President's Message Helping Our Veterans

Just Another Way to Make a Difference As your 2019 President, I have the opportunity every single day to observe the many facets of our Dental Society. Whether I am tasked to write a quick article, to make a phone call on a member’s behalf, to help recruit someone to a leadership position, or to just “show up somewhere,”

partnership for good. As we look back over the previous months at the cases we have done, we have had successes and we have had challenges. But the cases we remember the most are those that truly improved people’s lives because we improved their smiles. Please read the stories in this issue of the Nugget; it makes me so very proud to be a part of this work that we do.

Let me tell you about my friend Tom, and how our office got to know him. Tom was referred to us in 2016 with the Smiles for Big Kids program through the Dental Society. His cheery disposition and grateful attitude immediately bonded him with us, and thus began a wonderful relationship.

I see how much our members do and how they make an impact on our community and the people in it. While the purpose and mission of the Dental Society is to “be the recognized source for serving its members and for enhancing the oral health of the community,” the mission of the Foundation is “to promote the oral and general health of the public by serving as the charitable arm of the SDDS and the dental community.” The two missions work together, and, in the end, the Foundation is the heart of the Dental Society, and the dentists who volunteer their time and talent indeed are the heart of our two organizations.

Tom had partial dentures from the time he was a freshman in college—over 50 years ago. With the loss of supporting teeth, over time his dental health had deteriorated. In 2016 we extracted all the remaining upper

teeth and diseased lower teeth and made him an upper denture and lower partial. He was so thankful; and as he smiled, he radiated joy from the depths of his soul. We lost touch with our friend when he went through a stroke, hospitalization and a long rehabilitation. In 2018 Tom came to see us again. He still had a larger-than-life smile, but he was hurting. Oral Surgeon Dr. Don Kim donated his time and expertise; and we were able to do a full lower denture for Tom. He again smiles with confidence and joy. You can make a difference. I invite you to donate a case a month; it will do so much for others—and for you and your whole team as well. 

At our office, we give back by taking on a donated case per month, and Smiles for Big Kids is our source of choice. The Foundation, through grants and donations received, reimburses the laboratory costs (for dentures mostly), and we donate the work. It is a great

As you read through this marvelous edition of the Nugget and the Annual Report of the Foundation, I hope you will recognize how our Dental Society and our Foundation work hand in hand. Our SDDS members (1800 strong) support our Foundation. Through members’ volunteer efforts, their pro-bono donation of services, and their truly generous donations, our Foundation is able to continue to do the work for our community that is so important. Our Foundation, founded in 1969, has shifted in focus over the last 15 years. Our strategic plans, both for SDDS and the Foundation, are totally separate but integrate nicely. Our Foundation truly is “The Heart of The Dental Society!” Whether it is serving our community’s children, or the elderly, or adults in need, we have now become the “GO TO” resource for the community. We navigate patients to existing care, benefits and services. We educate parents about 1st Tooth or 1st Birthday. We advocate for parents to use their existing benefits, and show them how to learn what benefits they have. That said, what was once the job of our Foundation to adopt everyone and treat them, has now changed drastically. Most children have benefits, either private insurance or Medi-Cal/Denti-Cal. Many adults do as well. They just need to be aware of the benefits available to them. The pro bono work that our dentists do are patients who may be within their own practices who have fallen on hard times, can’t afford the co-pays of their insurance, and the elderly with fixed income. Our Smiles for Kids Day has gone from 40 offices open 10 years ago, to only 8 this year (and 200 kids – 90% had Denti-Cal and needed navigation and an orthodontic consult). Our Adopt-A-Kid program is significantly less. And our Smiles for Big Kids is now focused on the elderly and the extremely needy. This decrease in patients and need for pro bono work is a GREAT thing! And it’s mostly because of our partners and the sharing of knowledge and resources in which SDDS is involved. 1. DTI (see Dr. Delmore’s article) - our partnership with Sacramento County provides us with funding to do Puppet Shows for dental health education, recruit more providers (and SDDS members too!) and educate the community. 2. School nurses – they continue to learn how to help navigate kids to existing benefits and current care options. 3. The GMC plans (Access, Health Net and Liberty) – these are our heroes with Denti-Cal issues, problems, lack of follow up and benefit education. With SDDS being the “GO TO,” our phone rings constantly with issues parents have with getting their kids care. The plans are a huge help in navigation (and we have their cell numbers!). 4. And, of course and most importantly, our member doctors who continue to say “YES” for those who truly fall through the cracks. Thank you! SDDS member doctors – you truly are the HEART of our Foundation! 

One Nurse’s Journey

with the Smiles for Kids Program A Letter Written to SDDS Thank you for giving me the opportunity to sing the praises of the Sacramento District Dental Foundation, its members, and programs. What began for me in 2002 as a tentative, hopeful, partnership, has blossomed over time into a relationship grounded in deep affection and respect for an organization that does incredible work to meet the needs of the underserved in our community.

Over time, we have documented a five percent reduction in the number of students in need of urgent care… When this story began 17 years ago, I was tasked with hosting a dental screening at the two schools I served, schools that were home to many students who didn’t have routine dental exams, whose families often had no knowledge of how to access dental resources they were eligible for, or who were reluctant to due to their experience with dental services in their countries of origin. In short, there were a lot of kids with some serious dental

issues! After trying unsuccessfully to secure a dentist to conduct a screening, I reached out to SDDS, and you connected me to a wonderful man, the late Dr. Gordon Harris. Going forward until his death, Gordon faithfully served my community, screening literally thousands of children. As important as those screenings were to identify immediate dental needs to be taken care of, the longer term gain of the relationship that his gentle, friendly manner made upon my students was perhaps even more important. His annual presence, coupled with his friendly disposition, eased the fears of many, many children about visiting the dentist. He took great delight, and my students did as well, in remembering each other from year to year. It was especially fun to hear sixth grade students boast to their classmates that Dr. Harris had “been their dentist” since kindergarten! With Gordon’s passing, I thought I had reached the end of an era of such dedicated service and generosity of spirit, but I was wrong. Cathy, you reached out to me and let me know that other Foundation members wanted to fill those very big shoes, and connected me to Drs. Dennis Peterson and Dan Harlan. Both Drs. Peterson and Harlan

have shown that same kind of compassion and dedication to addressing the needs of Sacramento’s children. Their work, like Dr. Harris’, has made a lasting change in the dental health of the community I serve. Over time, we have documented a five percent reduction in the number of students in need of urgent care, which I believe is due, in large part, to the services provided as a result of the annual screenings. Not only are students being screened, but their families are being aided in getting their kids into the dentist for further treatment when needed as well, often times with a helping hand from SDDS staff. The annual gift of a new toothbrush, toothpaste, and dental floss is significant, too, as many children report that is the only new toothbrush they receive each year. Cathy, I am indebted to you and your organization for continuing to support me and my community, through screenings, supplies, puppet shows that provide oral health education in a fun, entertaining manner, advocacy, and advice. For 17 years, you and your team have made it easy for me to address the dental health needs of my community. I am truly thankful for the gift that is the Sacramento District Dental Foundation. 

Unraveling the Smile Within By Donald Rollofson, DMD SDDS Member

We first met Yee Her at the 2014 Smiles for Kids ortho screening clinic assisted by Dr. Jennifer Drew. She had been referred to our office for the screening by a school nurse that over the years had asked us to help several special patients. Yee was 17 years old and a senior in High School. She was rather quiet and had a self-conscious half smile… with extremely crooked teeth. We just knew there was a great smile hiding inside and we chose her as one of our SFK patients. With the help of our friends at Capital Oral Surgery, she had four bicuspids removed and her upper braces placed in June 2014.

As her upper teeth unraveled, we took numerous progress photos (see pics) and her debonding reveal was in April 2017. After nearly 3 years she was so happy… but we were blessed to have had the chance to find that smile that was hiding behind her crooked teeth. 

Big Smiles

Were Plentiful The 2019 Smiles for Kids day was all about helping children in our local area who do not yet have dental insurance and qualified for the program. These kids were able to receive dental exams, x-rays, teeth cleaning, sealants, f luoride treatments, and emergency dental work at no charge. The dentists at Jackson Creek Dental Group, their staff and people from the community volunteered our time to treat 42 local Amador County children. Pro bono services were provided totaling over $15,000.

What is the Bump Dinner? Several years ago, while relaxing and having a massage – trying to clear my head! – my mind wandered about how we can raise funds for the Foundation. What auction item could include the things I love? (Cooking, people, our team!). This is where the Bump Dinner was born. Fifteen years ago. The concept was to offer dinner for 12 on a sign up basis at the Holiday Party Silent Auction. Then people could “bump” people to the next monetary level. What started out at about $350 per person has now turned in to $1000 and above per person – so obviously, the number of courses has increased, as well as the wine selection. (Thank goodness UBER and LYFT have come into play as well.) The Bump Dinner is the most highly sought after offering at our Holiday Party’s Silent Auction. You will frequently see people hovering near the signup sheet to make sure they aren’t bumped off, or in hopes to sneakily bump off others. Once the guest list has been finalized and the

winners of the Silent Auction are revealed and notified, I get to work at setting up the menu for the night. Each year the menu has a theme that the courses revolve around, whether that be French, Chinese, or even something fun like Valentine’s Day! The Bump Dinner takes place at my house each year and is something I work hard to prep for the whole week leading up to it and enjoy tremendously. The table is set in a fine dining manner, with all the forks and glasses you could need. The SDDS Staff arrives early to help prep the food and prepare for the night. Dinner guests start to arrive around 6pm when cocktail hour begins. The SDDS Staff goes around with passed appetizers while the guests enjoy some drinks and socializing. At 7pm, the guests sit down and the multicourse dinner experience begins. Anywhere from 7-9 different courses are served throughout the night, and no one leaves hungry. It is truly a wonderful dining experience for all involved.

Introducing

A Puppet Show for the Younger Crowd By Art Grueneberger Puppet Art Theater Co.

Art Grueneberger began his puppet career working under Master Puppeteer Richard Bay. His puppet performance premiere was as lead puppeteer in “A Thousand Cranes,” a UNIMA-USA Citation of Excellence winner. Grueneberger formed Puppet Art Theater Co. in 1994 and began producing outstanding children’s productions that have been seen by thousands of audiences across the United States. In 2010 Puppet Art Theater Co. completed over 600 live performances across the State.

The Tooth Fairy has dedicated her career telling stories that teach children how to keep their teeth healthy. Her latest tale is about Clay the cave boy and Sabey his pet saber tooth tiger kitten. Join them as they go on an adventure of a lifetime learning how to keep their smiles healthy by brushing twice a day, visiting the dentist regularly, eating healthy foods and avoiding the Sugarsaurus Rex. The show is just right for preschoolers, TK’ers, and kindergarteners. Our first puppet show offering is called Sir Enamel and the Sugar Bugs, it teaches oral health to kindergartners-third grade aged children. There’s also a Game Show that serves fourthsixth grades in a competitive contest on oral health questions. All shows are so much fun and full of laughs.

Letter from a Patient Hello, my name is Mandy and I am a 43 year old mother of six children, with three still at home under the age of 18. I am a single parent and sole support of my three minor children. Being a single parent with no other support other than what I make isn’t always easy, some months we just get by. With rent, food, utilities, gas and personal needs for myself and my children there is no way I can even dream of being able to fix my smile. Growing up I was the youngest of four children born to alcoholic parents who didn’t have the time or care to teach me proper dental hygiene. As I grew I fell to the cycle of addiction taking my first drink at age 12. By the time I was 16 I started smoking marijuana and using cocaine. At the age of 17 I graduated to methamphetamine. At age 21 I had my first son had my last child at age 33. Though I thought my life was normal and I was a good parent, I know now my life was unmanageable. I hit my bottom on August 5, 2015. I woke to the police department in my house telling me they had called CPS and that my children would be placed in protective custody. That day I lost all hope; hope for life, hope for my children, I didn’t want to live anymore. Believing the lies that my addiction so loudly whispered in my ear that my children would be better off without me, I resolved to letting the State of California raise them. I prayed for the Lord to take my life; I couldn’t bear the emotional pain any longer. Little did I know that prayer was the beginning for me. The beginning of a life I could only have dreamed of. December 26, 2015 (the day after that prayer) my sister rescued me from the streets in the

Bay Area and brought me to Sacramento. I detoxed on her couch. I contacted the County where my children were being fostered and let the worker know that I would do whatever it takes to be allowed to be a mother to my children again. I entered Alpha Oaks

Treatment Center and completed my 90 days. The only thing I knew for sure leaving rehab was, that I never wanted to go back to being the person I was just 90 days earlier. I was at a loss and didn’t know what to do or where to turn, the journey in front of me seemed so huge and unreachable. That was when I decided that this program called Saint John’s is where I needed to be. On May 9, 2016 I entered Saint John’s Program and that was when my true journey begin. My healing started that day. I started learning about my addiction and behaviors and why I did things the way I did. I learned true love for the first time in my life, the staff and therapists were always there to support me and teach me how to do things differently. In June, I started working at the Plate’s Restaurant, building my self-esteem and self-worth every day. Through the help and support of Saint John’s on October 18, 2016 I was granted full custody of my five minor children and they moved into the program

with me. That was a whole new journey, I had to learn how to parent clean and sober, I had never done that before. With the staff at Saint John’s right by my side I have learned how to be the kind of daughter, woman and mother God intended me to be. In June of 2017 I became employed in Sacramento doing data entry. I love what I do and see so much potential for growth. My hope is to one day move into sales where I will be able to earn better wages. In July of 2018 I moved from Saint John’s to my very own apartment! I never thought this would be possible, but here I am! I’m really doing it!! There is one thing that would help to close the door on my past and that would be to have my mouth fixed. It would be a dream come true to not have pain anymore, or to not wake up to a swollen face and eye that is shut because of infection in my mouth. One thing Saint John’s has taught me is, if you need help ask for help. I have made it this far by learning how to ask for help when I need it, and I need help. I need help to fix my mouth. I have so much to live for and smile for these days. Every day is a worry for me. I have a very important job/mission to accomplish. I get to finish raising my children without a drink or drug in my body. I’m scared every day that infection will make me so ill that

I have so much to live for and smile for these days.

I won’t be able to care for my children. And I made them a promise, I would never leave them again. Any type of help you would be able to provide would be so greatly appreciated and truly a blessing. 

From Dr. Redford’s office Mandy was scheduled in April to start treatment with Dr. Redford. We have planned to do SRP, several crowns, root canals, upper and lower partials. We are going to make her smile fabulous again. She is a lovely lady with a great story. We are glad that we can help her.

Improving Nutritional Intake My wife was familiar with a parent of one of her students that had bilateral missing posterior teeth in both arches. The family has several children and they have limited resources. The parents always put themselves at the rear of the list when it comes to needs, hence replacement teeth was not something that could be afforded. Unfortunately the family has had multiple

setbacks, most recent of which was that the dad was diagnosed with recurrent cancer. He now has stage 4 Lymphoma. As a result of his treatments he has not been able to be employed for some time which compounds the problem further. I did a complementary exam. He had some periodontal issues on the molar teeth and because of the chemotherapy; they deteriorated

further and were removed. He also has some sensitivity to metals so Ultaire high strength polymer partials were treatment planned. One of his abutment teeth had extensive caries so the tooth was crowned with a ceramic restoration. End result was that one crown and two Ultaire polymer partials were provided without cost to the patient. The SDDS Foundation supported the patient by covering the cost of the laboratory services. The patient is holding his own now that his nutrition has improved. All around it was satisfying to help a person in need. I trust that each of you can find that certain person out there that need a helping hand during a difficult time in their life. Thank you SDDS and SFBK for providing these support services. 

Every Smile Counts

Saving a Smile One Tooth at a Time By Margaret Delmore, MD, DDS

Dr. Delmore is an SDDS Past President and a member of the Sacramento District Dental Society and Sierra Sacramento Valley Medical Society.

As dentists we know that cavities are almost 100 percent preventable. Yet dental problems are the reason that more than half a million of California’s school-aged children miss at least one school day per year. We know that cavities do not heal; they require an intervention by a dentist. In Sacramento County, approximately 47 percent of oral health-related emergency department visits for children from newborn to age 20 in 2016-2017 were for preventable conditions. The oral health of our child patients can be dramatically improved with a physician’s help. Because children are more likely to access medical care than dental care, with well-baby visits generally beginning soon after birth and continuing throughout childhood, the medical provider, has the unique the opportunity to be on the front lines of assisting young children and their families in preventing dental decay. The Dental Transformation Initiative (DTI), a federally funded component of California’s Medi-Cal 2020 Waiver, aims to address the state’s position as having one of the lowest utilization rates in the country for dental care services by Medicaid (MediCal) beneficiaries ages 0-20 years. This is being done by incentivizing continuity of dental care, increasing reimbursement rates for specific preventive dental procedures for Medi-Cal dental providers and through the implementation of local dental pilot projects. This funding and support demonstrate that our federal and state governments view access to dental care as critical to achieving better overall health outcomes for Medi-Cal beneficiaries, particularly children. Sacramento County is one of 13 entities in California to be funded by the California Department of Health Care Services through DTI to participate in a local dental pilot project. “Every Smile Counts!,” Sacramento County’s local dental pilot project, is implementing three pilots over the course of three and half

years to reinforce good oral health practices and increase utilization of dental services by 5 percent annually: A “Virtual Dental Home” Pilot Providers such as dental hygienists in alternative practice work under the auspices of a dentist to provide dental care in places where people work, play, attend school and receive social services. Community Outreach and Parent Education (COPE) Pilot Community health workers reach out to families to provide oral health education and help them gain access to the services they need. (SDDS is included in this project.) A Medical-Dental Partnership (MDP) Collaboration Pilot Provides more opportunity to identify and prevent oral health problems. Of the three pilots, the Medical-Dental Collaboration Pilot is the one in which physicians should take interest. It aims to engage the medical professional to: • Provide oral health education, stressing prevention – brushing, flossing, and advising parents to take their child to a dentist when their first tooth comes in or by their first birthday (1st Tooth or 1st Birthday) and encouraging regular dental check-ups. • Apply fluoride varnish in the medical setting (2 times per year) • Refer children to their assigned dentist • Conduct a caries risk assessment to document and advise on the positive and negative behaviors of the child/parents that influence the dental disease process “1st Tooth OR 1st Birthday” is an especially important mantra that we need to drill into all medical providers. The American Dental

Association, the American Academy of Pediatric Dentists and the American Academy of Pediatrics all recommend a dental visit for children by age one. How do we get our medical colleagues on board? Children Now a statewide advocacy, policy and research organization focusing on children’s issues, can help. Children Now will train primary care physicians and their staff to: 1) educate children and families about oral health; 2) navigate children and their families to a dental home; 3) administer and bill for preventive dental services (e.g., dental screening; caries risk assessment; fluoride varnish). The Medical-Dental Collaboration pilot program became operational at River City Medical Group (RCMG) owned surgery clinics in July 2018. RCMG is one of the area’s largest Medi-Cal dental providers. Through DTI funding, RCMG employs a Support Services Navigator (Dental Services Coordinator) who coordinates dental services and an Oral Health Educator (Social Worker) who explains the importance of regular dental visits and follows up with the family after a visit. Additional state funding also supports the hiring of two parttime registered dental hygienists in alternative practice to enhance the medical-collaborative team. Integrating a dental professional into the medical team can foster the bilateral transfer of learning. Here is a snap shot of staff roles at RCMG: Medical Assistant • Explains the benefits of and applies fluoride varnish • Warm hand off to the Oral Health Educator Social Worker: Oral Health Educator (& Connection to Dental Services Coordinator) • Connects with patient/parent/guardian immediately following a well-child visit • Provides oral health education and materials; explains the importance of regular preventive dental visits • Explains role of the Dental Services Coordinator (DSC) – The Navigator • Requests permission from the parent for the DSC to call and help set-up a dental appointment. Notes outcome of request in Centricity (EMR)

Dental Services Coordinator: Connection to Care – The Navigator • Receives referral note from Centricity • Follow’s DSC Protocol to provide patient/parent/guardian with the assistance with the following: • Appointment scheduling • Transportation arrangements • Dental provider change • Basic dental benefit education We should expect the physicians, as an important part of ensuring patients, especially children, maintain their oral health, look at the teeth, gingiva and soft tissues of all patients. Perform oral health screenings and identify factors impacting oral health and overall health. (I’m always impressed with the physicians that know the numbers of the teeth or letters for the primary teeth.) If there is a problem, communicate with the patient’s dentist. We don’t bite. Ask patients and parents if they have a dentist. If not, direct them to Sacramento Covered at (866) 850-4321 for assistance finding insurance and a dental provider. They should educate families on 1st Tooth or 1st Birthday and how cavities and dental disease are preventable. Ideally, they should promote oral hygiene by recommending brushing twice a day for at least two minutes and choosing water instead of sugary drinks. (Remember the phrase, “Rethink your drink.”) ‘Dentifying’ a John F. Kennedy quote: Ask not what the physician can do for you, but what you as the dentist can do for the physician and ultimately the patients. Here is what you as the dentist can do: • Open communication with the patient’s physician. Share dental visit information to ensure holistic and coordinated care including patient’s caries risk assessment so that the medical professional can reinforce the positive behaviors recommended by the dentist. • ‘Bee’ cross pollinators: dentists can attend physician functions (through the local medical society, the Sierra Sacramento Valley Medical Society—SSVMS for example) to develop relationships and expand knowledge. Invite physicians and medical professional to attend dental related classes, functions, etc. I

see our new dental school at California Northstate University (CNU) as ideal breeding ground for this type of early collaboration since many of the dental classes will be coordinated with the medical classes at CNU. Get the medical and dental professionals working together as part of their educational process. Cross pollinate early on. • Be an influencer with colleagues— talk about increase communication, attending classes, etc. I asked the physicians to up their dental knowledge, you may need to up your medical knowledge. Remember a good dentist treats the teeth. A great dentist treats the whole patient. • Attend learning collaboratives and/ or case review sessions. Don’t know of any—start one! • Consider presenting at gatherings of medical professionals—trauma cases, prevention, etiology of dental disease, public health, fluoridation, etc. • Gain skills/practice needed to be comfortable seeing very young patients – 1st Tooth or 1st Birthday. As dentists, we know children are one of our most valuable future resources and helping to ensure their whole body, including their mouth, is healthy will also help to ensure better life outcomes. We can’t let our children suffer from dental neglect. We need medicaldental collaboration to center and respond to the needs of families and children in order to move closer to health equity. & Stronger Together At Midtown Dental, giving back to our community is an important part of our team culture. Our staff collectively came up with the idea of partnering with WEAVE in Sacramento to help victims of domestic violence. We worked with WEAVE to organize a screening at the safe house prior to the event to identify the specific needs and to be better prepared to offer services on the day of outreach. Three dentists, three hygienists, and five other Midtown Dental team members provided services on August 22, 2018, while representatives from Covered California helped survivors connect them with their ongoing dental benefits. An important part of our effort was to emphasize ongoing maintenance and care for long-term oral health.

I am happy to report that our Foundation is strong: strong in its ability to get volunteers to donate time, strong in its ability to find those in need and help them get the care required, strong in the leadership that it provides in advancing the oral and general health in our community.

Big Kids program, started in 2008, provides care on an individually assessed basis, where our SDDS members donate dental services to those that are in uniquely difficult situations. The Foundation pays for the material costs (i.e. labs) providing the care thereby encouraging the volunteers to do more.

Whilst so much of what we do involves volunteering of time and talent, there is a material cost for all of our programs and each depletes the funds of our Foundation in their own way. Our Foundation, through a number of programs, provides outreach, education and care for our surrounding communities. Our flagship program, Smiles for Kids, continues to positively affect the lives of thousands of children each year and 2018 was no different. We screened and placed those children with needs in contact with their respective care providers and it has resulted in a smaller number of individuals that require hands-on care through our Smiles for Kids treatment day each February. This reduction in numbers is a measure of the success we have had in reducing the level of untreated oral disease in the children of our community. From a patient load of over 1200 in 2008 to 300 in 2018 represents an amazing 75% decrease. Congratulations to all the volunteers, school nurses and team members both clinical and administrative who work tirelessly behind the scenes to make this kind of change achievable.

Numerous studies have shown that the greatest effect we can have on the oral

the children grow. We have had nothing but positive feedback from those institutions that have had the show visit. In addition, our 1st Tooth or 1st Birthday Campaign has been recognized statewide and continues to educate the community on the value of early dental visits. Seeing a child by the first tooth or first birthday allows the parents to be educated on how best to care for the oral health of the infant to help prevent disease instead of treating it. While the needs of the young have been the focus of the Foundation for many years, we have not failed to understand that there exists a need within other groups. Our Smiles for

To that end, I am pleased to inform you that 2019 is the 125th anniversary of SDDS and the 50th anniversary of our Foundation. The SDDF will be holding a Dinner and Dance Gala with a silent and live auction on Saturday October 5, 2019. It will be a celebration of all that we have achieved and a reaffirmation of our commitment to help our community flourish with optimum oral health. Please put the evening aside and tell your friends to come and celebrate the amazing successes of our Foundation and support its ongoing efforts. The legacy of what SDDF does for our community will go far beyond any of us and I welcome you to become a part of that legacy. ď Ž

Smiles for Kids is a wonderful opportunity to “give back” as we are blessed with such a wonderful profession. The funny thing is that I always leave feeling like I got more out of it than I gave. Our largest and most far-reaching project, Smiles for Kids (SFK), partners member dentists with local schools to screen and provide dental eduction to thousands of children each year. From the screenings, nearly 1000 underserved children are treated by member dentists and their staff on SFK Day each February. Each year, a million dollars in pro bono treatment is provided to children in the five county SDDS area.

Puppet Show Dental Education

The 1 Tooth or 1 Birthday campaign targets not only member dentists and the public, but the medical community (especially pediatricians). The message? Kids need to be seen by a dentist early, before problems arise. This early education and prevention message — and the 1T1B magnet and logo — is seen on bus tails, in medical offices as well as dental offices.

Thanks to two very generous sponsors, we are able to offer two educational puppet shows that travel to local area schools informing and entertaining our community’s children. We hope the message will make it’s way back home and impact the entire family.

In 2005, the Foundation partnered with Star Refining and began Crowns for Kids® (CFK) to benefit Smiles for Kids. In this program, member dentists collect scrap metal from their patients’ dental treatment in CFK jars, the jars are collected and sent to the metal refinery (Star Group), and the refinery sends a check to the Foundation each month.

Patterned after SFK, Smiles for BIG Kids (SFBK) recognized that often the parents’ dental health needs are equally as urgent as their childrens’. SFBK focuses on the needs of the parents of SFK patients, underserved adults and the elderly. The Foundation has partnered with several local community organizations to identify eligible patients and, each year, member dentists provide much needed care to these individuals in the form of dentures, restorative treatment, and specialty treatment.

Definitely From the Heart SDDS, is for the members, the dentists… The Foundation is for the community… SDDS Foundation is the heart of the Sacramento District Dental Society. Along with the more than $1 million dollars in grants received for our Foundation programs since 2003, Smiles for Kids and Smiles for Big Kids, it can’t go unmentioned that the SDDS Foundation has received three other MAJOR donations… definitely “from the heart.” Just saying thank you cannot begin to show the appreciation and gratitude that we have for these three major donations – but our membership (and the readers of this edition of the Nugget) hopefully will make note and say a special thank you and prayer for these generous gifts from the heart. $217,000 (2013) Robert & Lolita Oates Charitable Trust Given to the Foundation by the parents of Past President Dr. James Oates and his brother, Edward Oates; given to the Perpetual Fund and the largest gift the Foundation has ever received. This donation will continue helping to provide opportunities for our Foundation’s goals and purposes now and in the future.

Geared specifically toward the dental assistants and also the EF curriculum, the SDDS Foundation is in partnership with FADE (the Foundation for Allied Dental Education) to help defray costs for those applicants who qualify. Applications will be available in June 2019. $39,000 (2014) The Yee Family Dental Student Scholarship This dental scholarship began in 2014 to assist our new dental students matriculating from CSUS. The Yee Family has a heart for higher education, and recognize the costs of dental school for future dentists. In honor of the partnership SDDS has with the CSUS Pre Dental Club, the Yee Family Scholarship wants to pay it forward, since more than 15 members of the Yee Family are dentists! 

$75,000 (2018) The Dr. Beverly Kodama/Delta Dental Foundation Scholarship for Allied Education Dr. Kodama (who served on the Delta Board of Directors) partnered with Delta Dental Foundation to establish a scholarship to help defray the educational costs for dental assistants and other allied team members. Dr. Kodama always felt that allied team members were the heart of a dentist’s

Where are they now? Past Recipients of the Yee Family Scholarship Allicia Lucich University of Nevada, Las Vegas Class of 2021 I have been busy at UNLV. I am currently second semester of my DS2 year and preparing for boards. We have our white coats and started seeing patients a few semesters ago. I received the four year Army Health Professions Scholarship Program. I am always involved, so I am the VP of the Hispanic Student Dental Association, Legislative Liaison of ASDA, Secretary of the Business Club, Public Relations Officer of the Sgt Ferrin Veterans Clinic and I am currently running for President of the Executive Council at UNLV. My kids are getting big and we are traveling a lot for pleasure (South Africa on safari) and business (I have gone to Arizona, Oregon, Chicago and Pittsburgh for ASDA conferences).

Kevin O’Brien University of the Pacific Class of 2022 Just finished our 3rd quarter at UOP! The culture here is so positive and collaborative. I have learned so much in only nine months and I can only imagine how much I will learn in the next 27. It has been fun sharing everything with my dad and comparing it to his practice. We are wrapping our pre-clinical coursework and seeing our first patient (my mom) for an initial exam next quarter! Another big update, is that Jess and I had a baby boy (Kai) in March. We are so excited to be parents!

Andy Dinh University of California, Los Angeles Class of 2022 Thus far, dental school has been quite the transition - both in terms of workload and lifestyle. Being surrounded by so many outstanding students has really propelled me to further evolve as both a student, and as a person. I’ve been blessed have been elected as the UCLA class of 2022’s Vice President where my primary mission is to promote unity, inclusivity, and a culture of service & giving back. Although daily life as a dental student is rigorous and challenging, being able to zoom out and focus on the big picture allows me to refocus my motivations and push myself. I’m very thankful for both familial and community support, especially from the SDDS, and it constantly reminds me who and what I’m doing this for.

MATTHEW SANDERS, DDS At Sunrise Orthodontics it’s so important to us that part of living a happy and fulfilling life includes being part of a community where we can give back to those in need. Dr. Matthew Sanders has been a part of the Smiles for Kids program since 2010 and has enjoyed getting to help each family we have been able to sponsor give their kids the smiles they deserve. This year, we got to help out at Smile Kingdom Dental and it was a huge turnout. We really enjoyed meeting so many new faces!

By California Employers Association Do you have any upcoming events where you might ask your employees to volunteer to help? For example, Smiles for Kids is a great event that SDDS members generously give their time, energy, office space and hearts to each year. Often your office staff will pitch in to help during this busy day. But the question is, are they working as your employee on this day and need to be paid or can they be considered volunteers and not be compensated?

Step 2 Have each employee sign a document prior to the event acknowledging that they understand they will not be paid, it is a charity event and they are not required to be there.

The State of California has taken a closer look at volunteers recently, so employers must be cautious in considering if employees should be paid for this event or if they can really be classified as a volunteer. Someone who is truly a volunteer is offering their services for public service, religious or humanitarian objectives. Smiles for Kids will meet that criteria but you must be very careful to ensure employees know that this is a charity event, not a work event and that their participation is not required, but strictly voluntary.

If you participate in the event and want to require your employees to be present, then they must be paid. Anytime you require an employee to participate in any event (charity work, training, CE, etc.) they must be compensated for their time. 

There are several steps that you can take to ensure that it’s clear to everyone that this is a volunteer event for charity. Here are some best practice suggestions when employees volunteer for events like Smiles for Kids: Step 1 Hold a staff meeting prior to the event to let employees know about it and inform them that it is voluntary.

Step 3 Ensure that no adverse action is taken against anyone who does not agree to volunteer for the event.

YOU ARE A DENTIST. You’ve been to school, taken your Boards and settled into practice. End of story? Not quite. Are you up to speed on tax laws, potential deductions and other important business issues? In this monthly column, we will offer information pertinent to you, the dentist as the business owner.

Billing Dentists: Learn How to Properly Bill When Another Dentist Performs Treatment As the dental-benefits analyst for CDA Practice Support, my job is to help dentists navigate the ever-changing benefits marketplace and this entails clearing up misconceptions that could place a dentist at risk. In my January Update article, I covered the risks involved in billing improperly during practice transitions. Here, we’ll cover how to properly bill dental-benefit plans when a dentist performs the treatment. As we explore the proper way to bill for the treating dentist, keep in mind that a claim is a legal binding document and, as such, all elements documented on the claim form must be accurate. In today’s dental-benefits marketplace, many plans have adopted contracting based on the contract of the treating dentist, not just the billing (owner) dentist. When billing a benefit plan, the information documented on the claim in the billing dentist or billing entity, treating dentist and treatment location sections must all be accurate. If the treating dentist documented on the claim differs from the treating dentist noted in the patient’s chart, the dental-benefits industry considers this billing practice to be fraudulent billing because the treating dentist is misrepresented on the claim. It is important to understand that many plans use analytic software to monitor a dentist’s claim-utilization patterns. These analytics can be used to compare the treatment patterns of

a dentist to their peers and identify a dentist who stands out.

them with the plan or listing them as an outof-network dentist.

One way a billing dentist might stand out is when they are a contracted dentist with a plan but have an uncontracted dentist treating enrollees of that plan in their practice, and yet the billing dentist bills the plan noting the contracted billing dentist as the treating dentist on the claim. Because the treatment was actually performed by the uncontracted dentist, this type of improper billing can cause a rise in that billing dentist’s claimutilization pattern when compared to a peer who is also a contracted dentist with the plan but has no one else working in their practice. This overutilization pattern can trigger a plan audit. If discovered, such billing can be costly, as the plan could recoup any monies paid beyond the out-of-network benefits under the policy because the treatment was provided by an out-of-network, not in-network, dentist. The billing dentist or billing entity also runs the risk of being terminated from the network due to a breach of contract.

If the treating dentist will contract with plans, remember that in most cases the plan will consider the dentist as out of network until the contracting process is completed by the plan. This means that many plans will treat the dentist applying to their network as out of network while the plan works to complete the application for that dentist. Do not assume that contracting is one-size-fitsall. The dental-benefits marketplace is very complex and plans have different contracting protocols, including for compensation.

Let’s look at how a practice can bill properly by registering all treating dentists in the practice with a plan to avoid the previously mentioned risks. When adding a new treating dentist to the practice, the billing dentist should contact the plans well in advance of their start date to learn how to add the treating dentist to the plan’s system of records by either contracting

Determining a plan’s contract protocol well in advance, including how long it will take the plan to process the contract, is critical for a smooth transition of adding a treating dentist into the practice. Also, keep in mind that not all plans will allow assignment of benefits when an uncontracted dentist performs the treatment, and this means the patient, rather than the practice, will receive payment for services provided. What’s more, many plans will decrease a patient’s benefits when they see an out-ofnetwork dentist. So if the goal of the practice is to have the treating dentist in network with plans due to the previously mentioned factors, it is important to try to align their start date with the time frame the plan provides to contract them.


If your practice plans to add a treating dentist, here are some things to remember: • The dentist who performs the treatment must be documented as such on the claim in the treating dentist section. Examples: – If the billing dentist (owner) is the treating dentist, then their name, license number and Type 1 NPI should be noted in the treating section of the claim. – If the (non-owner dentist) is the treating dentist, then their name, license number and Type 1 NPI should be noted in the treating section of the claim. • If the treating dentist will contract with the dental-benefit plan, the billing dentist should learn the plan’s contracting requirements and time frames. • If improper billing is done, the billing dentist could be found in breach of their contract with a plan and would take on the penalties of the improper billing. • The billing dentist should follow the contractual obligations noted in their contract with the plan for reporting any treating dentist working in their practice. Also, not all plans’ contracts are portable; in fact, many are location-specific, meaning that even if the dentist is currently contracted with a plan at another practice location, they will be required to sign a new contract with the plan if they wanted to be an in-network dentist at a new location. Plans’ contracting and compensation rates change from time to time. Committee Corner Pre-Dental Interns A Valued Program The UC Davis Pre-Dental Society and the Sacramento District Dental Society have partnered over the past few years to help guide pre-dental students by pairing them with local dentists in the community to shadow, and gain valuable knowledge and experience. The pre-dental shadowing program has been tremendously valuable for our pre-dental students in not only seeing firsthand the life of a dentist, but also by allowing our students to realize and develop a true passion for the field of dentistry. This program has also been wonderful in creating lifetime mentors between the dentists and our students. We would like to thank the

ASK THE BROKER Hey Young Dentists, Get out of the City!! In the 20 years that I have been involved in practice transitions, never has there been such a discrepancy in the interest of practices in the big cities as compared to the rural areas! Practices in small cities like Chico garner almost no attention, even after most of the town of Paradise had to move there after the fire! In the early 1980’s I remember hearing Jim Pride DDS, founder of Pride Institute, tell us young graduates to leave the city, make more money than our city counterparts and come into the city to take our counterparts out for dinner on the weekends while enjoying whatever entertainment the city provided! Even 35 years ago, many rural areas provided the opportunity to make more money doing the same amount of work. With the current rising prices of everything in the cities, that is even truer today, but the city trend is even stronger. Why is that? I believe the reasons behind the increased trend are the millennial mindset and the staggering student loan debt crisis. I believe millennials are more inclined to stay in the cities and work as associates. Some of that reasoning may come from the fact that their student debt is so much higher than previous dental graduates. However, I do not think they realize that Jim Pride would be even more correct today than he was 35 years ago. Let’s look at some of the reasons to consider a smaller city.  Less competition is one of the most obvious reasons. Many of our “rural” practices have higher collections and higher fees than our city practices because they do not have the competition.  Higher take home pay is often the case as rent and staffing is usually cheaper in the “rural” practices.  Stronger and more loyal patients is often the case for “rural” practices, especially if you plug into some of the local community and school activities.  Practice purchases cost less in the “rural” areas. If the normal metric for a practice sale is around 75% of collections in the city, the metric for the rural area might be closer to 55%. Also, the million dollar collections practice in the city probably nets around $300K, whereas the rural practice probably nets around $400K. Therefore you might be able to buy the same practice in a rural area for $200K less, but also take home $100K more per year. THIS DOES NOT EVEN TAKE INTO CONSIDERATION THAT THE COST OF LIVING IN A LARGE CITY CAN SOMETIMES BE A FACTOR OF DOUBLE THE COST OF LIVING IN A RURAL AREA! Utilities, groceries and transportation are generally higher in the city, but the biggest reason is obviously the cost of housing can be up to 4 times higher in the city! BY MATT CAREY New York — Manhattan was the host to an oral health initiative on a massive scale Feb. 1 when the New York County Dental Society hosted a Give Kids A Smile day during which volunteers screened 1,392 children from underserved neighborhoods. The event had 129 volunteers at five sites serving nine schools in East Harlem, West Harlem and the Lower East Side of Manhattan. There was a 65 percent increase in children treated from last year when 845 children were screened. Students received screenings, oral hygiene instruction, and fluoride treatment. The estimated cost of all care provided was $156,010. “It is quite rewarding to see the incredible number of wins that come from a program such as ours,” said Dr. Deborah Weisfuse, general chair of GKAS NYC 2019. “Besides the obvious one of helping children and their families learn about and move towards better oral health, I have really enjoyed watching the personal development of my leadership team, their increased engagement in GKAS NYC, and even greater engagement in our component.” Dr. Weisfuse was the first female president

The teacher had told her students a story about what her family does when trying new food. They don’t only take one bite, but three bites, before they decide whether they like a certain food or not. “Our daughter shied away from trying new food,” Dr. Alassaad said, adding that one day, his daughter wanted to try a fig from their backyard tree which she was never interested in before. “The braver approach to new tastes opened her mind to other new experiences,” he said. “Our daughter adored her teacher and listened to her instruction. Her teacher became very influential by being a role model.” In November 2017, Dr. Alassaad attended an oral health literacy panel in Chicago where the attendees were challenged to go back to their communities and find a way to contribute to oral health literacy. In going through this process these agencies have become cognizant of the significant community effort of the society, said Dr. Weisfuse. In 2018, Dr. Weisfuse, encouraged by U.S. Rep. Adriano Espaillat, created a multidisciplinary community task force that includes New York County Dental Society leaders and representatives from local gov-

The Yolo County Oral Health Program and the school district also provided the schools all the additional educational materials including giant plastic teeth models and toothbrushes for demonstration.

Planting a seed

The school district started implementing the curriculum in February while celebrating National Children’s Dental Health Month. Dr. Alassaad attended one of the classroom sessions and immediately saw why teachers and school nurses should take the lead when it comes to oral health education in schools. “I was glad I wasn’t the one teaching it,” he said. “I could not have connected with that many children in such a short period of time the way their teacher and nurse did. There’s already a relationship they’ve formed with their students.” Dr. Alassaad said the children were receptive and engaged, asking several questions to the presenters. Their excitement only grew when a tooth fairy and Mighty Molar the mascot showed up. To Dr. Alassaad, instilling good oral habits in children requires a three-pronge d approach: dentists during dental visits, parents at home and from educators in schools. “Statistically speaking, our effort seems small in the overall oral health picture,” Dr. Alassaad said. “But what we’re doing is planting the seed for something much bigger. It’s a success because this has never happened in our school district before. SDDS VENDOR MEMBERSHIP SUPPORT IS A WIN-WIN RELATIONSHIP! SDDS started the Vendor Member program in 2002 to provide resources for our members. No, Vendor Members are not exclusive, and we definitely have some competitive companies who are Vendor Members. But our goal is to give SDDS members resources that would best serve their needs. We suggest that members reach out to our Vendor Members and see what is a best “fit” for their practice and lifestyle. We currently have 37 Vendor Members. They pay $3,900 per year; that includes a booth at Midwinter, three tables at General Meetings, advertising in The Nugget, and much more. Our goal is to provide Vendor Members with the opportunity to connect with and serve our members. We realize that you have a choice for vendors and services; we only hope that you give our Vendor Members first consideration. The Vendor Members program and the income SDDS receives from this program helps to keep your dues low. It is a wonderful source of non-dues revenue and allows us to provide yet another member benefit. Additionally, we reach out to our Vendor Members for articles for The Nugget (nonadvertising!).
