Welcome to the Prospectus of Services for 2012/13 for County Health Partnerships and Bassetlaw Health Partnerships. We hope you find this a useful guide to the services we provide, where they are located and how to access them. Health Partnerships delivers services through County Health Partnerships and Bassetlaw Health Partnerships, both involving primary care in each of the respective localities. County Health Partnerships comprises Nottinghamshire Healthcare NHS Trust, Central Nottinghamshire Clinical Services (CNCS), who currently provide out of hours and primary care services through a social enterprise, and South Notts Integrated Clinical Services (SNICS), a primary care clinician led consortium. Bassetlaw Health Partnerships was formed towards the end of 2011 between GPs in Bassetlaw and Nottinghamshire Healthcare NHS Trust. On 1 November 2011, services formerly provided by Bassetlaw PCT (Bassetlaw Community Health) transferred to Nottinghamshire Healthcare NHS Trust. We have many achievements to be proud of in our first year: • Clinical Directors appointed in each locality resulting in strong relationships being developed and a distributed leadership approach is now emerging • Delivered 12% sustainable CIP over two years • Staff engagement score in the National NHS Staff Survey within the top 10% of Trusts nationally • 94% of patients reporting high levels of satisfaction • Podiatric Surgery – no patient now waits over 18 weeks. Sustainable service achieved by: - Reversing previous CIP by appointing new Podiatric Surgeon - Investing £80,000 in new equipment to improve efficiency - Redesigned theatre throughput with external efficiency review

• Lings Bar Hospital service redesign has reduced length of stay from 36 to 24 days • Delivered 8% extra activity last year and sustaining this • Physiotherapy waiting times in Mansfield and Ashfield have reduced to no more than 1 week, achieved through service redesign. Planning has commenced for the deployment of the new model in the south of the County • Lynne Cotterill, Clinical Director for Rushcliffe shortlisted for the Nursing Times, Nurse of the Year Award • £500,000 invested in IT equipment replacement and laptop deployment Most importantly, throughout a year of transition, we have continued to provide high quality, safe services to patients. Last year, over 97% of patients/carers said the quality of our services was either excellent or good. Almost 99% said the level of respect and dignity was either excellent or good. Moving forward, we have ambitious plans for service transformation. Our Clinical Directors will be at the forefront of our organisation. Their clinical leadership will drive service redesign, clinical excellence and they will play the central role in the partnership’s strategic issues with our commissioning colleagues. During our second year, we will develop new service models and test out new approaches to support the shift in care from hospitals into the community. We are confident we have now built the platform to create and sustain a radically different model of community support with our unique partnership and strong clinical leadership.

Children’s services Integrated Child and Family Health The Child and Family Health Teams provide universal and targeted health services to children and young people ages 0-19 years and work within integrated health visiting and school nursing teams. Health Visiting The Health Visiting Service works with families with children aged 0-5 years, delivering programmes of care tailored to meet the needs of the individual family unit. (This is a support scheme for parents who have had a baby die suddenly and unexpectedly, there is support from Consultant Paediatricians, Midwifery and Health Visitor Teams).

Paediatric Liaison Health Visiting team (LHV) The team is hosted by CHP, and covers the City and County of Nottinghamshire. The LHV Service liaises with Acute and Secondary Care of children aged 0-19 years, who attend Nottingham University Hospitals and Sherwood Forest Hospitals. The aim of the service is to facilitate all children accessing healthcare provision to be safeguarded by effective

Breastfeeding Promotion Service The Breastfeeding Promotion service has been commissioned to lead County Health Partnerships to achieving the UNICEF Baby Friendly Initiation accreditation. By achieving this award, pregnant women and breastfeeding mothers will receive optimum evidence based support to improve their breastfeeding experience, thereby enabling continuation of breastfeeding for as long as each mother wishes. As a result, breastfeeding initiation and prevalence is positively affected, improving both morbidity and mortality rates within the County. This is achieved by: • Regular training and interaction with health visiting teams, and Sure Start Childrens’ Centre staff • Partnership working with the County Council and GP practice colleagues • The development of a Breastfeeding Peer Support volunteer programme • Partnership working with neighbouring Trusts • Specialist breastfeeding support in the clinical area

All children are automatically referred to the School Nursing Service upon leaving the Health Visiting Service. Special School Nursing The service provides nursing support for children and young people at special schools with complex healthcare needs and supports their transition into adult services. Services include: • Ongoing clinical monitoring of children and young people who are ill or have life limiting conditions. All new school entrants receive a full assessment to determine the appropriate level of support for the child and identify any staff training that may be required • Routine and emergency medication administration (including via enteral route), storage and monitoring • Assessment and monitoring of children and young people with a tracheostomy who require total support • Routine catheterisation • Enteral feeding • General and child specific multi-agency training e.g. enteral feeding, emergency medication to support their care

Sure Start Children’s Centres Sure Start Children’s Centres are designed to offer parents to be and families with young children access to integrated early childhood services “when and where they need them”. They act as a service hub for the community, bringing together public, private and voluntary agencies to work collaboratively. The main aim is to improve outcomes for all young children and to provide early intervention to protect the most vulnerable children and their families. Children in Care and Adoption Health Team The Children in Care and Adoption Health Team provide a full and responsive health service specifically designed for children in care and on an adoption pathway across Nottingham City and Nottinghamshire County. The team aims to address health inequalities by offering a distinct and specialist service. The team works closely with partner agencies to meet the health needs of this specific community of children and young people and work towards alleviating the impact of neglect, abuse and poverty that children and young people may have experienced in early life. Services include: • Health assessments for all children in care • Ensuring children are registered locally with a GP and a Dentist • Integrating children into mainstream services • Ensuring immunisations are up to date • Providing training to a range of partner agencies • Providing health support in residential homes

Adult services Intermediate Care Intermediate Care is a range of rehabilitation services that are community based and delivered in partnership with Adult Social Care and Health. The service provides multi-professional assessment and up to 6 weeks rehabilitation for older people. Intermediate Care can be delivered either in the patient’s own home or in a residential care home, dependent on the level of need. Community Stroke in the South of Nottinghamshire The Community Stroke Team provides rehabilitation in patient’s homes, and provides advice in the workplace and within leisure facilities. The team promotes self care, health promotion and reducing dependency. The staff have expertise in home-based stroke rehabilitation for those with complex needs. Primary Care Rehabilitation Team The team provides standardised, individually prescribed exercise programmes to improve strength and balance, home hazard assessments and intervention, and referral for vision assessment, in line with the evidence base. Interventions will be individualised and aimed at promoting independence and improving physical and psychological function. Patients referred to the Community Falls Service must have a complete multi-factorial clinical assessment undertaken by a medical practitioner to exclude any medical causes for a fall. Community Matron Community matrons work closely with GPs, and nursing and therapy teams along with other agencies to identify and provide proactive management and care for patients with long term conditions who have complex needs (Very High Intensity Users, VHIUs). Each patient on their caseload has a personalised care plan and is supported in their own home to manage and monitor their condition. A Community Matron’s core role includes: • Case management through care coordination • Health promotion and education • Medicines management • Promotion of self care and self monitoring • End of life care

The overall aim being to reduce unnecessary hospital admissions, facilitate early discharge from hospital and support the patient’s level of independence. Adult Community Nursing Services The services provide appropriate and effective care management through Community Nursing Integrated Care Teams, working in defined community wards. Teams include the skills and expertise (including non-medical prescribers) of Community Matrons, District Nurses, Community Registered Nurses and Health Care Assistants. Key priorities of the service are: • Management of long term conditions • End of life care • Tissue viability services • Management of continence The service will: • Assess, promote, treat and manage continence. • Provide patient education to minimise risk of recurrence of venous leg ulceration and assess, treat and promote healing of venous leg ulcers. The service holistically assesses the needs of patients and carers to plan, implement, and evaluate nursing care interventions. The service promotes the physical and psychological wellbeing of patients and their carers, taking into account social care needs Continence (North) The service provides comprehensive specialist continence assessments for adults (aged 18 or above), to determine a diagnosis and offer individualised treatment programmes. Where treatment programmes have not

given an effective outcome, referral to other specialist services will be offered following treatment pathways. The service is delivered in specialist nurse led clinics and, where relevant, in patient’s own homes and follows the CHP Adult Continence Care Pathway. The service provides specialist advice and professional support and delivers continence training programmes for staff across healthcare and the independent sector (care homes).

Tissue Viability (North) The service provides specialist assessment, expert advice, education and training to health professionals. This takes place in hospitals and community settings (including the private sector). The aims of the team are to prevent unnecessary hospital admissions, facilitate early discharge, improve healing rates of wounds and reduce the incidence of pressure ulcers. Diabetic Specialist Nursing Service (North) Community Diabetic Specialist Nurses support patients in primary care who require additional management of their condition. COPD Chronic Obstructive Pulmonary Disease (COPD) Community Services provide case management, including integrated, multidisciplinary assessment and treatment through person-centred care plans, in line with case management strategies.

Pulmonary Rehabilitation programmes consist of exercise and education to help patients improve their quality of life by monitoring and managing their condition. These are delivered in each of the localities and patients offered a choice of venues.

Community Heart Failure Specialist Nurse The Heart Failure Specialist Nursing Services provide a community focused specialist service to support both the pharmacological and non-pharmacological management of people living with heart failure. The aims of these specialist services are to work in partnership with primary and secondary care health professionals, using evidence based guidelines, to improve quality of life, reduce unplanned hospital admissions and facilitate end of life choices among heart failure patients. Lings Bar Lings Bar is a 72 bedded Rehabilitation Hospital situated in Gamston. The hospital is all on one level and has three 24 bedded wards. The hospital cares for both men and women generally aged 60 and above, who require physical rehabilitation following an admission to an acute hospital. The Medical, Nursing and Therapy Teams work in collaboration with the patients to plan care and set goals ultimately giving them the best chance to regain their mobility and independence

in order to be safely discharged home from hospital. The care approach is that of nurse managed patient caseloads with direct support from a doctor and Consultant each week. The team is multidisciplinary and involves nursing, medical and therapy staff with support from Dietetic and Speech and Language Therapy Teams. The hospital works in partnership with both health and social care teams within primary and secondary care settings. The social care teams are housed within the hospital allowing for a seamless approach to patient care and integration of teams working together. The service is also commissioned to provide up to 15 non weight bearing beds for patients with predominantly lower limb fractures. Palliative care patients are also cared for in the service for those in the last days of life on either the Liverpool Care Pathway or Fastrack Pathway. Phlebotomy The Phlebotomy Service based in Mansfield and Ashfield, Nottingham North and East, Nottingham West and Rushcliffe areas provides a blood taking service to support GP employed Practice Nurses.

Palliative Care The Specialist Palliative Care Services work in partnership with the John Eastwood Hospice Charitable Trust, which owns and maintains the John Eastwood Hospice. Part of the partnership is the provision and management of a large team of volunteers working within the service. John Eastwood Hospice is purpose built to meet the needs of palliative care patients and their families, ensuring good disabled access to ensure a broad spectrum of needs can be met.

Specialist services ADULT SPECIALIST SERVICES Speech and Language Therapy A specialist service to adults aged 18 years and above, who present with acquired communication (speech, voice, language) and swallowing disorders. There are four pathways within the service: • Long term conditions • Stroke • Specialist medicine and dementia • Head and neck The service provides a range of assessment and therapeutic interventions to adults with acquired communication and swallowing problems. Musculoskeletal Therapy Service The service consists of a team of physiotherapists and occupational therapists who provide assessment, diagnosis and management for patients with a broad range of musculoskeletal presentations (joint, muscle and nerve problems). The service includes: • Individual assessment and personalised treatment • Group sessions where appropriate • Occupational therapists provide a limited upper limb splinting service to patients within Nottingham West, Nottingham North and East and Rushcliffe • Provision of exercise and information sheets to enable patients to manage their own condition • Access to a Clinical Specialist Physiotherapist to review complex cases • Within Rushcliffe, the service is provided under an Any Qualified Provider contract and provides an assessment, advice and guidance service only Falls Prevention (Newark and Sherwood and Mansfield and Ashfield) Provides standardised, individually prescribed exercise programmes to improve strength and balance, home hazard assessments and intervention, and referral for vision assessment, in line with the evidence base. Interventions will be individualised and aimed at promoting independence and improving physical and psychological function. Individuals and their carers will be offered information both verbally and in writing about what measures they can take to prevent future falls.

Patients referred to the Community Falls Service must have a complete multifactorial clinical assessment undertaken by a medical practitioner to exclude any medical causes for a fall. This may be carried out by other appropriate clinicians within their own professional competence and registration as appropriate to prevent duplication. Podiatry The podiatry service incorporates community, surgical and specialist (diabetes and musculoskeletal) teams who provide assessment, diagnosis, treatment, education and advice for a range of foot disorders with an emphasis on prevention and self-management. • Community Podiatry is based in health centres throughout the County and visits are provided to patients in their own home • Podiatric Surgery and outpatient clinics are provided at Park House (Carlton) and Newark Hospital • Specialist Podiatry input to Diabetes Services is provided at clinics at Nottingham University Hospital Trust sites (City Hospital and Queen’s Medical Centre) and the King’s Treatment Centre at King’s Mill Hospital (Sherwood Forest Hospitals Trust)

• Specialist Musculo Skeletal Biomechanics is provided at Park House (Carlton), Mansfield Community Hospital, Ashfield Community Hospital and Newark Hospital. Special Needs Dentistry The service provides high quality dental care and oral health advice to patients with special needs, and those who cannot be treated in general dental practice due to some impairment, disability or complex medical condition. Services include: • Full oral examinations and explanation of treatment options using behavioural management techniques, inhalation sedation and General Anaesthesia (GA) where appropriate • A direct access dental extraction service (exodontia service) for children in pain is provided using facilities at Nottingham University Hospitals, Queens Medical Centre campus • An emergency walk-in service at the Integrated Dental Unit (IDU) at the Nottingham Walk In Centre, Seaton House, for patients in pain Nutrition and Dietetics The Nutrition and Dietetics Department provides therapeutic and specialist dietetic services for referrals of all age ranges. The service provides information and advice on a range of conditions, including obesity, diabetes, gastroenterology, elderly care, stroke care, mental health, home tube feeding, paediatrics, hyperlipidaemia, food based allergies and malnutrition. The team treats patients on an individual and group education basis. The service trains and supports other NHS staff and volunteers who provide nutrition and dietetic advice. The community nutrition service also fulfils a public health role, supporting media campaigns and carrying out health promotion sessions by request.

CHILDREN’S SPECIALIST SERVICES Paediatric Physiotherapy The service provides assessment and treatment for children with recognised difficulties and impairments, including those with a neurological condition, including: • Cerebal Palsy • Duchenne Muscular Dystrophy • Spina Bifida • Toe walking and other gait anomalies • Profound and multiple learning difficulties (PMLD) • Developmental delay • Motor control difficulties • Syndromes This team is also responsible for delivering the service to children and young people registered with a GP in NHS Nottingham City. Paediatric Occupational Therapy and Therapeutic Support The service is primarily involved in the assessment and intervention of childhood conditions which may

inhibit progress of physical and functional development. This range of conditions will include: • Neurological conditions e.g. Cerebal Palsy • Neuromuscular conditions e.g. Muscular Dystrophy • Syndromes e.g. Retts • Children with functional and motor coordination difficulties • Orthopaedic conditions e.g. post surgery • Genetic conditions e.g. Osteogenesis Imperfecta • Children with moderate and severe learning disabilities

Children can be seen individually or in a group session, depending on their needs. All intervention is in relation to the identified needs of the child. This includes the following: • Assessment and observation • Individual care planning • Planned direct intervention • Home programmes • Monitoring and review • Information, support and advice to parents and carers and others working with the child

• Hearing impairment • Autistic Spectrum Disorder • Disorders of fluency

Paediatric Speech and Language Therapy (hosted by CHP covering the City and County) A specialist service for children with disorders of communication (speech, language, fluency, social interaction), and/or eating and drinking difficulties (dysphagia). There are five care pathways, which include: • Specific language impairment • Learning difficulties and severe and complex needs including: eating and drinking difficulties, acquired brain injury and high tech augmentative and alternative communication

Children’s Development Centre (CDC) The nursing team based at the CDC located within the grounds of the Nottingham City Hospital, supports a range of services including community nursing, support for consultant clinics and short breaks for the south of the County and City.

For all these care pathways the service provides specialist support for children whose first language is not English. Children meeting the service entry criteria will receive assessment, diagnosis and packages of care. These are delivered in a range of settings, according to the needs of the child.

The community children’s nurses are based within the CDC and manage a caseload of children and young people with a variety of complex health needs, including tracheostomy care, enteral feeding, oxygen therapy, venepuncture

Other teams Oral Health Promotion The team facilitate key oral health messages and good practice of oral health, in order to address health inequalities and improve the oral health of the local population. Healthy Schools Team The team work with educational settings to improve outcomes for children, young people and their families through the development of interventions that promote positive health choices and enhance wellbeing. Safeguarding The team provide specialist training, advice and supervision to staff around safeguarding and child protection to enable them to safeguard children and vulnerable adults. Infection, Prevention and Control The team provide specialist knowledge, advice and education and support all services and staff in ensuring that the

The information service provides a lending library and an enquiry service for families and staff caring for children and young people with physical, learning or emotional disabilities. Families do not need to be attending clinics at the CDC to access this service.

Innovative methods are used to seek and gain carers views to influence service improvement Self Care – self care and management of long term conditions is a key element of all services, supporting the care people take towards their own health

Self management programmes are provided for people with long term conditions including Expert Patient Programme, Health Peer Mentors, Regaining Confidence after Stroke and Looking After Me, a programme for carers Customer Relations – is a single point of access for comments and feedback on services, including compliments, concerns and complaints and the team support services to take action to build on good practice and improve services
