I had written a completely different editor’s letter. It was about friendship, the power of lasers and our beauty awards, next to a photo of me with our Clear + Brilliant–loving cover star Drew Barrymore. On the day we were to release the magazine to the printer, we received the devastating news that our friend and mentor Vic Narurkar, MD had passed away. I’d like to dedicate this month’s letter to him and his loved ones. Dr. Vic had one of those crazy-beautiful brains that make magic: he was always thinking, inventing and innovating. This annual Awards issue is often the most challenging one for me to review. Lots of products, lots of promises and lots of newness flood the submissions, and concurrently, tend to bring out the concerned pessimist in me.
kind of effect, we’d be slathering ourselves with it. The promise of a pill that can plump skin, erase wrinkles and give you a glowing complexion regardless of any other lifestyle factors is probably a long way off and may never actually be achieved in our lifetime.
However, the carefully vetted winners that make the cut definitely deserve the honors bestowed. They’ve been tested, they’ve gone head-to-head with the competition and they’ve proven that they are “the best.” But the sheer number of items submitted that promise the “anti-aging, look-better-now, does-everything world” always begs the question: How can this many things do this much this quickly?
In most cases, if the marketing matched the reality, the choice would be a no-brainer. What is right for everyone, particularly when it comes to aesthetics, is having a trusted resource to help us make smart, informed decisions. Simply put, the market is oversaturated, and setting a high standard to find what really stands out from the rest is necessary, which is exactly what these Awards do.
The truth is, they can’t. There are many incredible new products and treatments that deliver exactly what they say, but there are probably more than a thousand times that amount that don’t, and that number seems to grow every year. For consumers— even the most astute ones—this can be overwhelming and endlessly confusing.
When it comes to beauty products, selecting one that doesn’t work probably won’t do much damage other than trying your patience and wasting your money. It is a good reminder that, in the field of beauty, when it comes to more aggressive and often more effective aesthetic treatments, both surgical and noninvasive, the risk runs much higher and the permanent outcome that can go along with unfavorable results and complications can be very serious.
Who wouldn’t want to wash their face in a magical potion that takes 10 years off overnight, comes with guaranteed results, no downtime and costs only a fraction of the cost of a real facelift? Botox Cosmetic produces great results, but if there’s an overnight solution that can give the same

USES: XEOMIN is a prescription medicine that is injected into muscles and used to improve the look of moderate to severe frown lines between the eyebrows (glabellar lines) in adults for a short period of time (temporary). It is not known if XEOMIN is safe and effective in children under 18 years of age. WARNINGS XEOMIN may cause serious side effects that can be life threatening. Call your doctor or get medical help right away if you have any of these problems anytime (hours to weeks) after treatment with XEOMIN: • Problems with swallowing, speaking, or breathing can happen within hours to weeks after an injection of XEOMIN if the muscles that you use to breathe and swallow become weak. Death can happen as a complication if you have severe problems with swallowing or breathing after treatment with XEOMIN. • People with certain breathing problems may need to use muscles in their neck to help them breathe and may be at greater risk for serious breathing problems with XEOMIN. • Swallowing problems may last for several months, and during that time you may need a feeding tube to receive food and water. If swallowing problems are severe, food or liquids may go into your lungs. People who already have swallowing or breathing problems before receiving XEOMIN have the highest risk of getting these problems. • Spread of toxin effects. In some cases, the effect of botulinum toxin may affect areas of the body away from the injection site and cause symptoms of a serious condition called botulism. The symptoms of botulism include: loss of strength and muscle weakness all over the body, double vision, blurred vision and drooping eyelids, hoarseness or change or loss of voice, trouble saying words clearly, loss of bladder control, trouble breathing, trouble swallowing. These symptoms can happen hours to weeks after you receive an injection of XEOMIN. These problems could make it unsafe for you to drive a car or do other dangerous activities. Do not use XEOMIN if you are allergic to XEOMIN or any of the ingredients in XEOMIN (see the end of this Guide for a list of ingredients in XEOMIN), had an allergic reaction to any other botulinum toxin products such as rimabotulinumtoxinB (MYOBLOC®), onabotulinumtoxinA (BOTOX®, BOTOX® COSMETIC), or abobotulinumtoxinA (DYSPORT®) or have a skin infection at the planned injection site. Before receiving XEOMIN, tell your doctor about all of your medical conditions, including if you: • have a disease that affects your muscles and nerves (such as amyotrophic lateral sclerosis [ALS or Lou Gehrig’s disease], myasthenia gravis or Lambert-Eaton syndrome) • have had any side effect from any other botulinum toxin in the past • have a breathing problem such as asthma or emphysema • have a history of swallowing problems or inhaling food or fluid into your lungs (aspiration) • have bleeding problems • have drooping eyelids • have plans to have surgery • have had surgery on your face • are pregnant or plan to become pregnant. LIP SERVICE As many cancer patients know, seeking treatment comes with its own set of side effects. The same drugs used to combat cancer are simultaneously affecting skin cells and body function. When Guilford, CT plastic surgeon Beth Collins, MD began treatment after a breast cancer diagnosis, one of the first changes she noticed fellow patients were complaining about was an extreme case of dry mouth. Chemotherapy and radiation therapy damage the salivary glands, leaving lips chapped and Sahara Desert–dry. “It reminded me of my skiing days and how I used to use pure lanolin on my lips, which tastes awful, but it was the only thing that would keep them from cracking.” This sparked her to create Pour Le Bouche, her own line of all-natural lip balms, made of lanolin, honey and essential oils. “I brought it down to the Smilow Cancer Center radiation department and handed out buckets of it to patients, who would go through them so quickly. It helped replenish moisture they desperately needed.” Dr. Collins says using basic ingredients is best for patients with compromised immune systems, so there were no hormones, chemicals or dyes added. “Besides, there’s absolutely nothing better than lanolin. When lip balm manufacturers water it down with cheaper ingredients, like waxes, it just doesn’t work as well.”
Certain wellness cures are proving to be not so healthy for the mouth. In the Boulder, CO office of cosmetic dentist Lori Kemmet, DDS, there’s been a surge in dreaded cavities among patients. The culprit, in Dr. Kemmet’s opinion: the city’s rise in kombucha bars. “There is a lot of kombucha consumed here and it can be very sugary,” she says, adding that not all “wellness items” that are good for the body are always so good for the teeth. “With any product, we have to consider that the tiniest ingredient can sometimes wreak havoc. I recommend always looking at the ingredient list and avoiding sodium lauryl sulfate, artificial sweeteners—like erythritol—and fluoride.” Atlanta cosmetic dentist Ronald Goldstein, DDS agrees. “CBD gummies sound great, but your teeth do not like them! They are so ‘sticky’ that they can sometimes pull a filling out. And if they contain sugar, watch out for possible decay. When 16-year-old Estella Warren was chosen for Irving Penn’s illustration of the pursuit of a plump pout, she didn’t realize the magnitude of the job until she walked on set. Celebrity makeup artist Kevyn Aucoin artfully painted her lips with Dior’s Dazzling Orange, since discontinued, and veteran Vogue editor Phyllis Posnick expertly guided each element of the shoot, including a beekeeper’s colony flown in from New Mexico. “It was one of my first photo shoots in New York,” she says. “I hadn’t had my teeth fixed yet, I knocked one out skating and the dentist made a temporary fake tooth that I was extremely self-conscious about.” But it was Warren’s pillowy lips that drew both the prolific photographer and the buzzing bumblebees. “We tried many
different bees, and Estella made different shapes with her mouth,” Posnick once said. “In fact, she became very playful, and at one point, stuck her tongue out and flicked a bee onto it.” To keep the excitable insects calm, the beekeeper tricked them into hibernating by storing them in a cold fridge. “They were frightening at first,” says Warren, “but the beekeeper also used smoke to make them docile and kind of sleepy.” The oftenmimicked image remains one of Penn’s most memorable visual masterpieces, thanks to those somnolent bees and his meticulous direction: “He was so precise, and the lighting was extraordinary, probably the best I’ve ever seen,” says Warren. “I remember him touching my lips and saying, ‘Hmm, they are real. Good.’”

ANATOMY 101 Michelangelo, da Vinci, Dürer, Raphael: Through various media, Renaissance masters celebrated human hands as works of art and marvels of anatomical engineering, illuminating the intricacies of their shape and sinews. Today, plastic surgeons and dermatologists are taking a renewed interest in the subject, aiming to preserve aging hands— refining canvases, restoring curves—with respect to their distinct design. “The hands are incredibly complex, with 29 bones and 34 muscles controlling their movement; uniquely pliable and supple skin concealing critically important structures; and specialized receptors that allow us to perceive pain, temperature and contact,” says New York plastic surgeon Lara Devgan, MD. Their fine covering and supreme dexterity elevate their aesthetic value, but also prime them for early aging. “The skin of the hands, though it takes a lot of wear and tear, is quite delicate, with a thickness similar to the eyelids,” says New York facial plastic surgeon Dara Liotta, MD. What’s more, our hands are lacking in pilosebaceous units, and consequently, “the stem cells present within these follicles, which are vital for repopulation of the skin following injury,” says New York dermatologist Mitalee Christman, MD, explaining why our hands tend to heal more slowly than faces post-procedure. Seizing upon the hands’ built-in vulnerabilities is a steady environmental barrage. “Sun exposure when resting on the steering wheel, chapping when shoveling snow, blistering when pulling heavy loads— our hands accrue far more damage than other parts of our body,” adds Dr. Devgan. The most notorious
aggressor, ultraviolet rays, turns on pigment production, leaving hands mottled, and saps our collagen reserves, further thinning the skin to lax, crinkled, translucent effect. The fat pads of the hands naturally deflate with age, unmasking ropy veins and tendons (knobby knuckles, too, we’ve noticed). And then there are the insults we more willingly hurl: “Excessive handwashing and sanitizing with alcohol-based products break down the skin barrier, leaving hands dehydrated, cracked and inflamed,” says New York dermatologist Dana Stern, MD. Over time, our fingernails, too, can adopt a wizened look, likely due to sluggish circulation and decades of UV exposure. They tend to lose their luster and develop ripples and ridges, which “aren’t serious, but can cause nails to split faster,” notes Montclair, NJ dermatologist Jeanine Downie, MD. Senescent nails even suffer a slight growth slump with age, starting as early as 20, with a rate drop of 0.5 percent per year. Before diving headlong into a sea of solutions, let’s take a moment to acknowledge that a certain amount of maturity often becomes the hands. When a dermatologist recently posted a picture of a pair of wrinkled, discolored hands—the caption, detailing why they age at warp speed—an astute commenter (a former beauty editor) pointed out “how ‘important’ jewelry looks on old hands...as if they earned the right to wear it.” I loved that silver lining. Whether you choose to undo the ravages of time with one of the many methods we explore here, or simply appreciate the dignity in their ever-evolving form, no judgment. I have this clear memory of my father at my
age, holding up a hand to inspect its brawny backside, bending and unfurling his fingers to see the skin crinkle in response, and summarizing plainly that his hands were no longer young. Recently, I’ve found myself, at 42, doing the same, noting that familiar crepe-paper texture, the widening valleys between tendons, the blue veins like rolling hills. My Dad, it’s worth noting, has long been an avid outdoorsman, a woodworking craftsman, a fixer of all things broken— handy, in the most literal sense. Me? I type words and collect creams. Hoping to head off my genetic destiny, I paid a visit to New York dermatologist Robert Anolik, MD. When I shared with him my self-diagnosis—severe volume loss, moderate visibility of veins and tendons—he laughed, assuring me the problem wasn’t quite so dire and could easily be addressed with Restylane Lyft, the first (and, thus far, only) hyaluronic acid (HA) filler to win FDA approval for the correction of age-related volume loss in the backs of the hands. The soft, sugar-based gel is temporary, lasting up to six months in the hands; reversible if you don’t love it; and has been used safely in the face for more than 10 years, so it’s got a solid track record. What’s more, injecting the hands, Dr. Anolik says, is less risky than the face, because the blood vessels here are big, visible and redundant. In other words, they’re easier to avoid, and even if you did mistakenly enter one, the filler isn’t as likely to obstruct a crucial source of blood supply, precipitating skin death, as can happen in select zones of the face. Also good to know: Filler-related lumps and bumps are less common in the hands than the face. On the backside of my left hand, Dr. Anolik made a tiny needle prick about an inch up from my wrist, in line with my middle finger. Into the opening, he inserted

a blunt-tipped cannula under my skin and above the muscle, and began injecting the gel, fanning the cannula left and right to hit all the hollows. (Pretty painless—even without numbing cream.) After a vigorous hand massage to evenly distribute the HA, he repeated the process on my right hand, saving one spot for a traditional needle fill, so I could compare the two methods. Cannula won, hands down: It’s a single needle stick, and in my experience, produced less bruising and swelling. Both hands were a little tender the next day. (For the record, injecting Restylane Lyft into hands with a cannula is considered off-label. Still, “dermatologists use it routinely here, as it is felt to be safe, and it is, in fact, approved for use with a cannula in the midface,” Dr. Anolik says, adding that cannulas can give impressive results, “especially in the hands where you have fairly easy access to a broad treatment area from one entry point.”) It took just half a syringe of filler per hand to instantly elevate the sunken pockets where time had eroded my fat. (With Restylane Lyft, one to two syringes, at $550 to $1,000 per, is more the norm.) I’m partial to HA fillers, because I like knowing they can be dissolved if need be, and that they metabolize over time. But, they’re not the only option. Also greenlighted for hands, Radiesse is a thick biostimulatory filler made from calcium hydroxylapatite microspheres, proven to last up to a year in the hands. “It works very well, and in my opinion, offers a better value owing to its greater longevity and biostimulatory effects, promoting fibroblasts to build new collagen,” says New York dermatologist Estee Williams, MD. Most patients need one to two syringes at about $1,250 each. Sculptra Aesthetic is also being used

WHEN IS FAT THE BEST FIT? Autologous fat grafting—taking from the rich areas (using lipo) and giving to the poor (via injections)—is another way to revive withered hands. “It’s like spreading melted butter on a bagel—the fat settles into all the nooks and crannies in a soft, supple way,” explains Dr. Devgan. While she uses both synthetic fillers and nanofat (or “morcellized particles that allow for easier delivery and smoother results”), the latter has its advantages. “Fat is in abundant supply in most of us; it’s permanent; and its inherent stem cells may benefit skin texture and quality,” she adds. New York plastic surgeon Adam Kolker, MD calls fat grafting “the gold standard in hand rejuvenation.” But the procedure isn’t without downsides. It is surgery, after all—hence riskier, more time-consuming and more expensive than the average filler (figure $8,000plus for hands). Fat is also criticized for its inconsistency— patients may need multiple sessions to get the boost they’re after. This shortcoming has some surgeons abandoning fat for hand restoration. “It’s not as predictable as filler,” says Dr. Saltz, who relies on Radiesse when treating the hands. Chicago plastic surgeon Julius Few, MD also prefers synthetic fillers to fat for hands, generally reaching for reversible HA options. Same for Dr. Liotta, who doesn’t like to fat graft in such thin-skinned areas. In her experience, she says, “there’s always the risk of clumping—having one island of fat cells that takes next to an area that doesn’t.” Other complications include bruising, swelling and soreness, but experts agree this is a very safe procedure when performed by an experienced, board-certified plastic surgeon.
(off-label) in the hands ($750 to $1,200 per vial). The liquidy, poly-l-lactic acid infusion has a delayed gratification effect and “gives a gradual improvement to a larger portion of the hands (due to the way it diffuses), showing results in three weeks and lasting up to two years,” says New York dermatologist Ellen Marmur, MD. Last fall, however, Dr. Marmur conducted an IRB approved pilot study with 20 patients examining the safety and efficacy of Restylane Silk, a thin HA filler, for finger rejuvenation, and found it delivered significant improvement for more than three months with a high level of patient satisfaction. “Fingers age, like the hands, from overuse and loss of fat,” she says. “Our goal here, though, is more to improve skin tone and smooth wrinkles than restore volume.”
The use of filler in the fingers is not FDA-approved, nor is it wildly popular. “Only in the last six months or so have patients—and not a lot of them—begun asking me to inject their fingers, right where a ring would sit,” says Dr. Liotta. Some doctors warn that the fingers’ unique anatomy can make injections in the area extra-risky. “The compartments are small and tight, so you have to be careful not to overfill—you also have the digital joints, nerves and arteries to consider,” says Salt Lake City plastic surgeon Renato Saltz, MD. “I’d worry about scar tissue formation, foreign body reactions and infection. It’s not a procedure for the novice injector.” With this and every injection, trust only board-certified dermatologists and plastic surgeons.

To fade sun-induced speckling on the hands, doctors may reach for any number of energy-based devices. (Before committing to one, “have a dermatologist examine your hands for atypical spots and potential skin cancers,” suggests Los Angeles dermatologist Dr. Michael Kassardjian.) Intense Pulsed Light (IPL) therapy scores points because it can tackle a broad swath of flecks on fair skin, in three to five treatments, with little downtime. “It’s strictly for flat sunspots and works best on those that are smaller than a dime,” says Dr. Williams. After zapping, the spots darken and flake off over the course of several days, revealing unsullied skin beneath. For larger, more concentrated splotches, pigment-seeking Q-switched lasers, like the Ruby, Alexandrite and Nd: YAG, can safely treat a range of skin types, minimizing or all-out erasing marks in a single appointment. “The area develops a white frost or pink welt, which transforms within hours to a dark scab or flat pink stain, healing completely in about one week,” says Dr. Christman. For widespread dappling coupled with crepiness, there’s the Fraxel Dual, a resurfacing laser that creates microscopic wounds, instigating a repair process for stronger, smoother, more even-toned skin. The Dual has also been shown to eradicate precancerous lesions, “preventing their progression into full-blown squamous cell carcinomas,” notes New York dermatologist Kenneth Howe, MD. Expect about a week of downtime, during which skin looks sunburned, then bronzed and crumbly. For those concerned more about skin’s springiness than spots, radio-frequency tools, like Thermage and Pellevé, can thicken and tighten the skin of the hands with zero downtime, says Miami dermatologist Janelle Vega, MD. Sun avoidance and protection are extra crucial when recovering from light and laser treatments.

If you’ve ever watched a web of spider veins vanish before your eyes, you’re perhaps already familiar with sclerotherapy. (Thank you, Instagram.) During the treatment, doctors inject an irritant solution into offending veins, which damages their linings, causing them to collapse and shrivel, or close up altogether. Calves and ankles have long been popular targets, but hands, it seems, are the next frontier. “I find most people’s biggest complaint is the unsightliness of the veins,” says New York plastic surgeon Steven M. Levine, MD. With light sclerotherapy, “we can significantly shrink the visible surface level veins on the backs of the hands.” (Published data is scant, however.) These vessels may look important, but Dr. Levine contends they can be diminished without impacting the hand’s function: “Known as reticular veins, they have a great amount of redundancy, specifically within the deeper venous system below the muscles.” Due to their size (between 2 and 5 millimeters), the aforementioned disappearing act may not happen with hands. “The veins typically lighten with the inflow of the sclerosant, and then contract, but can take up to three weeks to fully resolve,” says Dr. Levine (following one to three treatments). The procedure generally isn’t painful enough to warrant numbing cream, but for those wanting to distract from the needle sticks, “we can use cold and mechanical pressure to overload sensory nerves,” says New York vein specialist Luis Navarro, MD. Following injections, hands are wrapped for a day or two to minimize swelling. “The treated veins usually don’t come back,” adds Dr. Navarro. “In the small percentage of patients who do see them recur, it’s five to 10 years later.”

I made them! I discovered JayJun on my first research trip to Hong Kong for Flower Beauty. I had popped into a drugstore in a subway station underneath a weird mall and grabbed their Baby Pure Shining sheet mask. I tried it and was instantly impressed. Two days later in South Korea, I had already arranged a meeting with the company, and we collaborated to launch three masks [JayJun x Drew Barrymore] in Asia. It’s almost like going into a wine shop. You’ll see a product on the shelf and something about the label speaks to you, or maybe you’re in the mood for that varietal. If you’re in the mood for a sheet mask, that’s what you’ll focus on. I’m not crazy about trying tons of color cosmetics—you can get those anywhere. For me, it’s all about the skin-care formulas—what’s that latest innovation you don’t have access to because it’s all the way on the other side of the world. CONGRATS ON WINNING A NEWBEAUTY AWARD FOR YOUR LIP DUO! WHAT INSPIRED IT?
What made me fall in love with lipstick was this mid-’80s double-ended lipstick pencil from Shiseido. Both ends had the same shade, but one side was a buttery finish and the other

And then later I would look at someone like J.Lo on TV and wonder why her lips could so prismatically capture the light. So I created a two-in-one, my Flower Mix N’ Matte Lip Duo, with one very wearable color on one side and an illuminating lip gloss in the same tone on the other. It won’t change the shade at all, but will pick up the light. Actually, it’s three-in-one: this end, that end, and the two ends together! HAVE YOU ALWAYS BEEN A BEAUTY OBSSESSIVE?
I grew up in a makeup trailer, so I knew what makeup was. All of it. But they don’t really do much makeup on kids. They put you in a chair and put a puff on your nose and make you feel a part of it. While we were making Firestarter, they would spray a lot of sticky, wet, viscous glycerin on my face to look like sweat. I was covered in fake blood before anyone ever put lipstick on me! And then in real life, at night, I was going out with a lot of adults who were certainly wearing makeup. There’s nothing like early ’80s rouge! Boy George and Adam Ant were the thing. At 10, I was at dance clubs wearing eye shadow in a straight line out to my temples. I was really into chartreuse and gold, glittery-fine sparkle, like olive-green snakeskin. matter if the date isn’t great, you get to take a moment and be feminine. Put on some music and dance while you get ready and do your makeup. I was filming all day yesterday in prosthetics and color contacts, looking at all the glues and adhesives, and fake noses and eye bags, and eyebrow additives. I’m into the theatrics, the art, all the crazy stuff you can do to transform your face, age yourself, change your hair, look like a different person. When I would work with Kevyn Aucoin, he was really into that. He loved making women into other women. He made me into Myrna Loy.
BEYOND SHEET MASKS, DO YOU HAVE ANY GO-TO SKIN FIXES?
I love the Clear + Brilliant laser. I do it once a year to slough away my sunspots. I also try to get a facial once a year, but I just don’t have the time to go more frequently. WHAT OTHER TRICKS HAVE YOU LEARNED ON SET?
They used to re-powder women all day long, and after 15 hours of hot lights baking it into your skin, the makeup looked so cakey. High-definition stopped people from piling it on, and everyone got inspired by the rice-paper sheets. It became more about blotting and lifting up the oil rather than trying to matte it with more and more powder. Then there was this perfect storm

Just behaving. Being as nice as I can. Not being a total A-hole. Just being in a good mood at work, not losing my cool when my kids lose their cool. Not sweating the small stuff. I’ve realized that even when the little things aggravate you and seem really

Everything became about tinted moisturizer, instead of heavy base makeup. And color correctors! Which was Ben Nye’s approach: he made up all the glamorous women in film in the ’40s, ’50s and ’60s, and then created his own line 50 years ago. His kids run it today. I love that legacy. Sometimes I like to think my girls will be involved in Flower Beauty one day. Olive would be a chemist and Frankie would come up with all the marketing.
big and monumental, or even very public within your own circle and you just wish you could hide your problems, you just can’t lose your cool. Be nice through all of it. That’s always when I feel the best, no matter what. Go put it out in some private corner and then show up and just be good to everyone. HAS BECOMING A MOTHER CHANGED YOUR IDEAS ABOUT BEAUTY?
I do feel like I’m on an upswing. I just went through a couple of hard years, and I can see it wearing on my face. It’s not about aging, it’s about how I am on the inside. There’s also a very long period when you’re raising kids when it takes it out of you. When it depletes your ability to take care of yourself because your new job is doing nothing but taking care of someone else and you love it. I’m not all about working from the inside out—I’m not big on meditation—but I do think your outside cannot hide your inside. I’m now determined more than ever to show my daughters that aging is a luxury. If we’re lucky, we are all going to age. I just want them to be at peace with who they are and not what they look like. If they are good, cool people, that’s all I care about. That said, I feel like people— myself included—have a 2019 new-battery recharge. I’m one of many who are coming out of a semi-difficult hibernation. Like in the spring. And hope springs eternal!
WHEN DID YOU START TAKING SKIN CARE SERIOUSLY?
When I was a teen, I washed my face with a bar of clear vitamin E soap. Nothing. Moisturizers always broke me out. They still do on my chin. My chin is like a completely different neighborhood on my face, and you don’ t go there after dark. Any product that goes on my chin will screw it up. In my late 30s, I started taking my skin seriously. I like scrubs and all of that stuff, and acids and tingling, but I’m still a firm believer in the most gentle face washes. I always want to have a lot of moisturizer and serum, and that’s why I love the new Flower Beauty skin elixir. The goal was to create a formula that makes your skin look like you’ve just left a workout class: pinky, plumped and dewy—like when you have your own body’s blush on your cheeks. or anyone who had braces before ortho game-changer Invisalign came around in 1997, there was no greater joy than when the day came to march into the orthodontist’s office to remove the wires and brackets that moved teeth at glacier speed. For me, it meant a freedom from the migraineinducing task of placing tiny rubber bands around sharp little brackets; a release from the discomfort of housing a mouth full of metal; and an instant makeover worthy of a She’s All That– style, slow-motion reveal. I don’t remember the exact day—somewhere around 1994—but I do remember naively thinking I was officially done going to the dentist, for good. Sadly, the business of wearing a retainer entered my life, and those dreams were short-lived. And because younger me was in charge of how often I wore my retainer, my chances of a lifetime with perfectly straight teeth were slim. Today, the teeth-moving game is light years away from what my post-pubescent nightmares were made of. Metal braces are still a viable straightening option, but there are many other solutions that promise a faster, more affordable and accessible process than ever before. More than 6 million people to date have been
treated with Invisalign—the brand name has become synonymous with clear plastic aligner trays. However, its patents began to expire in 2017, so it’s no longer the only name in the game. With today’s need for immediate gratification, it’s no surprise that teeth straightening is also getting the 1-800-Contacts treatment with a slew of new DIY teeth-straightening retailers—there’s SmileDirectClub (which Invisalign parent company Align Technologies owns a share of ), Byte, Candid, SnapCorrect and Smilelove, among others—changing the way people think about getting their teeth “fixed.” At Candid’s chic West Hollywood studio on trendy Robertson Boulevard, there’s no sign of the normal medical accoutrement that can evoke anxiety in the minds of patients who suffer from “dentaphobia.” There are no dental chairs, drills or sickle probes. Instead, the sleek, bright studio houses white, stylized textured pods reminiscent of Miami’s artsy Faena district, with nothing more than a comfy chair and a 3-D scanner. There are no dentists or orthodontists on site either, but just like at the Apple Store, a consultant greets clients and guides them on their direct-to-consumer journeys. A 10-minute 3-D scan creates images of the teeth, which are then sent to an outsourced orthodontist. Within a month, a box of custom aligners—

two sets for every month of treatment— arrives on the client’s doorstep. Candid, which also offers at-home modeling kits in lieu of an in-store scan, has nine retail shops in major cities like Austin, Boston, New York and San Francisco, and has plans to open 66 additional studios in 2019. SmileDirectClub, the biggest player in the direct-to-consumer aligner game, has 158 SmileShops throughout the U.S. and Canada. Clients can visit a brick-and-mortar shop just once or mail in a modeling kit from home to get started. The convenience factor is a big selling point, but the real appeal is in the fixed price point the DIY brands offer, generally less than $2,000 from start to finish, whether treatment lasts three, six or nine months—a far cry from the $5,000–$8,000 costs estimated for in-office straightening. But there’s one major drawback that both the American Association of Orthodontics (AAO) and the dental community find worrying: The total orthodontal visits required throughout the entire treatment, and this goes for all of the aligner services, amounts to zero. These dental startups rely on teledentistry to monitor patient progress, and depending on the brand, checkups are performed remotely by either a licensed dentist, orthodontist or dental technician. In October 2017, the AAO lodged complaints against online aligner

consider the overall time, compliance and cost involved.” Dr. Kravitz agrees, and adds there are some factors a scan or mail-in impression won’t be able to detect. “Patients who have had periodontal disease, large fillings, veneers and/or crowns are at risk of losing teeth, which they wouldn’t know unless they’ve had a full, in-person dental evaluation.” Dr. Cotca says the list of drawbacks associated with mail-order straightening is long and concerning, “from incomplete diagnosis and potential tooth loss to other complicated conditions such as the onset of temporomandibular disorder (TMJ/ TMD). Comprehensive hands-on evaluation, imaging, testing and data collection is an expert-level requirement.” Beverly Hills, CA cosmetic dentist Laurence R. Rifkin, DDS says there’s more to it than just moving teeth—dentists need to also assess if patients are wearing their trays for the recommended 22 hours a day. “Without X-rays, photographs and models, there’s no way to see what the preexisting bone is like or if there is any gum recession. Regular monitoring of patient compliance is also needed to determine how to properly schedule the right sequence of trays.” Despite warnings from the pros, the at-home straightening trend continues to grow exponentially. Dr. Cotca says one thing all patients, including those who take matters into their own hands, should never do is fall back on regular checkups. “Increased oral hygiene and maintenance during treatment adds to a healthier prognosis and better final result.”

companies with dental boards and attorneys general in 36 states, alleging the companies are creating medical risks by bypassing in-person checkups. The organization has also issued a consumer alert warning, cautioning consumers to think carefully before opting for a procedure “without an in-person evaluation or ongoing in-person supervision from a medical professional.” The companies advertise that their plans are best suited for those with mildto-moderate straightening needs, and the dentists we spoke to stress that in-person evaluations are always needed before any orthodontic treatment. “If you’ve had a regular cleaning and your dentist has examined your teeth and gives the OK to proceed, then those with mild orthodontic needs may benefit,” says Rockville, MD cosmetic dentist Joe Kravitz, DDS. Chevy Chase, MD cosmetic dentist Claudia Cotca, DDS says she cannot recommend DIY straightening and wants the public to know there’s no substitute for seeing a qualified dental professional in person. “Academic credentials, training, clinical skills and expertise are all vital in assessing and treating a patient,” she adds. New York cosmetic dentist Irina V. Kessler, DDS says buyer beware after having to correct some of these DIY treatments herself: “Moving teeth requires continuous patient monitoring. Not solely through photos, but also in person, where progress and potential complications can be assessed. I have redone many cases that were started with DIY straightening, and patients are lucky if they get away with minimum damages. Consumers need to

The type-A toxin activating FDA-approved neurotoxins Botox Cosmetic, Dysport and Xeomin has been shown to ameliorate everything from crow’s-feet and muscle spasms to chronic migraines and urinary incontinence. Fully defying its vanitydrug stigma, “more than half of the botulinum toxin now used in the world is for medical purposes,” says New York dermatologist Paul Jarrod Frank, MD. Aiming to add to its repertoire, Botox is currently being investigated as a treatment for depression, a host of pain disorders, even atrial fibrillation in cardiac patients—earning itself a reputation as “the product that keeps on giving,” says Chicago facial plastic surgeon Steven Dayan, MD.
Botulinum toxin first proved its medical value in the 1970s, correcting ophthalmological conditions like strabismus (crossed eyes) and blepharospasm (marked by uncontrollable blinking). In 1987, the story goes, Vancouver-based ophthalmologist Jean Carruthers, MD, wife of worldrenowned dermatologist Alastair Carruthers, MD, discovered a happy side effect of treating such eye issues with toxins: a softening of the lines between patients’ brows. The drug, which came to be known as Botox, was approved for therapeutic use in 1989, and as a fix for frown lines in 2002.

unrelenting tour de force, with doctors administering over 5 million neurotoxin injections worldwide in 2017, according to the International Society of Aesthetic Plastic Surgery (ISAPS). With a Taylor Swift–caliber fan club, Botox Cosmetic has become a household name—the Kleenex of toxins— and its manufacturer, pharmaceutical goliath Allergan, bears the bottomless pockets needed to keep the brand on the forefront of groundbreaking research. Still, the majority of injectors artfully use all three muscle relaxers in practice, appreciating their subtle differences and finessing them to beautiful effect. The doctors we spoke with also expect to welcome into their armamentaria several new neurotoxins in the near future, each poised to offer its own breakthrough advantages once greenlighted by the FDA. Over the next few pages, we’ll share all we know about the neurotoxins of today and tomorrow—fascinating theories on how they may squelch sadness; the science behind their ability to improve scars, refine pores, prolong blowouts, and out-contour the Kardashians; and why, despite prevailing no-big-deal notions, one should never underestimate the gravity and complexity of these “miracle poisons.”

THE BIG THREE: WHICH WRINKLE-BUSTER IS RIGHT FOR YOU? While Botox Cosmetic may be the most famous of the bunch, Dysport and Xeomin contain the same type of toxin, and work by identical means—blocking the release of a chemical messenger called acetylcholine to lessen muscle contractions for three to four months in most cases. “The overall gestalt of neurotoxins is that there is no one best—they all fill different niches,” says Dr. Frank, due to slight distinctions between final formulations. Beyond an unrivaled track record— “it is the most tried-and-true,” says New York dermatologist Macrene Alexiades, MD—Botox Cosmetic is famous for its potency. “Patients with very strong muscles, and those wanting a complete block, tend to prefer it because it really gives a precise and powerful correction,” says Philadelphia plastic surgeon Ivona Percec, MD, PhD. Dysport reportedly kicks in faster than its competitors— within a couple days opposed to a week. It also “diffuses a bit more into its

surroundings, making it ideal for treating a high forehead or a long stretch of crow’s-feet, or for softening the bands of the neck,” says New York dermatologist Heidi Waldorf, MD. Injectors commonly describe its effect as softer and smoother, “theoretically due to the diffusion,” notes Dr. Percec, “but the data isn’t definitive.” Xeomin is also known for its mild manner, but more so for its peerless purity, as it omits the complexing proteins present in other neurotoxins, “which may play a role in immunogenicity, or the fact that a small percentage of patients don’t respond as well to Botox [and Dysport] after many years of use,” explains Dr. Alexiades. The purpose of the proteins is debatable, but they’re thought to help stabilize the drugs. “While there’s very little evidence of immunogenicity in cosmetic doses, I have had a handful of patients—very few in more than 30,000 treatments—who’ve become inexplicably unresponsive to Botox,” says Dr. Dayan. In those cases, switching them to Xeomin

“usually remedies the problem.” Still, he adds, “it’s hard to say definitively if Xeomin is successful because of the lack of proteins, but some speculate it may be.” Such nuances are largely anecdotal, we should note, and experienced injectors say they can achieve similar results with all three toxins—essentially leveling the playing field between brands. Much comes down to individualized injection technique (a point of pride among doctors) and a little thing called reconstitution—the act of restoring these powdered toxins back to liquid form with saline prior to injection. “This step can differ among injectors, changing clinical outcomes,” says Miami dermatologist Joely Kaufman, MD. A high dilution can beget a quicker onset and more spread— across the board. Boosting the dose of any will give a stronger, more lasting effect. The bottom line: Find a doctor proficient in all toxins, not just one, who can handily manipulate them to suit your unique anatomy and aesthetic goals.

New York facial plastic surgeon Jennifer Levine, MD addressed this 60-year-old patient’s platysmal bands and neck lines by injecting 100 units of Dysport. “The platysma is a depressor of the lower face, so injecting a neurotoxin causes lifting. BEAUTY AND BEYOND: POPULAR USES FOR NEUROTOXINS The list of FDA-sanctioned (on-label) uses for botulinum toxins is concise, but doctors routinely inject them experimentally (off-label) to address countless other beauty and wellness concerns. (FYI: “Botox Cosmetic” and “Botox,” the therapeutic version, are the same drug vetted in different studies.)
FINE LINES Botox Cosmetic, Dysport and Xeomin are approved to treat frown lines; the big B is also cleared for forehead lines and crow’s-feet. “The upper face was the original gold-standard aesthetic use for toxins,” says Dr. Percec. Injecting the lower face was initially considered controversial due to its more intricate anatomy. (An errant needle stick or overdose around the mouth could leave one unable to formulate words or control drooling.) Over time, however, doctors found the drug’s benefits outweighed the risks in these areas. “Muscle strain increases the most around the mouth with age, and as women lose bony mass, our tooth position changes,” explains Dr. Percec. But, neurotoxins can help remedy the repercussions of these shifts: “We’ve now learned to treat lip pursing, the turned-down corners of the mouth, puckering of the chin— all of which are strain-related,” she says. Delicate areas, like the upper lip and neck, demand dilute microdoses of toxin (often combined with filler) for the most discreet effect, and require frequent touch-ups, because small doses lack durability. GUMMY SMILES Injecting minute amounts under and on the sides of the nose will reduce the extreme pull on the upper lip, helping it rest in a more
pleasing position. According to Birmingham, AL dermatologist Corey L. Hartman, MD, this area requires the precise placement of Botox Cosmetic or Xeomin. And, fair warning, with this treatment “you’re changing the quality of your smile—yes, you’ll show less gum, but the corners of the mouth may go up higher than they normally would, so small test doses are key,” says Dr. Frank. LARGE PORES “Micro-sized injections of toxin into the dermis target the arrector pili muscles, which control pore size,” says Dr. Hartman. Relaxing these muscles causes pores to shrink up a bit and secrete less oil. Some doctors inject toxin as part of an AquaGold treatment, during which a small stampingtype tool with hollow needles delivers actives into the skin; others apply toxin directly to the skin’s surface and then press it in with a microneedling pen. CONTOURING “By carefully injecting the muscles that pull the neck down, and purposefully not injecting those that lift the mouth, we can reshape the jawline,” says Dr. Kaufman. TIP FLIP Injecting toxin at the base of the nose “relaxes the muscle responsible for dragging its tip down with age, temporarily raising it back up,” Dr. Hartman says. It’s very important to limit the spread of toxin here, he notes, to prevent slurred speech and an inability to purse the lips or open the mouth. SLIMMING THE JAWLINE Injected into the masseters, or jaw muscles, “toxins can narrow the lower face and make it look more feminine,” says Dr. Dayan. The change comes on gradually

over several weeks. A high dose is needed to shrink them, but “results usually last at least six months, and sometimes longer,” notes Dr. Kaufman. CLENCHING AND GRINDING Lifelong teeth grinding can make masseters bulky. By weakening these muscles with toxin, says Dr. Percec, “it not only softens and slims the face, but also alleviates functional problems, like jaw clenching and teeth grinding.” It’s such a powerful fix, adds Dr. Alexiades, that “many of my patients have stopped wearing night guards.” EXCESSIVE SWEATING The sympathetic nervous system rules our sweat glands by pumping out acetylcholine. When toxins block the chemical, glands can’t contract, and perspiration subsides. “This is where Dysport shines for its ability to diffuse and capture as many sweat glands as possible,” explains Dr. Hartman. Toxins typically take aim at underarms, palms and soles, but can help in less obvious areas, too, like the folds beneath the breasts where sweat tends to pool, sometimes causing rashes and infections. Neurotoxins can stymie sweat on the scalp as well—the hairline, the crown and the nape of the neck—keeping strands clean between blowouts, says Boston dermatologist Papri Sarkar, MD. Results last up to six months. SCARS “Tension is the worst thing for a fresh scar, but when underlying muscles move, the skin stretches,” says New York plastic surgeon Alan Matarasso, MD. “By freezing the area with Botox [an off-label use], we remove the tension and get a better scar.” (Think of it as

a chemical cast.) Studies show Botox may also modify wound healing by other means, tamping down inflammation along with growth factors known to promote keloid scarring, says Dr. Frank. MIGRAINES Botulinum toxins are now a standard treatment for migraines that don’t respond to traditional oral medications. “With Botox [the first brand FDA-approved for migraines], we usually see about a 50 percent improvement in the frequency of migraines, and most patients say they’re better able to control, or abort, oncoming migraines with prescription triptan drugs,” says Boston anesthesiologist Edgar L. Ross, MD. “There’s evidence that botulinum toxins can affect pain-modulating molecules, like neuropeptides, all along the pain pathway, as well as prevent the release of pain molecules like substance P,” says Bethesda, MD neurologist Barbara Karp, MD. “There is also evidence that botulinum toxins can help suppress inflammation.” PAIN DISORDERS Neurologists are using toxins (off-label) to treat all kinds of aches, typically as part of comprehensive painmanagement regimens. Of their many advantages, botulinum toxins “can be safely combined with other treatments, and have very few interactions with medications,” says Dr. Karp, adding that some users are even able to curb their reliance on opioids. “In a study of botulinum toxin for the chronic pelvic pain of endometriosis, we’ve seen women decrease their daily use of analgesics and better tolerate pelvic floor physical therapy after injections,” she adds.

Botox Seeks FDA Approval for Major Depressive Disorder
S
ince the mid aughts, Botox has been moonlighting as an antidepressant, starring in a cache of small mental-health studies, and turning out remarkably consistent results: a 50 to 60 percent response rate; a 45 to 55 percent reduction in depressive symptoms; and a remission rate of approximately one-third, notes Chevy Chase, MD dermatologist Eric Finzi, MD, PhD, in an article published in Dermatologic Surgery in 2018. “People diagnosed with depression are reporting improvements in mood following treatment of facial wrinkling with botulinum toxin,” says Philadelphia psychologist David B. Sarwer, PhD. “These observations have grabbed the attention of both the doctors who offer these injections, as well as mental health professionals seeking alternative therapies for depression.” Botox will soon be entering phase three clinical trials for major depressive disorder, yet scientists still can’t say for sure how the drug works to combat the disease. “We do know that improvement in mood is not related to improvement in wrinkles, because if we treat someone young, who has no lines at rest, their depression can still improve,” says Dr. Finzi. The most probable explanation for the drug’s psychological success, he adds, is the facial feedback hypothesis, or the idea that our expressions can influence our emotions. “Many studies have shown that the mere act of frowning leads subjects to have a more negative appraisal of whatever they’re evaluating—a
story, a cartoon, a photograph. Subjects have also reported feeling sadder right after frowning,” Dr. Finzi explains. It seems the muscles responsible for our angry 11s may play a role in regulating our mindset, not just revealing it. “There’s something unique about the glabella [area between the brows],” says New York psychiatrist Sudeepta Varma, MD. In fact, she notes, studies show through brain imaging that “by quieting the glabellar muscles, we can reduce neural messages sent to the amygdala, or the fear center of the brain, which is known to be overactive in depression and anxiety.” Now, the glabella is arguably the most common injection site for anyone seeking smoother skin. And the doses used in depression studies are roughly equivalent to those used to minimize wrinkles. But according to Dr. Varma, that doesn’t necessarily mean every between-the-brow shot comes with a side of joy, as studies investigating Botox’s mood-altering prowess have looked only at individuals with depression. For that select set, however, “it’s reasonable to see if botulinum toxin treatment of the frown can help, as we know it’s safe, and doesn’t have the side effects of other medications for depression,” notes Dr. Finzi. But not every shrink is boarding the Botox bandwagon just yet. “While it’s fascinating to think botulinum toxins could be an accepted treatment for depression,” says Dr. Sarwer, “we need more research—large, randomized, controlled trials comparing toxins to placebo injections—before recommending them to patients suffering from major depression.”
New Toxins in the Pipeline Coming soon to a doctor’s office near you? A squad of new botulinum toxins, each with its own designer appeal. For the Bo-phobe or neophyte looking for a test run, there’s an innovative botulinum toxin type E from Allergan that works on demand (within 24 hours) and wears off in two to four weeks. The drug has been tested in frown lines and Mohs surgical scars (resulting from skin-cancer removal). It’s also being investigated for its painkilling and spasm-quelling properties when injected into affected muscles during breast augmentation and abdominoplasty surgeries. “Much of the discomfort following these procedures results from the muscle work we do—lifting up the chest muscles when placing breast implants, or putting hundreds of stitches into the abdominals to tighten them,” says Dr. Matarasso. This short-acting shot “could be good, because some people are still sore a couple weeks later.” Doctors anticipate it gaining approval around 2025. Nearer on the horizon is a neurotoxin from Revance Therapeutics shown to outperform all current options: “In many studies, we’re seeing it last seven to nine months,” says Dr. Carruthers, a lead investigator on the trials. The product contains the same active component as Botox Cosmetic, but adds a proprietary peptide thought to prolong the toxin’s reign. “We compared 20, 40 and 60 units of the Revance product to 20 units of Botox, and also to placebo, and the longevity was just spectacularly better with the Revance groups,” says Dr. Carruthers, noting that the 40-unit dose offered the best balance of longevity, natural appearance and lack of complications. “Subjects were thrilled with how they were looking at every single visit.” There were a few cases
of mild and transient eyelid drooping, Dr. Carruthers notes, which resolved before the drug wore off. “You’d think that if a product lasted twice as long [as Botox Cosmetic] that the complications would, too, but somehow that didn’t seem to be the case.” Attempting to measure up, Botox Cosmetic is being tested at higher doses (40, 60 and 80 units), and proving a link between quantity and longevity. But bigger isn’t always better. “I don’t advocate the higher dose, as it can lead to a more frozen look and I want patients to retain some muscle action,” says New York facial plastic surgeon Jennifer Levine, MD. On the flipside, Dr. Alexiades says she’s already giving patients five to six months of smoother skin—without sacrificing expression—by injecting larger doses of Botox Cosmetic over two sessions, so as not to “overdose on the first.” Dr. Carruthers also uses generous doses in practice when treating athletes and certain men with outsize facial muscles, but says “in a delicately muscled person, I certainly wouldn’t do this because it wouldn’t provide a natural look.” A third newcomer, Jeuveau from aesthetics company Evolus, won FDA approval for frown lines in February. The toxin hails from Korea where “it’s been used safely and effectively for years,” says Dr. Carruthers. As for its onset, duration, efficacy and risk profile, “it appears to have similar clinical properties to Botox Cosmetic,” she adds. Jeuveau’s edge will likely be its lower price point—a perk stemming from the fact that “the drug will only be used in aesthetics, thereby decreasing the amount of money spent on research studies,” says Dr. Kaufman. What’s more, adds Dr. Percec, with no therapeutic counterpart, its price won’t be swayed by insurance companies, which is ordinarily the case with toxins that work in both worlds.

Botox Cosmetic isn’t an anti-aging everything. Deep glabellar grooves and etched-in lip lines, for instance, won’t disappear with toxin alone. “As we age, keeping it natural generally requires combination therapy with toxins, fillers, resurfacing and tightening devices,” notes Dr. Waldorf. Plastic surgery, too, some would argue. Toxins may slow the descent of the brow or jawline to an extent, but “they’re not going to give you what a facelift does,” says Dr. Matarasso.
While the very idea of a paralytic going rogue inside your body is, indeed, alarming, doctors say the risk is exceedingly rare: “With highvolume toxins, like those used to treat medical conditions, such as cerebral palsy spasticity, this can potentially be a problem,” says Montclair, NJ dermatologist Jeanine Downie, MD. “But in all of the patients I’ve injected with standard cosmetic doses of Botox since 1994, I’ve never had an issue with a toxin spreading beyond the injection site.”

Pedaled at house parties like burp-able storage containers, and teased on placards fronting stripmall spas, “Botox is quickly losing credence as a real medical procedure,” laments Dr. Downie. In truth, neurotoxins are prescription drugs intended to be injected in sterile environments by experienced practitioners who not only know their way around a syringe, but also have the chops to handle any complication. If you haven’t yet pledged allegiance to an MD injector, consider these reasons for doing so. NEUROTOXINS AREN’T WITHOUT RISK.
This 35-year-old patient was bothered by her gummy smile. Philadelphia dermatologist Nazanin Saedi, MD treated her with Botox Cosmetic, injecting a total of six units to target the levator labii superioris alaeque nasi muscles, which lift the upper lip and dilate the nostrils. The dose was divided into two injections at the base of the nose. The “before” photo features the patient’s original smile; the “after” photo was taken two weeks post-treatment.
Pain, swelling and bruising are common and fleeting, but “when the product goes outside of the intended muscle, one may experience droopy eyelids, dropped eyebrows, crooked smiles, drooling, and excessive tearing or dry eyes,” says Dr. Hartman. And because there’s no undo button for Botox, such effects can linger for months. Getting injected by a board-certified dermatologist or plastic surgeon is the best way to sidestep would-be problems. DOCTORS ARE ANATOMY EXPERTS.
Anyone can push a needle, but MDs are versed in muscular interplay and anatomical nuances, and can better tailor treatments. A prime example: Everyone’s

keen on injecting the masseters for a slimmer look, but in jowl-prone patients, doing so can be detrimental, says Dr. Percec, “because the bulk of that muscle in the posterior jaw is actually creating a lifting effect.” These folks need to preserve the masseters, not erode them, to keep jawlines tight. Another hiccup often unforeseen by novice injectors: exacerbating eyelid hooding—a consequence of lowering the brow too much—in pursuit of a smoother forehead. “I prefer to treat patients with any brow or lid laxity over two appointments,” says Dr. Waldorf. “During the first, I create brow elevation, and at the second, I review the residual forehead movement and determine if more toxin is appropriate.” IT HELPS ENSURE YOU’RE GETTING THE REAL DEAL.
Counterfeit toxins are on the rise. Getting injected in a physician’s office “significantly increases the chances that products are obtained directly from manufacturers, and are, in fact, authentic,” says Dr. Hartman. (Use the “find a specialist” function on any toxin maker’s website to confirm your doc of choice buys straight from the source.) Is the MD experience more expensive? Yep, usually, because the price you pay is a reflection of your injector’s level of experience coupled with the cost of the toxin. Cheap isn’t a word we’d want associated with injections either.

If there is no excess skin of the lower lids, injection of a non-hydrophilic hyaluronic acid on the bone and into the fat pads on the bone can lift out the shadowy hollow and restore a youthful look.” “In Candi’s case, she had excess folds of skin on the upper eyes and lower, as well as hollowing in the lower eyelid area. She was in excellent health and a great candidate for skin removal for both the upper and lower eyelids. She also had hollowing in the lower eyelids, so two layers of hyaluronic acid filler were placed on the bone to ‘lift out’ the shadows for a more refined appearance. We also performed a few sessions of Botox prior to the procedure to aid in the lifting of some of the wrinkles around the eyes. “With breast lift or mastopexy surgery, some patients are just unwilling to accept the anchor scar. I remind my patients that we always have to make a trade in plastic surgery—give
something to get something. In mastopexy, we trade shape for scars. The only way to change a flat, long breast into a full, uplifted one is to remove the excess skin, which requires that we make incisions. Ordinarily, if you are happy with your volume tucked up in a bra, just tightening the skin, removing the excess and repackaging the volume can give a very nice-looking breast. Bringing the nipple up so that it looks forward instead of down, as well as getting more breast fullness, can be well worth the trade of the anchor scar. Most patients love their new look and feel that the compromise is worth the resultant shape improvement. All scars take six to 12 months to fully mature and become flesh colored or white. In my practice, we apply an exterior antibacterial protective mesh after wound closure to reduce stress and encourage healing. We have found that this improves scars significantly. Topical scar cream is started around four or five weeks post-surgery until the scars are completely mature. Additional interventions for scars that do not heal properly include injections, laser skin treatments and surgical scar revision.”

“PRP does not replace hair restoration surgery, but it can enhance it. I also have used it in patients who are not candidates for surgery, such as women with generalized hair loss. What patients report is less hair loss—‘less hair in my comb’ is a common comment. Many patients also enjoy a PRP facial, which can be done at the same time.”

There’s been a lot of talk surrounding Brazilian Butt Lift surgery risks. Is it still a safe procedure to undergo?

Meeting Drew Barrymore is a bit surreal—her presence in pop culture is all-encompassing and she makes everyone in the room feel like a friend she’s known for years. Spoiler alert: She’s as cool and down-to-earth in person as one would expect, and she takes an especially hands-on approach to everything she does, including our cover shoot.
Drew keeps “an arsenal of good eye creams” on hand to help with dark circles. “I like the Clinique Even Better Eyes Dark Circle Corrector,” she says. “It has a cooling metal tip, and the peachy tint is really flattering. You can almost wear it by itself in the daytime—it has such a good tone. Olay Total Effects 7 in One Anti-Aging Eye Transforming Cream is comparable. It’s a little pinky, but it performs the same way. I’m also obsessed with Juara Miracle Tea Complete Eye Crème. I noticed a difference within a week of using it.
