s important to include as much contact information for yourself as possible. List two phone numbers, like your cell phone and office number, if available, and specify which numbers can be reached by text. In addition to your own contact information, add contact information for at least two trusted additional caretakers. This could be your spouse, your child’s grandparents, aunts or uncles, or a long-time babysitter. If you can’t be reached or are out of town when an urgent situation occurs, having additional contact information for others who are invested in the care of your child is vital.
Creating an emergency plan for your child is the most important step in the process. But children grow quickly and are constantly changing, so keeping that plan updated is important. At a minimum, update the plan every year before school starts—but you can also update it when they have any sort of medical care, including check-ups and vaccinations. If you or any of your emergency contacts move or change phone numbers, be sure to update that contact information as soon as possible. If you have multiple children, create an individual plan for each child. Whenever you update one, take a minute to check the other for accuracy. Developing a written emergency medical plan for your child doesn’t take long, but it can mean everything in an emergency situation when every minute counts. Be sure your child receives the care that’s right for them by proactively creating a medical emergency plan, specifying that they receive care at a children’s hospital, and including the most important information so teachers, coaches, babysitters and healthcare professionals can provide the best care for your child. It may be the most important thing you do during your back-to-school preparations.

LEARN ABOUT LAZY EYE
azy eye is a term that is often used for three different conditions. One of those conditions is ptosis, which is a droopy upper eyelid that occurs after someone suffers nerve damage. If one eye does not see as well as the other, this is called amblyopia. Most commonly, the term lazy eye is for strabismus, when the eye either turns in (esotropia) or out (exotropia). If one eye turns up, this is called hypertropia. This condition is rare. Amblyopia causes more vision loss in children than all other causes combined. With amblyopia, the parts of the brain concerned with visual processing do not function properly. Vision is learned. In early development, a child relies more on the sense of touch to make sense of the world. Around age 3, vision begins to be the dominant sense, but the brain must learn how to see. Normally, vision is binocular, which means the eyes move and work together. This is essential for good vision. The eyes must work together with the brain. If the brain doesn’t get clear input and images from both eyes, it may decide to make things easier on itself and draw more visual information from the good eye because it focuses better. Subsequently, the brain ignores input and images from the eye that has poorer vision. Amblyopia occurs when the eyes fail to work together properly and one eye becomes more dominant. The problem originates in the brain when it does not fully acknowledge the images the amblyopic eye sees. This is called suppression. The brain perceives two mismatching images or blurry vision so it throws one image out to avoid creating double vision. This condition may cause problems with movement perception and visual acuity. There may also be loss of depth perception. The symptoms of amblyopia are not always obvious. Can amblyopia be treated? Although amblyopic vision is not readily corrected by either glasses or contact lenses, current vision research proves the brain can change, develop and recover from amblyopia with occupational, physical and visual therapies. This adaptive ability of the brain, which is called neuroplasticity, makes it possible to treat amblyopia at any age, even in teenage children and sometimes adults. Vision therapy offers eye exercises and other treatments for amblyopia. While patching the better-seeing eye is often recommended as a treatment protocol and is utilized with children who have amblyopia, it should be used sparingly. General consensus is to patch for only a few hours at a time. Another treatment uses atropine eye drops. These dilate the pupil and blur close-up vision in the better-seeing eye, thus encouraging the use of the eye with poor vision. The only way to definitively diagnose amblyopia is through a comprehensive visual acuity examination. As with other conditions, early detection is best in order to get a jump on treatment and prevent vision

Enjoy life to its fullest. But also learn about the resources available to you and your family for your retirement. Tri-For-Sight Supports UK Eye Research Fund

It’s difficult to imagine life without sight. Each year, thousands of people lose the battle with macular degeneration, diabetic retinopathy or glaucoma. The University of Kentucky Eye Research Fund supports work that helps doctors and scientists better understand and treat these diseases.
The Susan Bradley-Cox Tri-For-Sight Triathlon supports their work. This fun, short, local triathlon helps raise money for this research. All proceeds from Tri-For-Sight go directly to fund the UK Department of Ophthalmology and Visual Sciences Research Fund. What is a triathlon? It is an athletic contest consisting of three different events, typically swimming, cycling and long-distance running. Some events eliminate the swimming portion and replace it with kayaking. These events or races can be short in distance, like the Tri-For Sight, which is normally called a Sprint Triathlon. Next are the Olympic-level distances. The longest triathlon most people are aware of is the Iron Man. The half-Iron Man covers a total of 70.3 miles. A full Iron Man is over 140.6 miles, which is a bit extreme. There are even Ultra-Triathlons that are double the Iron Man distance. However, the most common events you will find are the Sprint and the Olympic triathlons. In a Sprint Triathlon, the combined activities of swimming, biking and running cover a distance of 15 miles. People of all ages, body types and athletic abilities compete in these events. We encourage you to come watch or consider participating in the Tri-For-Sight on Sunday morning, September 9th, as an athlete or volunteer. To be healthy in life, you have to stay active and eat right. This is what motivated Susan Bradley-Cox to begin participating in triathlons at age 42 years. Before this, she had never competed in such an endeavor. “I developed my active lifestyle as a young girl growing up in Paducah, Kentucky,
where I took an early interest in cheerleading, swimming and acrobatics (gymnastics),” she said. “These activities taught me an appreciation of physical activity and I now find emotional release through physical expression.” Susan noted when she was growing up, there was not much for young girls to do athletically. “Girls didn’t really compete back then,” she said. While she stayed active in college and early adulthood, it did not have much of an impact on her life. In 1982, Susan joined the Todd’s Road Stumblers, an informal group of runners who met each Saturday morning to run and enjoy each other’s company. One of the members, John Sensing, invited her to be part of a brand-new sport being formed called a triathlon. Susan was encouraged to do a triathlon at Spindletop Hall, a UK Faculty/ Alumni/Staff Club. At the time, she did not even own a good bike, so she borrowed one. She set out on her new adventure and has never looked back. In her first-ever triathlon, Susan won the women’s division. Since then she has been an inspiration for many people of all ages. “I feel I have been blessed with the ability to achieve physically and I want to share my talents and experiences with others,” Susan said. “I have been competing in Masters Swimming and triathlons since the ’80s. I have been blessed to win 11 National USAT championships. I have also had the opportunity to be part of the USA Triathlon Team since 1989.” Susan also qualified to participate on the National Team and was part of the team that first helped the sport of triathlon become an Olympic event. “I have had the opportunity to travel all over the world representing the United States, winning 12 ITU World championships in triathlon in my age division,” she said. “I competed in the 1986 Hawaii Ironman World

Championship, which was one of the highlights of my triathlon career.” Currently, Susan is the head coach of the Wildcat Masters Swim Team at UK and teaches water aerobics at Pinnacle in the winter and Spindletop Hall in the summer. “This allows me to inspire others to move their bodies and thus be healthier, happier people,” she said. “I am also the Lexington triathlon coach for the Kentucky Leukemia/Lymphoma Society. The participants on the team raise money for the cancer research at the University of Kentucky and I coach them to compete in a triathlon.” Susan says she receives many benefits from staying active. “Physical exercise makes me feel refreshed and positive, which results in a healthy release of tension,” she said. “This for me is a personal accomplishment, one that has helped me grow and feel better about myself, but also one I am happy to share with others.” Susan joins us in encouraging our Health & Wellness readers to get out and move. FOR PATIENTS WITH DIABETES
P
eople who have diabetes must be vigilant about their eyes. Diabetes is the leading cause of new cases of blindness among adults between the ages of 20 and 74 years, and 45 percent of patients with diabetes develop diabetic eye disease, which can lead to severe vision loss or even blindness, according to www.DiabetesSightRisk. com. One complication of diabetes that affects the eyes is diabetic retinopathy. In this condition, blood vessels become blocked and prevent areas of the retina from receiving blood and nutrients. This can lead to diabetic macular edema, which occurs when the damaged blood vessels leak fluid into the macula. At Family Practice Associates, we now offer fundus photography to screen for retinopathy in patients with diabetes. The procedure takes about five minutes and is covered by Medicare and most insurances. It takes color photographs of the retina and offers an automatic evaluation. The diagnostic report includes retinal images, which are reviewed and read by a board-certified ophthalmologist. A referral/screening plan clearly details next steps you can take to enhance your eye health. Sometimes you may need to see a retinal specialist, who will perform additional tests. With fluorescein angiography, the specialist injects a dye into your arm and takes photos as the dye passes through your eye. This test helps the specialist detect any closed, damaged or leaking blood vessels. A second special test is an imaging test called optical coherence tomography. It produces a cross section of the retina and is useful for revealing any swelling in the eye tissue. To keep tabs on possible diabetic eye disease, patients with diabetes are encouraged to get a dilated eye exam every year in addition to routine visits with their primary care physician. This type of exam often detects problems a regular vision test cannot. Your doctor will put drops in your eyes to dilate and enlarge the pupils, the openings at the center of the iris. Enlarging your pupils allows your eye doctor to see more of the inside of your eyes to check for signs of disease. Using a special magnifying lens, the doctor checks the retina and optic nerve for damage. The National Institute of Diabetes and Digestive and Kidney Disorders (www.niddk.nih.gov) says your eye doctor may conduct other tests to measure the pressure in your eyes; your side or peripheral

vision; and how well you see at various distances. You may also need to keep seeing a general ophthalmologist or optometrist if you have other medical conditions that affect your eyes, such as glaucoma, or if you wear eyeglasses or contact lenses. A dilated eye exam could be the deciding factor in preserving your sight. Early detection, timely treatment and follow-up care could reduce your risk of diabetes-related blindness by 95 percent. Another way to keep your eyes healthy is to keep your blood glucose and Hgb A1C numbers as close to your targets as possible. Also, be sure to regulate your blood pressure. High blood pressure can damage the tiny blood vessels in the retina. Follow your doctor’s recommendations and take your medications as directed. from Florida. On two different occasions while we were in Florida, our parents did have to go to the hospital. The grandchildren stayed with them as long as possible, but they needed to get back to work. Aging Life Care Solutions took over and stayed at the hospital with them. They had the authority through the paperwork we had signed to share all information from the doctor with the family and give updates until my arrival back to Lexington from Florida. What a blessing! Source of Yuma E. Coli Romaine Found Federal officials first announced on April 13 an E. coli outbreak linked to romaine lettuce grown and produced in the Yuma, Ariz., area. Federal investigators found the source of the outbreak of E. coli O157:H7 on July 28: canal water. The Food and Drug Administration (FDA), Centers for Disease Control and Prevention (CDC) and state partners collected sample of water, soil and manure in the Yuma growing region. The investigators used whole genome sequencing (WGS) to make the match. This process takes a bit longer than other testing methods, but it’s like DNA fingerprints for pathogens, say investigators at both the CDC and the FDA. In all, 218 individuals became ill from the outbreak in 36 states and in Canada; 27 of those developed hemolytic uremic syndrome, a type of kidney failure; and five died, according to the CDC’s final report. Laboratory testing of other environmental samples continues and the FDA is still working to discover how the E. coli bacteria entered the water and the ways it could have contaminated romaine lettuce in the region.

ALLEVIATE PAIN WITH MILD TREATMENT
he Pain Treatment Center of the Bluegrass is the largest freestanding facility in Kentucky dedicated to the treatment of pain. It hosts 11 physicians of varying specialties and subspecialties, all who have the goal of pain alleviation and management. Dr. Steven Ganzel, one of the doctors practicing at the Center, has recently added Minimally Invasive Lumbar Decompression (MILD) treatment to his list of services for those seeking relief from pain and suffering. “Vertos Medical, Inc. is the company that developed the MILD Treatment,” Dr. Ganzel said. “They supply the necessary instrumentation to those doctors trained in the technique. I trained in this specifically because I saw the benefit it was giving many patients, and now I’ve been offering it to those who meet the criteria and seeing a great deal of success.” What are the criteria to receive the treatment? “The person has to be 66 years of age and they have to have a thickening of the ligamentum flavum,” Dr. Ganzel explained. These ligaments that run along the lower spine tend to thicken with age and degeneration of the bone and discs. The ligaments start to take on more and more burden as those other structures are compromised. This usually leads to weakness in the lower back and legs, which is called neurogenic claudication. The pathways through the back narrow and the nerves are constricted. This can also be hastened by arthritis and other degenerative diseases. If the person has had a fusion or spinal surgery in the past 12 months, he or she is not a candidate for MILD treatment. Why is the procedure gaining momentum now? “When Vertos first introduced the procedure, it did well for three to four years, then went into a bit of a lull due to clinical trials and other issues,” said Dr. Ganzel. “Around 2016, it was given the full green light and is now picking up steam as a terrific procedure for pain relief.” What are the advantages of this procedure? “The biggest advantage is that the convalescence is minimal,” Dr. Ganzel said. “Minimally invasive means just that. We go through two small holes to do the procedure, and it takes less than an hour to do both sides. By de-bulking the ligaments, the patient will find tremendous relief, and they are usually up and about within a few days versus up to six months for some spinal surgeries. Anesthesia is usually not necessary. Some patients do require ambulatory services, which are provided by the clinic since we are a full-service facility..” How do people get a consultation to find out if they are candidates for MILD? “I have to have an MRI to know if there is thickening of the ligament,” Dr. Ganzel said. “If a patient has had an MRI elsewhere in the last year, that will work, but longer than that allows for too many changes in the body to develop, which might result in an improper clinical decision. Just contact us at The Pain Treatment Center and we will set up a consultation.” Dr. Ganzel adds the procedure has worked wonderfully and with great success for many qualified candidates. “I wouldn’t spend time training on it and offering it to my patients if I didn’t believe in it

Whether they tackled the biggest problems of the day first, wrote out a plan for the entire day or got in a game of tennis at 5:45 every morning, as Wintour still does, all are good reasons to ditch the snooze button. Having a regular morning routine can be the key to a productive day – especially active habits such as exercising. An early-morning exercise routine can enhance your metabolism, encourage consistency in your routine, increase physical and mental energy, improve sleep habits and help you develop self-discipline. Try a wide-legged child’s pose to open up the hips and thighs; a supine spinal twist to relax and stretch the spine and abdomen; a seated forward bend to stretch the shoulders and hamstrings; and a knees-to-chest pose to stretch out the lower back. Remember to take five to 10 full breaths before moving on to each new pose. Mini-But-Effective Calisthenics Think old-school gym class warm-ups as a full-body workout routine to get the heart pumping. Weave together a five-minute routine consisting of one minute of jumping jacks, one minute of squats, one minute of jumping rope, one minute of planking and one minute of pushups. See how many of each exercise you can do in 60 seconds. Challenge yourself to get in one more of each exercise every day.
Purposeful Walking A morning exercise routine doesn’t have to be 30 or 60 minutes to be beneficial. And you don’t always have to sweat. Set your alarm for just 15 minutes earlier, give yourself five minutes to get ready and then head out for a 10-minute walk around your neighborhood. Use the time to enjoy the sounds of nature, mentally make your to-do list and think about all the good you’ll do that day. If you’ve got the time, add an extra minute here and there throughout the week. Hydration Energy Blast Water can make an amazing difference in your energy level by fighting off fatigue, which can be caused by overnight dehydration. Make it a habit to immediately grab 8 ounces or more of water as soon as you get out of bed and have another dose of H2O before you head out in the morning. Meditation for Productivity Whether you have an important meeting first thing in the morning or a hectic schedule of errands, anxiety and stress can quickly zap your energy, making you less productive. Give yourself five to seven minutes (or longer if you’ve got the time) to sit quietly alone and silence your mind with a background of soothing music or the stillness of a dark room. If you allow yourself this ritual every morning, you just may find you’re calmer throughout the day with a clearer perspective. No matter how you begin your mornings, start tracking your progress as well as your goals you want to accomplish each day. WHAT IS A
SPORTS MEDICINE DOCTOR?
f you or your child sustain an injury during exercise, sports participation, or any type of physical activity, you may be advised to see a sports medicine trained doctor for treatment. A sports medicine trained doctor has specialized training in both the treatment and prevention of sportsrelated injuries and is also knowledgeable about preventing injury in active people. These specialized doctors are board certified and have extensive additional training through residencies and fellowships. Although sports medicine doctors do work with professional and collegiate athletes, they also treat children and teens involved in sports, as well as adults who exercise for personal fitness or work in physically demanding jobs and construction workers. Examples of injuries treated by a sports medicine doctor include: • Ankle sprains • Fractures • Knee and shoulder injuries • Muscular strains • Tendonitis • Cartilage injuries • Joint injuries • Concussions Many sports injuries do not necessarily require surgery. Treatment for sports injuries can include the use of immobilization with a cast or sling, physical therapy or injections. In some severe cases, a surgical procedure to repair a torn tissue or realign bones is necessary. Most sports medicine doctors also serve as team physicians for local teams and clubs. These physicians must fulfill many additional responsibilities, including: • Pre-participation physical examinations • Injury assessment and management • Care of sports-related and general medical needs of athletes • Sports psychology issues • Substance use issues • Education and counseling on illness and injury prevention • Coordinating care with other members of the sports medicine team, including athletic trainers, physical therapists and personal physicians, as well as other medical and surgical specialties • Communication with athletic trainers, coaches and school administration, as well as athletes and their families Sports medicine specialists are ideal physicians for the non-athlete as well. They are excellent resources for the individual who wishes to become active or begin an exercise program. For the “weekend warrior” or “industrial athlete” who experiences an injury, the same expertise used for the competitive athlete can be applied to return the individual as quickly as possible to full function. When Should You Call a Sports Medicine Doctor? If you or your child sustain a significant injury during exercise or sports, a potential trip to an urgent treatment clinic or emergency room may be necessary. Signs of a significant injury include severe pain, swelling, numbness and an inability to put any weight through the injured area. If none of these symptoms are present, a visit with a sports medicine doctor can usually be arranged very quickly. MORE HOSPITALS ADOPTING MASSAGE THERAPY TO ADDRESS PAIN OVERALL PAIN LEVEL, EMOTIONAL WELL-BEING, RELAXATION AND ABILITY TO SLEEP WERE IMPROVED.
M
anaging pain for patients in an acute care setting can be a tricky situation for hospitals. Medical professionals don’t want to undertreat, inadequately manage or overtreat pain. Any of these options could lead to unpleasant physical and psychological outcomes for patients and their families. And in the face of the nation’s opioid epidemic, hospitals and accreditation organizations across the United States are adopting complementary and integrative medicine (CIM) initiatives such as massage therapy to address pain treatment strategies, according to a study published in the International Journal of Therapeutic Massage and Bodywork. Study Overview and Results Conducted at Flagstaff Medical Center in Flagstaff, Ariz., a nonprofit community hospital that serves a large rural area of northern Arizona, researchers provided 53 hospital patients with one or more massage therapy sessions. The sessions averaged 30 minutes each and were administered by three licensed massage therapists employed by the hospital. The most common modalities included gentle Swedish massage, acupressure, craniosacral therapy and cross-fiber myotherapy with light pressure. The study included patients from medical, surgical and obstetrics units. Pain levels before and after massage therapy were recorded using a zero to 10 visual analog scale. The findings determined before massage, the mean pain level recorded by the patients was 5.18. After massage, the mean pain level was 2.33. In the end, qualitative data illustrated improvement in all areas. The most significant areas of impact reported were overall pain level, emotional well-being, relaxation and ability to sleep. This study shows integration of massage therapy into the acute care setting creates overall positive results in the patient’s ability to deal with the physical and psychological outcomes of his or her health condition, including a significant reduction of pain and an increase in relaxation, sleep and recovery time. The study report’s primary findings show a strong correlation between reduction of pain levels after massage therapy and statistically significant differences in pain scores before and after massage. The
perception among participants of improved pain levels and less need for pain medication underscore the promise of massage therapy’s positive effect on pain management protocols. COMMON SLEEP DISORDER WREAKS HAVOC ON THE BODY E VA LUAT I O N BY A S L E E P S P E C I A L I S T I S E S S E N T I A L
he National Sleep Foundation estimates over 18 million adults in the United States, or about one in every 15 people, suffer from sleep apnea. Obstructive sleep apnea is a sleep disorder that interrupts breathing, resulting in disruptive sleep. Individuals suffering from obstructive sleep apnea will experience a repetitive (partial or complete) airway collapse throughout their sleep, which prevents air from reaching the lungs. Obstructive sleep apnea is the most common form of sleep apnea disorders. It affects all ages and both sexes, although it is more common in males over age 40 years. Other risk factors include obesity, large neck circumference, retruded jaw and a large tongue, tonsils or uvula. Common symptoms of sleep apnea include: • snoring; • shortness of breath during sleep, sometimes causing a person to wake, gasping for air; • excessive daytime sleepiness; • difficulty concentrating; • cognitive decline; • morning headaches; and • chronic drowsiness, which can impact work performance and even the ability to drive safely. Because a lack of sleep can wreak havoc on the body, individuals who suspect a problem should ask their bed partner if they ever snore loudly, gasp for air or stop breathing. If so, they should consider an evaluation by a sleep specialist. Sleep apnea is diagnosed by a sleep study, also known as polysomnography. It measures multiple parameters, including apnea (temporary cessation of breathing for 10 seconds or more), hypopnea (reduction in breathing flow for 10 seconds or more) and minimum oxygen saturation. Findings from the study are used to calculate the apnea hypopnea index (AHI), which consists of the frequency of apneas and hypopneas per hour of sleep. A sleep study is typically performed by spending the night at a sleep center. After a diagnosis has been confirmed and depending on the level of severity, treatment options can be discussed. The most common treatment for obstructive sleep apnea is the use of a continuous positive airway pressure device, better known as a CPAP machine. Using a CPAP machine requires you to sleep while wearing a mask that covers the nose and/or mouth. The machine forces air into the nose and/or mouth during the duration of your sleep in order to avoid airway obstruction. While the CPAP option is a very effective treatment, it does not always fit within the lifestyle of those suffering from sleep apnea. Many patients stop wearing it due to: • discomfort from sleeping with a mask; • bothersome machine noise; • a poor-fitting mask; • nasal and oral dryness; and • the machine not being travel friendly. For those individuals seeking an alternative, other effective treatment options may be available. In fact, dental sleep appliances are the most common nonsurgical alternative for patients who have difficulty tolerating the CPAP device. Many medical insurance plans provide benefit coverage for oral appliance therapy. A dentist with specialized training in dental sleep medicine will help individuals find the right oral

appliance for their needs as well as properly fit and adjust the device. The oral appliance is a custom-made adjustable mandibular advancement device (MAD). While wearing the device, an individual’s lower jaw and tongue are positioned forward to maintain an open airway to improve breathing. The device is similar in appearance to a mouth guard with slight modifications. It offers an effective, non-invasive treatment that requires very little change to a patient’s lifestyle. Oral appliances make no noise, are easy to care for and are very convenient for travel. Individuals interested in this option will need to speak with their sleep physician to ensure it will meet their needs. Generally, those with mild to moderate cases make good candidates for oral appliance treatment. The effectiveness of the oral therapy can be measured by little to no snoring, waking up rested and feeling rested through out the day and ultimately by repeating the sleep study with the appliance in the mouth to compare the results with the initial sleep study. If left untreated, sleep apnea can lead to serious complications, which include but are not limited to: • high blood pressure; • stroke; • chronic acid reflux; • memory loss; and • increased risk of death mainly with untreated severe cases. Additional alternative methods of treatment may include surgery, such as tonsillectomies should a person’s tonsils be enlarged, and lifestyle or behavioral changes such as weight loss, avoiding alcohol, quitting smoking and sleeping in a different position. It is important to keep in mind these options may not provide a permanent solution and sleep apnea issues may return at a later time. A SIMPLE MINDFULNESS PR AC TICE
he fast pace of modern life is taking its toll on our mental and physical health. Multiple surveys in the past year have documented an alarming increase in perceived stress, anxiety, depression and suicide. Our health – our very lives – depend on our ability to manage stress in healthy ways at home, at work, in traffic, in relationships – and simply inside our own skin. We need simple tools to bring some calm to the chaos – some peace to the frenzy – some kindness to the aggression and competition. The S.T.O.P. mindfulness practice is one such tool. This practice can take as little time as one breath or as long as you like. “S” is for “Stop” and take Stock Aren’t there times when you just need a break, even for a minute? Make yourself a promise to recognize several times each day when you need some self-care and rejuvenation and simply stop. Step out of the unskillful, habitual reactivity of automatic pilot mode and into the present moment. Step out of the doing mode and into the being mode. Reconnect with yourself and your natural inner resources of resilience, relaxation, peacefulness and wisdom. Really tune in, paying attention to what is happening right now, right here, without expectation, without an agenda other than a curious, open inquiry into what is actually happening inside you and around you. Even if you don’t remember this entire sequence, just remember the word stop. Have the intention to truly inquire into the three primary domains of your experience: 1) Body – sense perceptions and physical sensations; 2) Mind – thoughts, images, plans, memories; and 3) Emotions and feelings. Bring some well-deserved self-compassion and kindness to yourself, especially if your experience is unpleasant, stressful or painful. Ask yourself, “What is my experience right now?” Simply notice what’s going on around you and inside of you. Take stock of the situation. Take your foot off the accelerator and slow down, grounding yourself with some conscious, natural breaths. In the process, your pleasant experiences may be more fully nurtured and your unpleasant experiences may be less onerous. Alternatively, you may ask, “What is absent from my experience right now? What have I forgotten about myself, my work, my colleagues, my family?” Allow experiences of kindness, compassion, generosity, awe and beauty and smell the roses along the way. “T” is for “Take” a Breath Take a normal, natural breath, directing your full attention to breathing. Even one breath experienced with your full, unhurried attention can counteract the stress response. Feel the physical sensations of each inbreath and each outbreath – sensations in the nostrils as the air moves in and out – sensations as the air moves back and forth across the upper lip – sensations as the air moves in and out of the back of the throat – sensations as the chest expands and contracts – and sensations as the belly expands and contracts. You may find it helpful to say to yourself “in” on the inbreath and “out” on the outbreath. Use your breath as an anchor to bring you into the present moment and help you tune intentionally into your natural state of calm awareness and restful alertness. “O” is for “Open” and “Observe” Expand the field of your awareness beyond your breathing, including a sense of the body as a whole, your posture, your facial expression and the sensations on your skin. Notice all your sense perceptions – touch, sight, sounds, smells and tastes. Expanding your awareness beyond your body, connect to the trees and all the green growing things you depend on for oxygen. Notice your thoughts and their fleeting, impermanent nature. Notice

that thoughts are not always facts and not necessarily true. Notice that you can intentionally choose to think your thoughts or let them go. Allow your emotions to surface, recognizing and naming them without judgment: “This is anger” – “This is joy” – “This is grief” – “This is happiness” – “This is anxiety” – “This is depression” – “I know you. I am experiencing you but you do not define me.” Naming your emotions without self-judgment helps cultivate emotional intelligence, magnifying the benefits of uplifting emotions and reducing the power of distressing emotions. Opening your heart to your own stress, difficult emotions and suffering can nurture your natural capacity for human affiliation and social support and your capacity to help relieve the stress and suffering of other people and all living things. “P” is for “Proceed”/new “Possibilities” After this intentional slowing down, stepping off the treadmill and out of the rat race, take the benefits of this practice into the next moment, the next task, the next meeting, the next conversation, the next relationship, informing ordinary daily activity with the physiological benefits of mindful self-care. Notice the world around you, experiencing how things really are, tapping into your intuitive inner wisdom for what you need right now – a call or text to a friend, a quiet moment alone, a bite of chocolate, a cup of tea. Then proceed with more clarity, from a place of choice and skillful responding rather than reactive, habitual autopilot. Proceed without any expectation of how others will act or speak or behave. Be realistic about your inability to control the pace at which other people are moving. Know with increasing confidence you can consciously choose the pace of your own mind and body, where you place your attention and whether you perceive your cup as half empty or half full. Feel your inner relaxation response naturally balancing your stress response. With an open, curious mind, experiment with the S.T.O.P. practice several times a day, anywhere, anytime, before each meal, before starting the car, turning on the computer, bathing, brushing your teeth, taking out the trash, during conversation, going to bed, waking up in the morning – anytime, anywhere. As you take control of where you place your attention, you will understand why mindfulness is also translated as heartfulness. Refining your ability to slow down and S.T.O.P. can help you promote resilience, manage stress, prevent burnout and cultivate compassion. Keeping a log of your practice can be extremely helpful. entures (sometimes called false teeth) stretch far back in history. These are partial or full replacements of missing teeth. Teeth can be lost for a variety of reasons: accidents, dental malformation, tooth decay, drug use, pregnancy. For some people, losing some or all of their teeth can be very socially debilitating, so much so that many are too embarrassed to ask a well-qualified dentist about options. When patients come to Adkins Family and Cosmetic Dentistry and ask about denture possibilities, the first thing to be determined is if they need replacement(s) in the maxillary arch (the top teeth), the mandibular arch (the bottom teeth) or both. How many teeth need to be replaced and their locations make a huge difference in how the dentures are to be made and their overall performance and effectiveness. This performance must answer several important questions. Can the patient chew food more readily? Are the teeth more aesthetically pleasing to the patient? Can the patient pronounce words and enunciate better (missing teeth often affect the ability to speak)? Does the patient feel better about himself or herself after wearing the dentures? Many dentures are designed with a palate, meaning they actually fit into the roof of the mouth, and cosmetic WE CAN GO teeth fit right into the spaces where teeth used to be.
