WHAT’S NEW, WHAT’S EXCITING WITH RSTI Hospitals are still dealing with the effects of the economic downturn and the prolonged debate over healthcare reform. If things continue as they are, the debate on what healthcare reform and potentially even healthcare delivery will look like in the future will be revisited. Even in the face of the current economic situation and healthcare reform, RSTI must keep moving forward in our efforts to provide the best educational opportunities needed to benefit the medical imaging community. Perhaps you have noticed that RSTI has been adding some newer high-end systems to the labs. These systems are quite expensive. We finance these systems by buying them outright or trading training credit for used equipment. To continue the improvements in the labs WE NEED YOUR HELP. You can direct your used equipment to RSTI instead of trading it in. In most cases we can match the tradein offer given to you by the new equipment salesman. If you are removing or replacing a system please give us a call. Maybe RSTI can help your facility dispose of old equipment and at the same time, you can help your training center.
Lately, we have had the honor of training our military men and women who are responsible for maintaining the imaging equipment in their hospitals. On-site training sessions have brought together military bio-meds from around the world to improve their skills. We hope you will join us in recognizing the service and sacrifice of our Armed Forces on Veterans Day, November 11th. As we go through Trade Show season, check our schedule to see what shows we will be attending. If you have an opportunity stop by and see us, we always enjoy visiting with our friends and students. Bring someone with you and introduce them to RSTI. Finally, in the past some of you asked if we have an instructor position open, the answer is usually no. Well, due to our growth we have a need for a special individual to join our first class training team. Inside on page 4you will find a detailed listing for the position.

Letter from the President Time seems to be flying by these days at RSTI. I am writing this near the end of September 2010, which means we are winding down our 25th year in business. Last year, 2009, was our 8th consecutive year of growth with digital training in every modality leading the way. While we have no intention of eliminating any analog training, it is clear at this juncture that both DR and CR have significant future growth potential for RSTI. This year we have added some great new equipment to our training labs, and our instructors have been busy developing curriculum to meet those needs. RSTI added the OEC 9900 in May of this year, and we have already run 5 FULL classes since its inception on 6/14/10. While the new classes like the Precision 500D, the Revolution XR/d, the GE Senographe 2000D and Bone Densitometry all continue to run FULL, RSTI does not stop there. We just purchased our first GE Senographe Essential FFDM system. It is a 2009 system, and we are working on the curriculum for the 11/1/10 class, which, by the way, is already FULL. As you may surmise, our costs have skyrocketed to keep RSTI’s leading edge approach to training.
Eleven years ago, when I began at RSTI, we would never have considered investment spending on relatively new equipment and with good reason. Everything was analog, so you didn’t need to be any more current than 10 years old or more. Today the world has changed and so has RSTI. Most of our equipment is now 5 or 6 years old, geared to be in our labs when the equipment is most likely to finish the extended warranty period. For example, our 500D was made in 2008. The Essential was made in 2009, and the OEC 9900 came off the line in May of 2010. I could go on, but you get the point. RSTI will continue to invest in your education whenever and wherever possible. Currently we have 21 full labs. Ten of them now have DR equipment. The other 11 have analog systems, so that we can make sure that your time spent here is the best, well-rounded education available anywhere in the world. Why train anywhere else? Additionally, the RSTI staff continues to shine. Anna Morrison is the best registrar around. She takes care of our students as if they were her own family. Those of you who have not experienced her organizational wizardry have a treat in store for you. Donna Hastings is the keeper of the keys. She creates and re-creates everything you need

“What we have is the best equipment available for the hands-on training that is crucial to the educational experience. “ from manuals and schematics to RSTI pencils. When you walk into an RSTI classroom for the first time, you know that you are among professional educators. She prepares every classroom to her high standard, so that when the instructor and his students walk in, everything is where it should be to begin. We rarely discuss our instructor staff, but I have nothing but high praise for anyone who can take the sophisticated curriculum we have and make it palatable for everyone in their class. It takes a real artist to take someone with no imaging in their background and couple him with engineers that have 20-30 years of experience. Getting them both through the class with two different levels of understanding and appreciation is a daunting task, and yet I watch our instructors successfully tackle it every day. Not only are our instructors professional educators, but they have all had years of actual field experience, so they are able to relate on the most personal level possible. For example, Todd Boyland, our digital guru, has installed and calibrated more than a dozen digital mammography systems. He also has been called upon many times to resolve field problems with many different PACS and Connectivity issues. Dale Cover has de-installed and installed more systems than I can count. His experience in the Army and in the field gives him some very unique problem solving skills that get called upon by OEM’s from time to time. He also develops much of the new course material for our new classes. Of course, he was trained at RSTI by the best,

Terry, Sr. himself. Dave Domanski worked as a field engineer for more than 10 years before we hired him. That experience coupled with RSTI training have made him a potent educator in Phase I and II. He also does all of the OEC classes. John Drew teaches many of the analog classes, including mammography, Proteus and Ultrasound. Additionally, he has been involved with de-installations and installations on all of this equipment, giving him that unique hands-on capability shared by all of our instructors. Last, but not leas,t is Mike Petelin. Mike has a degree in electrical engineering. From there he has mastered diagnostic imaging in the areas of CR, Bone Densitometry, and Mammography. Currently he is working on his Master’s Degree so that he will be able to teach on the college level. Hopefully, I haven’t bored you with this insight into RSTI. When selecting where to spend your time and money, we recommend that you comparison shop. Anyone who has done the legwork ends up at RSTI, sooner or later. What we have is the best equipment available for the hands-on training that is crucial to the educational experience. What we do is teach you to be competent, confident and customer service oriented. DAVE’S RAVE: The OEC 9900 Greetings from the labs at RSTI! Have you been frustrated by training with a serious lack of hands-on lab time? Well you won’t run into that here! In fact, I guarantee that at the end of the very first day of my OEC 9900 class the students will have this new 9900 completely taken apart. Do you have what it takes?
Cover’s Column: Getting Started In most of my past articles I discussed the benefits and value propositions of some of our most advanced classes. For example I have outlined how, after the completion of the Computed Tomography series of classes, you will be capable of handling most of the service needs of your CT system. In the productspecific Radiographic/Fluoroscopic classes I’ve talked about replacing X-Ray tubes, image intensifiers and pick-up tubes plus all the calibrations involved. Let’s take a moment however to look at proper preparation for these advanced training offerings. Being prepared for advanced education is the key to getting the most from these classes. You might try to take an astrophysics class without the required math classes beforehand but chances are the most you’ll get out of the class is an opportunity to look at some pretty pictures. I’m not comparing our classes to astrophysics class; I’m just trying to make a point.
RSTI’s not the only one that pushes the idea of prerequisites. All of the OEM’s have their own prep classes to get you ready for their advanced product classes. If you have looked into OEM training, you know that even the prep classes can be shockingly expensive when combined with the primary class you want to take. In most cases, RSTI classes can be substituted for the required OEM prep classes at a substantial savings. A major benefit of our basic courses is that they are hands-on while most of the OEM’s are using CD or web-based training. Besides meeting the requirements for more advanced training, our entry level training courses present topics and develop skills that will give you the foundation to get started quickly with the general service work involved in the modality you are training for. Your skills can be further and more efficiently improved during the advanced course due to your previous class work.

What is resolution? Resolution, by definition, is the number of pixels (horizontally and vertically) that can be displayed at any one time. As the medical imaging world continues to evolve toward digital capture we, as service professionals, need to be acutely aware of the benefits and implications of system resolution. Resolution of a captured image is directly related to file size. Special attention has to be made when adding a system with a high resolution detector such as a Digital Mammography system or a Mammography CR system. We need to take into account how many of these high resolution images are to be sent over our network and the amount of images that are anticipated to be archived over a period of time. To calculate an individual image file’s size, we use the following equation: [Height(# of pixels) X Width(# of pixels) X Color Depth X Bit depth ] ÷ 8 = Size of Image in Kilobytes A real world example is a Hologic Selenia©. The Pixel Dimensions are 3328 x 4096 with a color depth of one, or monochrome, and a bit depth of 14 bits. The division by eight simply changes the final result from bits to bytes. If we wanted the result in Kilobytes we would further divide by 1024; an additional division by 1024 will result in Megabytes. The calculation would be as follows: [3328 (Height) X 4096(Width) X 1 color X 14 bits] ÷ 8 = 23,855,104 Bytes

A typical Mammography study requires 4 views or images so a single patient can compile 91 Megabytes of image data in a single exam. If patient throughput is 10 per day, that would be 910 Megabytes of image data per day. We need to know if our PACS Archive has enough storage to handle this load. The moral of this story is that when a facility buys the latest and greatest in imaging technology, we need to be sure everything that this system will be utilizing, from network resources to storage, is robust enough to handle the load.

PACS TRAINING : TRIPLER ARMY MEDICAL CENTER RSTI recently concluded the second of a total of 3 PACS training programs at Tripler Army Medical Center in Oahu, HI. Tripler Army Medical Center is the headquarters of the Pacific Regional Medical Command of the Armed Forces administered by the United States Army in the State of Hawaii. It is the largest military hospital in the Asian and Pacific Rim region and serves a military sphere of jurisdiction that spans over 52% of the Earth’s surface.
During the 2-week and 3-week PACS training courses, soldiers and GS (General Schedule) staff from the Medical Maintenance and Radiology divisions from Tripler Army Medical Center, Brian Allgood Army Community Hospital Korea, and the BG Crawford F. Sams US Army Health Clinic Japan were trained on RSTI PACS Phases I & II. Students developed skills in the following areas:

2010 > SHOW SCHEDULE
CEAI held its first annual conference, “Balancing Healthcare’s Future,” on August 25-27 in Oak Brook, IL. By all accounts, the meeting was a great success. Fantastic job! There were over 60 vendors and 150 attendees. In addition, as many as 20 classes were offered. RSTI was happy to participate by having a booth and providing a two-session “Introduction to Imaging” course. My classroom was packed with students. It is always good to see such a strong desire for engineers to learn new skills. If you are from the Illinois area and are not currently involved with CEAI, I would highly recommend joining the organization. They have a great bunch of people over there.

SOCIETIES IN THE NEWS: FIME
Once again the FIME show was a great success. We have attended FIME for the last few years. This has become a very good show for us. This year was no exception. It is well attended by the South American and Latin markets. Over the last few years we have been able to meet and greet with many new clients. Since the show has grown in attendance each year, they announced that they will be opening the rest of the convention center to accommodate all the new exhibitors. It is a global event. RSTI training has become an interesting topic of discussion and has gained considerable interest. While we still talk with many people about used and refurbished equipment, this year, training seemed to be the more interesting topic. We look forward to attending again next year and for years to come.

Get Trained on New Technology Systems For the past few newsletters I have been emphasizing the RSTI X-Ray Certificate Series, Phases I-IV. These classes are required to get an engineer up to speed in our field. It is by no means all the training you will need in your career. It does however take years of OJT, “On-The-Job” training. Now I would like to talk about continuing your education with advanced training on newer systems. Getting trained on the new technology systems can follow a little different path than older systems. We still need the knowledge of how to produce X-rays by controlling kV, mA and time but there may be many new things we get introduced to as well.

New Updated Selenia Curriculum at RSTI RSTI has decided to update the curriculum and format of our Selenia digital mammography training program. The Selenia training formerly consisted of a 3 week program, which now will be condensed to 2 weeks. Today’s service professional has a difficult time getting away for 2 weeks of training, let alone 3 weeks. The reason for the change comes from service manager’s requests for a more condensed course, combined with the evolution of the Selenia software. The OEM recently released DROC software 3.4. This version of software combined with the past 2 software version upgrades has streamlined many of the calibrations including AEC calibration, pixel mapping, and detector calibration. The updated curriculum has eliminated many software applications and procedures that have become obsolete throughout the system’s software evolution. The end result will be a tightly-packed two-week hands-on training course, producing a fully qualified service professional. RSTI feels this change will benefit the students with updated curriculum, up-to-date software, less travel time away from home, and lower travel expenses.
