Prior Preparation  Send letter to candidate confirming start date, position, rate of pay, and initial work schedule  (Optional) Send W-4, I-9, and other non-confidential forms for employee to complete and bring in  Notify the staff of the new employee’s name, position, and start date  Notify internal departments as needed (or perform these functions accordingly as described below): o Payroll, to initiate pay process o Human Resources, to initiate personnel file and record o Benefits, to initiate paperwork o Corporate office, as needed o Radiology, to order radiology dosimetry badge o Marketing, to add to mailing list as needed o IT, to set up computer access o Telephone Communications, to set up telephone access  Assign the necessary items: o Timecard, or computerized time clock log-in and password o Operating software log-in and password access o Network email address if applicable o Locker o Mailbox o Telephone Extension o Security card o Keys if needed  Order the necessary items: o Business cards o Name tag o Identification card o Radiology badge o Uniforms  Collect the necessary documents: o Employee Policy Manual o Hospital Safety Manual o Work schedule o Training Program, with assigned mentors  Begin Personnel File o Resume, references, and employment application

Employee On-Boarding  Greet on first day of work and give another brief tour of the facility, introduce to team members  Give instructions on clocking in and calling in when absent or late  Demonstrate where to park, where to store belongings, where breaks can be taken, etc.  Introduce to Training Mentor  Present name badge, security card, radiology badge, email address, locker, mailbox, work schedule, uniforms, etc.  Present current copy of Employee Phone List  If applicable, give Biography to fill out and take photo to add to bulletin board
