President’s Message Ubuntu

In 2007 the Boston Celtics were the worst team in the National Basketball Association. The following year they beat the LA Lakers (YAH!!!!) to win the NBA title; the biggest one year turnaround in the history of the game. Some say adding Kevin Garnett and Ray Allen to the team made all the difference. You’ll get no argument from me; great players make great teams. But great players alone don’t win championships. Something more was needed in Beantown and their coach Doc Rivers knew who it was. He needed the savvy of someone who never played basketball, but knew something about teamwork. Wisely, he turned to the insights of the great South African Freedom fighter and President, Nelson Mandela, and he found “Ubuntu.” Naturally, a word so strange as Ubuntu, and traveling so far from where it first was used, won’t fit neatly in your pocket dictionary. You must first savor the sound of the three syllables; chant them over and over, until the power of the word can be appreciated. OO BOON TOO OO BOON TOO OO BOON TOO “I am who I am because of you. I am who I am because of you. I am who I am because of you.” Powerful word and powerful concept, Ubuntu emphasizes that the success of any individual

depends on the collective action of the group. The stronger the sense of shared goals, the more dramatic the results. Sports teams are a natural fit for this concept. There are not that many players on the team, they usually are all working together and they only compete for a finite period of time. The players’ willingness

The stronger the sense of shared goals, the more dramatic the results. to sacrifice their individual goals for those of the team can be sustained because this shared goal is reachable in less than a half a year’s time. But something can happen along the way that reinforces this sacrificing of personal gain. As the team gets better, so does the individual. As the team gets better so does the enthusiasm of the fans. Fan enthusiasm reinforces team enthusiasm and so on. It doesn’t matter if it is a team or a dental society when groups of people form alliances; they need more than great leaders if collective success is the goal. Unselfish volunteering by the rank and file member is the core ingredient guaranteeing success. Volunteering fosters friendships. Volunteering solidifies social ties that enable groups to extend their power and

influence. Ubuntu speaks to that collective action that individuals take. It reminds us that the ties that bind the group together are sometimes greater than the sum of their parts. Sociologists have a word for these ties that bind communities together: it’s called “social capital.” Social capital is a real resource that groups can accumulate and invest. Because this capital is generated by the trust and resource sharing that has taken place over time, there is a certain reciprocity of benefits that flows back and forth from the giver and the receiver. Our dental society offers a wealth of opportunity to make a difference. The most notable opportunity is our Smiles for Kids program. This year our volunteers provided over $1 million in dental services for kids who would otherwise not receive treatment. This month’s issue of the Nugget highlights the many ways our members volunteer. Their unselfish giving of both time and money has made a dramatic impact on our civic presence. As President of SDDS this year, I have had the opportunity to meet with several of our City and County elected leaders. I can assure you that the reputation of our Society is greatly enhanced by the wealth of social capital our volunteers have procured for us. Their good deeds make our community a better place and make our Society more successful. Speaking as your President, I humbly admit that I am who I am because of you. 

From the Editor’s desk Voluntarism Webster defines voluntarism as “the principle or system of doing by or relying on voluntary action or volunteers;� so, as often happens with Mr. Webster, one is left to contemplate the meaning of “volunteer.� Actually, the word may be used as a noun, as in “one who enters into... a service of his own free will;� or as a verb, as in “to offer one’s services.� Since your Editor is a veteran, and has heard endless variations on the military dictum, “Keep your eyes open, your mouth shut, and NEVER volunteer (at least that is the most printable version!).� We shall eschew any further examination of the verb form and instead concentrate our focus of this issue on the noun form and the wonderful members and othersSacramento-7.5x4.875.pdf who offer their valuable 1time3/9/10 and services so freely to the community.

Any discussion of voluntarism and our Society would of course have to mention the magnificent efforts of the “Smiles for Kidsâ€? volunteers, along with the amazing amounts of dental care provided on a pro bono basis. However, many more individuals associated with SDDS offer their services in additional ways and often in far distant locations. In this issue, we will highlight the stories of our projects, our Foundation and our volunteers. In addition, lest we think there is no reward for voluntarism other than personal satisfaction, we have an article from one of our vendor members, John Urrutia, CPA, on possible tax benefits for the individual who volunteers or donates. I am so proud of this month’s issue of the Nugget! What a huge testament and tribute to our Dental Society, our members, our projects (our/my tendency toward being a bit A.D.D.!), our members’ continual giving nature and, most of all, our interest in our community and the people within it. For sure… UBUNTU. Dr. Jones — what a wonderful article you wrote this month!

“I am who I am because of you.” As I begin my tenth year with the Dental Society, I can’t help but look at how far we have come in the last nine years. As you read the 2009 Annual Report included in this issue of the Nugget, I look back to April Fool’s Day 2001, (my first day at SDDS) and see the growth we have all made together. Our leadership, our staff and our members have really been the key element of our success at SDDS. Some of the wonderful successes I share with you: • Our membership is up from 62% of the market share to 82% • We are approaching 1600 members • Our SDDS budget is nearly $1 million (a 51% increase since 2001) • We have operating and building reserves • Our CE, OSHA, Licensure Renewal, Member Forums , MidWinter and General meeting programs’ attendance is growing every year • Our member benefits continue to increase exponentially (job bank, online link, website, vendor members) • Our members know we are here; we get the information they need • We do our best to help our members be successful • Our community partnerships are successful and model-worthy • Our Smiles for Kids program has grown exponentially

Working on a Saturday

I hate working Saturdays. It’s not the work really. It’s that the sun is shining and other people can play, go to a movie or just relax. I am, frankly, jealous. However, for the past six years there is one Saturday when I jump out of bed early, ready to go. That Saturday is Smiles for Kids Day. Smiles for Kids is a national event which allows local dentists to treat children in their community for free. In Sacramento, these children are then “adopted” by local dentists to finish their needed work – again, for free. In current economic times, when people are struggling and money is tight, the Sacramento District Dental Society is not afraid to ask us, its members, to “step up” and give back to the communities that support us every year. I am so proud of my office for volunteering to participate in this event annually. This year we supplied 20 team members, including three doctors, to assist in the event. We contribute tens of thousands of dollars in free dentistry every year, and generally adopt about four children for the completion of their work. This year, though it was our smallest SFK event to date, we saw eighteen children and adopted four for completion. We generated over $18,000 in free services and will contribute another $15,000+ with our adoptions. Not bad for a slow year. I have always enjoyed this event, though the pace is grueling and the work is hard, but this year was especially meaningful. One of our “adoptees” is a 15-year old boy. Teenagers often get passed over for adoption because they are not “cute” and their work is often more complicated. Our guy did not expect to be adopted — he did not even ask. We took him to make a point. Each child is intrinsically valuable. Not because they are tiny and cute, but because they are human and they need help. Our skills can help this young man feel

Perhaps someday he will pay the favor forward and be a good steward of the lesson we’re trying to teach — that all people deserve help because we are all in this together. One of the other children we adopted was a young girl with cancer. Her family has been completely “wiped out” by the healthcare costs associated with this disease. (Her brother, whom we also adopted, has an enormous amount of necessary work, too. It has gone

Smiles for Kids is such a meaningful and powerful way to carry forward our gifts to our fellow community members. untreated because the family’s entire focus is on “holding together” and saving their daughter.) We will need to treat this little one as part of an oncology team. Several meetings, letters and treatment modalities will need to be employed to safely treat her while she battles cancer. A lot of time, expense, and effort will be called upon from my team to make a difference. It won’t be a “one-day” thing. When I discussed this situation with my team, there was not a moment of hesitation. She was ours and of course we would help make her well. That is the spirit of Smiles for Kids. Year-round. Not just once-a-year. And finally, the last of our adoptees. This little girl broke our hearts. Her mother is a past drug addict who wrote to tell us about the beautiful little person whom she felt she had failed. Our princess had severe early childhood caries. Her front teeth were so

broken down that some of the kids at school called her pumpkin face. Even though the depth and prevalence of caries must cause her daily pain, and the teasing at school has got to be very destructive to her self-esteem, she presented with a sunshine attitude and a bright, cheery smile! Her mother says she awakens every day to her daughter singing and dancing. Mother also reports that our little miss will tell her she can’t eat certain foods because of her “sugar bugs” and bravely reminds mother to brush her teeth every day, even though it stings. We completed four anterior pulpectomies and crowns with resin facings to change her “pumpkin face” into a princess smile worthy of her amazing soul. What was really extraordinary, however, was not her smile transformation. She was just as her mother had said. Throughout the entire experience this little princess was a warrior. She was brave and cheerful and uncomplaining. The best moment of my day was when she jumped into my lap and gave me a giant “thank you” hug. What a rush!! We here at Magic Smiles cannot wait to finish up her work and make her as healthy on the inside as we possibly can. Smiles for Kids is such a meaningful and powerful way to carry forward our gifts to our fellow community members. Although it means a hard work day without pay on a weekend, my team is always willing to commit — year after year. I am so grateful to be part of this event and to bring comfort and health to the children who are growing up with my children every day. Charity should always start at home, locally. Setting my Alarm for a 4:30am Wake-up OK… I must admit, not too many events will find me setting my alarm clock for a 4:30am wake up on a Saturday morning... but “Smiles for Kids Day” did! Since I am “6-months new” to the dental industry after seven years in the pharmaceutical selling arena, I have found myself wanting to get involved in groups and events in the dental world, which is helping me to familiarize myself with those in the dental field and in my assigned geographic territory. Living in the East Bay Area and being assigned the Northern California territory, I’m able to be only a car ride away from many worthwhile events in the dental industry that are happening all around us.

I was placed at a very busy location, with several dentists, assistants, hygienists, all sharing space and equipment. The teamwork and cooperation was tremendous! All these very talented and highly professional individuals, in one place, with their egos left at the door was very impressive!! Having these mothers thank me for allowing their child to be treated, and then seeing kids boast that they did not cry when they had a tooth pulled, pulled at my heart strings!! I saw more than one parent in tears, because they were so thankful to have their child receiving such professional work by “top notch” dentists and staff. I felt honored to be a participant in such an event.

I’m so glad I had the opportunity to participate in “Smiles for Kids” Day! I volunteered to participate and my job was to “check kids in” and assure their parents had the correct paper work so their child could be treated. This meant being in Sacramento by 7:00am, which resulted in a “early morning” start for my Saturday! I had the chance to see many talented professionals come together with one goal, to treat children who otherwise may not have the opportunity to see a dentist.

The company I work for (SS White Burs), has been a successfully functioning rotary instrument company for over 160 years. How many companies, in today’s economic trend, can state that? Our CEO, Tom Gallop, is very passionate about the high quality products we can offer to dentists, but even more passionate about how we can help dentists solve “bigger problems” in their practice. He is a talented forward thinker, and has led us to be more than a rotary instrument company.

I became involved with Smiles for Kids because Cathy Levering met my boss, the owner of SS White, at a meeting in Chicago.

I felt honored to be a participant in such an event. He evidently told her, “If there is anything you need for your Smiles for Kids Day, call me!” She did, and he called me. It was exciting to see our fissurotomy burs being “put to use” at Smiles for Kids. Smiles for Kids Day made me realize how fortunate I am to have a job in a industry that is filled with talented people willing to “give back” to those in need.Will I be back next year to volunteer for Smiles for Kids Day?? Crowns for Kids™

To the SFK Program We were invited to attend the Smiles for Kids Day in January. We started out at the SDDS Executive offices at 7:30am and began our day visiting some of the dental offices in the Sacramento area that participate in this wonderful day of charitable giving. Some of the most needy and worthy children in this area receive free dental care once a year, with funds provided by the Smiles For Kids project of the Foundation. The kids had been chosen through screening at the local schools and the age range was anywhere from age two through 18. The first child that we saw was a little boy of two; some of his teeth had rotted away through lack of poor dental care and unfortunately genetically bad teeth. He had to have two of his baby teeth extracted and he

also needed fillings on two of the other teeth. It was heartbreaking to see this poor little boy, obviously in pain and totally hysterical, but after 30 minutes he was finished, had calmed down and his teeth were fixed! There were many other older kids having various work done on their teeth. Some managed to hold it together better than others, but what struck us the most was the care that the doctors and staff at all the dental offices we visited gave to these children Each and every one of them were willing and happy to give up their Saturday to come and help these families who were much less fortunate than themselves. On a personal level, to say that it was one of the most fulfilling days that we have ever been involved in is an understatement. They say

that in life you have to “give back to receive,” and this especially rang true for all who participate in this very special program. The families that were attended to have no means to pay for dental treatment and without this fantastic day of giving back to the community, these children would never have the opportunity of having their teeth fixed for free. Paying it Forward was a book written by Catherine Ryan Hyde. The idea behind the book was that anyone can do a favor for another person without any expectation of being paid back, but how does that work when it comes to taxes?

That’s not to say that your efforts are not needed or appreciated by those who benefit from your giving.

Your generosity may entitle you to some tax breaks.

Uncle Leo is that deductible? What about the guy by the freeway with their cardboard sign or the musician setup downtown who you throw a few bucks to for playing something you like? Sadly, none of these situations will get you a tax deduction. There are basically three areas where we are asked about the deductibility regarding charitable actions, Cash Donations, NonCash Donations and Volunteering. Cash Donations No deduction is allowed unless the taxpayer has either bank records (cancelled check or account statement) or written acknowledgment for the charity (documenting the contributions amount and date). Charitable donations of $250 or more in any one day are required to have the written acknowledgement. Non-Cash Donations Clothes, household goods, furniture, computers, etc. are welcomed items by a charity. However, it is always a good idea to take pictures of the items you are donating. Volunteering

If you are a volunteer worker for a charity, you should be aware that your generosity may entitle you to some tax breaks. Although no tax deduction is allowed for the value of services you perform for a charitable organization, some deductions are permitted for out-ofpocket costs you incur while performing the services. This includes items such as: • Away-from-home travel expenses while performing services for a charity (out-ofpocket round-trip travel cost, taxi fares and other costs of transportation between the airport or station and hotel, plus lodging and meals). However, these expenses aren’t deductible if there’s a significant element of personal pleasure associated with the travel, or if your services for a charity involve lobbying activities. • The cost of entertaining others on behalf of a charity, such as wining and dining a potential large contributor (but the cost of your own entertainment or meal is not deductible). • If you use your car while performing services for a charitable organization you may deduct your actual unreimbursed expenses directly attributable to the services, such as gas and oil costs. Alternatively, you may deduct a flat 14¢ per mile for charitable use of your car (for expenses incurred before Jan. 1, 2009, a higher amount if you used your car to assist in relief efforts related to the 2008 Midwestern disaster). In either event, you may also deduct parking fees and tolls.

• You can deduct the cost of a uniform you wear when you do volunteer work for the charity, as long as the uniform has no general utility (e.g. a volunteer ambulance worker’s jumpsuit). You can also deduct the cost of cleaning the uniform. Some people also try to do work for a third party and just have their income given directly to a charity. In this case the income is taxable to the donor and they also get the charitable donation, so a wash if they itemize. While all organizations welcome and need a straight monetary contributions, there are additional ways to donate. It could be lending your time or sending an employee to an organization for a day to help out at an event. It could be a simple phone call to the Director of the organization thanking them for their efforts. It could also be encouraging your employees to set up a team to participate in an event that one of your charitable causes is holding. While the payback on these efforts is typically the good feeling you get and the assistance that is welcomed by the organization, there could also be some side benefits (kind of an icing on the cake for you). In closing, we had our two older daughters volunteer their time for a variety of causes as they worked their way through junior high and high school. The additional benefit came when we were filling out their college applications and we were able to include the letters of recommendations from the organizations to whom they had given their time. Smiles for Kids 2010 January 30, 2010 Testimonials: When I turn on the TV to hear news, often I am saddened to hear what’s going on around us. You really want to go to Haiti or Chile and help those needy people but your family and responsibilities would sometimes hold you from taking on that task. Smiles for Kids gave me this perfect opportunity to help and to give back to my community. I have been volunteering for Smiles for Kids and GKAS for the last four years. However, 2010 was the first time I hosted Smiles for Kids site at my office. The SDDS staff did an outstanding job making us prepared for the day. They literally laid down a step by step plan from what to expect to what to do. The best thing about the SFK program I liked was that I didn’t have to personally go to schools to screen kids. I also have to thank my staff who volunteered to make this project a grand success. They were really excited to help the kids. Getting the CE units at the end was like icing on the cake! We were there a total of about three hours, and it will take another two appointments to complete all of the dental work that needs to be done. He had three fillings done today and had a cleaning, and the next appointments are to take care of the crown and root canal issues. Jourdan was incredibly grateful and appreciative that everyone had gone to such great lengths to make sure that he was taken care of. The entire way home he kept looking at his teeth in the mirror! The Smiles for Kids program is an incredible asset to our community. Through the generosity of the Sacramento District Dental Society, as well as dentists throughout our region, we are able to link children to critically needed services. It is difficult, if not impossible, for a child with significant dental health needs to meet the demands of the school day; Smiles for Kids removes that barrier for children, allowing them to focus on the business of learning.

This was the first time our office was a Smiles for Kids site. Although we were busier than we ever could have imagined, at the end of the day we were all very satisfied that we were able to provide care for so many children. It was also an eye-opener for us that there were so many underprivileged kids that had such severe oral problems. It really goes to show that we really need to step up on educating our community about oral health.

I have organized the Smiles for Kids program in our office for several years now. Each year the program grows in size but never loses that personal touch. For all of us here at Kids Care Dental it is a way of giving back to our community and the children in need. There are no words to explain the smile on a child’s face when they have finish treatment and are out of pain. The same feeling comes true when the parent is so thankful their child is taken care of in such a fun, loving environment. The feeling is good for children of all ages. My greatest experience was when a teenager came in with a black front tooth. She was so self conscious she wouldn’t smile. Once the treatment was complete she came beaming out of the operatory with the biggest, brightest smile I had ever seen. You could see the self confidence restored in her because she had a great looking smile. Thank you to all the offices and all the volunteers for a job well done!

Smiles for Kids is typically scheduled on a Saturday, but the care these children receive extends far beyond a one-day event. The children are“adopted” by a number of offices throughout the Sacramento area and their treatment is completed from start to finish. What really impressed me this year was the large number of volunteers (including dentists, hygienists, assistants and administrative staff) who donated their time. The parents are extremely grateful and continue to send personal thank you cards to individual participants. It’s a rewarding experience and I encourage everyone to get involved in 2011! Patients Treated in Smiles for BIG Kids (SFBK)… A recovering meth addict with a “bombed out”mouth is given a second chance at a beautiful smile. With the help of a volunteer oral surgeon and general dentist, through our Smiles for BIG Kids program, she can now smile with confidence and has the opportunity to get her life back in order. An emancipated youth who has been in foster care her whole life wakes up one morning with lots of pain and swelling and suspects that it is one of her wisdom teeth. She is on her own, in the process of getting her Masters and in no way able to afford an extraction of a wisdom tooth. In a matter of two hours, Smiles for BIG Kids was able to get her into a general dentist to make sure there was no infection (there wasn’t) and within a week the tooth was surgically removed. An elderly woman falls in her apartment and is not discovered for seven days. She broke her front teeth during her fall, which caused her a lot of pain, not to mention a broken smile. She is on a fixed social security income and in need of a removable partial or bridge. If it weren’t for Smiles for BIG Kids she would likely still be suffering with broken teeth. An 80-year old veteran from the Korean War has a tooth with an open cavity causing him pain. He is on a severely set income and has no insurance to pay for the crown he needs. He is now pain free, due to a partnership between SFBK and a volunteer general dentist.

A homeless mother of three children moves into a women’s shelter to begin the process of improving her life and the lives of her children. Before she became homeless she owned and operated her own pallet recycling company. During this time she suffered a couple of hard blows to the face, breaking and knocking out most of the teeth. It is her hope that through Smiles for BIG Kids she will have her front teeth replaced so she can smile again showing all her teeth.

By Wai M. Chan, DDS Oral health is part of total health. Having good oral health is critical to the overall health and well-being of an individual. One of the issues that has been repeatedly discussed at health care policy meetings and conferences is the issue of Access to Care. During those discussions, it becomes evident that it is not just Access to Care but also the Barriers to Access. The healthcare professionals who are in the trenches will agree that addressing the barriers will help to improve the access. In a medically and dentally underserved area, common barriers are: ignorance of the need and importance of oral health, finances, lack of transportation, languages, difficulty navigating the health care system, single parent with young children or elder care responsibility, citizenship status, and work schedules of the parents/care givers. Having a school-based health center helps in breaking down some of the barriers. It does not solve all the problems, but it is a small step in a long journey to address the barriers. There are numerous studies recommending school-based health care for the students. The most recent one is in December 2009, the California Assembly white paper titled “The critical connection between student health and academic achievement.” It stated: “Children with poor oral health and poor general health are 2.5 times more likely to report poorer school performance than those of good health.” It also stated that, “in California, the need for oral health is the most prevalent unmet health care need among children and adolescents.” Tooth decay is the most common preventable chronic childhood disease and tooth decay affects nearly 6 in 10 children in US. One of the recommendations is Establishing Schoolbased health center. UCLA Health Policy Research Brief Nov 2009 “Unaffordable Dental Care is linked 20 | The Nugget

to frequent school absence” stated that 73% of students who missed more than two days of school because of dental problem said they could not afford dental care. It suggested school-based dental care as one of the solutions. On December 1, 2009, at Assembly Select Committee Briefing, California Education Supports Project has a presentation by WestEd titled “Linking Health and Education for Student Success.” It stated that the health of California students has a direct impact on student dropout rates, attendance, learning engagement, academic performance and school district revenues. Sacramento Health Care Improvement Project reported in July 2008 that “Access to Dental Care is a Critical Need.” For students, when they are sick, when they are in pain or when they are missing school, they will not be learning. They cannot focus if they are sick and or in pain. If we can keep them in school and keep them healthy, provide the care and guidance, and be their role models, we can help to make them productive citizens and future leaders. It is a win-win situation for the school districts and the students they serve. The school will not lose funding because of students missing school. The students have their health care need taken care of in schools. When they stay healthy and are willing to learn, they can learn. The parents or caregivers do not have to take time off from work to bring their children to the doctors/dentists offices. A schoolbased health care center also strengthens the connection between the school and the parents/caregivers and the community. In Hiram Johnson High School, Family Nurse Practitioner Pauline Tracey has seen her share of students with toothaches and missing schools. She dreamed of having a school-based health center with a dental clinic. She is a person with a big heart and she does not take no for an answer. After almost

nine years of working on having a schoolbased health center, her dream has come true. The center was opened in December 2009. Hiram Johnson High School has the first Northern California School-based Health Center with a dental clinic. The one-chair dental clinic is open every morning and is staffed by volunteer dentists. It has a parttime dental assistant, intra-oral camera and digital radiography. If you are interested to help one morning a month, please contact Wai Chan at wmchandds@frontiernet.net. Other ways to get involved… Non-Profit Pediatric Dental Clinics Coming Soon to Sacramento

The Effort is working in partnership with the Children’s Dental Task Force and the First 5 Sacramento to create two pediatric dental clinics for underserved children. Both sites are in the final planning stages and construction is scheduled to begin by the end of April. Dr. Cassie Krupansky will be leading the clinics as The Effort’s Dental Director. Dr. Krupansky is a pediatric dentist who completed her residency at UC San Francisco. She has extensive experience in treating children with special needs, including those who are medically compromised or mentally disabled, and very young children with dental phobia. The clinic will focus on providing dental care to patients from a very young age and empower parents to care for their children’s teeth. The goal is to change oral health in Sacramento and help underserved children maintain healthy smiles.

These clinics will serve low-income families including Medi-Cal, Healthy Families and Healthy Kids members, and will also provide care for the uninsured. Private insurance will not be accepted. Uninsured patients below 200% of the Federal Poverty Level will pay on a sliding fee scale based on income status, and will be assisted in applying for public programs. The North Highlands Dental Clinic will be located at 6015 Watt Avenue at The Effort’s Birth & Beyond site. There will also be a medical clinic on-site with a Head Start program and county services next door. The Oak Park Dental Clinic will be located at 3415 Martin Luther King Jr. Blvd. as part of the Oak Park multi-service complex. This dental clinic will also be co-located with a medical clinic offering WIC services in the same building. The dental clinics will provide education, prevention and general dental care including

exams, x-rays, emergency care, fillings, extractions, cleanings, sealants and fluoride. As a non-profit, The Effort is seeking dental donations and is very thankful for the tremendous amount of donations already received from Sacramento District Dental Society members. Additional donations are still needed — especially handpieces, composite guns, instruments, dental materials and a digital pano. Please note that the clinics will be all digital, so donations of regular pano machines can not be accepted. Thank you for your help in spreading the word among your dental colleagues and helping to outfit dental clinics for the underserved. When it comes to providing assistance for an increasingly eclectic homeless population this organization really goes the extra mile. As the economy has dipped this organization has

seen an increase in the number of homeless due to job loss and ultimate loss of their homes. These individuals are not the chronic homeless that you have seen in San Francisco doorways and park benches. Each afternoon around 3:30pm you can find people lined up outside the Gathering Inn, where they are screened, take showers and get clean clothing before heading to one of the more than 60 area churches who volunteer to feed and house them for the night. During the course of a year the Gathering Inn will see more than 600 different individuals enter their program. Our goal would be to add a dental component to their existing healthcare program by involving volunteer dentists, dental spouses, and dental staff with screening and referral for appropriate dental care. We would expect to expand this program eventually to serve additional groups of individuals, including children, in the Placer County area whose dental access is so difficult or non-existent. In this initial stage, however, we will begin with one or two dentists and dental volunteers one morning each week at the Gathering Inn to screen potential patients and refer them to a list of volunteer dentists who agree to accept an occasional patient. We will need dentists willing to do screenings, as well as dentists who agree to treat an occasional patient in their practice

setting. Dr. Gabrielle Rasi has agreed to screen one morning a month, and we will need seven or eight more dentists for the screening team. However, any amount of volunteer time will be greatly appreciated. You will be given an opportunity to accept or reject any request for assistance. The Gathering Inn has the resources to make sure the patient is presentable when they deliver them to your office for care. Cover the Kids (CTK), Sacramento Children’s Health Initiative invites you to participate in ensuring all children and teens have access to affordable, comprehensive, and continuous health insurance. CTK is a broad-based collaborative partnership that is comprised of key representatives from local hospitals, county health service agencies, community clinics, faith-based community organizations, education, funding institutions, and businesses. CTK’s Board of Directors is co-chaired by Supervisor Roger Dickinson, Sacramento County Board of Supervisor and Mayor Kevin Johnson, City of Sacramento. Funding is provided by Blue Shield of California Foundation, California HealthCare Foundation, City of Sacramento, County of Sacramento, First 5 California, First 5 Sacramento, Kaiser Permanente, Mercy, Sierra Health Foundation, Sutter Medical Center, Sacramento, The California Endowment, The David and Lucile Packard Foundation, UC Davis Health System, and United Way California Capital Region. Of the 16,000 Sacramento County children ages 0–18 who are currently uninsured, it is estimated that 2/3 (10,720) of these uninsured children qualify for existing public health programs, but are not enrolled and 1/3 (5,280) do not qualify due to other factors such as income or citizenship status.

CTK has developed a comprehensive plan to provide medical, dental, and vision coverage to the nearly 26,000 uninsured children in Sacramento County. In September 2006, CTK launched the new Healthy Kids insurance product for children who are not eligible for any existing programs such as Medi-Cal and Healthy Families and whose incomes are between 251 - 300% of the federal poverty level (Family of four $5,500 per month). Healthy Kids is currently enrolling children. Undocumented children within the income limits for this program are eligible to receive services. The providers participating in Healthy Kids are Health Net, Delta Dental, Vision Service Plan and Kaiser Permanente’s Child Health Plan. Additionally, CTK staff are focused on reaching families throughout Sacramento County with children who are eligible for subsidized programs including: Medi-Cal, Healthy Families, Kaiser Permanente’s Child Health Plan, and Access for Infants and Mothers (AIM) Program, and assisting them with the enrollment process, as well as providing case management services to help them maintain coverage. From Your 2009 SDDS President… Thanks to careful planning, Cathy’s guidance and the hard work of the staff and your army of volunteer leaders, we had a great 2009 in terms of membership, financial strength as well as member satisfaction. It appears that, in the face of financial constriction, our members focused on shoring up their basic skill sets to meet their patients’ demands — locally! Our CE course, member forums, licensure renewal courses, monthly meetings and MidWinter Convention received great praise and were very well attended. Our attendance statistics and program success is higher than it has been in years. Thanks to all members who support our programs through their attendance! Last year, we had several companies approach us with the offer of shoring up our declining membership (not the case) in the face of the financial crisis. Much to their chagrin, we did not need anyone’s bail-out assistance. Our Membership Committee and staff rose to the occasion and saw our membership increase in 2009. At year end, we pride ourselves with 82% of the market share of member dentists. Thanks to our members for continuing to renew their membership and for continuing to see the value of membership in organized dentistry! I am proud to see that our Foundation is ‘taking off’, so to speak. Our SDDF Crowns for Kids program has taken on a life of its own. What a gift. Our Smiles for Big Kids has germinated and is finding its own spot in the forest, along with the already successful Smiles for Kids program. Despite the financial situation in 2009, we still found ways to be philanthropic. Thank you as well! Our partnership with other organizations such as First 5 Sacramento County has grown even stronger as we undertake projects such as fluoridation of our water, improving access to care to less fortunate children, and many other areas. These programs promise major leaps forward in oral health of the underserved and general population.

You are a dentist. You’ve been to school, taken your Boards and settled into practice. End of story? Not quite. Employee evaluations, hiring and firing, labor laws and personnel files are an important part of being an employer. Are you up on the changes that happen nearly EVERY January 1st? In this monthly column, we will offer information pertinent to you, the dentist as the employer.

From Taiba Solaiman (Risk Management Analyst, TDIC) Dentists have an ethical and legal responsibility to safeguard the confidentiality of patient information which includes name, date of birth, address, driver’s license number, Social Security number and credit card numbers, in addition to health and other personal information. The practice owner is responsible to ensure the information is accessible to those authorized and is restricted from generalized use. Keeping current on privacy requirements under state and federal laws will help dentists and the dental team protect their patients. Compliance can also help protect the dental practice from claims of improper disclosure or use of a patient’s information. Any business that stores personal information can be the victim of a data breach. Networks can be hacked, a laptop computer stolen or personal information inadvertently revealed in an e-mail or on a website. The thief can use a patient’s Social Security number and birth date to create a false identity and utilize it to commit fraud. According to a 2006 survey by the Federal Trade Commission (FTC), three percent of identity thefts involve a person’s healthcare information. A thief steals a patient’s dental identity to get free dental

services, prescriptions or to file false claims with insurance companies. Establish the following protocols to protect from data breach: • Develop a secure password system and train staff to understand why your security procedures are important. • Encrypt sensitive data, such as Social Security numbers. Encryption provides better protection than passwords alone. It is the most effective way to achieve data security. To read an encrypted file, one must have access to a secret key or password that enables you to decrypt it. These protocols must be applied to all forms of data storage that contain patient information including computer hard drives, lap tops, thumb drives, CDs and backup tapes.Physically destroy or electronically remove data from hard drives before disposing of computers. • All internet connections must have secure firewalls and anti spy/spam/virus programs. • Disable computer jacks, such as USB ports, to make it difficult to copy information onto portable media.

In the event that your office sustains a data breach, follow your state laws to find out how to respond. Reporting identity theft to local law enforcement will enable a more effective response. The Health Insurance Portability and Accountability Act (HIPAA) has provisions regarding data security breach notifications. The provisions were amended by the Health Information Technology for Economic and Clinical Health Act of 2009 (HITECH). As of September 23, 2009, patients must be notified any time their unsecured personal health information (PHI) may have been compromised through unauthorized acquisitions, access, use or disclosure. HITECH’s security breach notification requirements apply to covered entities. TDIC developed a sample letter for dentists to send to patients as notification of a data breach. This letter can be accessed at thedentists.com in the recordkeeping and forms section of the Risk Management link. The impact of losing electronic data is expensive, time consuming, and can be damaging to the operations and reputation of a dental practice. Often, office property insurance provides coverage for physical loss or damage to electronic data processing hardware, software and media. This does not usually cover costs associated with data being lost, stolen or damaged. TDIC offers Data Compromise Coverage to help dentists respond to loss or theft of patient information as an optional piece of the office property policy. Contact your TDIC broker to inquire about Data Compromise Coverage. Tooth Fairy dental education program

Calling all Toothfairies and Tommy Toothbrushers! As part of our community outreach, this summer the SDDS will be holding a Toothfairy Training Session. In carrying on the great work of our past Alliance members (and continuing the work of some of their teenaged children!), the purpose of this training will be to train high school and college students to help teach good oral health care to grade school and preschool children.

Requirements of a Toothfairy: • Must be 16 or older; • Must be willing to “dress up” (yes “Tommy Toothbrusher” can make an appearance as well!); • Must be available during the school day (community service time for high schools) • Must be able to STAY IN CHARACTER at all times while wearing the costume (even if you get pulled over for speeding!); Please call the SDDS Office if you, your staff members or your children (college age and in town?) are interested. And… special thanks to Samantha Adair, daughter of Dr. Terry and Deborah Adair, for her work this year as our Toothfairy. Thanks, Samantha!

The Dental Professional’s Community Service Obligation The Preamble to the ADA Principles of Ethics and Code of Conduct suggests that the qualities of compassion, kindness and charity are part of the ethical practice of dentistry and help to define the true professional. Section 3.A of the ADA Code provides that dentists “have an obligation to use their skills, knowledge and experience for the improvement of the dental health of the public and are encouraged to be leaders in their community.”

• Donating time or financial support to organizations that work to increase access to care, such as Sacramento District Foundation;

Many dentists find it rewarding and enjoyable to volunteer at one of the many clinics providing access to care to the underserved. What if you want to help, but time and other constraints do not permit you to address the access problem by volunteering outside of the office? There are many other ways to fulfill your ethical obligation as a dental professional. You might consider:

• Helping to educate your local school board about the damage caused by soft drinks in schools; or

Donating time or financial support to programs such as school dental health screenings or Give Kids a Smile (or Smiles for Kids!);

• Accepting patients enrolled in Medicaid; • Helping to educate your government representatives about the need to fund programs such as community health clinics or community water fluoridation;

• Contacting Sacramento District Dental Society and volunteering to serve on a committee. If you are already contributing your time, treasure and/or talent to make a positive impact on the world around you, please know that you are greatly appreciated. If you aren’t yet, but you are ready to begin making a difference, today is a great day to start. 

In memoriam Anatoly B. Ray, DDS Dr. Anatoly Ray passed away at the age of 81 on March 13, 2010. He was a first generation born in America to Russian immigrant parents and raised in the Russian community of Bryte. He obtained his DDS from University of California San Francisco in 1961, and set up his dental practice in Sacramento. He worked there for 40 years before retiring in 2000. Dr. Ray and his second wife, Barbara, were married in 1983. Together, they pursued their interests of traveling and gardening. Dr. Ray was a member of SDDS for 49 years.
