FEMALE CONDOMS Did you know female condoms can improve the health and welfare of millions of people worldwide? They are the only method women can initiate themselves for protection against both unwanted pregnancies and sexually transmitted infections, including HIV. Studies show that women as well as men become enthusiastic about them after trying them out. They can even increase sexual pleasure. The time for female condoms is now.

UAFC In the Universal Access to Female Condoms (UAFC) Joint Programme, four Dutch organizations cooperate to get female condoms worldwide on the map: Rutgers, Oxfam Novib, i+solutions and the Dutch Ministry of Foreign Affairs. We strive to make female condoms available, accessible and affordable for all through international activities and country programmes in Nigeria, Cameroon and Mozambique. We and our partners increase the distribution, support manufacturers, educate communities and enhance the commitment of governments and donors. ZAWADI SMARTLOVE Do not be surprised if you come across this character at a health rights related event. In 2011, our charming spokeswoman became world famous as a paper doll on which people wrote their message demanding female condoms. She still travels around the world to tell people about the importance of these contraceptive life-savers. SASKIA HÜSKEN Making female condoms available is only possible with the support of governments and donors. Convincing them of the need for female condoms is the job of advocates. Saskia Hüsken leads UAFC’s advocacy efforts and reflects on 2014: “Our partners keep going despite the many challenges.” WHAT DO YOU DO WITHIN THE UAFC PROGRAMME? “We are raising a voice for female condoms. Ideally every government and relevant donor puts increasing the accessibility of female condoms on their agenda. To achieve this, we try to make decision-makers aware of the demand for female condoms and the value of investing in them. Our advocates in Nigeria and Cameroon are doing this at country

level, but we also aim to make female condoms part of international agreements on improving health. Moreover, we support advocates around the world via our International Advocacy Platform. The Platform enables them to exchange experiences, stay up-to-date on international developments and learn from UAFC’s female condom programming and market intelligence knowledge.” WHAT MOTIVATES YOU TO FIGHT FOR FEMALE CONDOMS? “I strongly believe that women and men have the right to be informed about methods of contraception and disease prevention and that they should be offered a choice of safe commodities. Female condoms are the only woman-initiated method that protects against both 5


unintended pregnancies and sexually transmitted infections, including HIV. They are safe, they are effective and they can offer additional sexual pleasure.” WHAT ARE THE BIGGEST CHALLENGES IN FEMALE CONDOM ADVOCACY? “It can be a bit frustrating to meet reproductive health experts who still have not heard about female condoms. How can you not know about this great method? As well as this, it is a challenge to counter the persisting prejudices about female condoms. For example, there are people who say that female condoms are ugly. I do not find other methods pretty either. This is rather a weak excuse to not consider this product as a serious option. Furthermore, seeking support from decision-makers requires continuous energy. When an official is transferred to another department or when there are elections, our efforts can come to a stand-still and in some cases we have to start all over again.” HOW DID YOU TRY TO ACHIEVE YOUR GOAL IN 2014? “Advocates from UAFC and its partners worked hard to get female condoms more and more on the map. We had meetings with governments, made connections with 6

influential parties, contributed to international coalitions and made noise during relevant conferences. One of the highlights was our attendance at the International AIDS Conference in Melbourne. With some creative presentations we helped conference participants get acquainted with female condoms. We


built a hair salon where people could have their hair done while being counselled on female condoms. Of course we also used our famous ‘belly tray’ to show the various female condom products. Another milestone was the development of the publication ‘The Business Case for Female Condoms’, which analyses the cost-effectiveness of investing in female condoms. This report helps in our advocacy towards governments, donors and large implementing organizations to convince them to commit to female condoms. The publication was launched at the International AIDS Conference with several high-level stakeholders present.” LOOKING BACK AT 2014, WHAT MAKES YOU PROUD? “What makes me most proud is the energy and persistence of our partners in Nigeria and Cameroon. They keep going despite the many challenges and difficult situations, like the Ebola crisis and the security situation in Nigeria. I am honored to support their work. The commitment from the government in Cameroon to allocate a budget for the purchase of female condoms is a great achievement from our partner FESADE. The ownership by the Nigerian government of the national

commemoration of Global Female Condom Day shows how good our Nigerian partners are.” LOOKING FORWARD TO 2015, WHAT EXCITES YOU? “The Global Female Condom Conference that we are organizing together with PSI, CHANGE, and MatCH Research will be the highlight of our overall programme, but especially for our advocacy component. The conference itself will of course offer exciting sessions and discussions on advocacy. As well as this, the event will be a great advocacy moment towards donors and partners from around the world.”

Cameroon

In Cameroon, ACMS and FESADE are paving the way for female condoms. ACMS creates selling points and advances the supply chain. Unique is its approach to train hairdressers to educate their clients on female condoms. FESADE connects with policy makers and community leaders to generate acceptance and enthusiasm for the product. To make a case for female condoms, there is nothing stronger than the voice of the people. Three of UAFC’s partners shine their light on the (potential) power of female condom users: Titilola Abolade (ARFH, Nigeria), Estrella Alcalde (Pathfinder Mozambique) and Valérie Gystiane Tsemo (FESADE, Cameroon). CAN FEMALE CONDOM USERS HELP TO INCREASE THE ACCESS TO FEMALE CONDOMS? Titilola “Yes! When they speak out about their satisfaction with the use of female condoms and where they accessed the product, more people will want to try it. When there is more demand there will be more supply.” Valérie “I agree that the voice of female condom users is very important. They can play an important role in reducing the myths and misconceptions that still

persist around female condoms. Because they already conquered the taboos, they are an example to others.” Estrella “We sometimes hear decisionmakers, media and others say that people do not like the female condom. But when we are in the communities, we hear from all kinds of women and men that they do like it after having tried it. We need to highlight those stories to counter the dissenting voices out there!” 11


HOW CAN USERS HELP? Estrella “Health and rights associations could bring users together to establish a collective voice. In our projects we have arranged for users to speak to a broad audience at public events and on TV and radio. In schools and communities we show a documentary video in which young women talk about female condoms. This is a success because many people can relate to them.” Titilola “Users can share their personal experiences, for example in one-on-one or group discussions and chats on social media. In our campaigns towards communities and events for policy makers we let users tell their story. This proves to be a very effective way of awareness raising.”

Valérie “They can not only make female condoms known in their community, but also promote the outlets where they are available. As well as this, their personal experiences help us to create communication messages towards the different target groups.” ARE THERE BARRIERS FOR USERS IN PROMOTING FEMALE CONDOMS? Estrella “Yes. Culturally it is not very acceptable to talk openly about sex and sexual behaviour, especially for women. In some communities they may suffer stigmatization for that.” Titilola “Communities have a culture of silence on sexual and reproductive health issues, including the use of contraceptives.”

For example, men are considered to be the boss at home, so they make the decisions on sex and reproduction.” HOW CAN THESE BARRIERS BE BROKEN DOWN? Valérie “In our education activities in communities we try to raise discussion regarding traditional standards. For example, we ask women if they agree that they should not be allowed to protect them-

Titilola “We need to empower women and young people to enable them to make informed health choices and speak out on issues they believe in. Advocacy to key stakeholders at all levels, including men, is also essential. To counter traditional views, it is necessary to invest in research for adequate evidence that supports the use of female condoms.”

It was a female condom! Listeners stood in rapt attention while my mind wandered to the female condom I received at the hospital during my daughter’s vaccination. The nurse had not explained how to use it. This was an opportunity to learn how to use the female condom. I eagerly joined the crowd. The spectators chuckled as the lady used an object shaped like a woman’s private part to demonstrate how to use a female condom. It seemed simple. She explained that the female condom is good for family planning and that it can be very pleasant to use as well. My husband and I had talked about family planning after our daughter’s birth. I had been using daily pills but they caused some rather uncomfortable internal body heat. That evening, with the female condom from the hospital in my hand, I told my husband about my experience at the junction. He kindly supported trying the female condom. Next morning my concerned husband asked me how I had experienced having sex. Our first time with a female condom

was not painful. It only felt different. We continued to use female condoms and it has been enjoyable ever since! My husband and I value anything that will help us live a better life. The female condom is inexpensive and enjoyable to use. Most of all, after two years of using it, I did not get pregnant again. I stopped using the pills and the internal heat stopped. I was inspired to tell my neighbour who finally started using female condoms as well. Now she is grateful to me.

JUDIT BARNIOL With their knowledge on making health commodities accessible in developing countries, i+solutions completes UAFC’s unique puzzle. Its support to manufacturers and social marketing organizations stimulates the market for female condoms. Project Manager Judit Barniol looks back at 2014 with pride: “The snowball is rolling.” WHAT DO YOU DO WITHIN THE UAFC PROGRAMME? “We aim to bring good quality female condoms to countries that otherwise would not have access to them. We do this in the cheapest and quickest way so the products are available for as many people as possible. We support social marketing organizations in Nigeria, Cameroon and Mozambique by providing them with

female condoms and demonstration models and offering them training on how to distribute the commodities. We also offer support to manufacturers in getting their product on the market.” 23

WHY IS IT IMPORTANT TO SUPPORT MANUFACTURERS? “Female condoms need to be prequalified by WHO/UNFPA before they can be eligible for bulk procurement by international donors. By offering support to manufacturers we facilitate their application for prequalification. Also because of our support more new condom developers are interested in joining the market. Eventually this will contribute to more variety of products. With more variety we will meet the taste of more women. In this way the demand for female condoms will increase. More demand should make manufacturers willing to reduce their prices, which makes the female condom even more popular. We aim for a snowball effect.”

HOW DID YOU MAKE THE SNOWBALL ROLL IN 2014? “A major milestone was the completion of the functionality study for two female condom models, the new Cupid 2 and the Velvet Condom. In this study, carried out by MatCH Research in South Africa, all the technical parts of the models were tested, for example the fragility of the materials. We were happy to see that the models performed very well. This will increase their chances of getting prequalification from WHO/UNFPA. We also developed an e-course on Good Manufacturing Practices that teaches manufacturers how to make sure their chain of production meets quality standards. This will prepare them for the inspections by UNFPA during the prequalification process.”


WHAT WERE YOUR OTHER HIGHLIGHTS OF THE PAST YEAR? “We were very excited to launch the online Female Condom Market Intelligence portal. With this portal we created a transparent platform to provide manufacturers, donors and ministries of health with up-to-date information on all quality-assured female condom designs that are currently available on the market or are in the pipeline. Another big step was an assessment we performed of the supply chain management in Mozambique. It showed that the Ministry of Health and social marketing organizations did not have proper system to register the distribution of female condoms or calculate the needs for a particular region. This caused problems like stock outs in some areas and abundant supplies in others. Moreover, donors like UNFPA could not be approached with exact needs. By training the organizations and redesigning the registration we improved the system significantly. This should have an immediate impact on the amount of female condoms procured by donors in Mozambique.”

LOOKING BACK AT 2014, WHAT MAKES YOU PROUD? “Maybe it is not so visible, but I think we have achieved a lot. I hope at the end of the UAFC programme our work is reflected by having one or two more female condoms prequalified and leaving our in-country partners with the capacity and knowledge to distribute female condoms and other family planning commodities. With our achievements in 2014 we are on the right track to have a sustainable impact.”

LOOKING FORWARD TO 2015, WHAT EXCITES YOU? “Now that Cupid 1 has received prequalification, there are two models on the market now, together with the FC2. I am very curious to see how the market will react to this. The manufacturers need to develop new strategies to market their product, like targeting other segments of the population. Now we will see if more variety on the market indeed leads to more demand. If it all works out well, this can be an incentive for new manufacturers to come up with new models. Nigeria FORMER MINISTER OF STATE FOR HEALTH RECEIVES A POLICY BRIEF ON FEMALE CONDOMS.

The advancement of female condoms in Nigeria is in the hands of SFH, ARFH and EVA. As an expert in marketing, SFH is successful in selling, distributing and promoting the product. Meanwhile, ARFH and EVA are advocating towards the Ministry of Health, religious leaders and other influential parties, including through female condom ambassadors (‘advocacy champions’) and youth advocates. IN MEMORIAM

Damaris Mounlom 1940-2015

Lucie van Mens 1955-2014 On 17 July 2014, the world was shocked by the disaster involving flight MH17, killing 296 people. UAFC and the entire female condom community was shocked to learn that Lucie van Mens was among the victims. She was travelling together with several other participants to the International AIDS Conference in Melbourne. Lucie was UAFC Coordinator from 2008 to 2011. During this period she made a great contribution to the programme and to female condoms in general. After she left UAFC in 2011 she continued her mission to increase access to female condoms with the Female Health Company. 32

On 4 February 2015, the sad news reached us of the death of Damaris Mounlom, Coordinator of Femmes Santé et Développement (FESADE), after a period of illness. As the head of this UAFC advocacy partner in Cameroon, Damaris has been leading advocacy efforts on female condoms and reproductive health in general in Cameroon. She has been an example to many women in Cameroon and beyond and she has been instrumental in putting women’s empowerment on the national agenda. UAFC and partners will forever remember Lucie and Damaris. May their passion, energy and commitment continue to motivate us to keep pushing the case of female condoms forward and to build on the work that these two exceptional women have started.
